1.Expression of NCX and NCKX Isoforms mRNAs in Rat Pituitary Gland.

Na(+)-Ca(2+) exchanger plays a fundamental role in controlling the changes in intracellular concentration of Na(+) and Ca(2+) ions. Two different families of Na(+)-Ca(2+) exchanger, NCX (K(+)-independent Na(+)-Ca(2+) exchanger) and NCKX (K+-dependent Na(+)-Ca(2+) exchanger), are known and each family includes several isoforms. But little is known about their expression in pituitary gland. In this study, in situ hybridization with digoxigeninlabeled riboprobe and double-labeled experiments with immunohistochemistry were applied to investigate the expression of NCX and NCKX mRNAs and their distribution in normal rat pituitary gland. NCX2 mRNA hybridization signals were expressed in pars distalis, while both NCX2 and NCKX2 mRNAs expression were strongly observed in pars nervosa. NCX2 and NCKX2 mRNA were also expressed in supraoptic nucleus of hypothalamus. In pars distalis, 68.2% of growth hormone secreting cells was colocalized with NCX2 mRNA, whereas NCX2 mRNA was not found in S100 positive folliculostellate cells. These results suggest that NCX2 in pars distalis and NCX2 and NCKX2 in pars nervosa appear to be involved in endocrine function of pituitary gland.

2.A Case of Retroperitoneal Neurilemmoma.

Neurilemmoma is a relatively rare, which was first described by Stout in 1935. Neurilemmoma is a neoplasm arising from Schwann cells. It is usually solitary and may be benign or malignant, solid or cystic, encapsulated or diffuse. We report one case of retroperitoneal neurilemmoma with review of literature.

3.Factors Affecting Hospital Nurses Intention to Remain: Focusing on Role Conflict.

PURPOSE: The purpose of this study is to investigate nurses' role conflict and intention to remain and to identify factors that may influence this intention. METHODS: Data were collected from 172 nurses in a 600-plus bed hospital on nurses' intention to remain, which included 47 items and nurses' role conflict, which included 82 items from a self-reported questionnaire. Data were analyzed by descriptive statistics and stepwise multiple regression using SPSS 22.0. RESULTS: The mean of the role conflict frequency was 2.71(±.39) and the mean severity was 2.86 (±.47). The most significant item among the items of intention to remain was “a nurse's job is to help people.” The intention to remain employed showed a significant difference in the duration of desired working period and subjective job satisfaction. The subjective job satisfaction of nurses was the most influential variable as a factor affecting the intention to remain, followed by frequency of role conflict in nursing practice. CONCLUSION: These results suggest that inadequate nursing environments in nursing practice require improvement and support the idea that nurses with positive attitudes of the nursing organization and theirs leaders reduce nurses' related fatigue.

4.Changes of Tc - 99m - HDP Bone Scan Findings in Avascular Necrosis of the Femoral head ( ANFH ) Treated with Vascularized Fibular Graft.

Vascularized fibular graft is one of the treatment option for avascular necrosis of femoral head (ANFH) and has been known as providing a strut mechanical support and new vascular bed. It is very important to evaluate a healing process in femoral head after vascularization procedure. But a plain radiographic finding has a problem to assess pathological changes of ANFH treated with vascularized fibular graft. Authors analyzed chronogical changes of Tc-99m-HDP bone scan in 31 cases of ANFH treated with vascularized fibular graft from April, 1991 to May, 1994 and following results were obtained. l. Until 2 months after surgery, no change in size of cold defect area and no significant change in extent of RI uptake in reactive zone were noted. 2. During 2 months and 6 months after surgery, slight decrease in size of cold defect area and minimal changes in extent of RI uptake in reactive zone were observed. 3. During 6 months and 12 months after surgery, size of cold defect area decreased to 1/2 and changes in extent and degree of RI uptake in reactive zone were observed. 4. During 1 year and 2 years after surgery, size of cold defect area were minimal and sometimes faded away. RI uptake with decreased density in reactive zone broadened and moved to subchondral area. 5. After 2 years after surgery, cold area faded away and small increased RI uptake were observed in same site. In conclusion, Tc-99m-HDP bone scan which is economic and easily applicable in a patient is very useful for follow up study in ANFH treated with vascularized fibular graft.

