2.The Conceptual Analysis of Spirituality.

The purpose of this is to understand the aspect of human spirituality and its general meaning to nursing practice. This conceptual analysis has been defined thus : 1) spirituality is a basic human attribute 2) spirituality gives meaning to life 3) spirituality is a relationship between the omnipotent and the supernatural. Spirituality assumes these : 1) all human beings possess a spiritual nature 2) it is the source of human power 3) it assumes mutual relationship between the individual and others 4) conviction etc. And the result of spirituality leads to : 1) petition 2) peacefulness 3) hope 4) love between the self and the surrounding others and etc. The experience of the spirituality refers to the relationship between the self, others, and with the environment as well as the relationship with God. And that positive and assertive mentality and attitude will result in a desirable spiritual relationship between the caregiver(the nurse and the receiver.

3.Correlation of Expression of CD44, p53 and bcl-2 Protein, DNA Ploidy Pattern, and Clinicopathologic Prognostic Factors in Invasive Ductal Carcinoma of the Breast.

In this study of 64 cases of breast cancer with a clinical follow-up period of more than 5 years, several prognostic factors were evaluated. The purpose of this study was to determine whether any one parameter or group of parameters serves as adequate predictors of tumor behavior and patient's prognosis. Several prognostic factors included clinicopathological variables (patient's age, histologic grade, status of lymph node (LN) metastasis, and tumor size), expression of estrogen receptor (ER), progesterone receptor (PR), p53, bcl-2 and CD44 by immunohistochemistry, and DNA ploidy pattern. The results showed that the expression of ER and PR had a significant inverse correlation with the histologic grade (ER, p=0.05; PR, p<0.05). The expression of p53 protein showed a significant relationship with high histologic grade of tumor (p<0.05). The expression of bcl-2 protein was preferably seen in low histologic grade of tumor (p<0.05) and significantly associated with ER positive or PR positive tumors (ER, p<0.05; PR, p<0.05). This results suggest that bcl-2 protein might play significant roles in ER and PR. The CD44 expression showed a significant relationship with tumor size (p<0.05). The large size and aneuploidy pattern of tumor had a tendency to be associated with shorter patient survival. Cox's multivariate analysis showed that overall survival was affected by LN metastasis because of the shorter survival in patients with LN metastasis. In conclusion, tumor size, DNA ploidy pattern, and LN metastasis were themselves significant predictors of breast cancer survival rate.

4.Successful Control of Extended-spectrum Beta-lactamase-producing Klebsiella pneumoniae Outbreak in a Neonatal Intensive Care Unit.

BACKGROUND: Extended-spectrum beta-lactamase-producing (ESBL) Klebsiella pneumoniae is an important cause of nosocomial infections in neonatal intensive care units (NICUs). This study aimed to reduce ESBL-producing K. pneumoniae in the NICU by using infection control measures. METHODS: We performed prospective surveillance cultures, monitoring, and education for infection control in the NICU between May and August 2011. Specimens were collected from all infants and the environment including stethoscopes, thermometers, ventilators, incubators, etc. The anterior nares and hands of healthcare workers were also screened. We inspected infection control practices and provided feedback. The level of infection control awareness was measured using a questionnaire. RESULTS: The level of awareness and performance of hand washing increased significantly after intervention (both P<0.001). The environmental management of healthcare providers also improved significantly (P=0.001). The yield of ESBL-producing K. pneumoniae from clinical specimens decreased gradually throughout the study period (30.4% in May to 12.6% in August). Central catheter-related K. pneumoniae bacteremia decreased from 1.3/1000 to 0/1000 catheter-days. CONCLUSION: Infection control measures including education, monitoring, and surveillance can lower the incidence of ESBL-producing K. pneumoniae in the NICU.

5.A Structural Model of Hospital Nurses' Turnover Intention: Focusing on Organizational Characteristics, Job Satisfaction, and Job Embeddedness.

PURPOSE: This study was done to build and verify a model of clinical nurses' turnover intention using organizational characteristics, job satisfaction and job embeddedness. METHODS: The study participants were 389 hospital nurses. SPSS and AMOS 22.0 program were used to analyze the data and the modeling of turnover intention. RESULTS: A total of 41% of turnover intention was explained by job satisfaction, job embeddedness and organizational characteristics. Nurses with higher job satisfaction and job embeddedness showed lower turnover intention, while organizational characteristics had an indirect effect on their turnover intention. It was found that organizational characteristics had positive effects on both job satisfaction and job embeddedness, and job embeddedness played a mediating role between organizational characteristics and turnover intention. CONCLUSION: To reduce nurses' turnover intention, hospitals' organizational characteristics should be considered. Nurse managers should strive to increase nurses' job satisfaction and job embeddedness through an understanding of the factors of organizational characteristics such as organizational fairness, nursing work environment, motivation, organizational citizenship behavior, and transformational leadership.

6.Factors Influencing Health Promoting Behavior among Hospital Registered Nurses - Mood States, Resourcefulness, and Health Perception -.

