BACKGROUND : The recent studies shows that Helicobacter pylori (H. pylori) related chronic gastritis leads to dysplasia, intestinal metaplasia and gastric cancer. The aim of this study was to determine the association between precancerous lesion of stomach (dysplasia, intestinal metaplasia) and H.pylori infection in the elderly. METHODS : 469 patients were enrolled this study, Patients with intestinal metaplasia or dysplasia (case, n=148) were compared with controls (without intestinal metaplasia and dysplasia, n=321) about H. pylori positive rates H. pylori positivity was confirmed by histologic determination of endoscopic biopsy specimens. Results : 1) Among study populations H. pylori positive rates were 51.8% (55.8% in male, 40.7% in female). It was significantly higher in male (p=0.001). 2) There were no significant differences in sex distribution between cases and controls, but in old age, the cases were more frequent than in young age (p<0.001). 3) Age distribution of H. pylori positive rates were not significant between cases and controls. 47.5% in intestinal metaplasia, 43.4% in dysplasia, 21.3% in dysplasia+intestinal metaplasia were H. pylori positive. H. pylori positive rates were more higher in controls than cases, followed by in decreasing order by the intestinal metaplasia group, the dysplasia group, and those having both (p=0.003). 4) Age and sex adjusted H. pylori positive rates were more higher in controls (42.7%) than in cases (25.0%)(p<0.009). CONCLUSION : The prevalence of intestinal metaplasia and dysplasia was age dependent. But we couldn't explain the carcinogenic effect of H. pylori, because H. pylori positive rates were lower in cases than in controls

PURPOSE: This study was to analyze the effect size of complementary and alternative intervention studies in reference to dysmenorrhea and menstrual distress. METHODS: In order to conduct a meta-analysis, a total of 393 studies were retrieved from the database. Twenty-eight studies that were published from March 2001 to February 2011 were selected. RESULTS: Intervention studies included seven studies on aromatherapy, five on auriculotherapy, three on each Koryo-Sooji-Chim and moxibustion, two on each heat therapy and magnetic therapy and six on other therapy. The effect size of the intervention studies on dysmenorrhea and menstrual distress was greater than 0.48 for Koryo-Sooji-Chim, moxibustion, aromatherapy, auriculotherapy and other therapy. CONCLUSION: This study suggests that drug free therapy can reduce the levels of menstrual distress, despite the small number of intervention studies and randomized controlled trials.

BACKGROUND: Hepatitis A is an acute infectious disease of the liver caused by the hepatitis A virus (HAV). The purpose of this study was to investigate the HAV vaccination status and related factors among college students. METHODS: The study subjects were 503 students conveniently sampled from C university located in one district of Jeollanamdo. General characteristics and HAV vaccination related factors were collected by a self-reported questionnaire in October 2013. Chi-square tests and multiple logistic regression analyses were used to evaluate related factors with HAV vaccination. RESULTS: Of the 503 subjects without a history of HAV infection, 65 subjects (12.9%) reported that they were vaccinated. In multiple analyses, subjects who have had health screening (adjusted odds ratio, aOR=2.44 and 95% confidence interval, 95% CI=1.26-4.73) and those who were aware of HAV infection (aOR=6.00, 95% CI=1.81-19.91) or who perceived the benefits of HAV vaccine (aOR=2.05, 95% CI=1.07-3.93) were more likely to be vaccinated than those not involved in these factors. With regard to intention for HAV vaccine, 314 subjects (71.9%) reported that they have the intention to be vaccinated against HAV. In multiple analyses, higher grade level, high monthly allowance, awareness of HAV vaccine, perceived susceptibility of HAV, perceived seriousness of HAV and perceived benefits of HAV vaccine were significantly associated with the intention for HAV vaccination. CONCLUSIONS: The vaccination rate for HAV was found to be low in our sample of college students. To reduce health risks related with HAV, prevention efforts should consider these results.

