1.A Study on the Degree of Self-care Activities of Stroke Patients, the Family Burden, and the Educational Needs of Primary Caregivers.

The purpose of this study was to identify the degree of self-care activities of stroke patients, the family burden, and the educational needs of primary caregivers. A descriptive survey research was conducted in 97 stroke patients and their respective primary caregivers. Data were collected the using 26th to by interviews a structure questionnare from July August 5th, 1999. NIH stroke status was 12.15+/-8.40, the degree of self-care activities of stroke patients was 26.85+/-9.39. The score of objective burden of primary caregivers was 31.24+/-4.81 and subjective burden was 24.30+/-6.99. The score of the educational needs was 89.78+/-9.99. We present several imformations about clients from these scores as follows: The patients were convalescent and their depentant level of self-care was middle range: The objective burden was higher than the subjective burden: And the educational needs of family caregivers were very high. The Elain Mattis Educational Wants of Family Caregivers of Disabled Adults Questionnaire was used to assess the educational needs of primary caregivers of stroke patients. The results of this research are offered to help nurses understand the feelings of uncertainty about the new and unexpected role that family caregivers face and to help nurses meet the needs of families preparing to care stroke survivors at home.

2.Experience of the Collaborative Practice among Health Professionals in Korea

Purpose:
: This study aimed to understand and describe the experiences of collaborative practice among health professionals in Korea.
Methods:
: Data were collected through individual in-depth interviews conducted from January to March 2021 with 10 health professionals in Korea. All interviews were audiotaped and transcribed verbatim. The transcribed data then underwent qualitative content analysis.
Results:
: Through content analysis, 10 categories and 3 themes were formed. Three themes, “working based on knowledge,” “having difficulty in building collaborative relationships,” and “playing complementary roles” emerged.
Conclusion
: The limitation is that the study participants are 10 health professionals in Korea working at healthcare institutions. However, this study is of great significance in that it provided basic data for preparing measures for collaborative practice among health professionals in Korea.
3.Usefulness of Routine Renal Sonography Screening for Healthy Infants with Isolated Single Umbilical Artery .

PURPOSE: It is known that single umbilical artery is frequently associated with gastrointestinal or urogenital anomaly, however, routine renal sonography has been debated in healthy neonate with isolated single umbilical artery. This study is designed to determine the usefulness of routine renal sonography in apparently healthy infants with an isolated single umbilical artery. METHODS: Thirty healthy neonates with a single umbilical artery without a major anomaly from January 1995 to July 2002 were enrolled. The authors investigated the clinical background of babies and their mothers after renal sonography after 72 hours of age. When the abnormalities were found at the first renal sonography, the severity of hydronephrosis and degree of obstruction and renal function were analyzed by follow up renal sonography, voiding cystourethrography(VCUG) and technetium-99m-dimercaptosuccinic acid(DMSA) scan or technetium-99m-mercaptoacetyl-triglycerine (MAG3) scan. RESULTS: Among the 30 healthy patients with isolated single umbilical artery, five patients(16.7%) showed abnormalities on first renal sonography with one major(3.3%) and four(13.4%) minor renal anomaly(minimal or mild hydroneohrosis). One major renal anomaly(severe hydronephrosis) showed severe decreased renal function on MAG3 scan without reflux, and the other four minor regressed spontaneously on follow up study. CONCLUSION: The value of routine early renal sonograpy for detecting renal anomaly in healthy infants with an isolated single umbilical artery remained unclear because most of the anomalies would regress spontaneously in the follow up study.

PURPOSE: The purpose of this study was to determine handwashing-related factors in nurses. Specific purposes of this study were to examine 1) the applicability of the hypothetical model constructed for this study, on the basis of Theory of Planned Behavior(TPB), and 2) relationships between handwashing practice and other variables such as age, career, level of education, and level of recognition about handwashing. METHOD: Data were collected from 187 nurses in one university hospital in Kyung-Ki province. RESULT: The hypothetical model based on TPB was revealed applicable, but the degree of the variance explaining handwashing practice was small(26%). Perceived behavioral control(PBC) was the most strong variable explaining handwashing practice, and intention didn't have a significant effect on handwashing practice. Norm and PBC had significant effects on intention, but attitude was not a significant factor of intention. Age, career, and the level of awareness of handwashing practice were significantly related to handwashing practice. CONCLUSION: The result of this study suggests that we should focus on perceived behavioral control, awareness of handwashing practice and subjective norm about handwashing practice to increase the level of nurses' handwashing practice.

6.A Case of Tufted Angioma Showing a Good Response to Interferon-alpha.

A 13-month-old boy had irregularly shaped, erythematous to dusky red, slightly raised patches and plaques on the left antecubital fossa and arm for 7 months. The lesions were firm and tender. Histopathologically the lesion consisted of small, circumscribed, angiomatous tufts and lobules with a cannonball appearance scattered throughout dermis, a characteristic feature of tufted angioma. For 15 weeks, interferon-a intralesional injections have been performed and the lesions showed an improvement.

