1.A Case of Erythema Multiforme - like Eruptions due to Contact with Lacquer Tree.

Irritant or allergic contact dermatitis presents usually eczematous process or occasionally noneczematous lesions like in this case. We report a case of erythema rnultiforme-like eruptions in a 40-year-old female who presented multiple iris or annular shaped maculopapular eruptions on both hands and forearms after contact of lacquer tree. Being performed with standard patch test screening series, lacquer tree extracts and urushiol, she showed significant reactions to the lacquer tree and urushiol. Histopathologic findings showed both features of erythema multiforme and suh acute contact dermatitis.

2.T Cell Epitope Analysis of Structural Protein of Adenovirus.

Thelper (Th) cells play a pivotal role in the regulation of immune responses. Since Th epitopes in adenovirus have not yet been defined, in this study, it was attempted to search for Th epitopes of adenovirus antigens that are restricted by MHC class II (H-2E). Among candidate viral proteins to be screened for Th epitopes, structural protein was selected, since they induced strong IL-2 release from adenovirus immune lyrnph node (LN) cells and the presence of E1 protein, which contains immunodominant cytotoxic T lymphocyte epitopes, did not potentiate the T cell responses. To confirm the presence of Th epitopes in the structural protein, virions were trypsinized and the resulting polypeptides whose molecular weights were lower than 5,000 were fractionated by HPLC. Some of the HPLC fraction turned out to induce LN cell proliferation. Ten synthetic peptides were designed as candidate Th epitopes from the primary amino acid sequences of adenovirus hexon and penton protein which are major constituents of the virion. The selected sequences share the common features of other known H-2E' binding ligands. Among these ten synthetic peptides, peptide of hexon protein amino acid residue 709-721 induced noticeable proliferation of LN cells from preimmune mice, and also able to induce IL-2 secretion from adenovirus-specific T hybridomas, suggesting that the peptide was the most immunodominant Th epitope. Hexon protein 221-233 and hexon protein 676-688 are considered as epitopes also. This study revealed three epitope sequences from adenovirus structural protein that are presented by class II MHC, H-2E.

4.Tuberculin skin test and measles vaccination.

Although the incidence of tuberculosis in Korea in general has declined in recent years, it remains an importat health problem, especially in children. Inspite of the recent controversy over the necessity of tuberculin skin test prior to measles vaccination, this brief report shows 13.8% of positive skin test at 9 and 15 months of age at the time of measles vaccination. We feel, in Korea, practicing tuberculin skin test prior to measles vaccination is good way of detecting asymptomatic infection in young age who needs preventive therapy.

Extrapelvic endometriosis is a rare disease. The majority of extrapelvic endometriosis cases involve scar tissue following obstetric and gynecologic procedures. We have treated two cases of extrapelvic incisional endometriosis. A 39 year old female patient with cyclic vaginal spotting after laparoscopic assisted vaginal hysterectomy due to uterine myoma and a 35 year old female patient with a painful palpable abdominal mass after cesarean section. Both underwent complete excision and were proven to have endometriosis by pathology. Here we report on both cases and review the medical literatures.

7.The Protective Effect of Marly SkinR on Housewife' s Eczema.

BACKGROUND: It is important to avoid the direct contact with water of detergents as possible for the treatment and prevention of hand eczema. OBJECTIVE: The purpose of this study is to evaluate the protective effect of skin protector, Marly skin on housewifes eczema, METHODS: The patients with hand eczema apply skin protector, Maly skin, four times a day on their both hands. They are evaluated with the scores of subjective symptor is(itch, dryness, smarting) and objective signs(erythema, scaling, fissures), and skin replica which are taken from the specified site are analysed by using an image analyser and, scanning electron micrascipe. RESULTS: 4 weeks after application of skin protector, there are statstically significant decrease of total clinical symptom scores. Scanning electron microscopic findings of the skin replicas show marked changes between the beginning and the end of the study. On quantive image analysis of the skin replicas, the length of fissure is a little decreased and the depth of rrows is slightly increased than before application of skin protector, but these are not statistically significant. CONCLUSION: Skin protector, Marly skin, has a protective effect on the development of hand eczema.

9.MRI of the internal derangement of temporomandibular joint: Comparison with arthrographic and operation.

We retrospectively reviewed the MRI findings of 100 temporomandibular joints in 78 patients who had complained temporomandibular joint dysfunction. MRI findings were classified according to Wilke's staging criteria. And these findings were compared with arthrographic findings in 22 joints and surgical findings in 44 joints. According to Wilkes's staging. They were classified into 6 stages of abnormality: stage 0 (33 cases), stage I (19 cases), stage II (10 cases), stage III (18 cases), stage IV (6cases), stage V (14 cases). Among the 22 cases in which arthrography and MRI were done, both studies were well correlated in 10 joints. In 7 joints, MRI was superior to arthrography, which correctly demonstrated the meniscal displacement in 2 joints and . Meniscal deformity in 5 joints. In 5 joints, arthrography was superior to MR, which demonstrated the perforation (1 joint), adhesion (2 joint) and recapture of meniscus (2 joints). Compared with surgical findings, MRI correctly demonstrated the displacement of meniscus in all 44 joints. However, in case of the 10 meniscal perforation, MRI demonstrated the meniscal discontinuity in only 4 joints. Retrospective MR findings in 10 proven cases were the defect in posterior attachment in 4, far anterior meniscal displacement without recapture in 8m condylar spur in 4, and close bone to bone contact in 1. In conclusion, MRI as a primary Fiagnostic modality of temporomandibular joint derangment, is superior for the grading of displacement and deformity of meniscus but inferior for the evaluation of perforation, adhesion and recapture of meniscus to conventional arthrography. In case of the suspected meniscal perforation, arthrographic correlation is recommanded preoperatively.
