1.Two Cases of Chronic Otitis Media caused by Aspergillus terreus.

Fungi have been recognized as a significant cause of external otitis and it may be the primary pathogen or be part of a mixed infection. In the immunocompromised host, fungus is capable of producing infection in inner ear or middle ear. Otomycoses are most frequently caused by Aspergillus spp. and Candida sap. There are few reports that Aspergillus species other than A. fumigatus, A. niger and f. flavus have caused chronic otitis media. We report two cases of chronic otitis media caused by Aspergillus ferrous in Korea. One case is a 7-year-old girl who had recurrent serous otorrhea and otalgia for 4 years, was reattended otolaryngology clinics with otorrhea of 3 days durations and another is a 6-year-old girl who had serous otorrhea for 2 months and 3 day fever, was attended otolaryngology clinics with them. Microscopic appearance and colony morphology from ear discharge cultures revealed A. ferrous. The infection responded well to topical ketoconazole therapy. This report should help to raise medical personnel's awareness of such human opportunistic fungal ear infections.

6.Bronchiolo-alveolar cell carcinoma a review of 11 cases

Eleven patients with proved Bronchiolo-aveolar cell carcinoma were found in the chest department of thenational menical center from 1975 to 1981. The incidence of Bronchiolo-alveolar cell carcinoma is recentlyincreased as primary lung carcinoma. The result as follow. 1. The ratio of male and female was 5:6 and anincidence of 4.4% among total primary lung cancer patients. The highest incidence (3 of cases) was seen in thesixth decade, and the remaining cases were evenly distributed in the third, fourth, and fifth decades of life.Among them youngest was 29 years old and the oldest was 66 years old. 2. Clinical and radiological initialdiagnosis prior to the final diagnosis were as follows; pulmonary tuberculosis; 7 cases, pneumonia; 1 case,bronchiectasis; 1 case, and lung cancer; 2 cases. 3. Radiological examination of chest presented several pictures;most commonly, homogenous or patchy infiltrations; 6 cases, nodular or mass like densities; 2 cases, disseminatednodular or military patterns; 2 cases, and reticular pattern; 1 case. 4. Bronchogram reveald no contributablefindings except one case of complete tappering obstruction of the segmental bronchus. Therefore we arrive at theconclusion that early diagnosis will result in increased resectability and improved survival so aggressivediagnositic work-up for suspicious pulmonary infiltrate is necessary.

7.A Study on Motivation for Pregnancy of Married Women in Korea.

PURPOSE: This study is to define the structural patterns of pregnancy motivation of married women. METHOD: Q-methodology was used on 110 statements collected from interviews with women and a literature review. 38 Q-samples were selected. The Q-samples were administerd to 30 randomized P-sample. RESULT: Data analysis of Q-type obtained by QUANL program shows three types of subjectivity in the perception of pregnancy motivation. Type 1, "Family-connection strengthening", Pregnancy motivation is characterized by the tendency to make home, to tie family member, to connect couple firmly. Type 2,"Self-satisfaction", Women of this type take pleasure and happiness from child rearing in terms of being a mother and having maternal affection. Type 3, "Self-centered", This type of pregnancy motivation puts on high emphasis on self achievement and they make a family planning suitable for the maternal role. CONCLUSION: The stronger was women's pregnancy motivation, the more positive were women's pregnancy experience and maternal role attainment. Therefore, When nurses take care of pregnancy women, they should understand the women's psycho-social aspects and then apply a program of nursing intervention to the pregnant women, It is necessary to continue to study on social and psychological problems related to pregnancy.

8.High Sodium Intake, as Assessed by Urinary Sodium Excretion, Is Associated with Nonalcoholic Fatty Liver Disease or Sarcopenia

