1.Effects of a Refresher Program for Inactive Nurses on Nursing Professionalism and Nursing Clinical Self-Efficacy.

PURPOSE: The purpose of this study was to identify effects of a refresher program for inactive nurses on nursing professionalism and nursing clinical self-efficacy. METHODS: A pre-experimental design with one group pretest-posttest was used. The refresher program consisted of 150 hours and was provided to inactive nurses who were recruited from a refresher program at S institute. Eighty inactive nurses participated in the study. Data were collected between June and November 2009. To evaluate the effectiveness of the refresher program, researchers explored participants' nursing professionalism and nursing clinical self-efficacy through self-evaluation questionnaire before and after the refresher program. RESULTS: The refresher program was effective in increasing nursing clinical self-efficacy for inactive nurses (p<.001). However, no significant increase was found in nursing professionalism after completing the program. CONCLUSION: The refresher program had a positive impact on increasing the nursing clinical self-efficacy for inactive nurses.

2.Effects of Tailored Case Management using a Gatekeeper for the Depressed Single-household Elderly Population: Focusing on the Mental Health Case Management.

PURPOSE: The aim of this study was to evaluate the effects of tailored case management using a gatekeeper on depression and life satisfaction in the single-household elderly population. METHODS: The design of Quasi experiment was applied to compare the variables before and after the management. Ninety-seven people who had depression categorized by the Korean Geriatric Depression Scale. RESULTS: Depression (t=11.22, p<.001) and life satisfaction (t=-5.36, p<.001) were improved after management in comparison to the results of pre-tests, and the differences were statistically significant. The difference in the pre-test and post-test scores of the support system (chi2=13.89, p<.001) were significant, while the differences in the perception of depression (chi2=.02, p=.891) and coping methods (chi2=.34, p=.558) were not statistically significant. CONCLUSION: Tailored case management using a gatekeeper is effective to reduce the degree of depression and improve life satisfaction in the single-household elderly population having depression. This study offers a model of individualized as well as systemic mental health care for the community of single-house hold elderly people as an effective means for prevention of and early intervention in depression.

3.Effects of Cyclosporine on the Intrarenal Renin-Angiotensin System.

The present study was aimed at investigating the effect of cyclosporine A (CsA) on the renal renin-angiotensin systems. In rats chronically treated with CsA, the intrarenal expression of various genes of the renin-angiotensin system was assessed by Northern blot analysis. Along with the increases in plasma and renal renin activities, chronic CsA-treatment differentially affected the renal expression of renin-angiotensin system. The treatment with CsA for one week did not significantly alter the expression of either type 1 angiotensin II receptor (AT1A) or angiotensinogen gene, but increased the renin mRNA level. The three-week-treatment caused increases in the expression not only of renin but also of AT1A and angiotensinogen genes. Supplementation with L-arginine kept the expression of renin mRNA normal in the one-week-treated, but failed to prevent the alterations of the gene expression in the three-week-treated. Feedback control among components of the renin-angiotensin system also influences angiotesinogen. In the liver, the expression of angiotensinogen mRNA was decreased by the CsA-treatment for either one- or three-weeks. In conclusion, chronic CsA-treatment is associated with a differential expression of various genes for the renin-angiotensin system. L-Arginine may be effective in maintaining the normality of renin-angiotensin system only during early period after beginning the use of CsA.

4.The Effects of Exercise Program on Fatigue, Perceived Health State, Exercise-related Affect, Perceived benefits, and Self-Efficacy: From the samples of female college students.

The purpose of this study was to examine the effects of 6-wk low intensity exercise program on fatigue, perceived health state, exercise-related affect, perceived benefits, and exercise self-efficacy for female college student's. The subjects of the study consisted of thirty-four female college students. The research subjects were assigned to experimental and control group. The experimental group participated in 13-17 and 30-60 minute sesseions of exercise program over 6 weeks. Data analysis was done by t-test with SAS program. The results of this study are as follows. 1) The first hypothesis, "The fatigue of experimental group will be lower than control group", was supported. 2) The second hypothesis, "The perceived health state of experimental group will be higher than control group", was not supported. 3) The third hypothesis, "The exercise-related affect of experimental group will be higher than control group", was not supported. 4) The fourth hypothesis, "The benefits of exercise of experimental group will be higher than control group", was not supported. 5) The fifth hypothesis, "The self-efficacy for exercise of experimental group will be higher than control group", was supported.

