1.Influences of Social Support, Self-esteem and Hope on Health Conservation of the Vulnerable Elderly with Diabetes.

PURPOSE: The purpose of this study was to investigate the influence of social support, self - esteem, hope, and health conservation of the vulnerable elderly people with diabetes. METHODS: Participants were 100 vulnerable elderly people with diabetes living in D or K cities. Data collection was done through interviews from February to March, 2016. Social support was measured with the MOS-SSS (Medical Outcomes Study Social Support Survey), self-esteem with Self-Esteem Scale, hope with the Nowotny Hope Scale, and health conservation with the Sung's Health Conservation Scale. IBM SPSS 20.0 was used to analyze descriptive statistics, one-way ANOVA, independent t-test, Pearson correlation, and stepwise multiple regression. RESULTS: Factors affecting the health conservation of the vulnerable elderly people were social support, hope, education level and subjective health status. These factors explained 64.9% of the health conservation. CONCLUSION: It is necessary to construct a comprehensive nursing classification system for elderly people with diabetes in vulnerable class and to develop integrated health conservation program and nursing care as a new social support resource.

2.The Change in Regional Cerebral Oxygen Saturation after Stellate Ganglion Block.

BACKGROUND: Stellate ganglion block (SGB) is known to increase blood flow to the innervations area of the stellate ganglion. Near infrared spectroscopy reflects an increased blood volume and allows continuous, non-invasive, and bedside monitoring of regional cerebral oxygen saturation (rSO2). We investigated the influence of SGB on bilateral cerebral oxygenation using a near infrared spectroscopy. METHODS: SGB was performed on 30 patients with 1% lidocaine 10 ml using a paratracheal technique at the C6 level and confirmed by the presence of Horner's syndrome. The blood pressure (BP), heart rate (HR) and rSO2 were measured before SGB and 5, 10, 15 and 20 minutes after SGB. Tympanic temperature of each ear was measured prior to SGB and 20 minutes after SGB. RESULTS: The increments of the rSO2 on the block side from the baseline were statistically significant at 5, 10, 15 and 20 minutes. The rSO2 on the non-block side compared with the baseline, however, decreased at 15 and 20 minutes. The difference between the block and the non-block sides was significant at 15 and 20 minutes. The BP at 10, 15 and 20 minutes was increased and the HR was increased at 10 and 15 minutes. CONCLUSIONS: We observed an increment of the rSO2 on the block side from the baseline; however, the rSO2 on the non-block side decreased.

3.Nutrition Label Use and Its Relation to Dietary Intake among Chronic Disease Patients in Korea: Results from the 2008-2009 Fourth Korean National Health and Nutrition Examination Survey (KNHANES-IV).

BACKGROUND: Nutrition label reading is helpful in attaining a healthy diet. Nutrition label use and its related factors have been studied in many Korean articles, but their research samples were the general population and not chronic disease patients. This study aimed to explore the use of nutrition labels and its relation to dietary intake among chronic disease patients in Korea. METHODS: A total of 3160 respondents aged> or =20 years from the 2008-2009 Fourth Korean National Health and Nutrition Examination Survey (KNHANES-IV) participated in the study. Their chronic medical conditions included hypertension, hyperlipidemia, coronary artery disease, diabetes and/or stroke and they were interviewed regarding their use of nutrition labels. Nutrition intake was also estimated by 24-hour dietary recalls. RESULTS: Overall, 10.4% of the chronic disease patients reported to using nutrition labels. The nutrients they looked at first were total calories, fat and cholesterol. Factors related to reading nutrition labels were relatively higher education, high body mass index and lower frequency of alcohol intake in males and relatively younger age and higher education in females. Nutrition label users consumed lower total calories, protein and fat in the male CAD group, lower total calories and carbohydrate in both diabetes and total groups and lower total calories and fiber in the female stroke group. CONCLUSIONS: Our study found that, in Korea, a significantly lower rate of patients with chronic disease read nutrition labels. Strategies need to be developed to improve clinical application of nutrition labels in this population.

