1.A Case of Dilated Pore of Wliner.

We report herein a case of dilated pore occuring on the right philtrum of a, 65-year -old man. Histopathologic findings revealed irregular hyperplasia of the follicular wall focally in the lower portion but slightly atrophic in the ostial portion. This was treated by excision.

2.A Survey of Urban Middle-aged Women's Sexual Function and Sexual Distress.

OBJECTIVE: This survey assessed the prevalence and type of sexual dysfunction in middle-aged women regarding sexually related personal distress and examined the prevalence of perceived sexual dysfunction and perceived partners' dysfunction by women. METHODS: One-hundred ten healthy community-based middle-aged women participated voluntarily, and validated question- naires of the Female Sexual Function Index(FSFI) and the Female Sexual Distress Score(FSDS) were used. RESULTS: The percentage of women having sexual dysfunction and sexual distress were 67% and 32% respectively according to the cut-off of FSFI and FSDS. The average score of FSFI was 23.5(s.d.=5.7), which was lower than the cut-off of FSFI, while the average score of FSDS was 13.2, which was in range of a normal score. When considering the two concepts simultaneously, the women were categorized into 4 groups: sexually stressed dysfunction group (24.8%), sexually depressed group(42.2%), sexually healthy group(25.7%), and sexually hyperactive group (7.3%). The percentage of women reporting sexual problems was 24.3%, while the percentage of women reporting sexual dysfunction in their partner was 29.9%. CONCLUSIONS: The results indicate a high rate of sexual dysfunction and sexual distress in middle-aged women. However, considering the relationships between sexual dysfunction and sexual distress, almost two thirds were in the sexually depressed group among the women having sexual dysfunction.

3.A Study on the Degree of Self-care Activities of Stroke Patients, the Family Burden, and the Educational Needs of Primary Caregivers.

The purpose of this study was to identify the degree of self-care activities of stroke patients, the family burden, and the educational needs of primary caregivers. A descriptive survey research was conducted in 97 stroke patients and their respective primary caregivers. Data were collected the using 26th to by interviews a structure questionnare from July August 5th, 1999. NIH stroke status was 12.15+/-8.40, the degree of self-care activities of stroke patients was 26.85+/-9.39. The score of objective burden of primary caregivers was 31.24+/-4.81 and subjective burden was 24.30+/-6.99. The score of the educational needs was 89.78+/-9.99. We present several imformations about clients from these scores as follows: The patients were convalescent and their depentant level of self-care was middle range: The objective burden was higher than the subjective burden: And the educational needs of family caregivers were very high. The Elain Mattis Educational Wants of Family Caregivers of Disabled Adults Questionnaire was used to assess the educational needs of primary caregivers of stroke patients. The results of this research are offered to help nurses understand the feelings of uncertainty about the new and unexpected role that family caregivers face and to help nurses meet the needs of families preparing to care stroke survivors at home.

4.Subungual Exostosis.

Subungual exostosis are not uncommon, however, they are infrequently mentioned in the dermatologic literature. We report herein a rase of subungual exostosis in the great toe of 16-year-old female student which was confirmed by histopathologic and radiologic findings. Histopathologic examination showed that the mass was ooeered by a dense fibrous tissue which merged into a fibrocartilage cap and bone. Computed tomography showed a well defined, oval shaped, radiopacity of bony density capped by a radiolucency.

5.A study of the payment of nutrition counseling services.

This study was performed to investigate the cost of nutrition counselling service at present and to suggest the guideline for the payment. Data were collected using questionnaires and the cost of counseling was expected by analyzing the time consumed and needed for nutntion counseling. The results were as follows: In the case of diabetes, mean time of nutrition vounseling was 69.7+/-5.3min/case, but the fietitian asked 82.7+/-4.6min/case for counseling and in the cases of renal diseases, mean time of counseling was 64.3+/-4.1min/case and the time asked was 84.8+/-4.5min/case (P<001) It was found that time was not enough to educate or counsel the clients. The cost of counseling was not counted in 20.3% of the respondants and the mean nutrition counseling fee was 5,460.6+/-1,901.1won/case in out-patient and 6,408.9+/-3,547.7won/case in in-patient. The group education fee was 6,168+/-,813won/case. The estimations of the cost for nutrition counseling services using labor cost were 18,463.5won in case of diabetes and 18,463.5won/case for patients of renal disease and in group education 8,111.5won and 7,404.3won respectively.

6.The Classification of Standard Nursing Activities in Korea.

A nursing activity classification for hospitalized patients was performed based on an article review regarding nursing definition and nursing activity classification system. The study was conducted as follows: 1) Taxonomy was developed by the research team through the Delphi process and review article. The taxonomy consists of four nursing processes, (assessment, diagnosis, intervention and evaluation) and twelve nursing activity domains space (resperation, nutrition, elimination, exercise/alignment maintenance, comfort, hygiene, safety, spiritual support, counseling/ education, medication, communication, patient and information management). 2) First, nursing activities of the intervention process were listed and then classified by the nursing process of assessment, diagnosis, intervention and evaluation. The list consists of twelve nursing activity domains and 136 nursing activities. 3) A pilot study was conducted in two hospitals to verify validity and appropriateness of nursing activities. 4) The content validity index, which was calculated by 6 clinical practice experts, was 0.95. Also, a nursing activity classification system should also be developed in the department of community nursing and home health care nursing.

7.Development and Validation of Mini Dietary Assessment Index for Koreans.

