1.Adrenocortical Adenoma: A report of 7 Cases.

Primary tumors of the adrenal cortex are comparatively rare. In the medical literature they are most frequently reported as single case. Because of their infrequent occurence some features of their clinical behavior and pathologic anatomy are not widely known. For this reason the recording of the 7 cases from the Department of Pathology, School of Medicine, Chungnam National University is thought to be worth while. Of the seven tumors, five were associated with primary aldosteronism, one Cushing's syndrome, and the remaing one virilizing syndrome. All were benign. Females were affected more frequently than males. Adrenocortical adenomas tend to be small, weighing less than 42 gm. The tumors associated with primary aldosteronism were composed of zona fasciculata-like cells and "hybrid" cells. The tumor with Cushing's syndrome consisted of zona reticularis-like cells. The tumor with virilizing syndrome consisted of zone reticularis-like cells.

2.The Empirical Research on Relationship Between Traits of Medical and Non-Medical Staffs with Indexes of Computer Related Effectiveness.

In the complexity and diversity of modem society, there is an urgent need for an information system which can systematically collect, manage and analyze data. The purpose of this paper is to find the relationship between traits of medical and non-medical staffs with indicators of computer-related effectiveness. In order to fulfill this purpose, dependent and independent variables are extracted from previous findings, and hypotheses are formulated. To test these hypotheses we carried out field research. The sample size which we studied is 203 employees of a general hospital in Taegu. Some of the meaningful findings of this study are as follows: The educational level has a positive relationship with computer usage time. And the attitude about computers has a positive relationship with degrees of satisfaction. Nurses' group especially has the most positive relationship with degrees of satisfaction, and physicians' group, resident doctors' group, pharmacologists' group, non-medicals' group(administrative etc.) are ranked in order of satisfaction.

3.Train of Four Responses as Indicies of Onset and Recovery during Pancuronium Induced Neuromuscular Blockade .

The train of four (TOF) stimulation is commonly used in animal research and clinical practice to determine the degree of neuromuscular blockade. It has been suggested that relationship existed between the degree of single, or initial twitch depression and the amount of TOF fade responses within train. Although block produced by all non-depolarising muscle relaxants is associated with fade in response to tetanic or TOF stimulation, recent studies have shown quantitative difference in degree of these fade between various agent. However, more recently, the variability of this relationship with the same neuromuscular blocker during onset and spontaneous recovery of neuromuscular blockade has been demonstrated. And so, the degree of fade may be affect the reliability in assessment of degree of block and recovery of block in comparison to single twitch stimulation. In present study, we has assessed the effect of pancuronium 0.04 mg/kg on the pattern of TOF (T(4) ratio) and first twitch response in the train compared with control twitch (T(1) ratio) and, thereafter, the twitch responses of vecuronium 0.02 mg/kg on recovery period from pancuronium induced neuromuscular blockade in 14 rabbits. The changes in neuromuscular transmission were measured by recording the isometric contraction of the left gastrocnemius muscle evoked by supramaximal stimulation of the tibial nerve. The results were as follows: 1) The effect of pancuronium 0.04 mg/kg was variable, ranging from incomplete (64.3%) to complete (35.7%) neuromuscular block. 2) During the onset of action, there was greater reduction associated with T(1) than T(4), ratio in all cases. The changes of T(1) ratio decreased steadily in all cases, but the changes of T(4) ratio in cases of incomplete neuromuscular blockade was biphasic, showing initial decrease to 77.2+/-8.1% followed by increase to 100% of T(4) rato when T(1) ratio was 4.6+/-3.2%. 3) During the recovery period, T(4) ratio was increased linearly with T(1) ratio which reached 25% when T(4) ratio was approximately 22.5+/-18.8% in cases of complete neuromuscular blockade. In cases of incomplete neuromuscular blockade, T(1) ratio was increased linearly depending on time course, but the change of T(4) ratio was biphasic, showing initial decrease to 28.2+/-16.6% when T(1) ratio was 25%, followed by increased linearly with T(1) ratio. 4) The twitch response during the onset of the action and recovery after vecuronium 0.02 mg/kg on 75% recovery period of pancuronium induced neuromscular blokade was similar to that of pancuronium. The results of this study suggest that the TOF ratio and the depression of the single twitch bear no fixed relationship. The clinical use of TOF using the T(4) ratio only, as a monitor of the degree of block and recovery from non-depolarising block has been useful, but we would recommended that T, ratio of TOF should be considered to evaluated the incomplete neuromuscular block of pancuronium and vecuronium in rabbit.

4.Construction of MAGE - 3 Expressing Plasmid for Development of DNA Vaccine Encoding MAGE - 3 Cancer Antigen.