5.Establishment of Embryonic Stem Cell Line from Isolated Blastomeres from Mouse Preimplantation Embryos.

OBJECTIVE: The aim of this study was to investigate whether embryonic stem (ES) cells can be established from isolated blastomeres of mouse embryos. METHODS: Blastomeres were separated from mouse (C57Bl/6J) 2- or 4-cell embryos. Isolated blastomeres or whole 4-cell embryos were co-cultured with mitosis-arrested STO feeder cells in DMEM supplemented with recombinant murine leukemia inhibitory factor and ES-qualified fetal bovine serum. After the tentative ES cell lines were maintained from isolated blastomeres or whole embryos, some of them were frozen and the others were sub-cultured continually. Characteristics of tentative ES cell lines as were evaluated for specific gene expressions with immunocytochemistry and RT-PCR. RESULTS: One ES cell line (3.0%) was established from isolated blastomere of 2-cell embryo and one cell line (4.0%) from isolated two blastomeres of 4-cell embryo. And five cell lines (16.7%) were established from whole 4-cell embryos. Both cell lines from isolated blastomere and whole embryo expressed mouse ES cells specific markers such as SSEA-1, Oct-4 and alkaline phosphatase. Marker genes of three germ layers were expressed from embryoid bodies of both cell lines. CONCLUSION: This study suggests that mouse ES cells could be established from isolated blastomeres, although the efficiency is lower than whole embryos. This animal model could be applied to establishment of autologous human ES cells from biopsied blastomeres of preimplantation embryos in human IVF-ET program.

6.Long-term Follow-up Results of Hyperopic Refractive Change.

PURPOSE: To investigate the trend of refractive change in hyperopic patients according to increases in age. METHODS: Eighty-eight children who had hyperopia of more than +1.50 diopters (D) and could be followed up for at least 5 years were included in this study. We divided the patients into two groups according to the level of initial hyperopia and retrospectively analyzed hyperopic refractive changes over a 5-year period according to age at initial diagnosis, presence of esotropia, amblyopia, astigmatism and anisometropia. RESULTS: We gained the following formula about the aspect of hyperopic reduction in 88 patients over a period of 5 years: Diopter (D)=7.99-2.14 Ln (age). The presence of anisometropia and amblyopia did not affect hyperopic reduction. Hyperopic reduction amounts in the group with a hyperopic eye of more than +5D at initial diagnosis were greater than in the group with a hyperopic eye less than +5D, and greater in the group with an astigmatic eye of more than 1D. The presence of esotropia and the age at initial diagnosis did not affect hyperopic reduction. CONCLUSIONS: We show that emmetropization in hyperopic children occurs according to the following formula: Diopter (D)=7.99-2.14 Ln (age). Both the initial level of hyperopia and the concurrent presence of astigmatism affected hyperopic reduction. However, the presence of anisometropia, amblyopia, esotropia and the age at initial diagnosis did not significantly affect hyperopic reduction.

7.Multidimensional Factors Influencing Burnout in Intensive Care Unit Nurses

PURPOSE: This study aimed to identify multidimensional factors influencing burnout in intensive care unit(ICU) nurses. METHODS: A descriptive cross-sectional design was used. Data were collected during February 2016 from a convenience sample of 222 tertiary hospital ICU nurses and analyzed using t-test, ANOVA, correlation analysis, and multiple regression analysis. RESULTS: Burnout correlated positively with compassion fatigue (CF)(r=.37, p < .001), and negatively with compassion satisfaction (CS)(r=-.66, p < .001). The regression model explained 57% of the variance in burnout. For individual characteristics in the model, perceived health status (β=-.27, p < .001) and gender (β=.14, p=.028) were the most influential factors for ICU nurses' burnout. In the model with added work-related characteristics, nursing environment (β=-.22, p=.001), perceived health status (β=-.20, p=.001), and satisfaction with department (β=-.19, p=.007) were the most influential factors. Finally, for the model with psychological characteristics added, CS (β=-.56, p < .001) and CF (β=.35, p < .001) were the most influential factors. CONCLUSION: Results reveal that most ICU nurses have a moderate level of CF and a moderate to high level of CS and burnout, and that individual, work-related and psychological factors are relevant in ICU nurses' burnout. Programs or interventions to reduce burnout should be developed taking into account these multidimensional factors.

