1.Validity of the clinical recommendation of Hepatitis B virus vaccine to adults with negative HBsAg and AntiHBs.

BACKGROUND: Administration of hepatitis B vaccine has played a major part in the management of public health in this country. There were many researches to assess the immunogenecity, safety, long term effectiveness, dose, and route of administration of hepatitis B vaccine. But there was lack of validation of the recommendation that hepatit.is B vaccine should be administered in adults with negative HBsAg and AntiHBs, in an endemic area with high prevalence of HBsAg. Authors tried to establish a proper candidate for hepatitis B vaccination evaluating viral markers of hepatitis B in adults vaccinated. METHODS: 172 hepatitis B vaccinated subjects(35.0%) were drawn from 491 adults who have visited at health care center, Asan Kangnung Hospital from April 1st to May 31th, 1997. They were asked designed questions about the beginning year, frequency of vaccination, and the presence of family members with HBsAg in a direct line. RESULTS: Of the subjects(172), 2.3% were HBsAg(+), 63.4% AntiHBs(+), 68.6% AntiHBc(+). Among the HBsAg(-) adults(149) who had been vaccinated more than three times, 70.4%(105) were AntiHBs(+), of whom 66.6%(70) were AntiHBc(+) and 29.6%(44) were AntiHBs(-), of whom 56.8%(25) were AntiHBc(+). In the subjects(20) with family history of HBsAg, 80% were AntiHBc(+), which is higher than 61.2% in the group(129) without family history. Among the HBsAg(-) and AntiHBc(-) adults(54) who had been vaccinated more than three times, AntiHBs(+) group was not significantly differnt from AntiHBs(-) group in comparison of age and years after initial vaccination. CONCLUSIONS: The two thirds(68.9%) of vaccinated subjects showed AntiHBc(+), which means reflection of previous exposure to Hepatitis B virus. Accordingly, vaccination is not thought to be required for these. Consequently, all individuals are thought to have to be pretested for the presence of AntiHBc as well as HBsAg and AntiHBs in Korea.

2.Relation among Calcium Intake, Bone Metabolism Parameters, Serum Protein and Lipids of Female College Students in Chungnam.

The purpose of this study was to investigate the relationship among calcium intake, blood parameters related with bone metabolism, and serum lipids in healthy adults on self-selected diet. Subjects were consisted of 40 female college students residing in Chungnam. Anthropometric measurements, dietary intake measurements and blood collection were conducted. Serum concentrations of total protein, albumin, alkaline phosphates, leucine amino peptidase, BUN, calcium, inorganic phosphorus, and lipids were measured by biochemical analyzer and ICP spectrometer. The results were as follows. The mean age of subjects was 22.34 years and weight, height and BMI were 52.89kg, 161.29cm and 20.34, respectively. The daily mean energy and calcium intakes were 81.75% and 64.38% of RDA. The mean animal 1:2. The mean serum concentrations were 6.54g/dl(total protein), 4.12g/dl(albumin), 123.24U/iota(alkaline phosphates), 36.59U/iota(leucine amino peptidase), 8.26mg/dl(calcium), 3.29mg/dl(inorganic phosphorus), 60.73mg/dl(triglyceride), 138.49mg/dl(total cholesterol), 65.95mg/dl(HDL-cholesterol), and 60.39mg/dl(LDL-cholesterol). There were no significant differences among calcium intake, bone metabolism parameters, and serum lipids when analyzed by Pearson's correlation coefficient. More systematic studies are required to investigate the roles of calcium in healthy persons on self-selected diets containing different levels of calcium.

3.The Study of Pytoestrogen Intake and Bone Mineral Density of Vegetarian and Nonvegetarian Postmenopausal Women.

