1.A Case of Eccrine Nevus.

Eccrine nevus is an unusual entity with various clinical manifestations. Histologically the eccrine apparatus appears simply increased in number and/or size, but structurally normal. We present a case of eccrine nevus on the dorsum of the left 3rd and 4th fingers in a 20 year-old female. The lesions were 3 × 2cm sized, well demarcated, smooth surfaced, brownish plaques with mild hyperhidrosis. Histological examination revealed hyperkeratosis in the stratum corneum and hyperpigmentation of basal layer, and an increased number of eccrine sweat glands and many eccrine coils in the dermis was seen.

The origin of osteoclast-like giant cell tumor (OGCT) of the salivary gland has been debated because the prototypic cells of osteoclast-like cells and mononuclear stromal cells are largely unexplained in this gland. Bone marrow-derived CD14+ and CD45+ monocyte-derived multipotential cells (CD14+/CD45+ MOMC) may be one of the possible origins of OGCTs of salivary glands, which have never been explored in salivary OGCTs. We present a case of OGCT accompanied with carcinoma ex pleomorphic adenoma in the parotid gland of a 67-year-old Korean female. The tumor presented as a rapidly growing cervical mass comprising a central area of carcinoma ex pleomorphic adenoma and a peripheral circumferential area of OGCT. The immunohistochemical staining pattern was phenotypically consistent with bone marrow-derived CD14+/CD45+ MOMC. This case is the first report of a salivary OGCT in Korea.

5.Clinical significance of cerebrospinal fluid pleocytosis in pediatric refractory status epilepticus.

PURPOSE: Refractory status epilepticus(RSE) is a serious neurological emergency in children. The mortality is high and the neurological outcome is not good. This study aimed to evaluate the clinical significance of cerebrospinal fluid(CSF) pleocytosis in refractory status epilepticus in children. METHODS: From January 1999 to January 2006, 25 out of 37 children with refractory status epilepticus had spinal tapping. We retrospectively analyzed the data from these children's medical records. We compared the results between groups with and without CSF pleocytosis, and between a group with first seizure and a group with epilepsy . RESULT: Six out of 25 children had CSF pleocytosis. The group without CSF pleocytosis had a higher mortality rate and required higher doses of antiepileptic drugs as compared with the group with CSF pleocytosis. The group with CSF pleocytosis had much worse neurologic segualae. However, except for the children with CNS infection, the overall prognosis between the group with and without CSF pleocytosis was not significantly different. All children with CSF pleocytosis came in with first seizures. CONCLUSION: In children with RSE, a CSF study must be perfomed as soon as possible to exclude the possibility of CNS infection. A CSF study is even more important in cases of first seizure or CNS infection suspected. Mild CSF pleocytosis without evidence of infection does not seem to affect the prognosis, so physicians should therefore be more cautious in selecting antibacterial or antiviral agents for it.

6.Phenomenological Study on Mothering Experiences of the Married Immigrant Women in Urban Areas.

PURPOSE: This study was done to explore the lived experience of pregnancy, childbirth, postpartum and early child rearing in married immigrant women in urban areas. METHODS: Data were collected from February to April, 2011 through in-depth interviews. Nine immigrant women were selected in a metropolitan area through the purposive and snowball sampling method. After obtaining IRB permission and informed consent from the participants, all interviews were recorded with MP3 recorder and transcribed for analysis. Using the phenomenological approach, data collected through in-depth interviewing were analyzed following Colaizzi style. RESULTS: Data analysis revealed 4 categories and 10 themes related to pregnancy, childbirth, postpartum and early child rearing of married immigrant women in urban areas. These four categories were 'A clumsy foreigner's life with nostalgia', 'Crisis in the crisis', 'Unprepared for motherhood', and 'Living together with new family without regrets'. It was a common finding that they were not ready to be mothers because of the very short time for adjustment in Korea. Compared to rural, immigrant women urban women had various in social support systems. CONCLUSION: The results indicate that adequate education is needed for immigrant women regarding motherhood, and that this education should be culturally appropriate for these women.

7.Low-Grade Endometrial Stromal Sarcoma with Inferior Vena Cava Extension: First Report in Korea.

Low-grade endometrial stromal sarcoma (LGESS) with intravascular extension is very rare, with only 26 cases having been reported. We experienced a case of LGESS with inferior vena cava (IVC) extension. A 60-year-old female presented with left leg edema. She had a history of total hysterectomy, and was diagnosed of leiomyoma at that time. On imaging study, tumor masses were located around both common iliac veins (CIV), and within the CIV and IVC. The pelvic masses on both side and IVC mass were resected, and then the patient received adjuvant hormonal therapy and radiotherapy over the remnant pelvic masses. LGESS with IVC extension is difficult to distinguish from intravascular leiomyomatosis. LGESS is a malignant disease and commonly recurs, even in early stages. Accurate diagnosis, complete resection, proper adjuvant therapy and close follow-up are very important.

8.A Study on Health Promotion Behavior and Contraception in Married Immigrant Women.

PURPOSE: The purpose of this study was to identify health promotion behavior and contraception in married immigrant women. METHOD: Seventy-three immigrant women who were married with Korean men were recruited in October 2008. Data was collected by using questionnaires through a personal interview. The collected data was analyzed by descriptive statistics, Pearson's correlation, t-test, and ANOVA. RESULT: The lowest scoring part among health promotion behaviors was exercise. Stress management, eating habits and personal relationships followed. Health promotion behaviors were influenced by the age at marriage, frequency of going out, and health education experiences. Only 35.6% of subjects used contraception. The popular contraceptive methods were IUD, condom, and coitus interruptus. Most subjects got information about contraception from family and friends. 18.1% of subjects had no source of information. The women's education level had a significant influence on their contraceptive knowledge. CONCLUSION: Programs for exercise and contraception are needed to improve health promotion behaviors in married immigrant women. Nurses are an important part in health promotion. The role of nurses must be expanded in all communities through various intervention programs.
