1.Intrapulmonary Cystic Lymphangioma.

Solitary intrapulmonary cystic lymphangioma in newborn or infant is an extremely rare disease. We describe a case of solitary intrapulmonary cystic lymphangioma in a 4-month-old boy with dyspnea and tachypnea. It was in the left lower lobe and type 1 congenital cystic adenomatoid malformation was suspected radiologically. The resected cyst was unilocular with a thin wall, and was 9.5cm in size. Histologically, the cyst was lined by flattened endothelial cells supported by a minimal fibrous stroma.

2.CLINICOPATHOLOGIC ANALYSIS OF THE PLEOMORPHIC ADENOMA.

The author studied on the 197 cases of the pleomorphic adenoma which had been diagnosed with biopsy during the period of 1985 to 1996 at the Kyungpook National University Hospital. This study contains the statistical analysis of the clinico-pathological findings such as sex, age, location, chief complaint, duration, treatment method, size, recurrence, and treatment department in relation to anatomical location. The result were as follows : 1. The treatment department in oxter were ENT(16.2%), GS(22.8%), OMFS(18.8%), PS(12.2%). 2. The incidence of pleomorphic adenoma were parotid gland(57.8%), palatal salivary gland(20.3%), submandibular gland(17.2%), other minor salivary glands(4.5%). 3. The tumors were more common in females than males, with a male to female ratio of 1 : 1.85. 4. The peak age of ocurrence was in the forth decade. 5. The duration of the most cases were less than 5 years. 6. Although the size of the tumors was variable, the incidence was highest between 3cm and 4cm(25.4%). 7. The most prevalent chief complaint was swelling(94.9%). 8. Conservative treatment was performed 30.0%, and radical treatment was employed 70.0%. 9. The recurrence rate is 8.1%, and recurrence used to be relatively common with conservative treatment. 10. The rate of malignant transformation is 1%. 11. The anatomical location has no relationship with the factors of sex, duration, and malignant transformation. 12. There was statistical significance on the anatomical location to the treatment department, age, chief complaint, size, treatment method, recurrence (P<0.05).

3.Primitive Neuroectodermal Tumor Associated with Mature Cystic Teratoma of the Ovary: A case report.

Malignant transformation of benign cystic teratomas of the ovary is rare, with an incidence rate of 1-3%. The most common malignancy associated with benign cystic teratoma of the ovary is squamous cell carcinoma, which represents 90% of malignant transformations, followed by adenocarcinoma and carcinoid. Primary malignant neuroectodermal tumor in a mature cystic teratoma is extremely rare and microscopically can be subclassified as differentiated, primitive, and anaplatic. We experienced a case of primitive neuroectodermal tumor (PNET) associated with mature cystic teratoma of the ovary in a 11-year-old girl. Grossly, the ovary was a unilocular cyst which contained sebum material and disclosed several large, yellow to gray-tan polypoid excresences attached to the inner cystic wall. Except a large, fish-flesh, soft mass, the other polypoid masses were covered with sebum and contained small pieces of cartilage and tooth, and microscopically represented as mature teratoma. The large, fish-flesh, soft mass disclosed sheets and lobules of closely packed small cells with thin fibrovascular septa, which frequently replacing and encroaching the mature cystic teratoma. The small tumor cells were uniform and made up of round to oval nuclei with coarse chromatin and prominent nucleoli, very similar to those of PNET of the central nervous system.

4.A Study on Expression of p53 Protein according to Histologic Types, Degree of Malignancy and Differentiation of the Ovarian Surface Epithelial Tumors.

p53 gene alterations in the ovarian cancers are regarded as early events in the whole process of carcinogenesis. This study is intended to compare p53 protein expression rate in the ovarian surface epithelial tumors according to histologic types, degree of malignancy and differentiation. 134 cases of ovarian epithelial tumors including 26 cases of serous cystadenoma, 7 cases of serous borderline malignancy, 15 cases of serous cystadenocarcinoma, 40 cases of mucinous cystadenoma, 21 cases of mucinous borderline malignancy, 20 cases of mucinous cystadenocarcinoma, 4 cases of endometrioid carcinoma, and 1 case of clear cell carcinoma were studied. Immunohistochemistry using monoclonal p53 antibody(DO-7) was applied to the routine formalin-fixed paraffin embedded tissue. The results were as follows; 1. No immunohistochemical positivity of p53 protein was found in all 66 cases of benign serous and mucinous tumors studied. 2. There was no significant difference of p53 protein expression between serous and mucinous malignant tumors. 3. The expression rate of p53 protein exhibited a statistically significant difference between borderline(42%) and malignant(74%) ovarian surface epithelial tumors (p<0.05). 4. The expression rate of p53 protein of poorly differentiated carcinomas(100%) was higher than those of moderately (88%) and well differentiated carcinomas(60%), but there was no statistical significance. In summary, p53 protein expression might be a good indicator of malignant transformation of the ovarian surface epithelial tumors.