PURPOSE: The spectrum of melanoma antigen gene (MAGE)-expressing tumor is very wide and the gene of MAGE express antigens that are targets for specific recognition by cytotoxic T lymphocytes derived from tumor-bearing patients. All of these characteristics represent MAGE as tumor vaccine can be useful for cancer prevention or treatment. Here, we detected MAGE-3 gene expression in cancer cell lines and evaluated recombinant MAGE-3 protein producibility of MAGE plasmid to develope MAGE DNA vaccine. MATERIALS AND METHODS: MAGE-3 gene expression of cancer cell lines was evaluated by reverse transcription-polymerase chanin reaction (RT-PCR). Two kinds of MAGE-3 expressing plasmids were constructed and their MAGE-3 protein producibility was evaluated by immunohistochemistry and immunoblotting using monoclonal anti-MAGE-3 antibody. RESULTS: Among 13 cell lines, SNU484, AMC-HN-3, AMC-HN-4, AMC-HN-7, HeLa, NCI H1703 and HT29 expressed MAGE-3 mRNA. In order to make MAGE plasmid, cDNA that showed 100% DNA homology with MAGE-3 gene was cloned into pcDNA 3 plasmid and pSecTag plasmid. Intracytoplasmic and secretory recombinant MAGE-3 was produced by MAGE-3 containing pcDNA 3 plasmid and pSecTag plasmid, respectively. CONCLUSION: In this study, we showed high expression frequency of MAGE-3 in cancer cell line, and established two kinds of plasmid that produce recombinant MAGE-3 in cell lines. We expect these plasmids will be used in cancer treatment or MAGE-3 function study in future.

5.MR Imaging of the Knee using Fat Suppression Technique: A Preliminary Report.

PURPOSE: The purpose of this study is to evaluate the usefulness of fat suppression technique for MR imaging of the knee. MATERIALS AND METHODS: Twenty-eight knees of 26 patients were imaged at a 1. 5 T MR system. Sagittal and coronal T2-Weighted spin echo images (SET2) and sagittal fat suppression SET2(FSSE) were obtained in all cases. We used a chemical shift imaging method for fat suppression. We compared FSSE with SET2 in terms of the conspicuity of lesions of menisci, cruciate ligaments, cartilage, bone and soft tissue of the knee. RESULTS: Meniscal lesions were detected on FSSE and SET2 as well. FSSE depicted the lesion more conspicuously in 6 cases. For the depiction of ACL tear, SET2 was superior to FSSE in 5 cases. FSSE was better for the visualization of the normal structure of cartilage and it also depicted the cartilaginous lesions more conspicuously in 3 cases. Though bone bruise could be detected on both techniques, FSSE was better. CONCLUSION: FSSE could provide the improved delineation of menisci, cartilage, bone bruise and other soft tissues except the injuries of anterior cruciate ligament. Although FSSE is a reliable method, it can not replace SET2. It may be used as a complemental method in the imaging of the knee.

6.Development of the Health-Information System using a Multimedia Tool.

BACKGROUND: As people have been interested in healt,h and as the microcomputers with multimedia steeing have been supplied more and more, there is increasing need of multimedia information about health. The CD-ROM, which is a representative multimedia tool, is capable of interacting between information-provider and users, offering audiovisual interface, and saving great deal of knowledge. METHODS: We developed the health-information system to provide ordinary people with hea1th information using multimedia tools. It, has 7 parts ; self-diagnosis, exploring human body, drug information, hospital information, first-aid treatment, Q and A, and medical common sense. We got image data such as figures, photos, X-rays, electrocardiograms and pathologic specimens with scanner and film scanner, also moving image(video) with video-capturing program. RESULTS & CONCLUSIONS: We hope the multimedia health-information system to become a useful methodology of patient education and to be used as an information source in medical information network in the near future.

10.Histone Acetylation Level and Histone Acetyltransferase/Deacetylase Activity in Ejaculated Sperm from Normozoospermic Men.

PURPOSE: The aim of this work was to evaluate nuclear histone acetylation level and total histone acetyltransferase (HAT) and deacetylase (HDAC) activity in ejaculated sperm and their relevance to conventional sperm parameters. MATERIALS AND METHODS: Thirty-three normozoospermic men were included in this study. Semen samples were processed by swim-up and then immunostained by six acetylation antibodies (H3K9ac, H3K14ac, H4K5ac, H4K8ac, H4K12ac, and H4K16ac). Our preliminary study verified the expression of HAT/HDAC1 in mature human sperm. From vitrified-warmed sperm samples, total HAT/HDAC activity was measured by commercially available kits. Nuclear DNA integrity was also measured by TUNEL assay. RESULTS: The levels of six acetylation marks were not related with conventional sperm parameters including sperm DNA fragmentation index (DFI) as well as HAT/HDAC activity. However, sperm DFI was positively correlated with HAT activity (r=0.038 after adjustment, p<0.02). HAT activity showed a negative relationship with HDAC activity (r=-0.51, p<0.01). Strict morphology was negatively correlated with acetylation enzyme index (=HAT activity/HDAC activity) (r=-0.53, p<0.01). CONCLUSION: Our works demonstrated a significant relationship of acetylation-associated enzyme activity and strict morphology or sperm DFI.
