1.Clinical Evaluation of ASCUS.

OBJECTIVE: To evaluate the significance of atypical squamous cells of undetermined significance(ASCUS) by following-up cervical cytologic smears or histologic finding. METHODS: Two hundred fifty three cases were diagnosed as ASCUS on cervical cytologic smears from January, 3, 1997 to september, 16, 1998 at cytopathology laboratory, College of Medicine, Keimyung Univemity. These cases were evaluated by cervical smears or subsequent biopsies. RESULTS: The results were as follows, On repeat cervical smears, inflammation was 19.7%, normal was 11,1% and HSIL was 1.6%. On histologic diagnosis, CIS was 13,4%, cervicitis was 12,6%, invasive cervical cancer was 4.0%, and severe dysplasia was 3.2%. CONCLUSION: A diagnosis of ASCUS on smears need careful follow-up.

3.The Relationships of Pain cognition, Performance Status, and Hope with Health-related Quality of Life in Cancer Patients.

PURPOSE: The purpose of this study was to determine the relationships of pain cognition, performance status, and hope with health-related quality of life. METHODS: Patients(n=149) with various cancer diagnoses completed the SF-36 standard Korean Version and the Herth Hope Index. The Perceived Meanings of Cancer Pain Inventory was used to measure the cognition dimension of pain, whereas the Brief Pain Inventory Korean version was used to represent the sensory dimension of pain. RESULTS: The patients in the pain group had significant differences in the three dimensions(loss, threat, spiritual awareness) of pain cognition. There were statistically significant negative correlations between the three dimensions(loss, threat, and spiritual awareness) of pain cognitions and SF-36 dimension, and the positive correlations between challenge dimension and SF-36 dimension. Hope had the positive correlation with SF-36 dimensions. CONCLUSION: Pain has a negative impact on health-related quality of life, especially on physical health. However, patients who ascribed more positive meaning to their pain, tended to have a higher quality of life. Therefore, nursing intervention to reinforce the positive aspects of pain cognition is to empower patients to create a sense of control and assume an active role in pain management and quality of life.

5.Nursing Needs of Parents with Hospitalized Child.

The main purpose of this study is to identify nursing needs of parents who have hospitalized child. Research design is a descriptive survey. The subjects for the study were 79 parents who have hospitalized child at a pediatric ward of one of the general hospital attached to a university in J city. The data was collected by researchers using a structured questionnaire. The questionnaire was Liken type 5 point scale, composed of 5 categories with 61 items. The data was analyzed by SPSS/PC. The results of the study were as follows : 1) Mean score of nursing needs of subjects were 3.93 at 2nd day of admission and 3.99 at 7th day of ad mission. 2) Among the categories, the highest nursing need at the 2nd day and 7th day of admission was 'direct nursing', in desending order, 'facilities and environment', 'education and counseling', 'nursing assessment'. The lowest nursing need was 'reference'. 3) Differences between the nursing needs of subjects at 2nd day and 7th day were as follows ; (1) By categories, there was a significant difference only in the 'reference' category(t=2.74, P=.008). (2) By items, there were significant differences in items of 'to check necessary materials(t=2.31, P=.024)', 'to understand family function and family relationship(t=2.12. P=.041), 'to set up study room(t=2.22, P=.030)', and 'to mediate parent's meeting group(t=3.89, P=.000)'. The above result indicated that nursing needs of parents with hospitalized child were above average, especially very high in items about disease process, and items directly associated with treatment and nursing care. So, nurses have to focus on information about the patient's state of disease, treatment, test and procedure, and in efficiently giving direct nursing care to implement more effective care for the hospitalized children and their parents. And some future research is needed to identify the difference of degree of nursing needs of parents with hospitalized child according to admission duration using a different sample and a longer sampling interval.

7.Pathologic Correlations of Various Indications for Large Loop Excision of Transformation Zone(LLETZ) in Patients with Cervical Lesion.

