2.Homeostasis of complement system and the severity of coronary artery disease

Objective To investigate if the relative ratio between C1q and C3a, C5a had a relationship with the extent of coronary artery disease ( CAD) which had never been evaluated in humans.Methods Fifty-three patients scheduled for elective percutaneous coronary intervention ( PCI ) from February, 2016 to April, 2016 at Fuwai hospital were prospectively enrolled.According to the clinical and angiographic characters patients were divided into two groups:acute coronary syndrome ( ACS) group ( n=24), and control group (n=29, 19 patients with stable angina and 10 patients without CAD confirmed by angiography).In all individuals, fasting venous blood was collected by EDTA tubes after admission and strictly before PCI.The plasma level of C1q was measured by immune turbidimetric analysis, C3a and C5a were measured by ELISA tests.Differences between groups were assessed using t test, Mann-Whitney Utests, chi-squared test or Fisher exact test depending on the type of data respectively.Multivariate logistic regression analyses were conducted to evaluate the adjusted effect of C1q, C3a, C5a, C1q/C3a and C1q/C5a on ACS.Results Compared with control group, ACS group has an elevated circulation level of C3a (4 531.14 μg/L vs.4 179.95 μg/L, t=1.381,P=0.173) and C5a (6.44 μg/L vs.4.42 μg/L, t=0.133, P=0.108) but a decreased level of C1q (176.98 μg/ml vs.200.60 μg/ml, t=-2.022, P=0.048).The relative ratio of C1q/C3a was significantly decreased in ACS patients(4.05 ×10 -2 vs.4.97 × 10 -2 , t=-2.484, P=0.016).According to the multiple logistic regression analysis, lower relative ratio of C1q/C3a level proved to be independently associated with ACS ( OR=0.937, P=0.047, 95% CI:0.879-0.998).Conclusions The decreased relative ratio of C1q/C3a level proved to be independently associated with ACS.C1q/C3a ratio could be used as an important index reflecting the complement system homeostasis status which might have potential clinical value in evaluating the prognosis of patients with CAD.

Postoperative gastrointestinal dysfunction (PGD) is one of the most common complications following major surgeries under general anesthesia (GA). Despite ongoing research and new drug treatments, abdominal distension within 24 h postoperatively occurs in 8%-28% of all surgeries. We aim to analyze the effectiveness of preventing PGD by preoperatively stimulating Neiguan (PC6), Zusanli (ST36) and Shangjuxu (ST37) bilaterally twice a day compared with sham-acupuncture treatment and standard treatment.
4.Clinical observation of preventing and treating constipation by electroacupuncture intervention in ischemic stroke patients at acute stage.

OBJECTIVETo observe the efficacy of preventing constipation by electroacupuncture intervention in ischemic stroke patients at acute stage.
METHODSThree hundred and forty ischemic stroke patients who matched the inclusive criteria were randomly divided into an electroacupuncture (EA) group and a routine treatment group, 170 cases in each one. In the EA group, based on routine western treatment, rehabilitation training and common acupuncture, Tianshu (ST 25) and Zusanli (ST 36) were selected and connected to SDZ-V Hwato brand electric acupuncture apparatus respectively, given stimulation of dense-disperse wave (10 Hz/50 Hz) with electric current of 0.1 to 1.0 mA, once every day and 5 times a week in the first 2 weeks, 2 times a week in the latter 2 weeks. In the routine treatment group, only basic treatment was applied. In the two groups, the course of treatment was 4 weeks and 8 weeks' follow-up was required after treatment. The actual numbers who independently defecated less than 3 times a week after 1 to 4 weeks' treatment and in the 4th and the 8th weeks' follow-up and condition taking laxative medicines during the 12 weeks in the two groups were observed and compared.
RESULTSCompared on the numbers of constipation cases after 1 to 4 weeks' treatment, those in the EA group were all less than those in the routine treatment group [24.7% (42/170) vs. 36.5%(62/170), 18.8% (32/170) vs. 30.6% (52/170), 10.5% (17/162) vs. 27.5% (44/160), 9.3% (15/162) vs. 19.4% (31/160),P<0.05, P<0.001]. In the 4th and the 8th weeks' follow-up, numbers in the EA group were still less than those in the routine treatment group [9.0% (14/156) vs. 18.1% (28/155), 7.7% (12/155) vs. 16.9 (26/154) both P<0.05)]. During the 12 weeks, the number of cases who took laxative medicines in the EA group was less than that in the routine treatment group [21.2% (36/170) vs. 34.7% (59/170), P<0.05].
CONCLUSIONThere is clinical significance to electroacupuncture at Tian-shui (ST 25) and Zusanli (ST 36) coibined with basic comprehensive treatment for preventing and treating constipation symptom in ischemic stroke acute stage.

