PURPOSE: Hepatitis B virus infection is prevalent in Korea. Active immunization of Hepatitis B has been proceeded extensively for the last decade. This study was performed to estimate the negative rate of anti-HBs after immunization and to evaluate the effect of revaccination among Korean children between 1995 and 1997, as well as to determine a proper revaccination schedule. METHODS: Anti-HBs was measured with enzyme immunoassay. Hepatitis B revaccination was performed on nonresponders, those who were previously vaccinated and whose anti-HBs titers were under 10mIU/mL. Group A was revaccinated one dose of the same amount of the previous vaccine. Group B was given three doses of the same amount of the vaccine. Group C was given one double dose of the vaccine. The seroconversion rate and geometrical mean titer were measured one month later. RESULTS: Negative anti-HBs rate was 14.5% in children who had been vaccinated. The seroconversion rate was high in all revaccination groups(86.7-94.7%). Three doses of revaccination had no significant difference from one dose of revaccination. There was no positive effect of double dose of revaccination in comparison with the usual dose. CONCLUSION: In our study, one dose of hepatitis B vaccine is as effective as three doses or double dose of hepatitis B vaccine for the nonresponsive children. We recommend administering one dose of hepatitis B vaccine to those children whose three anti-HBs titer is under 10mIU/mL and examining of the anti-HBs titer one month later after revaccination.

The causes of hypocalcemia in patients suffering from severe infection including sepsis are largely uncertain. So we measured serum albumin, total protein, cholesterol, PTH, and calcitonin of the infection-associated hypocalcemic children and compared with those of normocalcemic children suffering from tsutsugamushi disease. All had normal renal functions and had been admitted to the department of pediatrics of Gyeongsang National University Hospital. Hypocalcemic patients were hypoalbuminemic. Serum total calcium was inappropriately more decreased compared to the decreased amount of serum albumin in the hypocalcemic group. We also observed more frequent incidence of hypocholesterolemia (<100mg/dL) in this group. And serum PTH was appropriately elevated in the hypocalcemic patients. Serum calcitonin was elevated in both groups, but 6 times higher in the hypocalcemic group than in the normocalcemic one. Serum total calcium was positively correlated with serum albumin and negatively correlated with serum PTH and calcitonin.

PURPOSE: Abdominal CT scans in patients with intraperitoneal fluid were retrospectively studied to identify characteristic features useful for differential diagnosis of various causes. MATERIALS AND METHODS: One hundred and seventy patients with intraperitoneal fluid collection were classified as categories of hepatic disease, carcinomatosis, and infectious disease. We analyzed sites of fluid collection, the presence of peritoneal thickening, omental and mesenteric fat infiltration, and lymph node enlargment. RESULTS: Intraperitoneal fluid was present in subhepatic space, subphrenic space, paracolic gutter, mesentery, and fossa of the gallbladder in decreasing order of frequency. Fluid in the gallbladder fossa was the most frequent in hepatic diseases. The fluid collection in subhepatic and subphrenic space was less frequent in infectious diseases. Peritoneal thickening was noted in infectious diseases, and carcinomatosis. Omental fat infiltration and enlarged lymph nodes were the most frequent in carcinomatosis (58% and 44%, respectively), whereas, mesenteric fat infiltration and enlarged lymph nodes were the most common in infectious diseases (61%, and 26%, respectively). CONCLUSION: The location of peritoneal fluid collection showed some lesion specific characteristics, and CT features of fat infiltration and enlarged lymph nodes of peritoneum, omentum, and mesentery were helpful for differential diagnosis between carcinomatosis and infectious diseases.

PURPOSE: The purpose of this study was to investigate the status and characteristics of visiting nurses' management for elders with cognitive impairment living in a community focused on health centers in a metropolitan city and five medium-sized cities. METHODS: Data were collected from 47 visiting nurses working in a metropolitan city and 47 visiting nurses working in five medium-sized cities from November to December 2012. RESULTS: There were no statistically significant differences in knowledge, attitudes, and nursing behaviors of cognitive impairment between two groups (F=2.13, p=.148; F=3.64, p=.060; F=0.28, p=.595). Among the elders referred to a physician in a metropolitan city by visiting nurses, 42.4% were diagnosed as mild cognitive impairment and 15.2% were diagnosed as severe dementia. The major intervention programs which visiting nurses currently applied for elders were medication and exercise intervention programs, and the intervention programs which they would want to apply in the future were playing, music and recall intervention programs. CONCLUSION: The cognitive impairment screening test can be done effectively by visiting nurses. This study also suggests to develop various kinds of intervention programs to improve cognitive function for elders living in a community.

PURPOSE: The purpose of this study was to identify the effects of the program to promote drinking knowledge, attitudes, and coping behavior in preschool children. METHODS: A quasi-experimental with non-equivalent control group pretest-posttest design was conducted. The participants were 123 five year old children from G city. They were assigned to the experimental group (n=77) or the control group (n=46). A pretest and posttest were conducted to measure main variables. For the experimental group, the drinking prevention program was given for 6 weeks. Data were analyzed using χ2-test, t-test, and ANCOVA. RESULTS: After the intervention, preschool children in the experimental group reported significant differences in drinking knowledge (F=9.25, p=.003), drinking attitudes (F=19.57, p<.001), and coping behavior (F=16.38, p<.001) compared to preschool children in the control group. CONCLUSION: The results of this study indicate that a drinking prevention program for preschool children is effective in increasing drinking knowledge, attitudes, and coping behavior. This drinking prevention program is recommended as an effective intervention for preschool age children to postpone an early introduction to drinking.