Background/Aims:
We explored whether high sodium intake, assessed by urinary excretion, determines the risk of sarcopenia and nonalcoholic fatty liver disease (NAFLD).
Methods:
We analyzed 10,036 adult participants with normal kidney function from the Korea National Health and Nutrition Examination Survey (2008–2011). NAFLD was identified using the fatty liver index, and the muscle mass was evaluated using dual X-ray absorptiometry. The dietary sodium intake was estimated using Tanaka’s equation.
Results:
The mean 24-hour urinary sodium excretion was 144.2±36.1 mmol/day (corresponding to 3.3 g/day Na) in the total population. The 24-hour urinary sodium excretion showed moderate accuracy in predicting NAFLD (area under the receiver operating characteristic, 0.702; 95% confidence interval [CI], 0.692 to 0.712). A cutoff value of 99.96 mmol/day (corresponding to 2.30 g/day Na) for urinary sodium excretion in predicting NAFLD showed 76.1% sensitivity and 56.1% specificity. The results of multiple adjusted models indicated that the participants with the highest urinary sodium excretion had a significantly higher risk of NAFLD (odds ratio, 1.46; 95% CI, 1.27 to 1.66; p<0.001) and sarcopenia (odds ratio, 1.49; 95% CI, 1.28 to 1.73; p<0.001) than those with the lowest urinary sodium excretion. The association between a higher 24-hour urinary sodium excretion and NAFLD was independent of sarcopenia.
Conclusions
Participants with a high sodium intake, as assessed by sodium excretion, had a substantial risk of NAFLD and sarcopenia
9.Comparison of the QIAGEN artus HBV QS-RGQ Assay With the Roche COBAS AmpliPrep/COBAS TaqMan HBV Assay for Quantifying Viral DNA in Sera of Chronic Hepatitis B Patients.

BACKGROUND: Hepatitis B virus DNA quantification is essential for managing chronic hepatitis B (CHB). We compared the performance of artus HBV QS-RGQ (QIAGEN GmbH, Germany) and CAP/CTM v2.0 HBV assays (Roche Molecular Diagnostics, USA) in CHB patients. METHODS: A comparative evaluation between two assays was performed with 508 clinical serum samples. Precision, linearity, and the limit of detection (LOD) of QS-RGQ assay was evaluated by using the WHO standard 97/750 and clinical samples. RESULTS: Detection rates and viral loads as determined QS-RGQ assay were significantly lower than those from the CAP/CTM v2.0 assay (52.8% vs 60.6%; 3.55±1.77 IU/mL vs 4.18±1.89 IU/mL, P<0.0001). The kappa coefficient between qualitative results was 0.79 (95% confidence interval, 0.74 to 0.85). Bland-Altman plot found a mean difference of (QS-RGQ − CAP/CTM v2.0)=−0.63 log₁₀ IU/mL (95% limit of agreement, −1.48 to 0.22). Repeatability and total imprecision (% CV) of the QS-RGQ assay were 1.0% and 1.1% at 2,000 IU/mL, and 0.7% and 1.4% at 20,000 IU/mL, respectively. Linearity of this assay ranged from 31.6 to 1.0±10⁷ IU/mL, and the LOD was 2.95 IU/mL. CONCLUSIONS: The artus HBV QS-RGQ assay showed good performance but significantly decreased detection rate and viral load compared with CAP/CTM v2.0 assays. This assay recommends using plasma; however, we used stored serum because of the retrospective study design. Usually HBV DNA quantification is performed in plasma or serum, but sample type and clinical relevance of quantitative values should be considered when determining the clinical application of this reagent.

10.Factors Associated with Sleep Duration in Korean Adults: Results of a 2008 Community Health Survey in Gwangju Metropolitan City, Korea.

Short (< or = 6 hr) and long (> or = 9 hr) sleep durations are both associated with risk factors for cardiovascular diseases and diabetes, depression, learning problems, accidents and excess mortality. However, little is known about factors associated with sleep duration in Korean adults. This study examined sleep patterns in Korean adults and identified factors associated with short and long sleep durations. This study analyzed cross-sectional data collected from 4,411 Korean adults aged 19 yr and older who participated in a community health survey conducted in Gwangju, Korea. Multinomial logistic regression analysis was used to identify associations between socio-demographic and health-related factors and short or long sleep durations. Of the population, 37.2% and 4.0% reported short and long sleep, respectively. Short sleep was associated with older age, lower levels of income, night or shift work, heavy smoking, and depression or anxiety; long sleep was associated with younger age, being divorced or widowed, heavy smoking, underweight, depression or anxiety, and poorer self-reported health. In conclusion, a relatively high prevalence of short sleep duration is identified in this population of Korean adults. Factors associated with short or long sleep may act as potential confounders of the relationship between sleep duration and health outcomes.
