PURPOSE: This study aimed to explore nursing students' experiences of observing surgeries in the operating room. METHODS: The data were collected through written scripts of nursing students who participated in the operating room practice. Data were analyzed via qualitative content analysis. RESULTS: Five themes emerged from the data: ‘preparing with the thrilled hearts toward unknown area’, ‘drawing back in front of the mirror of realities’, ‘becoming an audience at a lifesaving orchestra’, ‘reaching a tipping point of knowing’, and ‘redrawing the trajectory of dream to be a nurse. CONCLUSION: This study will prove helpful in describing their needs for systemic and emotional support. Findings indicated that major contributions of surgical observations to nursing students are more integrated understandings of nursing and the determinations of their career preferences.

This is a case report of a cystic hygroma with polycystic kidney in a fetus which was suspected by ultrasonography and was confirmed by autopsy. Recently, we have experienced this case in 25-year old woman repeatedly and we report that with a brief review of relevant literature.

PURPOSE: The purpose of this study is to explore the experience of university students who witnessed of school violence during their formative years. METHODS: The participants in this study included 30 university students who had witnessed school violence. Analysis of the data was performed using content analysis according to Downe-Wamboldt. Data were collected from September 2015 to February 2016. RESULTS: Based on this study, there were four domains of school violence including witnessing, type of reaction, aftermath and future measures. According to the analysis, these four domains were classified into 13 variables, 32 categories and 73 significant statements. CONCLUSION: The results of this study suggest that further research is needed to develop measures to address problems that have developed for students who have witnessed school violence.

PURPOSE: this study was to evaluate Cognitive Function Test for the Korean population. Digit Forward (DF), Digit Backward (DB) and Trail Making Test-A(TMT-A) and the translated version of the Trail Making Test-B (TMT-B) were used. The study examined the performance of Korean normal adult population whose age ranged from 20 to over 80. It was predicted that performances of the Korean population would be different from that of the United States population due primarily to their differences in language, and education. METHOD: Normal Korean adults at the Daejeon Metropolitan City and Chungchung Province (N=298) participated in this study. Seven age scores were evaluated; 20~29, 30~39, 40~49, 50~59, 60~69, 70~79, &>80s. The effects of age, gender, and years of education was examined, which yielded significant age and education effects. The scores were further specified accordingly in terms of years of education (0, 1~6, 7~12, 13~16, &>17), and gender. RESULT: As the age increased cognitive function score decreased. As the years of education increased, cognitive function scores increased. There was no difference in cognitive function scores according to gender. Age and year of education had significant effects on cognitive function and explained 52-58% of variants of each test. CONCLUSION: The data of the cognitive function tests are expected to be utilized for research purposes such as basic and clinical studies, as well as practical purpose such as cognitive assessment for traumatic brain injury, stroke, and elderly and nursing education for assessment tools.

OBJECTIVE: Our purpose was to determine normal amniotic fluid alpha-fetoprotein level in midtrimester Korean pregnant women whose neonatal outcomes were normal. METHODS: Amniotic fluid alpha-fetoprotein(AFAFP) levels were measured by specific radioimmunoassay (RIA) in midtrimester pregnant women for various indications of amniocentesis from May 1992 to July 1999 at Seoul National University Hospital. Normal ranges were obtained from 640 singleton pregnancies in which neonatal outcomes were normal. RESULTS: Median values of AFAFP in Korean pregnant women were 13,250ng/mL, 12,900ng/mL, 11,150ng/mL, 9,430ng/mL, 8,019ng/mL, 6,800ng/mL, 5,850ng/mL, 5,750ng/mL, 5,210ng/mL, 3,420ng/mL at 15 week, 16 week, 17 week, 18 week, 19 week, 20 week, 21 week, 22 week, 23 week, 24 week, respectively. CONCLUSION: This determination of the median values and the normal range of AFAFP level by each gestational week in uncomplicated Korean women could be used reference values for prenatal diagnosis of various disorders like open neural tube defect.

Group occupational health service programme started in 1990 is one of the measures to cope with limited human and financial resources in occupational health. The programme has expanded rapidly to include 52 institutions, private as well as public, all over the country. In spite of its potential impact on health of employees and practice of occupational health in small and medium sized industries, comprehensive evaluation in terms of quality has not been tried. This study has aims to develop the criteria to assess the quality of newly developed group occupational health service programme, and to investigate the quality of institutions, and finally to develop policies for the quality improvement. 1) Criteria development : By defining occupational health services, in particular for small and medium sized industries, as one of the primary health care, followings are included as core elements of qualitative occupational health programme ; accessibility, continuity, comprehensiveness, technical quality, intersectoral collaboration, emphasis on preventive services, community participation, and adequacy. Again each element is divided into five major components of national health system infrastructure developed by the World Health Organization ; development of health resources, organized arrangement of resources, delivery of health care, economic support, and management. In turn, each component is categorized into three aspects of quality assessment, structure, process and outcome. Expert panel selected several criteria for each category to evaluate the programme. Criteria were modified according to each group of interviewees, to produce two sets of questionnaire, one for chief operating officer and another for nurses in the institutions, and the chief operating officer and workers in the workplace. 2) Subject : Of all 52 institutions, 25 voluntarily participated in the survey. At individual institution, chief operating officer and practicing nurses were interviewed in depth. After intensive education for interviewees, every interview was performed with standardized guideline and questionnaire. The quality of the 'Group occupational health service programme' was found to be lower than expected. Especially In continuity, comprehensiveness, technical quality, community participation and adequacy, lower quality in structural aspect was commonly identified throughout all the institutions. Quality in terms of accessibility and continuity highly varied among institutions. To improve quality of the programme, more comprehensive and systematic programme such as accreditation has to be introduced. In addition, human resources, governmental fund and information systems for individual workers are to be developed. As a long range plan, integration of occupational health services into the national health systems and pooling of financial resources and planned allocation should be considered.