BACKGROUND: Pyoderma gangrenosum is a rare disease in which a painful nodule or pustule breaks down to form a progressive enlarging ulcer. Until now, only 8 cases of pyoderma gangrenosum have been reported in Korea. Therefore, we thought it necessary to perform a clinical analysis of pyoderma gangrenosum in Korea with a review of literature. OBJECTIVE: Our purpose was to find the clinical features of pyoderma gangrenosum in Korea. METHODS: Fourteen cases with pyoderma gangrenosum were investigated by reviewing medical records. Results There were 6 males and 8 females. The onset age was between 4 years and 65 years, and most(9 cases) had developed the condition between the ages of 20 and 60. Thirteen cases involved the extremities and 3 cases had whale body involvement, Seven cases(50%) had multiple lesions. All cases had pain at the lesional sites. Two cases were classified as the bullous type and the others were ulcerative in nature. The histological fmdings were non-specific. Dense inflarnmatory infiltrates composed of lymphocytes and predominant neutrophils were found in the epidermis and dermis associated with ulceration. Two cases were treated only with systemic steroids, and 2 cases with dapsone and steroids, 2 cases with colchicine and steroids, and 1 case with steroids, dapsone and colchicine. One case was treated with colchicine and anti-Tbc drug, 1 case with dapsone, 3 cases with antibotics, 1 case with the anti-Tbc drug and 1 case with anti-cancer drugs. Systemic disease was present in 5D% of the cases. The associated diseases were Behcets disease(3 cases), tuberculosis(2 cases), systemic lupus erythematosus(1 case), pancytopenia(1 case), iron deficiency anemia(1 case), acute leukemia(1 case), and colon adenoma(1 case). Recurrence developed in 2 cases and positive pathergy reactions were observed in 3 cases. CONCLUSION: Pyoderma gangrenosum was eccompanied with systemic disease in 50% of the cases and the most common therapeutic drugs were steroids. It is therefore impartant to detect the presence of any underlying disease and to treat this alongside pyoderma gangrenosum.

PURPOSE: The purpose of this study was to explore emotional labor experienced by nurses in ambulatory care setting. METHOD: The phenomenological method developed by Giorgi was used for this study. The participants were 9 nurses who had experienced emotional labor. Data were collected between May and August 2010 by face-to-face interviews. The interview was recorded and then transcribed. RESULTS: The constituents associated with the meaning of the nurses' experiences of emotional labor in ambulatory care setting were as followings: bearing down of suffering emotions coming up from the bottom, feeling loneliness when having to undertake care alone by oneself, having conflict between nursing professional and services, managing mind by means of both internal and external resources, getting ridding oneself of conflict with forced emotions over time. CONCLUSIONS: The results of this study should contribute to a deeper understanding of the meaning of emotional labor experienced by nurses in ambulatory care. The results also highlight the need to develop programs for nurses in ambulatory care setting to help them express their real action.

This study was done to investigate satisfaction of fifth-grade(n=264) and sixth-grade(n=117) students from school foodservice in five elementary schools in Won-ju. On a scale of 1 to 5, with 1 being very poor and 5 being very good, overall satisfaction from school foodservice was found to be 3.21+/-0.98('so-so~satisfaction'). The highest satisfactory factor was neatness of foodservice staffs(3.79+/-1.00), while the lowest satisfactory factor was the students' overall opinions of school foodservice(3.03+/-1.06). The biggest unsatisfactory factor was found to be 'bland food'. 'Enhancement of taste'(49.08%) caused the most complaints against school foodservice. Soup was identified as the type of food most likely to be left over, and there was some significant difference between fifth-graders and sixth-graders. The reason cited most often for leftovers was unfavorable menus(33.42%). Students were more likely to try foods which were unfamiliar to them and also foods they previously disliked as a result of using school foodservice(28.87%). Also significant, when the meal director was present during the meal service, satisfaction was markedly higher than when the director was not present.

