Congenital cystic adenomatoid malformation (CCAM) of the lung is a rare pulmonary malformation, associated with local aberrations of parenchymal tissue development, which have been classified lung bud anomalies. Lung bud anomalies include infantile lobar emphysema, congenital cyst of the lung, congenital cystic adenomatoid malformation, pulmonary sequestration and bronchogenic cyst. CCAM can enlarge rapidly by ball-valve air entrapment by cysts, lead to mediastinal shift and compression of the heart. compression of opposite lung may result in pulmonary hypoplasia and CCAM causes acute respiratory distress in newborn. We report a neonate with CCAM who were successfuly managed during perioperative period.

3.MR Imaging in the Evaluation of Mullerian Duct Anomalies.

PURPOSE: To assess the role of MRI in the diagnosis of uterine anomaly. MATERIAL AND METHODS: MRI(n:15), hysterosalpingography(n:7) and ultrasonography(n:7) were performed in 15 patients with suspected MullerJan duct anomaly. Nine cases were proved by operation and six cases were diagnoed with imaging and clinical findings. According to Buttram and Gibbons modified classification, the anomalies were 4 cases of class I, 2 cases of class III, one case of class IV, and 8 cases of class V. RESULTS: MRI enabled accurate diagnoses of anomalies in all cases, but HSG and USG showed wrong diagnoses in 3 of 7 cases and in 1 of 7 cases. CONCLUSION: MRI, especially T2-weighted images parallel to long axis of uterine corpus, was very useful in diagnosis of the Mullerian duct anomaly, because it could depict exactly the external fundal contour, intercornual distance, septum, transverse vaginal septum, and associated abnormalities such as hematocolpos and hematometra.

4.The Effects of Preceptor Nurses' Self-leadership on Role Recognition and Job Satisfaction.

PURPOSE: The purpose of this study was to examine the effects of preceptor nurses' self-leadership on role recognition and job satisfaction. METHOD: The participants were 171 preceptor nurses worked in one of three general hospitals in Seoul. The data were collected from April 12 to May 12, 2011 and analyzed using descriptive statistics, t-test, ANOVA, pearson correlation, Scheffe, and regression with SPSS Win 19.0 program. RESULT: The results of the study were as followed. The average score for preceptor nurses' self-leadership was 3.55, for role recognition, 3.81 and for job satisfaction, 3.28. The factors of the preceptors' self-leadership affecting role recognition were self-expectation (beta=260, p=.002) and goal setting (beta=199, p=.030), and those factors accounted for 18.9% of explanatory power. The factors of self-leadership influencing job satisfaction were self-expectation (beta=18.3, p=.021), and rehearsal (beta=-168, p=.030), and those factors accounted for 31.3% of explanatory power. Among the factors of self-leadership, self-expectation influenced both role recognition and job satisfaction. CONCLUSIONS: The results of the study indicate that to improve preceptor's role recognition and job satisfaction, there should be special training programs to enhance preceptor's self leadership.

5.Brain MRI in children with delayed development: emphasis on white matter maturation.

To analyzed the progression of white matter maturation and white matter pathology, MR imaging of the brain was obtained in 38 children with delayed development. Children with developmental delay showed a high incidence of MR abnormalities(34/38, 89.5%). Delayed pattern of myelination and gray-white matter differentiation was seen in 13 patients. Twenty-two patients had white matter patholgy, including 14 with white matter hypoplasia, seven with focal small infarction, five with periventricular leukomalacia, and three with high signal intensities on T2 weighted image. Associated structural abnormalities were also evaluated. The most common lesions in decreasing frequency were cerebral atrophy and dysgenesis of the corpus callosum, pachygyria and/or polymicrogyria, porencephalic cyst and Leigh's disease. Twenty-three of 34 children had multiple abnormalities on MRI. The MRI was useful in depicting the progression of myelination and other white matter lesions, and serial follow-up MR is recommended for patients with delayed or lack of myelination and gray-white matter differentiation.

6.The Relations between Participation in Voluntary Service Activity and Health-Related Quality of life among the Elderly.

OBJECTIVES: This study was performed to identify the relations between the old's participation in voluntary service activities and health-related quality of life. METHODS: The subjects of the study were 318 elderly people aged over 65 years. They were interviewed with the use of a questionnaire from May 1 to June 30, 2012 to collect data. To verify the data collected, this study carried out a linear regression analysis. RESULTS: The health-related quality of life of the elderly who were participating in voluntary service activities was statistically significantly higher than that of those who were not. CONCLUSIONS: This study has meaning in that it discovered that the elderly people's participation in voluntary service activities had a positive effect on their health-related quality of life.