5.Mother's Prenatal Care and Delivery Services Satisfaction according to Mode of Delivery.

PURPOSE: This study aimed to provide fundamental information about childbirth and antenatal care for pregnancy women and to find differences in mother's antenatal care and delivery service satisfaction between vaginal delivery and cesarean section. METHODS: This study was conducted in 4 residential areas and a study sample of 184 postpartum mothers who agreed to collect data. Data was collected from September 1 to October 20 2007 and a structured questionnaire were recruited by the survey. The data was analyzed by t-test and chi-squire test using SPSS/WIN 12.0. RESULTS: There was a significant difference in delivery place between vaginal delivery and cesarean delivery. Only 10.7% of vaginal delivery group delivered in general hospitals, however 24.5% of the cesarean section group delivered in general hospitals. Early antenatal care also showed statistical difference in mode of delivery. 43.5% of vaginal delivery mothers visited hospitals for the detection of pregnancy but 28.3% of cesarean section mothers did that. Vaginal delivery mothers more satisfied with her own delivery method and suggested a vaginal delivery to others. CONCLUSION: These results suggest that cesarean section mother's sensitivity of early antenatal care was less than vaginal delivery mother. Satisfaction related to delivery care services were higher in the vaginal delivery group.

6.The Effect of Crystalduringloid Administration on Blood Pressure and Heart Rate Change during Epidural Anesthesia in Cesarean Section.

BACKGROUND: Recently, epidural anesthesia for elective and emergency cesarean section is increasing in popularity and graudally replacing general anesthesia. However, the complications from the epidural anesthesia such as hypotension, dyspnea, nausea and vomiting have often been reported. This study was designed to estimate the efficacy of crystalloid administration on blood pressure and heart rate change during epidural anesthesia in cesarean section. METHODS: Forty ASAI and II patients scheduled for cesarean section under epidural anesthesia were randomly allocated to receive either only maintained fluid administration(group I) or 15 ml/kg crystalloid solution over 15 minutes(group II). Epidural anesthesia was performed in a lateral decubitus position. According to the patient's height, 20~23 ml of 2% lidocaine was injected through epidural catheter at the L3~4 interspace. RESULTS: Cardiovascular responses and complications after induction of epidural anesthesia were compared and results were as follows. The incidence of hypotension was 14/20 (70%) in the groupI and 4/20(25%) in the group II(p<0.05). Systolic blood pressure after induction of epidural anesthesia in the group I significantly decreased from 6 min to 12 min as compared to the group II. The mean heart rate after induction of epidural anesthesia in the both group was no significant difference to baseline heart rate. CONCLUSIONS: Considering above results, we conclude that a prophylactic crystalloid solution infusion is effective for minimizing and managing hypotension associated with epidural anesthesia in cesarean section.

7.A Clinical Study on Sepsis in Children: esp. about buffy coat smear.

A clinical and laboratory study was conducted on 58 children who had been admitted to out pediatric department from February to September 1979, under the clinical diagnosis of septicemia. Following results were obtained: 1. Boys were affected more frequently than girls(1.3:1), and highest incidence was noticed in newborn period, comprising 50% of the total cases. 2. The common clinical manifestations, in order of frequency, were fever, lethargy, jandice, poor sucking and abdominal distension, and hyperbilirubionemia was the most common assocaited disease, followed by anemia and leukemia. 3. Clinical course revealed death in 6 patients(10.3%), discharge against advice in 7cases(21.1%) and complete recovery in the remaining 45 cases(77.6%). 4. Peripheral blood count showed leukocytosie in 40%, leukopenia in 12.1% and thrombocytopenia was seen in 45% of cases. 5. Gram staining of buffy coat smear showed bacteria in 34.5% and especially high ratio was noticed in newborn infants(48.3%). 6. Positive blood culture was seen in 39.7% with highest ratio of 45% in newborn infants. In positive blood culture group, toxic granules were noted in 32.8%, shift to left in 25.9%, hrombocytopenia in 18.9% and positive buffy coat smear in 15.5%. In summary, thrombocytopenia, toxic granules, shift to left were quite helpful in early diagnosis of sepsis. In addition to diagnostic value, examination of buffy coat smear could aid physician to select appropriate antibiotic regimen especially in sepsis of newborn period.

