2.The Effect of Supportive Nursing Intervention on the Anxiety and Nursing Satisfaction of Mothers with Children Undergoing Open-Heart Surgery.

PURPOSE: The purpose of this study was to identify the effect of supportive nursing intervention on the anxiety and nursing satisfaction of mothers with children undergoing open-heart surgery. METHODS: The research design involved a non-equivalent control group pretest-posttest non synchronized design. The subjects consisted of 43 mothers of children who were expected to undergo open heart surgery, and were divided into two groups; an experimental group of 22 and a control group of 21. The experiment was administered to the experimental group a total of four times, following supportive nursing intervention protocols. The research tools used were Spielberger's State Anxiety Inventory for anxiety and Park Jung-Eun's nursing satisfaction. The data were analyzed using the chi-square-test, and t-test with SPSS/WIN 12.0 program. RESULTS: Hypothes 1, 'The experimental group who are given supportive nursing intervention will have a lower anxiety score than the control group' was supported(t=5.658, p=.000). Hypothesis 2, 'The experimental group who are given supportive nursing intervention will have a higher nursing satisfaction score than the control gropu wasl supported(t=-9.549, p=.000). CONCLUSION: The supportive nursing intervention protocol was effective in reducing anxiety and increasing the nursing satisfaction of mothers with children undergoing open-heart surgery.

3.A Comparison of Patients' Nursing Service Satisfaction, Hospital Commitment and Revisit Intention between General Care Unit and Comprehensive Nursing Care Unit

PURPOSE: The study aim was to compare nursing service satisfaction, hospital commitment and revisit intention between patients on general care units and comprehensive nursing care units. METHODS: Participants were 201 hospitalized patients in a WHO tertiary hospital and a general hospital. Data were analyzed using SPSS/WIN 23.0 program. RESULTS: There was a significant difference in the satisfaction with nursing service between patients on general care units and comprehensive nursing care units (t=14.73, p < .001). There was a significant difference in the hospital commitment between general care units and comprehensive nursing care units (t=7.52, p < .001). There was a significant difference in the revisit intention between patients on general care units and patients on comprehensive nursing care units (t=6.01, p < .001). There were significant relationships among nursing service satisfaction, hospital commitment and revisit intention. CONCLUSION: The findings reveal that patients on comprehensive nursing care units were more satisfied with the nursing service, hospital commitment, and revisit intention compared to general care unit patients. Nursing service satisfaction and hospital commitment were factors affecting hospital revisit intention. Therefore, these results are important data for complementing and expanding the comprehensive nursing service system to improve satisfied with nursing service and hospital commitment to increase hospital revisit intention.

4.Developing Dish-based Food Frequency Questionnaire for the Epidemiology Study of Hypertension Among Korean.

The purpose of the present study was aimed to identify dish items applicable in developing dish?based food frequency questionnaire (DFFQ) for a hypertension study of Koreans. The 24-hour recall data of 4,401 subjects aged 20~65 years from the 2001 Korean National Health and Nutrition Examination Survey were used for the analysis. Logistic regression model was used to identify the nutrient related with hypertension. Energy, fiber, sodium, calcium, carotene, vitamin B1 and vitamin C were associated with hypertension. Selection the top 30 dish items for these seven nutrients was performed based on their degree of contribution in supplying nutrients in terms of the cumulative percent contribution (cPC), as well as on their degree of explanation for between?person variation, in terms of the cumulative regression coefficient (cMRC). Rice supplied 43% of total energy consumption. Korean cabbage and radish kimchi were two major sources of sodium and it also covered the 27% of between person variation of sodium intake. Soybean paste soup and single item of orange?colored fruits supplied 43% of total vitamin C intake for Koreans and it covered the almost 79% of between person variation among the Korean population. Korean cabbage kimchi was the major source of fiber, calcium, sodium, carotene, and vitamin C for Koreans. In summary, the top 30 selected dish items supplying the 78% to 89% of the 7 nutrients. Those items also covered the 79% to 94% of between person variation of the 7 nutrients consumption. Therefore, the selected 30 dish items in each categories of nutrient could be applicable in developing dish based food frequency questionnaire for hypertension study.

