1.The Experience of Mechanically Venticanted Patients.

This study was aimed by applying phenomenal way of study which is the qualitative way of study in order to provide basic data for the whole nursing of the patients with artificial recipiratory organ who are in dangerous situations called mechanical ventilation aid by confirming and understanding the significance of the experience of the objects who experienced artificial recipiratory organ. The patients who agreed to participate at the study after being given the explanation, who are able to exchange minds as they are separated from the recipiratory organ and tube after having been fully recovered to the ability of breathing at present after the experience of mechanical ventilation aid in the room for serious patients of one general hospital at Sung Nam City, Kyongido province, and they are 5 in number who could repeat the statement about the experience, and it has been from Aug. 1998 through Apr. 1994. The data have been collected from the participant patients who had the feeling of trust and intimacy favorably with the researcher as they have been taken care for by the researcher when they were in artificial recipiratory organ in the room for serious patients in the hospital where the researcher has been assigned, and the details of conversation with them have been recorded directly and immediately and in order to prevent details of conversation from being omitted, they are tape-recorded by the consent of the patients. Phenomenal way of analysis suggested by Giorgi was used based on the data described from the words of the patients and the details of observation of the researcher, and the results of the study are as the below; The significance of the experience of the patients who had artificial recipilatory organ through confirmation by deep interivew and observation including participation: 1) The sense of loss: loss of mind exchange, loss of sleep, loss of guidance, loss of sense, loss of role, loss of self-concept, loss of self-control, loss of self-decision and loss of human resources. 2) Felling of inconvenience: Pain, agony 3) Psychological discords: confusion, grudge, felling of burden, fear, instability and sadness 4) Unconsciousness reaction: dream 5) Adaptation: reception, longing and feeling of gratitude and others. The significance in the science of nursing centered by the above results are as the follows; The patients with mechanical ventilation aid have been experiencing bigger pain than anyother patients in the room of serious patients due to environmental stress of the room of serious patients and the situational stress of having artificial recipiratory organ together with psychological pain due to the disease, and I think without any suitable arbitration they might be handicapped in the thought and action. The nurse who takes care of the patients with artificial recipiratory organ would be able to provide the suitable nursing arbitration which can decrease effectively the stress if the nurse has the concrete understanding about their stress as the person in intimate relationship nearest them. But until today the study on the significance of the experience of such patients has been scarcely conducted to the ones with artificial recipiratory organ and no study has been conducted in Korea. This study has been made with the effort of trying to understand deeply the experience of the patients in psychological, physical, social and spiritual views of the patients equipped with artificial recipiratory organ, therefore based on the experience of the patients with artificial recipiratory organ confirmed by this study, I think this study would be meaningful as the basic data for preparing the way of nursing arbitration effectively for the wholeman nursing for the patients who are given the mechanical ventilation aid.

MIN GLASSES is specially manufactured in order to enhance satisfaction of wearer and therefore increase the effectiveness of treatment of an amblyopia and a diplopia. One of the lens is specially grounded and coated to blur vision. Cosmetically, the eye appears to wear regular glasses through the blur lens. We prospectively studied 58 amblyopic children and 10 diplopic adults. Each patient was treated with MIN GLASSES for cosmetic problems. We checked linear visual acuity of pre- and post-treatment. Questionnaires were given to the patients and parents regarding cosmetic satisfaction with treatment. In amblyopic children. 44 patients(75.9%) showed visual acuity improvement of 1.61 +/- 1.50 lines by Han's visual acuity chart. Patient compliance was 87.9%(51 patients) with patient satisfaction based on a questionnaire response. Parents were 87.9%(51 parents) positive response. In diplopic adults, 10 patients(100%) were positive response with satisfaction and demonstrated to be free of symptom of diplopia. MIN GLASSES is effective in high compliance satisfaction of amblyopia and diplopia, and improvement of visual acuity of amblyopia.

5.Phase II Study of Paclitaxel and Cisplatin as Second-line Chemotherapy in Advanced Non-small Cell Lung Cancer.

BACKGROUND: To evaluate the efficacy and safety of paclitaxel and cisplatin against advanced non-small cell lung cancer (NSCLC) as a second-line chemotherapy. SUBJECTS AND METHODS: Twenty-five patients were enrolled. The patients received 200 mg/m2 paclitaxel as a 3-hour intravenous infusion and 60 mg/m2 cisplatin as 30-minute intravenous infusion with vigorous hydration on day 1 every 28 days. The response was assessed every 2 cycles. RESULTS: All 25 patients were assessed for their response and toxicity. Partial responses were observed in 5 patients. The overall response rate was 20% (95% confidence interval, 4%~36%) and the median response duration was 4.5 (range, 2-11) months. The median time to progression was 3.3 (range, 0-14) months. The median overall survival of all patients was 7.4 (range, 1.3-39) months. The hematologic toxicities were minor and easily controlled. CONCLUSION: The combination chemotherapy of paclitaxel and cisplatin as a second-line treatment has a moderate efficacy with an acceptable toxicity in patients with advanced NSCLC.

6.Two Cases of Ectopic Gallstones Treated by Endoscopic Therapy.

Two cases are herein reported involving patients with ectopic gallstones which were discharged into the stomach and duodenum through a cholecystoduodenal fistula and successfully removed by endoscopic therapy. In the first case, a 75-year-old man was admitted with epigastric pain. Simple abdomen film demonstrated a round laminated calcification and air biliarygram in the RUQ. Endoscopic examination revealed a fistula on the posterior wall of the duodenal bulb and a brown stone (about 5 cm in diameter) was found in the second portion of the duodenum, It was demolished through endoscopic electrohydraulic lithotripsy (EEH1) and discharged with the stool. In the second case, a 55-year-old man was admitted with epigastric pain. A CT scan revealed an ovoid laminated calcification in the dependent portion of the stomach. Endoscopic examination revealed a fistula on the anterior wa11 of' the duodenal bulb and a black pigmented stone (about 2.5 cm in diameter) was found in the stomach. This stone was removed orally by an endoscopic snare. These patients were discharged and remained asymptomatic.

BACKGROUND AND OBJECTIVE: Assessment of quality of life (QOL) of patients with chronic illness requires reasonable tools which reflect the patients' cultural and behavioral properties. We developed the quality of life questionnaire for adult Korean asthmatics (QLQAKA) on the basis of the Korean life style and evaluated its reliability and validity. METHODS: The QLQAKA consisted of four domains; symptoms (six items), daily activity (five items), emotion (three items) and reaction to environmental stimuli (three items). Patients answered each item according to a five-response scale. The reproducibility and validity of the questionnaire was estimated from the responses of 244 patients who visited the clinics in 15 institutes within a 2-week interval. RESULTS: Items with the most frequent complaints were dyspnea (87%), difficulty in sputum discharge or throat clearing (87%), limitation in strenuous activity (84%) and coughing (82.4%). The QLQAKA reflected the changes of patients' status very well. The value of minimal important differences, such as the clinically significant minimal change in the QOL score, was 0.5. The questionnaire was also highly reproducible with the value of intraclass correlation coefficiency and intraclass standard deviation as 0.940 (p<0.001) and 0.180, respectively. The changes of mean total QLQAKA score correlated weakly with the changes of FEV1 and PEFR values. CONCLUSION: The adult version of QLQAKA was valid and may be a reproducible tool for evaluating and monitoring Korean adult asthma patients.