There is some evidence that phytostrogen plays an important role in bone metabolism in postmenopausal women. In this study, we investigated the phytoestrogen intake levels and the relation between dietary phytoestrogens intake and bone mineral density of vegetarian (n = 77) and omnivore postmenopausal women (n = 122). Vegetarian women, all of them were seven day adventists, who had been on vegetarian diet (almost lacto-ovo vegetarians;a few vegans) over 20 yrs. The average age of vegetarians and omnivores were 62.3 yrs and 60.2 yrs, respectively and, there was no significant difference. However, body weight (p < 0.001), body mass index (p < 0.001) of vegetarians were significantly lower than those of omnivores. The mean daily energy intake of vegetarians and omnivores were 1386.1 kcal (76.3% of RDA) and 1424.5 kcal (76.9% of RDA), respectively. The mean calcium intake of vegetarians (456.7 mg, 66.3 % of RDA) was not significantly different from that of omnivores (453.5 mg, 65.2 % of RDA). The mean daily isoflavones (daidzein + genistein) intake of vegetarians and omnivore were 33.9 mg and 23.9 mg, respectively. The vegetarians consumed significantly greater quantities of isoflavones (p < 0.05) and lignans precursor (p < 0.05). In the vegetarians, intake of isoflavones was significantly positively correlated with BMD of femoral neck, after adjusted for age and BMI. Also lignan precursor intake of vegetarians was significantly positively correlated with BMD of spine, after adjusted for age and BMI. In conclusion, in omnivore post menopausal women, intake of phytoestrogen such as isoflavones and lignans was little low, and it is not clear that positive association with bone mineral density. But in vegetarian postmenopausal women, phytoestrogen intakes be important factors related to bone mineral density.

4.Validity and reproducibility of a food frequency questionnaire to assess dietary nutrients for prevention and management of metabolic syndrome in Korea.

Little data exists on metabolic syndrome (MetS) related with intake, especially for the South Korean. The purpose of this study was to develop and evaluate a food frequency questionnaire (FFQ) for nutritional assessment in the population with MetS in South Korea. Randomly selected female participants, mean age 21.9 years (n = 38) were invited to answer the FFQ twice (FFQ1 and FFQ2) over a nine-month interval and to complete twelve-day diet records (DR) during the months between in South Korea. The correlation coefficients for nutrient intake between FFQ1 and FFQ2 varied from 0.253 (niacin) to 0.573 (cholesterol), and the energy intake-adjusted correlation coefficients ranged from 0.187 for protein to 0.662 for iron. The energy intake-adjusted and de-attenuated correlation coefficients for comparison of FFQ1 and the DRs ranged between 0.277 (vitamin B1) and 0.768 (fiber), and between 0.229 (zinc) and 0.859 (fat) for comparison of DRs with FFQ2. The percentages of study subjects classified into the same quartiles in both the DRs and FFQ1 ranged from 15.8% (vitamin B6) to 47.4% (calcium), and for the same quartiles in DRs and FFQ2 from 13.2% (vitamin B1) to 44.7% (potassium). The FFQ has reasonably good validity and reproducibility. Further research is needed for an assessment of reproducibility and validation of present FFQ in the subjects with MetS.

5.A Comparative Study on Food Habits and Nutrient Intakes among High School Students with Different Obesity Indexes Residing in Seoul and Kyunggi-do.

The purpose of this study was to compare food habits and nutrient intakes among high school students with different obesity indexes who are residing in Seoul and Kyunggi-do. a total of 533 subjects were assigned to one of the following groups based on BMI : underweight, normal-weight and overweight. Food habits and nutrient intakes were evaluated based on questionnaires and 24hr-dietary record. The results were as follows. There was no significant differences in the general environmental characteristics of subjects with different BMI. However, the father's BMI was significantly higher in overweight female students compared to underweight or normal weight subjects. The menarch age was significantly decreased as the obesity increased. The self-satisfaction score for physical type was decreased as the BMI increased. The overweight group skipped dinner more frequently the other groups among male students. There was no significant difference in the scores of nutrition knowledge, nutritional attitude, and self -satisfaction of subjects with different BMI. The nutrient intakes of overweight group were the lowest among the three groups. In conclusion, obesity among female students may be related to family member's obesity. Also overweight students tend to have lower self satisfaction for their body image and undesirable food habits. Therefore, proper nutrition education is required to maintain desirable food habits for overweight students.

7.A Study on Sodium and Potassium Balance of College Women in Seoul.

The purpose of this study was to investigate Na and K balances in healthy adult women. Anthropometric assessments, biochemical analysis of blood, 3-day dietary flood records and collections of 3-day food, 24-hr urine and faces were performed to evaluate intakes and excretions of Na and K in 20 college women living in Seoul. The mean BMI and blood pressure of the subjects were 21.08 and 110.25/67.50mmHg, respectively. Mean daily intake of energy was 1578.84 kcal, 79% of Korean RDA. Also, daily intakes of Na and K ware 120.86mEq and 44.20mEq. The urinary and fecal excretions of Na were 99.88 and 4.45mEq/day, and those of K were 30.41 and 8.66mEq/day, respectively. The body retention, retention rate, and apparent absorption of Na were 17.11mEq, 13.23%, and 96.31%, and those of K were 5.82mEq, 8.69%, and 80.12%, respectively. The urinary and fecal Na/K ratio were 3.48 and 0.52. There were significantly positive correlations between 1) urinary Na, K excretions and intakes of Na or K, 2) urinary K and BMI, 3) serum K and serum globulin, and 4) urinary Na excretion and serum haptoglobin level, respectively. The results of this study show that Na intake was higher and K intake was lower than those of other advanced nations. Therefore, nutrition education show instruct people to reduce Na intake and to increase K intake.

