1.Tailgut Cyst with Glomus Coccygeum: Report of a case.

Tailgut cyst of retrorectal space is uncommon and its classification and prognosis are unclear. The lesion usually consists of a multiloculated cyst lined by squamous, transitional, and glandular epithelium. Disorganized fascicles of smooth muscle may be seen in the wall. Glomus coccygeum, normally located at the tip of coccyx, is incidentally discovered in the presacral mass including tailgut cyst. We report a case of tailgut cyst in a 35 year old female who had internal hemorrhoid and intermittent anal pain for 3 years. Pelvic computerized tomography revealed a 3x2 cm sized mass in the presacral area. The mass was multicystic and lined by squamous, transitional, and peudostratified ciliated columnar epithelium with inflammatory cells and scattered smooth muscle in the wall. Dermal adnexal structures were not present, which excluded dermoid cyst. A glomus body was identified in the surrounding soft tissue.

2.The Epidemiological Study on the Ocular Disorders of the Rural Aged Population.

OBJECT: It is to evaluate the priority area for effective eye care service to the aged population of rural comniunity at present and in future. METHOD: Number of aged group surveyed on 60 and over 60 years age that, counted 531 among the total 580 patients. Visual acuity was tested and refractive error was measured by Retinornax (Nikon, Japan) . Ocular examination was performed to determine the causes of visual impairment. This survey was conducted under the auspices of the Korean Foundation for Prevention of Blindness with assistance of the Seoul National University Postgraduate School of Public Health from 1994 to 1996 at the Public Health Center of Chun-Cheon, Kang-Won Do(Province). RESULTS: Age distribution showed the sixties (60-69) 49,3%, the seventies (70-79) 33.9%, and the eighties (80) 16.8%. Proportion of visual acuity of 0.7 or upper in the better eyes constituted 32.2% and of 0.2 or worse 19.4%. Causes of visual impairnnente consisted of refractive errors 35. 9%, cataract 35. 9%, macular degeneration 7, 4%, and corneal opacity 3.9% in order. Contributed modes of care were spectacles 27.9%, surgeries 23.9% (cataract, operation, pterygium removal etc.), and medical treatment 23.9%. DISCUSSION: Surgery of the aged population in rural comrnunity of Chun-Cheon revealed the most. prevalent. causes of visual impairment as the refractive errors and cataract. It is suggested that. the most effective eye care would be provided systemically based 0 the result of the epidemiological study on the various ocular disorders.

3.Fetal Heart Rate Telemetry System for Monitoring of High Risk Pregnancies.

OBJECTIVE: Our purpose was to establish whether FHR recording tape and telemetry system at home interpreted by remote telemedicine link is comparable to interpretation at hospital in a high-risk patient population. Patients and METHODS: Twenty women whose pregnancy was suspected as high risk were entered to study. All pregnancies were at term(7 at 38th weeks, 7 at 39th weeks, 6 at 40th weeks). Remote FHR monitoring at hospital was made for consecutive 6 days, using FHR-telemetry system at home which already developed and used by authors from 1991. Patients recorded FHR sound using recording tape for 5 days. On the 5th day, real-time transmitting of FHR sound to the hospital was also done via telephone lines after recording of FHR sound. On the 6th day, FHR test(NST) was performed at hospital. The signal loss(%), baseline FHR(bpm), and FHR variability were compared using computerized FHR analysis system(HYFM-I) after re-play of 5-days recording tapes at hospital with real-time transmitting data and in-hospital FHR test results. RESULTS: The signal loss was highest with real-time transmiting data when comparing with in-hospital FHR analysis. No significant differences were observed among 5-days data of FHR recording tapes. CONCLUSIONS: Frequent antenatal visit of high-risk pregnancy patients were in diHuities of their pregnancy outcomes. And many rural areas of our country are without adequate access to basic maternity services. From this point of view, a fetal telemedicine service is needed, and in fact, technically and clinically feasible. Our demonstration suggests that such a service reduces the need for hospital visit of high risk pregnancy women. Further evaluation in a variety of clinical settings is now indicated, along with cost-benefit analysis.

7.Treatment of Total Hyphema Following Trauma and Surgery with Tissue Plasminogen Activator.