8.Expression of Metallothionein-1, 2 in Folliculostellate Cells of Rat Anterior Pituitary Gland.

Metallothionein (MT) family, intracellular zinc binding proteins, has been suggested to play an important regulatory role in zinc metabolism. The present study utilized light and confocal microscopic methods to investigate the expression pattern of MT-1, 2 and to determine the distribution and extent of colocalization of MT-1, 2 relative to subpopulations of S100 positive folliculostellate (FS) cells and somatotrophs in normal rat anterior pituitary. By light microscopy, MT-1, 2 immunoreactivity was observed both in pars intermedia and pars distalis of anterior pituitary. Confocal microscopy revealed that most MT immunoreactivity was colocalized in S100 positive FS cells, not in somatotrophs. This is the first report that FS cells in pars distalis contain MT-1, 2. These results suggest that MT family may be involved in regulation of hypophyseal endocrine functions and can be used as new markers of FS cells.

9.Job Description of the Nurses Who Work in Operating Room Using DACUM Technique.

PURPOSE: Developing a curriculum(DACUM) is a method of analyzing job focused competency, which is obtained from the data of an expert belonging to a certain career. In this study the DACUM method was used to analyze the jobs of operating room nurses. METHOD: Through the DACUM workshop which was arranged by two DACUM facillitators, a definition of the role of operating room nurses was developed and then duties and tasks of operating room nurses' were identified. For the workshop, a DACUM committee with 10 operating room nurses was organized. Finally, the duties and tasks which were identified were validated by 422 nurses for importance, difficulty, and frequency. RESULTS: Thirteen duties and 105 tasks were identified on the DACUM chart, where importance, difficulty, and frequency of tasks were represented by alphabet letters A, B, and C as higher degree of importance. The determinant coefficient(DC) showed that the most important duty was assisting with operations(DC=6.61), and the least, managing operating materials(DC=4.22). For tasks, the most important ones were assisting in orthostatic surgery(DC=7.60), and assisting in thoracic surgery(DC=7.38), and the least important making gauze ball(DC=2.39), and saving of operation site((DC=3.27). CONCLUSION: The results suggest a need to develop an education program using the DACUM chart as a basis for the development and as a clinical career ladder and for curriculum of operating room nursing.

10.Phenomenological Study on Burnout Experience of Clinical Nurses Who have Turnover Intention.

PURPOSE: This study was aimed to understand the meaning and essentials of the experience of burnout for hospital nurses with turnover intention. METHODS: The design was a qualitative research of phenomenological study. Participants: Seven hospital nurses who had worked over three years and had experiences of turnover intention in a hospital with over 400 beds were included. RESULTS: Nine meaningful themes related to burnout experiences and four theme clusters of 1) battery warning sounds almost out; 2) the player who hit the drum and double-headed drum; 3) the target flying arrow without a break; and 4) the pendulum swaying to turn over. Registered nurses (RNs) felt burnout with an overload of work and by the thought that it was illegal action for registered nurses to receive insufficient rewards for their work. RNs also experienced there were no problem solving strategies to verbal violence by patient and medical team. CONCLUSION: The findings show that burnout experiences for those who had turnover intention was developed from the insight that insufficient training to do work independently with over-load for nurses was not ethical. It suggests that it is necessary to rethink training systems for nursing and hospitals to relieve turnover intention.
