1.Study on Therapeutic Effect and Mechanism ofQi-Zhu Er-Zhu Er-CaoTang in Treatment of Chronic Atrophic Gastritis Precancerous Lesion Based on Pathway of NF-κB (p65) and CyclinE

This study was aimed to observe the therapeutic effect and mechanism ofQi-Zhu Er-Zhu Er-Cao Tang(QZEZECT) in the treatment of chronic atrophic gastritis (CAG) precancerous lesion. A total of 56 clean grade healthy Wistar rats were randomly divided into 6 groups. In the negative control group (NC), model group,Wei-Fu-Chun(WFC) group,Ren-Zhu Jian-Wei Ke-Li(RZJWKL, RZ) group, there were 10 rats in each group. In the high-dose QZEZECT (QZ-H) group and low-dose QZEZECT (QZ-L) group, there were 8 rats in each group. CAG/PLGC model was established by MNNG in the model group, WFC group, RZ group, QZ-H group and QZ-L group. Intragastric administration of corresponding decoctions at the dose of 0.39 g·kg-1, 3.2 g·kg-1, 54.4 g·kg-1, 13.6 g·kg-1 were given to rats in the WFC, RZ, QZ-H and QZ-L groups once a day for 28 consecutive days, respectively. The same volume of normal saline was given to the NC group and the model group. Histomorphological changes of gastric mucous membrane in rats of each group were observed. Expressions of NF-κB/p65 and CyclinE protein were detected. The results showed that compared with the NC group, the degrees of infiltration and dysplasia and expressions of NF-κB/p65 and CyclinE significantly increased in the model group with statistical significance (P < 0.01). Compared with the model group, the expression of NF-κB/p65 and CyclinE in the WFC group reduced with statistical significance (P < 0.05). The expression of NF-κB/p65 and CyclinE in the RZ and QZ-L group obviously reduced with statistical significance (P < 0.01). The overall effective rates of WFC group, RZ group, QZ-H group and QZ-L group were 50%, 63.3%, 43.3% and 73.3%, respectively. It was concluded that QZEZECT can treat CAG precancerous lesion, which may take effect by improving and inhibiting pathologic changes of the transcription factor of inflammation.
2. Correlation between gestational age and ADC value in normal fetal brain region detected using DWI with b value as 1 000 s/mm2

Objective: To investigate the relationship between ADC value of normal fetal brain region obtained on ADC image generated with DWI of b value as 1000 s/mm2 and gestational age. Methods: A retrospective analysis of 106 pregnant women who underwent routine MR and DWI examinations due to suspected fetal abnormalities with prenatal ultrasonography was performed. Among them, 28 pregnant women were in the second trimester (13-27 weeks) and 78 pregnant women were in the third trimester (≥28 weeks). All fetuses were confirmed as normal neurodevelopment with prenatal routine MR and DWI examinations, as while as postnatal follow-up. DWI was performed with b value of 1000 s/mm2. ADC maps were generated based on DWI,and ADC values of frontal lobe, parietal lobe, temporal lobe, occipital lobe white matters, basal nucleus, thalamus, cerebellar hemisphere and pons were measured. The correlation between ADC values in different fetal brain regions and gestational age was analyzed using linear regression. Results: ADC values of different parts of normal fetal brain were in order from high to low: Parietal white matter ([1.84±0.08]mm2/s), occipital white matter ([1.82±0.08]mm2/s), frontal white matter ([1.77±0.03]mm2/s), temporal white matter ([1.70±0.13]mm2/s), cerebellar hemisphere ([1.41±0.10]mm2/s), basal ganglia ([1.40±0.13]mm2/s), thalamus ([1.35±0.04]mm2/s), pons ([1.27±0.08]mm2/s). Except for frontal white matter, ADC values of the rest of the normal fetal brain regions were negatively correlated with gestational age (all P<0.05). Conclusion: Most of ADC values in normal fetal brain different regions decreased with increasing gestational age, which can reflect the developmental process of fetal brain.
3.Formula Optimization and Quality Control of Amlexanox Nasal Thermosensitive Gel Spray