OBJECTIVE: This study was performed to evaluate the effects of mutiple indications and age on the likelihood of finding cervical lesion in large loop excision of transformation zone(LLETZ). METHOD: We reviewed the medical records of 471 evaluable LLETZ of cervix for cervical neoplasia at the Yonsei University College of Medicine, from April 1996 to August l998. The indications for the LLETZ were grouped as being for treatment(biopsy-proven disease)(indication A), significant discrepancy among cytologic and histologic results(indication B), unsatisfactory colposcopic finding(transformation zone not fully visualized)(indication C), possible microinva- sion(indication D), and combinations of the above. RESULTS: The prevalence rates of cervical lesion in performed LLETZ for indication A alone, for indication B alone, for a combination of indication A and C, for a combination B and C, and for indication D alone were 85.7%, 56.5%, 82.3%, 42.6%, 95%, respectively. The data were then reanalyzed to determine the likelihood of finding high-grade disease on LLETZ specimen. Overall, those with preoperative high-grade cytologic or histologic characteristics were much more likely to have high grade disease(78.1%) than were those with preoperative low-grade cytologic or histologic characteristics(27.2%)(p<0.001). The influence of age on the likelihood of identifying diasease on the LLETZ is undertermined. CONCLUSION: Neither age nor the preoperative grade of disease are good discriminators of the likelihood that disease will be found on a LLETZ specimen, However, patients with preoperative high-grade disease are much more likely than those with preoperative low-grade disease to have high-grade dysplasia or invasive cancer on LLETZ.

8.The Effects of Hospital Organizational Culture and Work Environment on Nurses' Intent to Stay at the Current Hospital

PURPOSE: This descriptive study was conducted to identify the effects of hospital organizational culture and work environment on nurses' intent to stay at work. METHODS: Participants of this study were 234 nurses who have worked at a local general hospital in the Chungcheong province for at least 6 months. The data were collected from July 17 to 28, 2017, using self-reported questionnaires and analyzed with multiple regression using SPSS version 22.0. RESULTS: Clinical experience (β=.38, p<.001), institutional support (β=.33, p<.001), and relation-oriented culture (β=.24, p<.001) affected intent to stay at work among nurses. These variables accounted for 34.8% of the variance in intent to stay at work among nurses. CONCLUSION: This study confirms the effects of the work environment and organizational culture on nurses' intent to stay at work. Efforts for improving the work environment and organizational culture need for retaining nurses in a hospital. We suggest further research to identify the other factors associated with intent to stay at work among nurses.

9.Effects of Isoflavones Supplemented Diet on Lipid Concentrations and Hepatic LDL Receptor mRNA Level in Growing Female Rats.

The purpose of this study was to examine the impact of isoflavones on lipid concentrations and hepatic LDL receptor mRNA level in growing female rats. Twenty four rats (body weight 75 +/- 5 g) were randomly assigned to one of two groups, consuming control diet or isoflavones supplemented diet (57 mg isoflavones/100 g diet). All rats has been fed on experimental diet and deionized water ad libitum for 9 weeks. The concentration of triglyceride and total cholesterol were measured in serum and liver. Serum HDL cholesterol was measured. Hepatic LDL receptor mRNA level was tested by RT-PCR. Supplementation of isoflavones did not affect weight gain, mean food intake and food efficiency ratio. Serum total cholesterol and non-HDL cholesterol of isoflavones supplemented rats were significantly lower than those of control rats (p < 0.05). But hepatic cholseterol level was not influenced by supplementation of isoflavones. Hepatic LDL receptor mRNA level not significantly different between control group and isoflavones supplemented group. Therefore, isoflavones may be beneficial on serum cholesterol and non-HDL cholesterol lowering in growing female rats.

10.Effects of Soy Protein and Isoflavones on Bone Mineral Density in Crowing Female Rats.

The purpose of this study was to examine the impact of soy protein and soy isoflavones on bone and mineral density in young female Sprague-Dawley rats. Fifty eight rats (body Weight 75+/-5 g) were randomly assigned to one of four groups, consuming casein, soy protein concentrate, soy protein isolate (57 mg isoflavones/100 g diet) or casein added isoflavones (57 mg isoflavones /100 g diet). All rats were fed on experimental diet and deionized water ad libitum for 9 weeks. Bone mineral density (BMD) and bone mineral content (BMC) were measured using PIXImus (GE Lunar Co, Wisconsin, USA) in spine and femur on 3, 6, 9 weeks after feeding. The serum and urine concentrations of Ca and P were determined. Diet did not affect weight gain and mean food intake. Food efficiency ratio was lower In soy protein groups. The serum concentration of Ca and P were not changed by soy protein and isoflavones. Urinary Ca and P excretion were not significantly different. Spine BMD was significantly increased by soy protein isolate on 3 and 6 weeks after feeding. Femur BMD was significantly increased in the groups of soy protein isolate and isoflavones adding on after 9 weeks. Therefore, soy protein with rich isoflavones may be beneficial on spine and femur BMD increasement in growing female rats.