5.Effect of acupuncture and moxibustion on depressive states of stroke patients' spouses.

OBJECTIVETo verify the clinical efficacy of acupuncture and moxibustion on depressive states of stroke patients' spouses.
METHODSForty-four subjects who were stroke patients' spouses and according with inclusive criteria with mild or moderate depressive states were randomly divided into an acupuncture-moxibustion group and a blank control group, 22 cases in each group. In the acupuncture-moxibustion group, acupuncture was applied at Baihui (GV 20), Shenting (GV 24), Sishencong (EX-HN 1), Hegu (LI 4), Zusanli (ST 36) and Taichong (LR 3), and suspended moxibustion was used at Shenque (CV 8), Guanyuan (CV 4) and Zhongwan (CV 12). The treatment was given twice a week for continuous 8 weeks. In the blank control group, neither acupuncture nor moxibustion treatment was given, and the patients were only treated with health and psychological guidance. Before treatment and after 8-week treatment, scores of self-rating depression scale (SDS) and numbers of insomnia severity grade were observed.
RESULTSIn the two groups, the scores of SDS were both reduced than those before treatment (both P<0.05), and the decrease in the acupuncture-moxibustion group was more obvious (P<0.05). After treatment, the number of insomnia severity grade in the acupuncture-moxibustion group was improved than that before treatment (P<0.001), and the improvement was evidently superior to that in the blank control group (P<0.05). The numbers of insomnia severity grade of the blank control group before and after treatment had no statistic significance (P>0.05).
CONCLUSIONThe acupuncture and moxibustion intervention plan has clinical treatment significance on the improvement of mild and moderate depressive states for the stroke patients' primary caregivers who are the patients' spouses.

6.Clinical studies on patient-controlled intravenous analgesia for postoperative pain relief in breast cancer patients with radical mastectomy

Objective To investigate the analgesic effect and adverse reactions of patient-controlled intravenous analgesia (PCIA) in breast cancer patients with radical mastectomy.Methods A total of 210 breast cancer patients who underwent radical mastectomy was randomly divided into two groups,experimental (group A) and control (group B) groups (n =105 cases per group).Patients in group A was used PCIA for 48 hours analgesia,while group B weas applied routine intramuscular injections of pethidine.Visual analogue score (VAS) at 4,8,12,24,and 48 hours after operation were recorded.Pulse,respiration,and blood pressure were monitored and side effects e.g.existed skin itching,nausea,vomiting,and respiratory repression were observed.Results The VAS of group A patients on 4,8,12,24,and 48 hours were2.02 ± 1.47,1.73 ± 1.38,1.68 ± 0.91,1.44 ± 0.65,and 1.21 ± 0.61,respectively;and the VAS of group B patients were 6.95 ± 1.96,6.42 ± 1.57,5.63 ± 1.66,4.99 ± 1.62,and 3.72 ± 1.46,respectively.The VAS was significantly lower in group A patients than in group B (P < 0.05).The incidence of skin itching,nausea,vomiting,and respiratory repression was also distinctly decreased in group A than in group B (P <0.05).The overall satisfaction of patients in group A (96.2%) was remarkably higher than in group B (67.6%) (P <0.01).Conclusions Patient-controlled intravenous analgesia pump can more effectively alleviate the degree of pain,reduce the incidence of skin itching,nausea,vomiting and respiratory repression,improve the satisfactory degree for analgesia in breast cancer patients with radical mastectomy compared to traditional intramuscular way.
7.Correlation of anti-stress ability of nuclear power plant operators with physiological and psychological indexes

Objective To explore the correlation of the anti-stress ability of nuclear power plant operators with their heart rate,blood pressure and mental health.Methods A total of 136 male nuclear power plant operators received the continuous performance test (CPT) and mental health test (MHT),while 37 male senior students were used as the control group.Heart rate and blood pressure were recorded before and after the test.The nuclear power plant operators were divided to high score group and low score group by CPT scores.The correlation of the anti-stress ability and blood pressure,heart rate and their mental health was analyzedd.Results The scores of CPT and MHT test with the operators were significantly higher than those of the control group(t =- 1.25,6.25,P < 0.05).After the test,the heart rate,systolic pressure and diastolic pressure of the operators were all significantly lower than those of the control group ( t =- 2.07,3.28,2.74,P < 0.05 ).The CPT score of the high score group was lower in the heart rate,systolic pressure and diastolic pressure than the low score group(t=-3.15, -2.78, -2.54,P <0.05),while the scores of CPT and MHT were better(t =0.63,6.90,P < 0.05).CPT scores were negatively correlated with the increasing range of the heart rate,systolic pressure and diastolic pressure( r =- 0.69,- 0.94,and - 0.62,P < 0.05 ),and positively correlated with the MHT scores ( r =0.54,P < 0.05 ).Conclusions To a certain extent,CPT and MHT test can reflect the anti-stress ability and be used as one of the bases to select nuclear power plant operators and to assess their competence.
8.A preliminary study on related factors of mental health in nuclear power plant operators