8.Effects of Osmolality and Osmotic Agents on Viability and Proliferation of Human Peritoneal Mesothelial Cells.

High glucose activates protein kinase C, induces reactive oxygen species generation, and upregulates expression of transforming growth factor-beta1(TGF-beta1) and fibronectin by human peritoneal mesothelial cells(HPMC). High glucose also induces premature senescence in mesothelial cells. Mesothelial cells shrink after exposure to hypertonic medium and intracellular uptake of amino acids increase to ensure subsequent volume increase. Based on these observations, new and more biocompatible peritoneal dialysis solutions that are glucose free and/or iso-osmolar have been developed. We investigated the effects of different osmolality and different osmotic agents including glucose, mannitol, and icodextrin on viability and proliferation of HPMC. HPMC were obtained from the omental tissues of consenting patients undergoing Cesarean section or elective abdominal surgery. All experiments were performed using cells in the 2nd or 3rd passage. Near-confluent HPMC grown in culture dishes were incubated with serum-free medium for 48 hours to arrest and synchronize cell growth. Lactate dehydrogenase(LDH) release was measured for cell viability and [3H]-thymidine incorporation for proliferation of cultured HPMC, after exposing HPMC to different concentrations of glucose, mannitol, and icodextrin for up to 96 hours. High glucose and mannitol at concentrations up to 100 mM(375 mOsm) did not increase LDH release up to 96 hours compared to control M199. When HPMC were exposed to 2, 4, 7.5, and 9% of icodextrin for 24-96 hours, LDH release did not increase. Glucose at 30, 50, and 100 mM significantly inhibited [3H]-thymidine incorporation by HPMC at 24 and 48 hours. Mannitol at 30, 50, and 100 mM for 24 hours and at only 100 mM for 48 hours also significantly inhibited cell proliferation. Icodextrin 9% (305 mOsm) inhibited cell proliferation compared with control M-199 at 24 hours. In conclusion, high osmolality per se dose not appear to increase HPMC death. However, high osmolality appears to inhibit HPMC proliferation at early stage. In addition, high glucose appears to inhibit HPMC proliferation independent of osmolality since high glucose continues to inhibit cell proliferation at 48 and 72 hours when mannitol at the same concentration did not. Icodextrin 9% of which osmolality is 305 mOsm inhibits HPMC proliferation at early stage but does not appear to increase HPMC death.

9.A Case of Thyroid Hematoma Complicated with Unexpected Airway Obstruction by Blunt Cervical Trauma.

Traumatic thyroid hematoma is a rare problem in cases of blunt trauma, but it can be a life-threatening condition. We report here on a case of thyroid hematoma that was complicated with unexpected airway obstruction due to blunt trauma on the anterior neck. A 23-year-old man who developed neck painful swelling and dyspnea was admitted to the emergency room with a blunt neck injury after motor-bicycle accident, and he hit his anterior neck against the handle-bar. He had no previous thyroid pathology and other medical problems. On the initial examination, his vital signs were stable and the oxygen saturation was 97.8%. A computed tomography (CT) scan of the neck showed fragmentation and extensive hematoma within the right thyroid gland and dislocation of the trachea. No definite bleeding focus was identified on the angiography that was done through the carotid artery. Two hours later, the patient experienced sudden respiratory distress and the oxygen saturation dropped rapidly. The patient needed emerg encyendotracheal intubation and surgical intervention. He successfully underwent right lobectomy of the thyroid gland, and was discharged on the 6th hospital day without any complications. The thyroid function was normal after 3 months.

10.Tinea Incognito Caused by Application of 0.03% Tacrolimus (Protopic(R)) Ointment in Atopic Dermatitis Patient.

We report a case of tinea incognito in a 29-year-old man after applying the 0.03% tacrolimus ointment. He was known atopic dermatitis patient and has been treated with intermittent application of tacrolimus. For his facial pruritus 0.03% tacrolimus ointment was prescribed, and after one week he developed annularly grouped erythematous plaques and patches around the ointment-applied area. The KOH smear revealed multiple fungal hyphae. Dermatologists should be aware of the possibility of topical tacrolimus to be the causative agent for tinea incognito.