5.Depression and Anxiety in the Epileptic Patients: the Association With Demographic and Seizure-Related Variables.

BACKGROUND: Depression and anxiety are common psychiatric symptoms in patients with epilepsy, exerting a profound negative effect on health-related quality of life. Several issues, however, pertaining to their association with psychosocial, seizure-related and medication factors, remain controversial. In the present study we have investigated the association of depression and anxiety with various demographic and seizure-related variables in patients with newly-diagnosed and chronic epilepsy. METHODS: We investigated 80 patients with epilepsy (46.3% males, mean age 36.1+/-11.4 years, range 18~64; mean disease duration, 13.4+/-12.2 years). Depression and anxiety were assessed in the interictal state with the Beck Depression Inventory, 21-item version (BDI-21) and the state and trait subscales of the State-Trait Anxiety Inventory (STAI-S and STAI-T), respectively. The association of depression and anxiety with various variables was investigated using univariate analyses and multiple linear regression analyses. RESULTS: Duration of epilepsy, the number of side effects and the number of GABAergic anti-epileptic drugs were correlated with depression, together accounting for 25.1% of the variation of the BDI-21. The STAI-S index was significantly associated with occupation status and the number of GABAergic anti-epileptic drugs accounting for 27.4% of the variation of the STAI-S. Finally, the number of side effects was correlated with trait anxiety. CONCLUSIONS: Our study shows that the duration of epilepsy, occupation status, GABAergic anti-epileptic drugs and the number of side effects are risk factors for the development of depression and/or anxiety.

6.Effects of Fatigue, Nursing Professionalism, and Nursing Work Environment on Nursing Intention for COVID-19 Patients among Nurses in a Designated COVID-19 Hospital

Purpose:
This study aims to identify the factors-along the lines of fatigue, nursing professionalism, and the work environment-that affect the nursing of COVID-19 patients by nurses at a designated COVID-19 hospital in Korea.
Methods:
Data were collected from March 7 to March 31, 2022 via structured questionnaires submitted by 162 nurses, and analyzed using t-test, ANOVA, Scheffé test, Pearson correlation, and hierarchical regression analysis.
Results:
In model 1, the variables among the general characteristics which significantly affect nursing intention are “6 months to less than 1 year of nursing experience for severe COVID-19 patients” (β=.29, p=.001), “having the volition to provide nursing support for future COVID-19 patients” (β=0.28, p<.001), and the “intention according to the hospital situation”(β=.35, p<.001). In model 2, fatigue (β=-.18, p=.007) and nursing professionalism (β=.43, p<.001) affect nursing intention. The total explanatory power of Model 2 is 47.0% (F=16.93, p<.001, R2 =.47).
Conclusion
To increase nursing intention for COVID-19 patients, intervention strategies should reduce nurses’ fatigue and introduce competency-strengthening programs as to improve nursing professionalism.
7.Head Elevation in Spinal-Epidural Anesthesia Provides Improved Hemodynamics and Appropriate Sensory Block Height at Caesarean Section.

PURPOSE: We aimed to determine whether head elevation during combined spinal-epidural anesthesia (CSE) and Caesarean section provided improved hemodynamics and appropriate sensory block height. MATERIALS AND METHODS: Forty-four parous women undergoing CSE for elective Caesarean section were randomly assigned to one of two groups: right lateral (group L) or right lateral and head elevated (group HE) position, for insertion of the block. Patients were positioned in the supine wedged position (group L) or the left lateral and head elevated position (group HE) until a block height of T5 to light touch was reached. Group HE was then turned to the supine wedged position with maintenance of head elevation until the end of surgery. Hemodynamics, including the incidence of hypotension, ephedrine dose required, and characteristics of the sensory blocks were analyzed. RESULTS: The incidence of hypotension (16 versus 7, p=0.0035) and the required dose of ephedrine [24 (0-40) versus 0 (0-20), p<0.0001] were greater in group L compared to group HE. In group L, the time to achieve maximal sensory block level (MSBL) was shorter (11.8+/-5.4 min versus 20.1+/-6.3 min, p<0.0001) and MSBL was also higher than in group HE [14 (T2) versus 12 (T4), p=0.0015]. CONCLUSION: Head elevation during CSE and Caesarean section is superior to positioning without head elevation in the lateral to supine position, as it is associated with a more gradual onset, appropriate block height, and improved hemodynamics.

