1.Adenocarcinoma Arising in Sacrococcygeal Teratoma: A case report.

We experienced a case of adenocarcinoma arising in sacrococcygeal teratoma. The patient was a 52-year-old woman. She was admitted due to one month of sacral pain. She had a sacral mass since birth. On physical examination, anal fistula was present at the perianal area and pus drainage was noted. MR image showed multiple variable-sized cysts with inhomogeneous density. Resected specimen, mesuring 12.5 7.0 cm in diameter, showed multiple variable-sized cystic lesions admixed with grayish solid portion. The cysts contained mucoid material. The microscopic examination showed mature teratoma composed of cysts lined by pseudostratified ciliated columnar epithelium, intestinal mucosa, mature cartilage, bone, and fat tissue. A moderately differentiated adenocarcinoma developed from the cystic area in the mass.

2.Validity and reproducibility of a food frequency questionnaire to assess dietary nutrients for prevention and management of metabolic syndrome in Korea.

Little data exists on metabolic syndrome (MetS) related with intake, especially for the South Korean. The purpose of this study was to develop and evaluate a food frequency questionnaire (FFQ) for nutritional assessment in the population with MetS in South Korea. Randomly selected female participants, mean age 21.9 years (n = 38) were invited to answer the FFQ twice (FFQ1 and FFQ2) over a nine-month interval and to complete twelve-day diet records (DR) during the months between in South Korea. The correlation coefficients for nutrient intake between FFQ1 and FFQ2 varied from 0.253 (niacin) to 0.573 (cholesterol), and the energy intake-adjusted correlation coefficients ranged from 0.187 for protein to 0.662 for iron. The energy intake-adjusted and de-attenuated correlation coefficients for comparison of FFQ1 and the DRs ranged between 0.277 (vitamin B1) and 0.768 (fiber), and between 0.229 (zinc) and 0.859 (fat) for comparison of DRs with FFQ2. The percentages of study subjects classified into the same quartiles in both the DRs and FFQ1 ranged from 15.8% (vitamin B6) to 47.4% (calcium), and for the same quartiles in DRs and FFQ2 from 13.2% (vitamin B1) to 44.7% (potassium). The FFQ has reasonably good validity and reproducibility. Further research is needed for an assessment of reproducibility and validation of present FFQ in the subjects with MetS.

3.Issues pertaining to Mg, Zn and Cu in the 2020 Dietary Reference Intakes for Koreans

In the current years, it has now become necessary to establish standards for micronutrient intake based on scientific evidence. This review discusses issues related to the development of the 2020 Dietary Reference Intakes for Koreans (KDRI) for magnesium (Mg), zinc (Zn), and copper (Cu), and future research directions. Following issues were encountered when establishing the KDRI for these minerals. First, characteristics of Korean subjects need to be applied to estimate nutrient requirements. When calculating the estimated average requirement (EAR), the KDRI used the results of balance studies for Mg absorption and factorial analysis for Zn, which is defined as the minimum amount to offset endogenous losses for Zn and Mg. For Cu, a combination of indicators, such as depletion/repletion studies, were applied, wherein all reference values were based on data obtained from other countries. Second, there was a limitation in that it was difficult to determine whether reference values of Mg, Zn, and Cu intakes in the 2020 KDRI were achievable. This might be due to the lack of representative previous studies on intakes of these nutrients, and an insufficient database for Mg, Zn, and Cu contents in foods. This lack of database for mineral content in food poses a problem when evaluating the appropriateness of intake. Third, data was insufficient to assess the adequacy of Mg, Zn, and Cu intakes from supplements when calculating reference values, considering the rise in both demand and intake of mineral supplements. Mg is more likely to be consumed as a multi-nutrient supplement in combination with other minerals than as a single supplement. Moreover, Zn-Cu interactions in the body need to be considered when determining the reference intake values of Zn and Cu. It is recommended to discuss these issues present in the 2020 KDRI development for Mg, Zn, and Cu intakes in a systematic way, and to find relevant solutions.

Cholesteatoma of the urinary tract is an extremely rare disease occurring in the bladder and renal pelvis. We report the radiologic findings in two cases of renal cholesteatoma involving the renal pelvis, calyces and ureter, and correlate these with the histopathologic findings.

6.Efficacy of Acyclovir on Replication in Infected Tissues and Virus Reactivation from Explanted Tissues in Mouse Encephalitis Model of Herpes Simplex Virus Type 1.

To investigate viral pathogenesis and in vivo efficacy of acyclovir (ACV) in mouse HSV-1 encephalitis models, female BALB/c mice aged 5 weeks were inoculated with strain F either intranasally (IN) or intracerebrally (IC). ACV-treatment by intraperitorneal injection with 0, 5, 10 and 25 mg/kg b.i.d. for 6days was commenced 1 h after infection. Body weight and signs of clinical disease were noted daily up to 2 weeks. ED50 of ACV in IN infection was 5mg/kg and 14.1 mg/kg in IC infection. Tissues of cental nervous system were collected from 2 mice per group everyday up to 5 days p.i. and the virus titers were measured. In IN infection model, high titers in eyes and trigeminal nerves were observed. ACV-treatment showed significant reduction of the titers in all the isolated. In IC infection model, cerebrum, cerebellum and brain stem showed high virus titers. ACV-treatment showed less significant reduction of virus titers than that in IN infection model. Reactivation of explanted trigeminal nerves from mice 30 day p.i. was monitored. In all of ACV treated mice reactivation was observed, i.e. even the highest dose of ACV did not inhibit the establishment of viral latency.