Improving dietary patterns, na, in turn, improving nutritional status, is now viewed as a key to improve public health and to prevent chronic diseases. There has been a peat needs to assess diet quality to identify nutritional risk group, however, little research has been done on methods to assess overall diet quality. The purpose of this study was to develope a mini dietary assessment (MDA) index for evaluation of overall dietary quality. A 10-component system was devised based on dietary guidelines and food Tower for Koreans. The system contained 4 food elements of which use is encouraged, such as milk, meat, vegetables, fruits, and 3 food elements of which use is limited, such as fat, salt, or sugar. Also the included were elements on dietary regulation and variety. A subject is to check one of 'always' 'generally' 'seldom', which has score of 5, 3, and 1, respectively, so the total possible index score is 50. This index is to use without dietary survey and is to use even by non-nutritionist. A sample of 432 healthy males and females in their 30's and 40's contributed diet intake data based on 24-hour recall for the validation of MDA. The mean MDA score was 31.34 of a possible 50 points. The main nutrients for each MDA component was correlated very well with the results of 24-hour recall. Also, very good correlation was found between healthy eating index (HEI) score and MDA score. However, some of MDA components were needed a modification of term or/and statement. So the Uh was revised and another effort for validation was made with new sample of 169 subjects and even better correlation was found. The revised MDA could be used with minor modification to assess diet quality and to screen nutritional status.

8.The Morphometric Study of the Pons and Cerebellum in Korean using MRI.

PURPOSE: To evaluate the size of normal pons and cerebellum in vivo and the change in size according to age, and to compare those with measurement of the diseased pons and cerebellum. MATERIALS AND METHODS: 121 normal adults(M:F=54:67), 5 patients with OPCD and 19 patients with WallerJan degeneration were studied. The normal group was divided into 5 subgroups according to the age (ranged from 20 to 72 years). 1.5T GE Signa MR unit was used. On axial plane, the AP(A) and transverse(B) diameters of the pons, the size of the middle cerebellar peduncle(C), and transverse diameter of the posterior fossa(D) and the cerebellum(E) were measured. On midsagittal plane, the longitudinal(F) and AP(G) diameters of the basis pontis were measured. The ratios of E/D and F/G were calculated. The student t test was used for statistical analysis. RESULTS: C, E and F/G were 15.5mm+/-1.3, 99.8mm+/-4.3 and 1.63+/-.10, respectively. F/G, H/I, and H/J were larger in male(p<.01). All data of the pons showed no statistically significant differences among age groups. E of the seventh decades was shorter than that of the third decades(p<.05). C(12.7 mm+/-1.4) in OPCD and F/G(1. 81+/-.10) in Wallerian degeneration(p<.01) showed the most significant differences when they were compared to the normal. CONCLUSION: Although the cerebellum decreased in size with age, the pons maintained its size up to eighth' decades. The measurement of middle cerebellar peduncle on axial plane (C) and the ratio of basis pontis on midsagittal plane (F/G) were important in the evaluation of OPCD and WallerJan degeneration, respectively.

9.The Effects of a Neurological Special Nursing and Early Rehabilitation Program on the Early Recovery of Patients with Acute Spontaneous Intracranial Hemorrhage and on the Level of Satisfaction of Patients' Families.

This study was performed to evaluate the effects of a special nursing program on the recovery of patients with spontaneous intracranial hemorrhage, and on the level of satisfaction of their families. The experimental group comprised twenty patients selected from A university medical center, and the control group, nineteen patients selected from B university medical center. Patients with cormorbidity were not included. Between 1 October, 1995 and 30 April, 1996, personnel specialized in neurosurgical nursing provided the experimental group with a special nursing program, while the control group was offered general nursing services. The key elements of this special nursing program were close observation to identify potential risk factors ; crisis intervention to prevent gaps in medical service delivery ; patient-centered care emphasizing early recovery, including specific protocols for the prevention of disability and for rehabilitation ; and patient education and counseling to maximize satisfaction. The average length of stay in ICU, neurological status, functional status, impact of complications, and satisfaction level were measured through medical records, the Glasgow Coma Scale, Functional Independence Measurement, patient assessments, and questionnaires, respectively. These data were analyzed by paired t-test, independent-samples t-test, and ANOVA, and the results were as follows : 1) The average length of stay in ICU of the experimental group was significantly shorter than that of the control group. 2) The degree of neurological recovery of the experimental group between admission and the second week after operation was not significantly different from that of the control group. 3) The impact of complications on the control group was greater than on the experimental group. 4) Between the first and second week after operation, the functional status of the experimental group changed more noticeably than that of the control group. 5) The degree of satisfaction with the nursing services was higher among the families of patients in the experimental group than among the other families. These findings indicate that a special nursing program might improve the recovery of patients with spontaneous intracranial hemorrhage and increase the satisfaction of their families. It is therefore suggested that in order to provide comprehensive medical services to patients with spontaneous intracranial hemorrhage, personnel such as neurosurgical nursing specialist be appropriately utilized. In addition, the current medical insurance fee schedule especially in relation to the cost of rehabilitation, should be adjusted to cover broader services.

10.Factors Influencing Health-Promotion Behavior in Adolescents.

PURPOSE: This descriptive correlation study was done to investigate adolescent behavior that promotes health and to identify factors influencing this behavior. METHOD: The participants (702) were a conveniently selected sample of second grade high school students. Data were gathered by self-administered questionnaires and the collected data were analyzed using descriptive statistics and stepwise multiple regression with the SPSS program. RESULTS: 1) The score for health-promotion behavior was 2.49 (total possible score = 4). By subcategories, emotional support was the highest (3.02). 2) Of the factors influencing health-promotion behavior, self-efficacy was found to account for 37.5% of the variance, followed by family function, social support, perceived health status and internal health locus of control. Overall, these factors accounted for 50.6% of the variance. CONCLUSION: The findings of this study showed that health-promotion behavior in adolescents was slightly above average. As self-efficacy and family function were identified as major factors in predicting health-promotion behavior, there is a need to consider interventions that will improve self-efficacy and family function.