OBJECTIVES: Our purpose was (1) to determine whether amniotic fluid concentrations of interleukin-6 are of value in the diagnosis of histologic chorioamnionitis of preterm placenta and in the prediction of significant perinatal morbidity and mortality in patients with preterm premature rupture of membranes and (2) to compare the diagnostic performance of amniotic fluid interleukin-6 with that of amniotic fluid microbial culture for these outcome variables. METHOD: The relation among placental histologic finding, perinatal outcome, amniotic fluid culture, and amniotic fluid interleukin-6 concentrations were examined in 65 patients with preterm premature rupture of membranes who delivered preterm neonates within 72 hours after transabdominal amniocentesis. Interleukin-6 level was determined by enzyme-linked immunosorbent assay. Receiver-operator characteristic curve, Mann-Whitney U test, and Fisher's exact test were used for analysis. RESULTS: 1) Patients with acute histologic chorioamnionitis had significantly higher median amniotic fluid interleukin-6 concentrations than those without histologic chorioamnionitis (median 12.6 ng/ml, range 0.03 to 142.2 ng/ml vs median 0.5 ng/ml, range 0.03 to 16 ng/ml; P < 0.0001). 2) Amniotic fluid having interleukin-6 concentrations higher than 3.2 ng/ml had a sensitivity of 78% (35/45) and specificity of 95% (19/20) in the diagnosis of acute histologic chorioamnionitis and sensitivity of 74% (25/34) and specificity of 65% (20/31) in the prediction of significant neonatal morbidity and mortality. 3) These sensitivities were significantly higher than those of amniotic fluid culture, but there were no significant difference in specificities between amniotic fluid interleukin-6 and culture (histologic chorioamnionitis: 78% vs 51%, p<0.01; significant neonatal morbidity and mortality: 74% vs 47%, p<0.01, respectively). CONCLUSIONS: Test of amniotic fluid interleukin-6 is of value and more sensitive than amniotic fluid culture for the antenatal diagnosis of histologic chorioamnionitis and for the prediction of perinatal outcome in patients with preterm premature rupture of membranes.

BACKGROUND: The prevalence of dementia is estimated to be as high as 11.3% among coummunity-dwelling persons over 65 years of age in Korea. The fear of dementia for both patients and families is growing because of the chronicity and progressiveness of the disease. Though the recognition, evaluation, and management of patients with dementia are important for the care of older pateint, unfortunately, MMSE takes 5 to 10 minutes to administer and is cumbersome to carry for all elderly patients in busy office. The Clock Drawing Test has been known as valid, easily administered and low cost screening procedure for the congnitive impaiment associated with dementia in general geriatric clinic population. In this study, we assessed the usefulness of this measure as a screening test for dementia, on the basis of the correlation between clock drawing and MMSE-K. METHODS: The subjects for this study consisted of 47 elderly who visited to two elderly clubs in Seoul from April 1996 through June 1996 or the outpatient department of Family medicine of the Ehwa Mok-dong hospital. They received the Mini-Mental Status Exam and the Clock Drawing test. Three observers evaluated them by ratings from 10(best) to 1(worst). Inter-rater reliability of scores from the Clock Drawing Test was assessed and Pearson correlation coefficients were used to examine the relationships between scores from the Clock Drawing Test and MMSE-K. RESULTS: Their mean age was 75.4 years with range of 65 to 91 years. There were 34 women and 13 men. Correlations between individual raters was highly significant(y=0.8939, 0.8552, and 0.8698, respectively ; P<0.01). Compared to scores on MMSE-K, rating from clock drawings by each of 3 raters revealed statistically significant correlations(y=0.7164, 0.7398, and 0.6884, respectively ; P<0.01). CONCLUSIONS: The clock drawings can be rated reliably by even untrained observers and performance on this task relates significantly to performance on MMSE-K of severity. We confirmed the usefulness of clock drawings as a simple, easily administered, low cost, and reliable general screening tool for dementia in a general geriatric clinic population.

In many previous studies for the relationship between air pollution and birth outcome, the exposure was based on the data for environmental monitoring site. We reviewed the epidemiologic studies that evaluated the health effects of air pollution on birth outcome such as low birth weight and preterm births. We identified the air pollution exposure during pregnancy was related with low birth weight and preterm birth, although there are differences between studies for the critical period of vulnerability. also suggests that the indoor and personal exposure to VOCs during pregnancy may contribute the adverse pregnancy outcomes. The biological mechanisms whereby air pollution might influence health of fetus are not clearly established. Controlling for potential confounders and valid assessment of exposure are the problem remained in these epidemiologic studies. In the future, more studies are need to investigate the effect of air pollution on preterm birth or stillbirths, considering the various exposure period, and the biological mechanism. also results should be taken into account for future advisories and evaluation of environmental policy .