PURPOSE: This study was done to examine the effects of mood state, resourcefulness, and health perception on health promoting behavior of hospital nurses. METHODS: Convenience sampling was conducted for nurses working at three University hospital and 336 nurses were selected. Data were analyzed using SPSS 18.0. RESULTS: The results of the analysis showed that the score for nurses' mood state was 41.76+/-18.90; for resourcefulness, 11.30+/-20.63; for health perception, 3.32+/-.77, and for health promoting behavior, 111.55+/-17.76. Mood state, resourcefulness, and health perception were significantly correlated with health promoting behavior. The overall explanatory power of the effects of nurses' mood state, resourcefulness, and health perception on health promoting behavior was 27.8%. CONCLUSION: Study results indicate that better mood state, higher resourcefulness, and higher health perception result in more health promoting behavior.

Adenoid cystic carcinoma arising from the submucosal glands of the trachea and bronchial tree is rare. The histopathology and natural history of bronchial adenoid cystic carcinoma have been well documented, but detailed descriptions of its cytomorphology are few. We report a case of primary bronchial adenoid cystic carcinoma in a 20-year-old female, diagnosed by bronchial brushing cytology. The cytologic specimens showed large clusters of small cells arranged around cystlike spaces containing globular basophilic material.

8.Significance of Expression of p16, Cyclin D1, Rb, and p53 Protein and Correlation with Clinicopathologic Prognostic Factors in Invasive Ductal Carcinoma of the Breast.

The retinoblastoma (Rb)/cyclin D1/p16 pathway is an important constituent of cell cycle regulation. Perturbations in this pathway due to a variety of genetic aberrations have been reported in many human cancers including breast cancer. We examined the significance of immunoexpression of p16 protein, cyclin D1 protein, Rb protein (pRb), and p53 protein in 128 cases of invasive breast carcinoma. The results were correlated with survival rate and clinicopathological variables, including age, histologic grade, lymph node status, tumor size, estrogen receptor (ER), and progesterone receptor (PR) content. Abnormal expressions of p16 and pRb which were defined as negative staining were seen in 21% and 43% of tumors, respectively. There was a significant inverse relationship between p16 and pRb expression. There was no correlation between p16 staining and any other parameters, including survival rate, cyclin D1, p53, and clinicopathologic variables. Surprisingly, there was a trend for tumors which were positive for pRb to be grade III ductal carcinomas. Cyclin D1 positivity was noted in 46% of cases. The expression of cyclin D1 protein was significantly higher in lower histologic grade, higher ER and PR expression. The expression of p53 protein showed a significant correlation with high tumor grade. In a Cox multivariate analysis, neither p16, pRb, cyclin D1 nor p53 was an independent predictor, but tumor size and lymph node status were independent predictors of patient outcome.

9.A Study of Stress on the Family Caregivers for the Hospitalized Elderly.

This study explored the variables to influence the stress of the family caregivers providing care for the hospitalized elderly. The subjects for this study were 104 family caregivers selected from the caregivers in the in patient departments of two major medical centers in Jeonju city. Data were collected from December 18th to 27th, 1995 using a Liker Scale Questionnaire and interview. Data analyses included percentage, mean, standard deviation, t and F-test. Results were following: 1. The mean of this sample is 21.48, and the standard deviation is +/-5.64 in the perceived stress. The major variables to influence on the stress of caregivers for the hospitalized elderly were the stay (16-24hrs/day) in hospital (F=7.99, P=0.006), no other helpers (t=-2.56, P=.012), perceiving severely on illness (F=3.90, P=.0232) perceiving moderately on physical health status (F=6.20, P=.0029). 2. Regarding the perceived social support of the family caregivers, the higher support the group perceived the lower stress (F=3.54, P=.0326).

10.The Effects of Solution-Focused Group Counseling on the Stress Response and Coping Strategies in the Delinquent Juveniles.

PURPOSE: This study was conducted to examine the effects of solution-focused group counseling on the general stress, stress response and coping in the delinquent juveniles. METHOD: The subjects consisted of a convenience samlple of 60 delinquent juveniles who had been placed under probation in Kwang-ju, and recruited from Oct., 2000 to Dec., 2000. Solution-focused group counseling was conducted for the experimental group once a week for 120 to 180 minutes for 6 week. The control group received no group session. For both groups the level of general stress, stress response and coping were measured before and after the experiment. RESULT: General stress significantly decreased in the experimental group and increased in the control group, but showed no significant difference between the two groups. Stress response decreased significantly in the experimental group and increased in the control group, but showed no significant difference between the two groups. The problem-focused coping significantly increased in the experimental group and decreased in the control group, and showed a significant difference between the two groups. The emotion-focused coping significantly decreased in the experimental group and increased in the control group, and showed no significant difference between the two groups. CONCLUSION: This study showed that solution-focused group-counselling has a positive effect on variables like the general stress, stress response, emotion-focused coping and problem-focused coping. Further study needs to be conducted to verify results.