7.Gastrointestinal Emergencies in Neonates: What We Should Know

Newborn gastrointestinal tract emergency diseases include various disorders that occur anywhere along the digestive tract. Most of them are congenital malformations, not acquired diseases. Many of them cannot be confirmed on prenatal examination, and consequently, postnatal evaluation is required. Unlike adults, in newborn babies, detailed evaluations with computed tomography (CT) and magnetic resonance imaging (MRI) are often limited, so making the diagnosis based on early abdominal radiography is most important. Therefore, it is necessary to be familiar with the radiologic findings that may be seen on imaging studies. In addition, it is important to understand abdominal ultrasound and fluoroscopy findings, which can be used in addition to simple radiography and indications that can be used for further diagnosis and appropriate treatment. Therefore, this study describes and organizes the various imaging findings that may occur in neonatal gastrointestinal emergencies.
8.Comparison and Implication of the Contemporary Blood Pressure Guidelines on Korean Population

Background and Objectives:
This study compared the potential impacts of the 2017 American College of Cardiology/American Heart Association (ACC/AHA) and the 2018 Korean Society of Hypertension (KSH) guidelines on prevalence of hypertension, recommended antihypertensive treatment, and achievement of target blood pressure (BP) in Korean population.
Methods:
We analyzed the 2007–2017 Korea National Health and Nutrition Examination Survey data to calculate guideline-specific hypertension prevalence and treatment implications on 59,767 adults aged 20 years or older by sex and age.
Results:
The prevalence of hypertension was markedly higher 46.3% by the ACC/AHA guideline due to the lowered BP cutoff than 25.9% by the KSH guideline; the increase was most pronounced in young adults. Yet, there was only a marginal 1.6% increase in the percentage of adults suggested pharmacological approach by the ACC/AHA guideline, but selectively in the older subgroups. Overall, 45.6% of Korean adults treated for hypertension failed to meet BP goal according to the KSH guideline; the underachievement extended to 61.7% of participants according to the ACC/AHA guideline.
Conclusions
The lowered BP threshold, 130/80 mmHg, by the 2017 ACC/AHA guideline, in conjuncture with 10-year risk calculation largely driven by age, would increase pharmacological treatment preferentially in very old individuals, while increasing prevalence and uncontrolled rate mostly in younger subgroups. Adoption of lower BP cutoff to the KSH guideline would require validated cardiovascular disease risk assessment tools accounting for risk distributions specific to Korean population.
9.Effect of Maternal Factors on the Concentrations of Minerals and Immunological Substance in Breast Milk.

Flavonoids contained in grapes are potent antioxidants that may protect against oxidative stress and reduce the risk of chronic diseases related with free radical damage. In this study we investigated the effect of daily grape juice supplementation on blood pressure (BP), plasma lipid profiles and the generation of free radicals in 67 healthy volunteers (29 smoker, 38 nonsmokers). The daily 480 ml of grape juice supplementation for 8 weeks resulted in a significant decrease in diastolic BP by 6.5% in smokers and systolic and diastolic BP by 11.2 and 3.7% in non-smokers. Plasma total cholesterol, HDL- and LDL-cholesterol levels in smokers and total cholesterol in non-smokers were significantly increased after the intervention. Plasma triglycerides and conjugated dienes were not affected by grape juice supplementation. Levels of free radical determined by reading the lucigenin-perborate ROS generating sources, decreased significantly by 18% compared to the beginning of the study. The results indicated that the consumption of grape juice may reduce BP and free radical generation in smokers, which was possibly exerted by flavonoids. Our findings suggested that the grape juice has protective effect on chronic disease due to the overproduction of free radical in smokers.

10.The Efficacy of BIS Monitoring for the Preventing of Withdrawal Movement on the Intravenous Injection of Rocuronium.

BACKGROUND: Arm withdrawal movements are often observed on the administration of rocuronium. This study used the BIS to predict the depth of sedation and the prevention of withdrawal movements. METHODS: With the approval of the institutional review board and the informed consent of the subjects, sixty patients, with ASA physical stati of 1 to 2, were randomized into four groups. After the induction of anesthesia using 0.5 mg/kg of thiopental sodium, each group received 0.6 mg/kg of rocuronium when their BIS values were 85 (Group I), 75 (Group II), 65 (Group III), or 55 (Group IV). The blood pressure, heart rate, BIS value at the time of loss of eyelash reflex, incidence of withdrawal movement and grade, and the correlation between the BIS value and loss of eyelash reflex were observed. RESULTS: No difference were observed in the incidence of withdrawal movement and grade between the four groups. The correlation coefficients of the BIS value and time of loss of eyelash reflex were both 0.33 (P<0.05). CONCLUSIONS: It may be impossible to rely solely on the BIS value to insure patients reach a deep anesthetic state after induction of anesthesia with thiopental. The withdrawal movement on injection of rocuronium can not be prevented with BIS monitoring.