7.CT and MR Findings of Genitourinary Tuberculosis.

Genitourinary tuberculosis is a disease spread hematogenously from a small tuberculous abscess of the lung. The renal cortex is initially involved, and multiple granulomas form. Ultimately the cortex may cavitate and communicate with the collecting system, allowing downward extension of the infection and subsequent focal caliectasis with infundibular stenosis, ureteral fibrosis, and calcifications of urinary tract organs. The female genitourinary organ is also infected by the hematogenous spread of tuberculosis, the most common lesion be-ing hydrosalpynx with salpingitis. The clinical and radiologic features of genitourinary tuberculosis may mimic other acute abdominal diseases, and the diagnosis of tuberculosis remains difficult. This report describes the ways in which computed tomography and magnetic resonance imaging are valuable aids in the recognition and diagnosis of genitourinary tuberculosis.

8.A Study on Mean Blood Lead and ZPP Levels of Children around Cheonan-city, Chungcheongnam-do.

PURPOSE: Recent years have witness a dramatic increase in public awareness of the health risks associated with lead exposure-particularly concerning young children. The lead poisoning in children was considered a rarity, usually resulting from unique circumstances such as in appropriate use of lead-based body cosmetics or direct administration of lead-containing folk medicine or contaminated atmosphere. The increasing concern about children with lower levels of lead exposure has developed, but there are no data regarding the mean blood lead levels and the incidence of lead poisoning in Korean children. So, this study was conducted to measure the blood lead and ZPP(zinc protoporphyrin) levels of 125 children around Cheonan city. METHODS: 125 children around Cheonan city (from 1 month to 15 year-old, 68 males and 57 females) were studied. We analyzed the mean blood Zinc Protoporphyrin(ZPP) and lead values and excluded children with hematologic disorders in this study. RESULTS: 1) The mean blood lead level was 4.15 microgram/dl (95% CI: 3.82-4.488) and the mean ZPP level was 53.48 microgram/dl (95% CI: 48.44-58.52). Both were within normal range. 2) The mean blood lead level of 68 males was 4.23 microgram/dl and 4.06 microgram/dl in 57 females. The mean blood ZPP level was 49.88 microgram/dl in males and 57.77 microgram/dl in females. There was no significant difference in males and females. 3) There was no siginificant correlation between blood lead and ZPP concentration. CONCLUSIONS: We concluded that the mean blood lead level of 125 children around Cheonan city was 4.15 microgram/dl. That was within normal range and the same values with United States. It is important that there must be a national counter plan and that pediatricians continue to pay attention to lead poisoning in children.

9.Immediate provisionalization using one-piece narrow diameter implants for restoration of edentulous narrow spaces: Case reports.

The aim of this case study was to report the clinical outcome of immediate provisionalization using one-piece narrow diameter (3.0mm) implants in missing maxillary lateral or mandibular incisors. The present study included 36 patients who were treated with 62 one-piece narrow diameter implants. After implant placement, immediate provisional restorations were delivered. All implants showed favorable osseointegration and after progressive loading from 3 months to 9 months (average of 5 months), final restorations were completed without failure in all cases. A survival rate of 100 % (62 of 62) was observed up to 23 months of observation (average of 12.6 months).

10.Oscillometric Blood Pressure of Upper and Lower Extremities and Aortic Blood Flow Velocity in Neonates.

PURPOSE: We studied the blood pressure difference between the upper and lower extremities in healthy newborn infants and the effect of isthmic narrowing of the aorta on making a possible difference. METHODS: The blood pressure was measured with an oscillometric blood pressure device from four extremities of 62 healthy infants aged 1-15 days. A Doppler echocardiography was performed for each infant to measure the aortic blood flow velocities in the ascending, just above and below the aortic arch isthmic narrowing. RESULTS: The blood pressure readings were as follow : right arm 69.7 (+/-7.5)/42.7(+/-7.2)mmHg, right calf 69.5(+/-6.1)/42.6(+/-6.3)mmHg, left arm 69.0(+/-7.3)/43.1(+/-6.3)mmHg, and left calf 68.9(+/-7.4)/42.9(+/-5.6)mmHg. The estimated pressure gradient between the ascending aorta and aorta below the isthmus was 2.4+/-1.3mmHg and between opposite sides of the isthmus was 1.2+/-0.8 mmHg. CONCLUSION: Unlike in childhood and adolescence, the blood pressure in the lower extremities of healthy newborn infants is not higher than in the upper extremities. But the isthmic narrowing of the aortic arch does not explain this phenomenon.