Delayed clearing from the total hyphema resulting from ocular trauma and surgery causes intractable glaucoma and corneal staining by blood-pigments such as hemoglobin and small amounts of hemosiderin. The management of hyphema includes supportive medical care such as bed rest and surgical procedures such as paracentesis and removal of blood clot to enhance the secretory function of trabecular meshwork. However, surgical intervention has many complications and should be used only when it is absolutely necessary. We used the recombinant intraocular tissue plasminogen activator as a method for minimizing complications and sequeles in three patients with total hyphema. Hyphema resolved in all patients who had been treated with 10ug of recombinant tissue plasminogen activator following 5 days medical treatment. In study, we experienced a case of vitreous hemorrhage. In conclusion, we recommend that eyes with complicated large or total hyphema may be considered for intracameral tissue plasminogen activator as a first procedure comparing to surgical intervention, but further invesigation is needed for timing and dosage of drug usage.

8.2023 Survey Results on the Perception of Diabetes Mellitus among the General Public

Background:
According to the Diabetes Fact Sheet published by the Korean Diabetes Association, as of 2020, one in six adults (16.7%) over the age of 30 were diagnosed with diabetes. In this rapidly increasing prevalence of diabetes in Korea, the severity of diabetes to raise awareness of the importance of awareness, early detection, and active management. A public diabetes awareness survey was conducted in 2022 and 2023, and the survey contents by year were compared and presented.
Methods:
This survey was a structured online questionnaire conducted in October 2023, targeting a total of 1,000 adult men and women aged 20 or older nationwide through population proportional allocation according to age, sex, and residential area.
Results:
The survey results indicated that 91.4% of respondents perceive diabetes as a serious disease, and 68.3% of those without a diabetes diagnosis have expressed concerns about diabetes or hyperglycemia. However, awareness of ‘prediabetes’ and ‘glycated hemoglobin’ was only 61.3% and 38.1%, respectively. On the other hand, only one-third of the respondents reported maintaining an appropriate weight and waist circumference, eating regular meals, and engaging in regular exercise. While awareness and perceived necessity of insulin injections were high, the willingness to undergo injection therapy was only about 50%, indicating a need to provide information on the benefits of insulin therapy and its positive impact on quality of life.
Conclusion
Therefore, raising awareness about the severity of diabetes, along with education and intervention, is necessary to facilitate early detection and proactive management of diabetes.
9.Effects of Soy Isoflavone Supplementation and Exercise on Bone Mineral Density and Urinary Deoxypyridinoline in Postmenopausal Women.

We studied the effects of soy isoflavones supplementation with exercise on bone mineral density and the urinary excretion of deoxypyridinoline as an index of bone resorption rates in postmenopausal women. A total of 67 postmenopausal women were assigned to Isoflavone (90 mg/day) or placebo groups. These groups were further divided into groups that undergone a regular exercise or a rather sedentary state performing daily activity only. Four groups were Placebo-control group (n = 16), Placebo-exercise group (n = 16), Isoflavone-sedentary group (n = 19) and Isoflavone-exercise group (n = 16). After the intervention, we compared anthropometric mesurement, dietary recall, bone mineral density (femoral neck, lumbar spine), urinary deoxypyridinoline between the groups and between the pre and post studies. There were no significant differences between the four groups in terms of average age, height, weight, period after menopause at the baseline. The average age of the subjects were 55.2 yrs, average height, weight, period after menopause were 154.7 cm, 59.3 kg, 5.58 yrs, respectively. After eight week intervention period, there were no significant differences between the four groups in bone mineral density, but urinary deoxypyridinoline excretion was significantly decreased both in Isoflavone-sedentary and Isoflavone-Exercise groups. These results suggest that Isoflavone supplementation alone or with exercise may be preventive measures through the decrease of bone reabsorption rate in post-menopausal subjects. Whereas exercise alone did not appear to be an effective measure in bone loss with these subjects.

10.Intraoperative Detection of Rate Dependent Left Bundle Branch Block.

Rate dependent left bundle branch block (RDLBBB) is an uncommon case. RDLBBB is defined as an intraventricular conduction defect that may return, if only temporarily, to sinus rhythm at lower heart rates. It appears when the heart rate exceeds a certain critical value. Although RDLBBB is usually benign, its diagnosis and treatment have clinical importance for association of RDLBBB and myocardial ischemia and infarction. Therefore, in the case of detection of intraoperative RDLBBB, a clear differentiation should be done as soon as possible. Also it is important to start appropriate treatment and to do clinical follow-up examination. We report a case of intraoperative RDLBBB during general anesthesia for laparoscopic cholecystectomy in 82 years old female patient who has a history of hypertension.