7.Cloning of Chromosomal Band Specific cDNA - cDNA related with neural development-.

Recently, surmountable amounts of genes are being cloned without information about them and it has become neccessary to develop new techniques for discovering genes with more informaiion like as chromosomal location and possible functions. We have developed one such a method and applied it to search for genes that may be related with the neural development. The mRNAs were extracted from cerebral cortex of 18 week old human fetus, cDNAs were made by reverse transcription from these mRNAs and Uni-amp cDNAs having Uni-amp adapters at both ends were made for subsequent PCR. To observe the distribution of the Uni-amp cDNAs on the chromosome, fluorescent in situ hybridization was performed with biotin labeled Uni-amp cDNAs. Among the chromosome bands showing strong hybridization with the cDNAs, 7q22 was microdissected from the chromosome hybridized with unlabeled Uni-amp cDNAs and amplified by PCR with Uni-amp primers. These amplified cDNA fragment were subcloned to vectors and the nucleic acid sequences were analysed. As a result, 46 different clones were obtained. They were categorized as 12 clones of well characterized genes, 14 clones showing low homology with known genes, 13 clones of simply registered uncharacterized human cDNAs, 7 clones of unknown genes. In situ hybridization histochemistry of 34 novel genes, except 12 known genes, were performed on developing and adult rat tissue sections to see the tissue specificity and developmental expression of these genes. The expression of several novel genes were restricted to the nervous system. From these results, it may be suggested that our technique is very useful to clone the genes expressed in the developing human braine with confirmed chromosomal location. In addition, this cloning technique can be used to discover the new genes related with neural development in combination with functional screening methods.

8.A Case Report of Neuroleptanesthesia in a Parturient with Sick Sinus Syndrome.

Inappropriate sinus bradycardia associated with degenerative changes in the sinoatrial node has been designated as the sick sinus syndrome. Patients can be asymptomatic but often complain of palpitations and syncopal episodes. A 30 year old parturient, whose pulse rate was about 45-55 beats/min, underwent cesarean section under neuroleptanesthesia. Authors report this case with the review of the relevant literatures.

9.A Case of Diabetic Nephropathy without Microalbuminuria in Type 1 Diabetes.

Diabetes is a rapidly increasing heath care problem all over the world due to increased prevalence during past decade. Diabetic nephropathy develops in 25-30% of patients with type 1 diabetes and is the leading cause of end stage renal disease. Diabetic nephropathy is characterized by persistent proteinuria, decline in renal function, hypertension and increased cardiovascular morbidity and mortality. Early detection of diabetic nephropathy risk is an important goal because early diagnosis and treatment prevent advanced renal damage and other diabetic complications. Increased urinary albumin excretion rate is widely accepted as the first clinical sign of diabetic nephropathy. However, reduced glomerular filtration or hypertension could be the first manifestation in some diabetic patients. We need improved markers and predictors of diabetic nephropathy risk. We report a case of diabetic nephropathy and decreased glomerular filtration rate (GFR) without microalbuminuria occcured in type 1 diabetic patient.

10.Modified Tei index in patients with Kawasaki disease by tissue doppler imaging.

PURPOSE: A quantitative and easily measured Doppler index of combined systolic and diastolic ventricular myocardial performance (Tei index) was recently proposed as a potentially useful predictor of global myocardial performance. However, presence of heart rate fluctuation makes it unreliable. Therefore, the modified Tei index was introduced by using tissue Doppler imaging (TDI) which enables measuring contraction and relaxation velocities from myocardium simultaneously. The purpose of this study was to investigate the effectiveness of the modified Tei index in the evaluation of global cardiac function. METHODS: Forty eight patients in the acute phase of Kawasaki disease (KD) were studied. These patients were divided into two groups according to the modified Tei index. TDI and conventional echocardiography were performed. Systolic velocity, systolic displacement, E' velocity, E' displacement, A' velocity and A' displacement were measured at the base, mid-septum and the apex of the interventricular septum. Ejection fraction (EF), Tei index and modified Tei index were estimated. RESULTS: Modified Tei index correlated negatively with systolic displacement, E' displacement and A' displacement at the base. Modified Tei index correlated positively with ESR and CRP. EF and Tei index were normal in KD patients. CONCLUSION: Modified Tei index is a safe, feasible and sensitive index for evaluating global ventricular functions in spite of normal EF and Tei index in KD.