Objective:To optimize the formula of amlexanox nasal thermosensitive gel spray and establish the quality control meth-od. Methods:Amlexanox nasal thermosensitive gel spray was prepared by a cold dissolving method, and poloxamer 407 (P407) and poloxamer 188 (P188) were used as the carrier materials. Central composite design-response surface methodology was used to optimize the formula with the amount of P407 and P188 as the influencing factors and the gel temperature and the viscosity before gelling as the indices. The content of amlexanox was determined by HPLC. According to Chinese Pharmacopoeia (2010 edition), the other indices of the preparation such as appearance, pH, viscosity, content, total spray times of each bottle and the content of each spray were deter-mined as well. Results:The optimum ratio of P407 and P188 was 17. 0% and 0. 9%,respectively . The average recovery of amlexanox was 98. 8% and RSD was 1. 7%(n=9). The quality of 3 batches of amlexanox nasal thermosensitive gel spray met the related require-ments. Conclusion:The formula and preparation process of amlexanox nasal thermosensitive gel spray are reasonable and feasible with controllable quality, which is worthy of further research.
4.Formula Optimization of Ropivacaine Hydrochloride Transdermal Gel

Objective:To optimize the formula of ropivacaine hydrochloride transdermal gel. Methods:The steady transdermal rate and cumulative transdermal percentage in 24 h of ropivacaine hydrochloride gel were used as the indices, an orthogonal design was applied to select the optimal formula, and Design Expert 8. 0. 5. 0 software was used to analyze the results. Results: The optimal formula con-tained 2% carbomer, 10% propylene-glycol and 5% Azone. The steady transdermal rate of the optimal formula was 0. 6951 mg·h-1 · cm-2 . The cumulative transdermal percentage in 24 h of the optimal formula was 91. 04%, which was 22. 79% higher than that of ropiva-caine hydrochloride solution with the same concentration. Design Expert 8. 0. 5. 0 software could predict the steady transdermal rate and cumulative transdermal percentage in 24 h of the optimal formula. Conclusion: The preparation design is reasonable, and the gel has promising properties, which is suitable for skin local application.
5.Analysis of detection of prenatal coagulation four indices,D-dimer and platelet in pregnant women

Objective To investigate the clinical significance of the antenatal detection of four indices of coagulation(PT,APTT, TT,FIB),D-dimer(DD)and platelet(PLT)in the judgment of parturient coagulation function and bleeding risk.Methods 795 pregnant women and childbearing age non-pregnant women with physical examination in this hospital from June 2012 to September 2013 were collected and divided into the normal pregnancy group(early pregnancy,middle pregnancy,late pregnancy,parturient pe-riod),obstetric accident group,pregnancy complicating other disease group and healthy control group.At the same time,PT, APTT,TT,FIB,DD and PLT were detected and the related statistical analysis was conducted.Results DD and FIB in the normal pregnancy group were significantly higher than those in the healthy control group(P<0.05),PT and APTT were significantly low-er than those in the healthy control group(P <0.05),TT had no statistically significant difference between these two groups(P >0.05 );DD and FIB in the obstetric accident group and the pregnancy complicating other disease group were significantly higher than those in the normal pregnancy group(P <0.05),while PT was significantly lower than that in the normal pregnancy group(P <0.05).PLT in the obstetric accident group and the pregnancy complicating other disease group was significantly lower than that in the normal pregnancy group(P <0.05).The sensitivity and specificity of the combined detection of 3 indices in diagnosing the ob-stetrical accidents were greatly superior to those of the single index detection or the combined detection of two indices.Conclusion The prenatal combination detection of four coagulation indices,DD and PLT can preliminarily judge the coagulation status of preg-nancy period in pregnant women,help to treat abnormal bleeding of pregnant women an possess the important guidance significance in formulating the labor scheme and preventing the occurrence of obstetric accidents.
6.Effects of Three Herb Extracts on Cell Proliferation and Collagen Synthesis in Cultured Human Dermal Fi-broblasts

Objective:To investigate the effects of three herb extracts on cell proliferation and collagen synthesis in cultured hu-man dermal fibroblasts. Methods:The human dermal fibroblasts were isolated and cultured with tissue explant culture technique. Im-munohistochemical staining was performed to observe vimentin in fibroblasts. The proliferation of fibroblasts was determined by MTT as-say. Hydroxyproline in the cultural medium was determined by a digestion method. Results:The cells were typical fibroblasts, and the immunohistochemistry staining of vimentin in the cells was positive. The three herb extracts could promote the proliferation of the fibro-blasts at appropriate concentrations in a time-dependent manner. After the 72-hour culture in the medium, 0. 312 5-5. 000 0 mg·ml-1 Aloe vera gel extract, 0. 156 3-2. 500 0 mg·ml-1 Bletilla striata extract and 0. 075 0 mg·ml-1 salvianolate showed significant effects on the proliferation of fibroblasts (P<0. 05 or P<0. 01 vs the blank group). Compared with that in the blank group, the amount of hydroxyproline in the three herb extracts cultural medium was also increased (P<0. 05 or P<0. 01). The effect of salvianolate on the cell proliferation was closely related with time and concentration, and the high concentration group could inhibit the growth of fibroblasts as the extension of time. Conclusion:Aloe vera gel extract, bletilla striata extract and salvianolate can promote the proliferation of fi-broblasts and the production of hydroxyproline, which may contribute to the wound healing.
7.Effect of different pressure CO2 pneumoperitoneum on early postoperative cognitive function in female ;patients undergoing gynecological laparoscopic surgery