5.Influence of Hospital Ethical Climate and Nursing Professionalism on Patient Safety Management Activity by Nurses

PURPOSE: The purpose of this study was to investigate the influence of hospital ethical climate and nursing professionalism on patient safety management activity by nurses.METHODS: A cross-sectional correlation study design was used. Participants included 142 nurses from two tertiary hospitals in Seoul and Gyunggi-do, South Korea. Data were collected in April and May 2019 using self-report questionnaires and analyzed using descriptive statistics, t-test, ANOVA, Pearson correlation coefficients and multiple liner regression analysis with SPSS/WIN 21.0.RESULTS: The patient safety management activity correlated positively with hospital ethical climate (r=.32, p < .001) and nursing professionalism (r=.40, p < .001). The patient safety management activity was the factor that most influenced nursing professionalism and position, explaining 21% of the variance (F=12.06, p < .001).CONCLUSION: It is necessary to provide education on professionalism. It is necessary to continuously provide job education and training to nurture competence and quality in professional nurses.

6.Impact of Biomedical Ethics Awareness and Ethical Values in Nursing Student on Their Attitudes towards DNR.

PURPOSE: The purpose of this study was to identify how nursing students' awareness of biomedical ethics and ethical values affect their attitudes towards a do-not-resuscitate (DNR) order. METHODS: This cross-sectional correlation study was conducted with 275 nursing students enrolled at two universities in North Chungcheong Province and North Gyeongsang Province of South Korea. Data were collected in April 2017 using a self-reported questionnaire. Data were analyzed using descriptive statistics, t-test, ANOVA, Pearson correlation coefficient and stepwise multiple regression with the SPSS/WIN 23.0 program. RESULTS: The students' attitudes towards DNR were positively correlated with attitudes that seek ethical values but negatively correlated with their biomedical ethics awareness. The explained variance for attitudes towards DNR was 20%, which was significant (F=13.01, P < 0.001). CONCLUSION: These findings suggest that nursing students' biomedical ethics awareness and ethical values were associated with their attitudes towards DNR. Curriculum organization and various educational programs should be developed and applied to help nursing students develop ethical values and awareness of biomedical ethics.

Accordingly, providing regular education programs and appropriate compensation schemes, by raising nurses' awareness of medical institutions' certification systems, is necessary.

9.Propofol-Fentanyl Total Intravenous Anesthesia for Coronary Artery Bypass Graft.

BACKGROUND: Total intravenous anesthesia (TIVA) is by definition a technique involving the induction and maintenance of the anesthetic state with intravenous drugs alone. In particular, propofol and opioid and muscle relaxants allow enhanced control of the state of anesthesia for the entire duration of the surgical procedure. We evaluated the clinical usefulness of TIVA with fixed fentanyl concentration 3 ng/ml using isoconcentration nomogram and titrated propofol for coronary artery bypass graft. METHODS: Anesthesia was induced using 1% propofol mixed with lidocaine 0.5 mg/kg and ephedrine 10 mg (150 ml/hr) until loss of consciousness in 19 patients undergoing coronary artery bypass graft. Infusion rate of propofol was adjusted in response to blood pressure and pulse rate. To achieve constant fentanyl concentration, infusion rate of fentanyl was changed timely according to isoconcentration nomogram. Infusion of propofol and fentanyl was discontinued 15 and 30 min before predictable end of surgery, respectively. Intraoperative hemodynamics, recovery profile and postoperative analgesic requirements were checked. RESULTS: Overall intraoperative hemodynamics including cardiac index and PCWP showed no significant changes compared with preinduction control value except during CPB period. Average flow rate of propofol and fentanyl was 3.4 0.2 mg/kg/hr and 2.8 0.4 g/kg/hr, respectively. Spontaneous eye opening time was 96.4 min after discontinuation of fentanyl. More than 80% (16/19) of patients did not require any analgesic during first postoperative 24hrs for pain relief. CONCLUSIONS: TIVA with propofol and fentanyl (3 ng/ml) could be a suitable and safe anesthetic technique for coronary artery bypass graft.

10.Effects of Job Stress, Physical Activity, Subjective Happiness on Wellness among Bus Drivers

Purpose:
This study aimed to identify the factors that influence the wellness of bus drivers by examining the relationships between job stress, physical activity, subjective happiness, and wellness.
Methods:
Self-report questionnaires were distributed during October 2017.
Results:
Based on a multiple regression analysis, the significant factors found to affect the wellness of bus drivers were physical activity (β=.48,p<.001), subjective happiness (β=.25, p=.001), and job stress (β=-.17, p=.025). The total explanatory power of the study variables for wellness was 45.7% (F=33.04, p<.001).
Conclusion
These findings highlight a need to develop workplace wellness programs to reduce job stress and improve physical activity and subjective happiness among bus drivers.
