2.Mucous Gland Adenoma of the Bronchus; Light Microscopic and Ultrastructural Features.

We present a case of mucous gland adenoma arising from the main bronchus of the lower lobe of the left lung in terms of clinicopathologic, immunohistochemical, and ultrastructural aspects as well as review of related literatures. The patient, a 31-year-old female, was admitted to Catholic University Medical College Hospital with complaints of coughing and purulent sputum for about seven years. The chest CT showed a severely calcified tumor in the left lower lobe of the lung. Grossly, the calcified tumor arising from the main bronchus protruded into the lumen and showed bronchiectasis of the lower lobe and atelectasis of the upper lobe of the lung. The tumor was pale brown-gray and sharply circumscribed and showed some small cystic spaces filled with mucoid material. Microscopically, most of the tumor showed dystrophic calcification. The growth pattern of the tumor is composed of cysts, tubules, and glands lined by cytologically bland columnar, cuboidal, or flattened mucus secreting cells. Electron micrograph of tumor cells showed some round or oval mucous granules measuring 0.5-1.8 micrometer.

3.Influencing Factors on the Stages of Change of Exercise in Patients with Low Back Pain.

PURPOSE: The objective of this study was to analyze the factors affecting the stages of exercise behaviors changes of low back pain patients. METHODS: The participants in this study were 220 low back pain patients who have been treated by therapeutic exercise or had the exercise treatment under the doctor's advice. The tools used for this study were the stages of exercise behavior change, intrinsic motivation questionnaires, and physical activity social support scale (PASS). RESULTS: The group that did exercise (preparation stage, maintenance stage, action stage) was 75.9% but the group that did not exercise (precontemplation stage, contemplation stage) was 24.1%. Social supports from family members and friends and motivation affected the exercise behaviors changes. And 34.6% of the exercise behavior change can be explained by the social support (family, friend) and motivation. CONCLUSION: There were two recommendations for maintaining the exercise behaviors of low back pain patients based on theoretical background. First, we recommended more exercise programs which reinforce social supports from family and friend for constant exercise behaviors of low back pain patients. Second, more exercise programs for the low back pain patients who have similar health goals or problems were needed for motivating them to join the exercise programs.

4.A Case of a Solitary Type of Congenital Self-healing Reticulohistiocytosis.

Congenital self-healing reticulohistiocytosis (CSHRH) typically presents at birth or in the first few weeks of life as a widespread eruption of cutaneous red-brown papulonodules that resolve spontaneously without involvement of other organs. While multiple lesions are common, a solitary lesion is rare. We describe a solitary type of CSHRH in a full-term, male neonate. He had an erythematous papule with a yellowish crust on the left heel without any systemic symptoms. Four weeks later, the skin lesion had disappeared spontaneously.

5.Treatment of Recalcitrant Acquired Reactive Perforating Collagenosis with Low-dose Allopurinol.

Reactive perforating collagenosis is one of a range of essential perforating disorders and is characterized by the transepithelial elimination of altered collagen. Two types have been recognized: the inherited, or childhood form and the acquired, or adult form. Acquired reactive perforating collagenosis is associated with systemic diseases, especially diabetes mellitus and/or renal failure. We report that a low-dose allopurinol showed dramatic effects on acquired reactive perforating collagenosis, which was associated with hypothyroidism and chronic renal failure caused by diabetic nephropathy.

6.A Case of Bullous Eosinophilic Cellulitis.

Eosinophilic cellulitis is a rare recurrent inflammatory dermatosis, characterized clinically by well-circumscribed erythematous plaques. The histopathologic features of the lesions are dermal edema, infiltration of eosinophils and development of 'flame figures' in the dermis. A 64-year-old female presented pruritic erythematous plaques, with associated bullae, on her right hand and wrist. Histopathologic examination showed infiltration of eosinophils and characteristic flame figures in the dermis. The clinical presentation and histopathologic alterations are consistent with the diagnosis of eosinophilic cellulitis. Herein, we report a case of bullous eosinophilic cellulitis which is an uncommon variant of eosinophilc cellulitis.

7.Comparative Study of Aminolevulinic Acid Photodynamic Therapy Plus Pulsed Dye Laser versus Pulsed Dye Laser Alone in Treatment of Recalcitrant Viral Warts.

BACKGROUND: Recurrent recalcitrant warts are likely to increase gradually, and require more aggressive and effective treatment for remedy. According to recent foreign research materials and literature, aminolevulinic acid photodynamic therapy (ALA-PDT) is reported to prove to show high curative performances, but there has been little relevant research in Korea to date. OBJECTIVE: This study will evaluate the efficiency and side effects of ALA-PDL and PDL in treating recalcitrant viral warts. METHODS: 120 warts from 44 people were selected and sorted by their locations such as hand, foot, and periungual area, and then randomly divided into 2 groups: ALA-PDL group and PDL group. The ALA-PDL group used 20% 5-ALA as a photosensitizer, and PDL for the light-source, the other group used PDL. The setting of PDL being used in both groups is as follows: wavelength of 585 nm, spot size of 7 mm, and energy density of 7.0~10 J/cm2. RESULTS: After 3 treatment sessions, the complete clearance of ALA-PDL and PDL groups was 44/59 (74.6%), 18/61 (29.5%), partial clearance-good was 8/59 (13.6%), 20/61 (32.8%), partial clearance-poor was 7/59 (11.9%), 21/61 (34.4%) and no response was 0/59 (0%), 2/61 (3.3%). The average treatment sessions were 2.32 and 2.97 times. ALA-PDL group showed pain in 27/59 (45.8%) for the first and second treatment sessions, and postinflammatory hyperpigmentation (PIH) in 11/59 (18.6%). The PDL group showed only PIH in 3/61 (4.9%). CONCLUSION: PDT is an efficient treatment modality in recalcitrant viral warts. Especially in the case of periungual warts, it is very safe and cosmetically satisfactory without serious side effects such as nail deformity.

