4.Distribution of Antibodies to Coxiella burnetii in Patients with Unknown Fever and Atypical Pneumonia.

Coxiella burnetii is the causative agent of Q fever worldwide in human and animals. While several clinical cases of Q fever were reported in Korea till the middle of 1990s, nobody has reported a case thereafter. However possibilities for an outbreak have still been raised. In this study, antibody titers to C. burnetii in patients with unknown fever and atypical pneumonia were tested by an indirect immunofluorescence method using the phase II antigen. In addition, the validity of a PCR method in indentifying C. burnetii directly from human sera was tested. Among the 560 specimens from atypical pneumonia patients, 23 sera (4.29%) reacted positively to the phase II antigen of C. burnetii. IgG antiphase II antigen titers were 1:16 in 16, 1:32 in 2, 1:64 in 2, 1:128 in 2, and > or =1:256 in one serum. IgM and IgA antibodies anti-phase II antigen were detected in 6 and 3 sera at 1:16, respectively. And each two sera had IgM antibodies at 1:32 and 1:64. Anti-phase II antigen IgG antibody titers in the patients with unknown fever were 1:16 in 5, 1:32 in 2, 1:128 in 1, and 1:256 in 3 sera. However, IgM antibody wasn't detected in this group. Of the 202 sera from abattoir workers, 5 (2.47%) reacted with phase II antigen. Among 448 sera of healthy controls, anti-phase II antigen IgG titer of 1:16 was found in 7 and 1:32 in 1 and 1:64 in 3 sera. In the case of IgM titer, two sera were reactive at 1:16 and 1:32, each. Significant differences among the test groups were not noted in the present study. The PCR assay to detect C. burnetii com-1 and plasmid genes did not show reliable specificity and sensitivity for the diagnosis of Q fever. So, the usefulness of the PCR for laboratory diagnosis of Q fever still remains controversial.

5.Hematological Differences between Roller Pump and Centrifugal Pump in Cardiopulmonary Bypass.

BACKGROUND: Prolonged extracorporeal circulation entails tremendous threats of red cell lysis, severe bleeding problems due to platelet injury and activation, and endothelial damages by sequestered leukocytes. In consideration of these problems, a new centrifugal pump was developed and tested clinically to evaluate its effectiveness. METHODS: We compared the effects of a centrifugal pump with those of a roller pump on hematological responses during cardiopulmonary bypass (CPB) in 20 coronary artery bypass surgery patients. The patients were divided into two groups of 10 each. The studied parameters included WBC counts, platelet counts, plasma Hb and D-dimer. Blood samples were taken after sternotomy, at 60 min, 120 min and 180 min after CPB start, and at 2 hr after CPB stop. RESULTS: No differences between the groups were found in bypass time, aortic cross clamp time, extracorporeal circulation flow and hematocrit. The centrifugal pump group demonstrated less platelet depletion (p<0.05), hemolysis (plasma Hb, p<0.05) and fibrinolysis (D-dimer, p<0.05). These differences were CPB time dependent and became statistically significant after 120 min bypass. CONCLUSION: We conclude that roller pump still can be safely used for standard cardiac procedures with bypass time less than 120 minutes and the centrifugal pump has significant potential to be safely applied to CPB for long ypass time in order to avoid postperfusion syndrome.

6.Factors of Physical and Psychological Symptoms in Women after Miscarriage.

PURPOSE: The study aimed to explore the health consequences that women experienced after miscarriage and the factors related to them. METHODS: A convenience sample consisting of 102 women who had miscarried within 2 years was used. Women were recruited from hospitals and enterprises in Seoul and Gyeong-Gi Province. Participants completed a self-administered questionnaire containing a physical and psychological symptoms checklist developed for this study. RESULTS: More than 40% of the miscarriages occurred after 9 weeks of pregnancy and 35% of women were found to have had a previous miscarriage prior to this study. Psychological symptoms were more prevalent and prolonged than the physical symptoms, furthermore, the frequencies of the symptoms experiencedwere not consistent with the duration of symptoms. Employed women and women with early miscarriages complained of more physical symptoms; however, psychological symptoms were not different according to women's characteristics. Employment was a significant factor affecting physical symptoms. CONCLUSION: Health care professionals need to inform and educate women and the family of the potential health changes during the recovery after the miscarriage. Health consequences due to miscarriages also need to be incorporated in women's reproductive health care. Nursing care should consider factors of maternal age, employment status, and obstetrical conditions upon the apparent social changes.

7.Characteristics of Epidemic Multidrugresistant Salmonella enterica Serovar Typhimurium DT104 Strains First Isolated in Korea.