Objective To explore the status of nuclear power plant operators in mental health and its correlation with emotional stability,liveliness,anxiety and urgency.Methods 255 male operators were randomly selected from three nuclear power bases,meanwhile 61 undergraduates were used as control group.The mental health and neurobehavioral evaluation system of Chinese nuclear power plant operators was developed by Second Affiliated Hospital of Soochow University,which was used to assess mental health of the subjects.The scores of mental health personality factors were recorded,together with four main personality factors including emotional stability,liveliness,anxiety and urgency.Results The score of lie was lower than 8 which showed all inspected groups were normal. 1.57% (4/255) operators had psychological disorders,3.92% ( 10/255 ) had poor mental health,27.84% ( 71/255 ) had general mental health,66.7% (170/255) had excellent mental health,whereas 9.84% (6/61)for control group had psychological disorders.Obvious difference was observed in the final scores between the nuclear power plant operators and control group.The former gained higher scores on mental health,emotional stability,and lower scores on anxiety and urgency ( t =3.437,4.423,- 2.493,- 2.093,P < 0.05 ).Both groups aged over 27 years and with length of service over 5 years were awarded higher scores on mental health,emotional stability( t =2.585,2.349 ; t =2.606,2.947,P < 0.05 ),lower scores on anxiety and urgency (t =- 3.407,- 2.138 ; t =- 2.941,- 2.256,P < 0.05).The mental health was positively correlated with emotional stability and liveliness ( r =0.721,0.650,P < 0.05),but negatively correlated with anxiety and urgency(r =- 0.809,- 0.693,P < 0.05 ).Conclusions The majority of nuclear power plant operators had excellent psychological quality,but some factors should be paid more attention,such as different ages and length of service time.
9.Preliminary study on psychosomatic status of nuclear power plant operators

Objective To understand the operators'psychosomatic health status in nuclear power plant;and provide the scientific basis of measures for preventing and reducing mental disorders in operators.Methods The Psychosomatic Health Battery(PSHB) was used to assess the psychosomatic health status in 109 operators who were random selected from Qinshan nuclear power plant,etc.They were tested from lie,emotional stability,liveliness,tension,apprehension,mental health,such as psychopathic deviatesuch 7 personality traits.Results Lie < 8,all inspected groups were normal.Psychopathic deviate:98.2% for normal group 0.9% for both of groups occurred possible mental health problems and confirmed mental health problems ;Mental health:80.7% (88/109) for fine mental health ones,29.4% (32/109) for those with excellent mental health,51.4% (56/109) for good mental health ones,13.8% (15/109) for general mental health ones,5.5% (6/109) for poor mental health ones.Age factor could influence the mean values of the factors of apprehension,tension,mental health and psychopathic deviate.Correlation analysis showed that there was a correlation between tension and psychopathic deviate(r = 0.664 ,P < 0.01),and the other correlation coefficient was between apprehension and mental health(r =-0.789 ,P < 0.01).Conclusions There is an excellent condition of psychosomatic health in most of the operators,however,there are still a very small percentage of psychosomatic disorders among these operators,to improve the quality of their psychosomatic health,psychological counseling should be particularly strengthened to those with problems of psychosomatic health.
10.Evaluation of personality features of nuclear power plant operators:investigation with Minnesota Multiphasic Personality Inventory

Objective To explore the personality features of nuclear power plant operators and the influencing factors thereof.Methods Minnesota Multiphasic Personality Inventory was used to examine the personality features of 136 nuclear power plant operators randomly selected from 2 cooperative units,all males.The results were compared with the nationwide norms and subsequently an inter-block contrast analysis was carried out.Results Obvious difierence was observed in the final scores between the nuclear power plant operators and nationwide norms.The former got higher scores on hysteria(t=3.05,P<0.05).and lower scores on hypochondriasis,depression,morbid personality,masculinity-femininity,paranoia,psychasthenia,schizophrenia,hypomania,and social introversion(t=7.47,7.47,7.3 1,2.23,15.09,16.15,19.28,7.88,11.10,P<0.05).The scores on hypochondriasis,depression,schizophrenia,and social introversion of those with the length of services over 3 years were all significantly higher than those ofwith the length of services less than 3 years(t=3.25,2.51,2.76,3.00,P<0.05).The scores on hypochondriasis,depression,psychop8thicdeviate,and social introversion of the operators aged over 30 were all significantly higher than those of the operators aged below 30(t=2.36,2.35,2.01,2.54,P<0.05).Conclusions The psychological quality of the nuclear power plant operators is superior to that of the general population.