Primary angiosarcoma of the breast is extremely rare and is observed in 0.0005% to 0.05% of primary breast tumor cases. The diagnosis of this tumor is difficult due to its undefined characteristics. Radiologic findings are often nonspecific and appear completely normal in one-third of patients with primary angiosarcomas. The prognosis is usually poor, and the treatment choices include mastectomy or wide excision. Radiotherapy and chemotherapy produce varying results. We report a patient with primary angiosarcoma of the breast to further our understanding of the characteristics of this tumor and facilitate the correct diagnosis of breast angiosarcoma.

PURPOSE: To report the results of 134 cases in which add-on stereotactic core biopsy (ASCB) was performed in patients with mammographically detected breast lesions, and to evaluate the usefulness of this procedure. MATERIALS AND METHODS: We analyzed the results of ASCB of 134 breast lesions in 125 patients, performed during a 41-month period. The mammographic findings were suspicious malignant lesion in 38 cases, benign lesion in 18, and indeterminate lesion in 78. Surgical excision was performed in 23 cases, and follow-up mammography in 39. We analyzed the pathologic results according to each mammographic finding and correlated the results of core biopsy with those of surgical excision. We also evaluated the mammographic changes seen during follow-up, and associated complications and procedural difficulties. RESULTS: Samples were adequate for pathologic diagnosis in 95% of cases (127/134). ASCB revealed malignancy in 47% of cases (18/38) in which this was suspected on the basis of mammographic findings, and in 5% of cases (4/78) in which these findings were indeterminate. The pathologic results of core biopsy and of surgical excision agreed in 78% of cases (18/23). In two of five false-negative cases, ASCB revealed the presence of atypical ductal hyperplasia. The mammographic findings in these five cases were suspicious malignancy in three, and indeterminate in two. Specimen radiography showed calcifications in four cases. The size or extent of mammographic lesions did not change during the mean follow-up period of 17.3 months. In 13/125 patients (10%), the complications and procedural difficulties noted included arterial bleeding, dizziness, syncope, patient movement, and instrument failure. CONCLUSION: ASCB is accurate, safe and useful, but surgical excision should be considered when the ASCB result is either atypical ductal hyperplasia or benign but with mammographic diagnosis of suspicious malignant or indeterminate lesions.

PURPOSE: To investigate the trend of refractive change in hyperopic patients according to increases in age. METHODS: Eighty-eight children who had hyperopia of more than +1.50 diopters (D) and could be followed up for at least 5 years were included in this study. We divided the patients into two groups according to the level of initial hyperopia and retrospectively analyzed hyperopic refractive changes over a 5-year period according to age at initial diagnosis, presence of esotropia, amblyopia, astigmatism and anisometropia. RESULTS: We gained the following formula about the aspect of hyperopic reduction in 88 patients over a period of 5 years: Diopter (D)=7.99-2.14 Ln (age). The presence of anisometropia and amblyopia did not affect hyperopic reduction. Hyperopic reduction amounts in the group with a hyperopic eye of more than +5D at initial diagnosis were greater than in the group with a hyperopic eye less than +5D, and greater in the group with an astigmatic eye of more than 1D. The presence of esotropia and the age at initial diagnosis did not affect hyperopic reduction. CONCLUSIONS: We show that emmetropization in hyperopic children occurs according to the following formula: Diopter (D)=7.99-2.14 Ln (age). Both the initial level of hyperopia and the concurrent presence of astigmatism affected hyperopic reduction. However, the presence of anisometropia, amblyopia, esotropia and the age at initial diagnosis did not significantly affect hyperopic reduction.

Aplasia cutis congenita is a congenital localized or widespread absence of the skin. We report a case of aplasia cutis congenita, type V, in a 6-day-old male infant born with the stellate and linear skin lesions covered by granulation tissue and soft capsule with slightly elevated erythematous edges on the trunk and lower extremities without any associated family history. The patient had amniotic bands and were diagnosed as aplasia cutis, type V. The patient received conservative treatment such as antiseptic dressing and prophylactic systemic antibiotics with healing of the ulcer.