The purpose of this study was to verify the validity of the Patient Severity Classification Tool by examining the correlations between the APACHE III and the Patient Severity Classification Tool and to propose admission criteria to the ICU. The instruments used for this study were the APACHE III developed by Knaus and thePatient Severity Classification Tool developed by Korean Clinical Nurses Association. Data was collected from the 156 Medical ICU patients during their first 24 hours of admission at the Seoul National University Hospital by three trained Medical ICU nurses from April 20 to August 31 1999. Data were analyzed using the frequency, X2, Wilcoxon rank sum test, and Spearman rho. There was statistically significant correlations between the scores of the APACHE III and the Patient Severity Classification Tool. Mortality rate was increased as patients classification of severity in both the APACHE III and the Patient Severity Classification Tool scored higher. The Patient Severity Classification Tool was proved to be a valid and reliable tool, and a useful tool as one of the severity predicting factors, ICU admission criteria, information sharing between ICUs, quality evaluations of ICUs, and ICU nurse staffing. 1) This paper was awarded the first prize at the Seoul National Hospital Nursing Department Research Contest.

Objective: To investigate the effect of tumor necrosis factor (TNF)-alpha on the production of macrophage migration inhibitory factor (MIF), which might have important roles in the immune mediated inflammatory response of Behcet's syndrome. Methods: Sixty two patients with Behcet's syndrome and thirty healthy controls were included in this study. The concentrations of TNF-alpha in sera were determined by enzyme-linked immunosorbent assay (ELISA). Peripheral blood mononuclear cells (PBMCs) from eleven patients with Behcet's syndrome were cultured for 48 hours with various concentration of TNF-alpha. The concentrations of MIF in sera and culture supernatants were determined by ELISA. Results: Serum levels of TNF-alpha were significantly higher in patients with Behcet's syndrome than in healthy controls. TNF-alpha dose-dependently increased MIF production from PBMCs in patients with Behcet's syndrome. Serum levels of TNF-alpha tended to correlate with serum levels of MIF, although did not reach statistical significance. Conclusion: Upregulation of MIF production by increased levels of TNF-alpha in patients with Behcet's syndrome might be related to the pathogenesis of Behcet's syndrome.

10.Study on the Relationship between Skinfold Thickness and Geriatric Depression in Older Adult Woman.

PURPOSE: This research was aimed at evaluating the differences in depression by skinfold thickness, and the relationship between two variables. METHODS: Research design was a non-experimental descriptive survey. Using a caliper, we measured skinfold thickness on the triceps, and then on the suprailiac, and the mid-thigh. Depression was evaluated using the Geriatric Depression Scale-Short Form (GDS-SF). Data were collected for one month from Sept 1st to 30th, 2011. There were 52 subjects, 25 of which were from elderly welfare centers and 27 were hospitalized. RESULTS: Subjects were an average age of 76.4+/-4.45, and the incidence rate of depression (> or =6) was 36.5% and those who had a sum of 3 skinfolds over 62 mm were 22.7%. The GDS-SF was significantly different in only the suprailiac skinfold thickness (F=7.25, p<.05). CONCLUSION: Findings indicate that depression is different based on suprailiac thickness in older adult women at elderly welfare centers and those who were hospitalized in an medical ward. It suggests that the suprailiac skinfold may be an indicator of abdominal obesity when considering depression in elderly women. Further study is needed to evaluate a cutoff score of skinfold thickness in obesity for older adult women.