Little data exists on metabolic syndrome (MetS) related with intake, especially for the South Korean. The purpose of this study was to develop and evaluate a food frequency questionnaire (FFQ) for nutritional assessment in the population with MetS in South Korea. Randomly selected female participants, mean age 21.9 years (n = 38) were invited to answer the FFQ twice (FFQ1 and FFQ2) over a nine-month interval and to complete twelve-day diet records (DR) during the months between in South Korea. The correlation coefficients for nutrient intake between FFQ1 and FFQ2 varied from 0.253 (niacin) to 0.573 (cholesterol), and the energy intake-adjusted correlation coefficients ranged from 0.187 for protein to 0.662 for iron. The energy intake-adjusted and de-attenuated correlation coefficients for comparison of FFQ1 and the DRs ranged between 0.277 (vitamin B1) and 0.768 (fiber), and between 0.229 (zinc) and 0.859 (fat) for comparison of DRs with FFQ2. The percentages of study subjects classified into the same quartiles in both the DRs and FFQ1 ranged from 15.8% (vitamin B6) to 47.4% (calcium), and for the same quartiles in DRs and FFQ2 from 13.2% (vitamin B1) to 44.7% (potassium). The FFQ has reasonably good validity and reproducibility. Further research is needed for an assessment of reproducibility and validation of present FFQ in the subjects with MetS.

7.Bilateral Elastofibroma: Report of a case.

Elastofibroma is a rare benign tumor-like condition manifesting as firm and spherical mass with poorly circumscribed margins of fibroelastic tissue, occuring in the subscapular region or the chest wall of elderly persons. It is not a true neoplasm but rather a reactive or degenerative process causing abnormal elastogenesis. It is unilateral in the majority of cases and the right side is affected more commonly than the left. We report a case of bilateral elastofibromas removed from both subscapular regions of a 73-year-old female farmer. She was presented with tender masses on the bilateral subscapular areas for seven years. Microscopically, it consisted of a mixture of intertwining broad eosinophilic collagen bundles and elastic fibers associated with a few fibroblasts and mature fat cells. The elastic fibers had a degenerated beaded appearance or were fragmented into serrated globules in a linear arrangement.

9.Factors Related to Nurse Staffing Levels in Tertiary and General Hospitals.

BACKGROUND: Adequate staffing is necessary to meet patient care needs and provide safe, quality nursing care. In November 1999, the Korean government implemented a new staffing policy that differentiates nursing fees for inpatients based on nurse-to-bed ratios. The purpose was to prevent hospitals from delegating nursing care to family members of patients or paid caregivers, and ultimately deteriorating the quality of nursing care services. PURPOSE: To examine nurse staffing levels and related factors including hospital, nursing and medical staff, and financial characteristics. METHODS: A cross-sectional design was employed using two administrative databases, Medical Care Institution Database and Medical Claims Data for May 1-31, 2002. Nurse staffing was graded from 1 to 6, based on grading criteria of nurse-to-bed ratios provided by the policy. The study sample consisted of 42 tertiary and 186 general acute care hospitals. RESULTS: None of tertiary or general hospitals gained the highest nurse staffing of Grade 1 (i.e., less than 2 beds per nurse in tertiary hospitals; less than 2.5 beds per nurse in general hospitals). Two thirds of the general hospitals had the lowest staffing of Grade 6 (i.e., 4 or more beds per nurse in tertiary hospitals; 4.5 or more beds per nurse in general hospitals). Tertiary hospitals were better staffed than general hospitals, and private hospitals had higher staffing levels compared to public hospitals. Large-sized general hospitals located in metropolitan areas had higher staffing than other general hospitals. Occupancy rate was positively related to nurse staffing. A negative relationship between nursing assistant and nurse staffing was found in general hospitals. A greater number of physician specialists were associated with better nurse staffing. CONCLUSIONS: The staffing policy needs to be evaluated and modified to make it more effective in leading hospitals to increase nurse staffing.