BACKGROUND: Salmonella infections continue to cause gastrointestinal and systemic diseases throughout the world. S. Enteritidis and S. Typhimurium were traditionally known as typical food poisoning Salmonella agents, the isolation rate of which has been increased recently in Korea. S. Typhimurium phage type DT104 has become an important emerging pathogen. Isolates of this phage type often possess resistance to ampicillin, chloramphenicol, streptomycin, sulfonamides, and tetracycline (ACSSuT resistance). The mechanism by which DT104 has accumulated resistance genes is of interest, since these genes interfere with treatment of DT104 infections and might be horizontally transferred to other bacteria, even to unrelated organisms. METHODS: All the isolates included in this study were identified as S. enterica serovar Typhimurium according to the Kauffmann-White serotyping scheme and were definitive phage type DT104 according to the phage typing scheme described by Anderson, et al. A total of 63 isolates of S. enterica serovar Typhimurium DT104 were characterized by antimicrobial resistance analysis, pulsed-field gel electrophoresis (PFGE) with the restriction enzyme XbaI. RESULTS: Four hundred ninety six S. Typhimurium isolates were divided into 28 different phage types and DT104 was the second most common phage type in Korea. A total of 63 S. Typhimurium DT104 isolates were grouped into 7 resistance phenotypes. Fourty one (65.1%) isolates were resistant to the ACSSuTTic core alone or to additional drugs as well except twenty two (33.9%) isolates were resistant to the ASSuTeTic. Four PFGE subtypes A1, A2, B1, and B2 were observed among DT104 isolates and type A1 was prevalent. CONCLUSIONS: We concluded two distinct clones were present among Korea multidrug resistant S. enterica serotype Typhimurium DT104 and multidrug resistant S. Typhimurium phage type DT104 has been an important emerging pathogen in Korea.

8.Concept Analysis of Preparatory Grief in Terminal Cancer Patients.

PURPOSE: The purpose of this study was to conceptualize and clarify a concept of "preparatory grief" in terminal cancer patients. METHOD: A hybrid model of concept development was applied to develop a concept of preparatory grief, which included a field study carried out in Busan, Korea. Participants of this study were 8 cancer patients. RESULTS: On the basis of our literature, research and clinical experience, the concept of preparatory grief emerged as a complex phenomenon playing an important role in five areas; physical, emotional, interpersonal, religious, and transcendental dimensions. Two new attributes were defined through a field phase; trust of the post-mortal world and a serene state of mind. Indicators reflected attitudes of sadness, worry, regret, capability to adapt and hope. The results of preparatory grief were loss of energy and interest, emotional chaos, contemplation, taciturnity and restoration. CONCLUSIONS: Differentiating among preparatory grief and other symptoms in cancer patients is essential because of therapeutic implications. Understanding preparatory grief is necessary in order to manage cancer patients for promoting quality of life so that its application may have a positive impact on the patient's life.

9.Seroprevalence and Risk Factors of Brucellosis among Slaughterhouse Workers in Korea.

OBJECTIVES: The incidence of zoonoses in Korea has increased recently. However, the study of high risk groups for zoonoses has not been conducted to date in Korea. Thus, we did this study to obtain data on brucellosis among slaughterhouse workers in Korea. METHODS: We evaluated the structure of slaughterhouses and the process of slaughtering by reviewing the relevant literature and doing field studies. We visited 73 slaughterhouses and 62 residual products handle houses across the country. In addition, we conducted a questionnaire survey of the work activities, and obtained blood samples in order to determine the seroprevalence and risk factors of brucellosis. The titers of brucellosis antibodies were measured using the standard tube agglutination test (SAT). We diagnosed subjects as seropositive for Brucellosis if the titers were more than 1:160. The data collected was evaluated using SPSS ver. 17.0. RESULTS: We included 1,503 subjects and obtained 1,482 blood samples among them: 849 workers involved in slaughtering, 351 handlers of residual products, 190 inspectors and their assistants, and 92 grading testers and their assistants. The seroprevalence of brucellosis among the slaughterhouse workers was 0.8% (95% CI=0.4-1.5). Broken down, the seroprevalence of brucellosis among the workers involved in slaughtering was 0.7% (95% CI=0.3-1.6), the handlers of residual products was 1.7% (95% CI=0.7-3.9) respectively. Risk factors for contracting brucellosis among slaughterhouse workers were being splashed with cattle blood around the mouth, cattle secretions around the body and not putting on protective apron while at work. CONCLUSIONS: An educational program is needed for high risk groups on zoonoses about the prevention of infection. Thus, effective working guidelines for workers who participate in the slaughter of animals must be developed in order to protect them from zoonoses.

10.A Case of Nevus Lipomatosus Superficialis without External Manifestations.

Nevus lipomatosus superficialis is a rare type of benign hamartomatous skin lesion characterized by the presence of mature adipose tissues in the dermis; it usually manifests as a protruding papule or groups of papules or nodules that have smooth or wrinkled surfaces. We report a case of nevus lipomatosus superficialis without external manifestations in a 49-year-old woman.