8.Influence of Clinical Nurse Specialists' Emotional Intelligence on Their Organizational Commitment and Turnover Intention.

PURPOSE: The purpose of this study was to identify the influence of clinical nurse specialists' emotional intelligence on their organizational commitment and turnover intention. METHODS: A descriptive study with second data analysis was conducted with permission. The original data was from the policy research on the status of clinical nurses with extended role hosted by the Hospital Nurses Association in 2007. RESULTS: The mean score of emotional intelligence, organizational commitment, turnover intention were 58.66 (+/-6.63), 46.71 (+/-3.54), 12.87 (+/-3.98) respectively. Emotional intelligence had 40% positive influence on organizational commitment (F=50.58, p<.01, adj.R2=.16) and 24% negative influence on turnover intention (F=16.68, p<.01, adj.R2=.06). CONCLUSION: The results recommend to develop a program that enhances clinical nurse specialists' emotional intelligence. The program could be utilized as an institutional retention strategy to increase clinical nurse specialists' organizational commitment and decrease their turnover intention.

9.Seroprevalence of hepatitis B virus markers among adult in a Youngdong area of Kwangwon Province.

BACKGROUND: Hepatitis B is a leading cause of chronic hepatitis, cirrhosis, and hepatocelluar carcinoma. It is a well-known fact that Korea is one of the hyperenedemic areas of Hepatitis B virus among East Asian countries. Many pnst reports have shown that positive rate of HBsAg in Korea was about 6-14%. But precise data for Yongdong area of Kwangwon Province in Korea was lacking. Here authors report on the prevalence of viral markers of hepatitis B among adults in this area. method: Sera were collected from 7469 residents in Yongdong area of Kwangwon Province who visited the Health Care Center, Asan Kangnung Hospital from September 11th to August 31th. HBsAg, AntiHBc-IgG, and AntiHBs were tested by radioimmunoassay. RESULTS: Positive rate of HBsAg of total study subjects was 8.5%, and was significantly decreased as age increased, and higher in males(10.2%) than in females(6.6%). Positive rate for one or more markers among HBsAg, AntiHBc-IgG, and AntiHBs was 84%, and was significantly increased as age increased, and higher in males(86.2%) than in females(81.2%). All negative rate for HBsAg, AntiHBc-IgG, and AntiHBs was 16.0% and all positive rate was 0.5%. Positive rate of AntiHBc-IgG without HBsAg and AntiHBs was 15.7% Positive rate of AntiHBs without HBsAg and AntiHBc-IgG was 11.7%. Positive rate of AntiHBc-IgG and AntiHBs was 72.1% and 60.2%, respectively. CONCLUSIONS: There was no significant difference in positive rate of HBsAg between this study for residents in Yongdong area of Kwangwon Province and past many reports. 84% of the total study subjects showed serological evidence of previous exposure to hepatitis B virus (HBV) naturally exposed or by vaccine immunization. 16% of the total study subjects was a proper candidate for hepatitis B vaccination.

10.Inbreeding Coefficients in Two Isolated Mongolian Populations: GENDISCAN Study.

GENDISCAN study (Gene Discovery for Complex traits in Asian population of Northeast area) was designed to incorporate methodologies which enhance the power to identify genetic variations underlying complex disorders. Use of population isolates as the target population is a unique feather of this study. However, population isolates may have hidden inbreeding structures which can affect the validity of the study. To understand how this issue may affect results of GENDISCAN, we estimated inbreeding coefficients in two study populations in Mongolia. We analyzed the status of Hardy-Weinberg Equilibrium (HWE), polymorphism information contents (PIC), heterozygosity, allelic diversity, and inbreeding coefficients, using 317 and 1,044 STR (short tandem repeat) markers in Orkhontuul and Dashbalbar populations. HWE assumptions were generally met in most markers (88.6% and 94.2% respectively), and single marker PIC ranged between 0.2 and 0.9. Inbreeding coefficients were estimated to be 0.0023 and 0.0021, which are small enough to assure that conventional genetic analysis would work without any specific modification. We concluded that the population isolates used in GENDISCAN study would not present significant inflation of type I errors from inbreeding effects in its gene discovery analysis.