8.The Relationship among Nurses' Perception of Super-leadership, Self-leadership and Organizational Commitment.

PURPOSE: This study was designed to assess the level of super-leadership and self-leadership, identify correlations among variables and identify the factors influencing organizational commitment of clinical nurses. METHODS: Participants were 217 nurses working in four hospitals located in Seoul and Gyeonggi Province. Data were analyzed using descriptive statistics, independent t-test, one way ANOVA, Scheffé test, Pearson Correlation, and Multiple Regression. RESULTS: The most influential factor for organizational commitment was super-leadership (β=.28, p<.001), followed by education (β=.16, p=.007), the number of unit nurses (β=-.18, p=.003), self-leadership (β=.17, p=.008), and the number of patients being under 10 (β=.12, p=.046). These factors accounted for 23% of the variance in organizational commitment (F=13.46, p<.001). CONCLUSION: The results of this study suggest a need for strategies to improve organizational commitment of nurses by enhancing nurse' self-leadership through nursing managers' super-leadership.

9.Longitudinal Change in Trabecular Bone Score during and after Treatment of Osteoporosis in Postmenopausal Korean Women.

BACKGROUND: The aim of this study was to evaluate the longitudinal changes of trabecular bone score (TBS) during and after bisphosphonate (BP) treatment in postmenopausal Korean women with osteoporosis. METHODS: We analyzed 191 patients who took BP and underwent bone mineral density (BMD) test for the period from January 2010 to December 2015. The mean follow up period during treatment and after treatment was 22.8 months and 18 months, respectively. The TBS and BMD values were evaluated by the percent changes relative to the baseline. RESULTS: In 191 patients, who treated with BPs, L-spine BMD increased 3.65±0.5% and TBS increased 0.26±0.4% from baseline during first 1 year. At 2 to 4 years, the changes of BMD and TBS from baseline gradually increased up to 9.3±3.25% and 2.69±0.98% and both results showed statistically significant correlation. In 86 patients who stopped BPs, L-spine BMD decreased -0.54±1.07% and TBS increased 0.33±1.96% from baseline during 3 years follow up period. CONCLUSIONS: Lumbar spine TBS increase over time with BPs treatment although the changes were less than that of BMD. Also, it preserve for years after stopping treatment, as the changes of lumbar spine BMD. The results of BMD and TBS showed significant correlation during treatment but not during drug withdrawal.

10.Assessment of Left Ventricular Function in Kawasaki Disease by Tissue Doppler Echocardiography.

PURPOSE: Tissue Doppler Imaging(TDI) is a modern echocardiographic technique used for quantitative assessment of myocardial tissue velocities. The purpose of this study is to estimate the left ventricular function by TDI in Kawasaki disease(KD). METHODS: TDI and conventional echocardiography were performed in 33 patients(3.2+/-2.4 years) in acute phase, 33 patients(4.2+/-4.0 years) in subacute phase of KD and 60 children(5.4+/-3.9 years) in controls. Systolic velocity, systolic integral, E' velocity, E' integral, A' velocity and A' integral at the base, mid-septum and the apex were measured. Ejection fraction(EF) and myocardial performance index(MPI) were estimated by conventional echocardiography. Myocardial function was compared in KD patients of the acute and subacute phase with controls by TDI and conventional echocardiography. RESULTS: E' integral at the base(4.59+/-3.87 cm vs 6.91+/-4.29 cm, P<0.05), mid septum(3.52+/-3.13 cm vs 5.49+/-3.47 cm, P<0.05), apex(2.52+/-2.40 cm vs 3.80+/-2.48 cm, P<0.05), A' integral at the mid septum(1.24+/-1.19 cm vs 2.11+/-1.64 cm, P<0.05) and apex(1.14+/-1.32 cm vs 1.90+/-1.59 cm, P<0.05) were lower in the acute phase patients than in the controls. Systolic integral at base, mid septum and apex were significantly lower in KD patients of the acute phase than in the controls. There was significant negative correlation with right coronary artery diameter and systolic integral, E' integral, A' integral at mid septum, E' integral, A' integral at apex in the acute phase of KD. EF and MPI were not significantly decreased in KD. CONCLUSION: Myocardial velocity was not different, but systolic and diastolic integrals at the left ventricular septum were significantly decreased in the acute phase of KD by TDI. The data will need to be compared with febrile controls.