Objective To investigate the effect of different pressure CO 2 pneumoperitoneum on early postoperative cognitive function in female patients undergoing gynecological laparoscopic sur-gery.Methods Ninety female patients,aged 40-60 years,ASA physical status Ⅰor Ⅱ,scheduled for elective gynecological laparoscopic surgery,were randomly divided into three groups (n = 30). The pressure of CO 2 pneumoperitoneum were set at 6-8,9-1 1 and 12-14 mm Hg in groups L,M and H,respectively.All of the patients were tested by the neuropsychology and questionnaire review to estimate whether the patient got cognitive decline at 24 h before the operation.The venous blood sam-ples 10 minutes before anesthesia (T1 ),at the end of surgery (T2 ),6 hours after surgery (T3 ),24 hours after surgery (T4 )and 72 hours after surgery (T5 )were collected for determination of serum concentrations of NSE and S100βprotein.The pH,PaCO 2 and PaO 2 were recorded before anesthesia (Ta ),before pneumoperitoneum (Tb ),1 hour after pneumoperitoneum (Tc ),2 hours after pneumo-peritoneum (Td )and 1 hour after stopping pneumoperitoneum (Te ).Results Scores of these tests in three groups were not different and there was no patient with cognitive decline after surgery.Com-pared with group H,the concentration of NSE at T2 and T3 was significantly lower in groups L and M (P <0.05).Compared with group H,the concentration of S100βprotein at T2 was significantly lower in groups L and M (P <0.05).Compared with group L,pH at Tc and Td was significantly decreased in groups M and H (P <0.05).Compared with group L,PaCO 2 was significantly increased at Tc-Te in groups M and H (P <0.05).Conclusion Different pressure of CO 2 pneumoperitoneum has no ob-vious effect on the early cognitive function,but low (6-8 mm Hg)CO 2 pneumoperitoneum can reduce the release of NSE and S100βprotein after operation.
8.Vertical Transmission of Hepatitis B Virus in Human Early Embryos

Objective To detect the expression of HBV in human early embryos of infertility patients combined with hepatitis B virus (HBV) infection after undergoing in vitro fertilization and embryo transfer (IVF-ET) treatment,so as to provide reliable evidence for the vertical transmission of HBV through human germ cells.Methods We collected the abandoned early embryos of patients combined with HBV infection after undergoing IVF-ET treatment in the department of reproductive medicine of the First People's Hospital of Yunnan Province,total genomic DNA was obtained by single cell PCR,and then polymerase chain reaction (PCR) was used to detect the expression of HBV-DNA and covalently closed circular DNA of hepatitis B virus (HBV-cccDNA) in early embryos.Results Both HBV-DNA and HBV-cccDNA were detected in the early embryo among the mother or father or parents HBV infected patients.The positive detection rate of HBV-DNA in mother,father and parents HBV infected patients was 78.9%,76.3% and 100%,and the positive rate of HBV-cccDNA was 63.2%,78.9% and 100%.The difference between the mother and the father was not significant (P>O.05).However,the positive rate in parents HBV infected patients group was significantly higher than the mother or father group,showing a significant difference (P<0.05).Conclusion HBV can be transmitted from mother or father to early embryos via IVF-ET treament,and the risk of vertical transmission increases significantly when the couple are HBV infected.
9.The compliance of second level prevention of coronary heart disease patients and its nursing countermeasures

Objective To analyze the compliance and nursing countermeasures of second level prevention of patients with coronary heart disease.Methods A total of 70 coronary heart disease patients in our hospital were divided into ideal compliance group (20 cases) and poor compliance group (50 cases), influencing factors of compliance and nursing strategy were analyzed.Results Good compliance rate of coronary heart disease prevention was 28.6%, and poor compliance rate was 71.4%.Level of education, family support, monthly income, the two grade prevention knowledge, adverse drug reactions, drug type, payment of medical expenses, the reexamination condition had statistically significant difference (P<0.05), and the remaining indicators had no significant difference (P>0.05);Logistic regression results showed that the adverse drug reaction, family support, two grade prevention knowledge were independent influencing factors (P<0.05).Conclusion Strengthening education and two level prevention knowledge propaganda can significantly improve the compliance of patients with coronary heart disease, so it is worthy of clinical promotion.
