1.Clinical application of standardized rating scales for attention deficit hyperactivity disorder in school-age children

Attention deficit hyperactivity disorder(ADHD) is one of the most common psychiatric disorders among school-age children.The diagnosis and treatment of ADHD rely on a comprehensive evaluation of symptom,functional impairment and comorbidity.Therefore the standardized rating scales is widely used clinically.The standardized rating scales can be categorized into: Broad-band scales,evaluating the general misbehavior and sociopsychological functions andNarrow-band scales,evaluating ADHD specific symptoms,and also functional impairment rating scales.Useful comorbidity and functional impairment rating scales are in need domestically.This paper reviewed the published literature about the clinical application and progression of frequently-used rating scales for school-age ADHD evaluation,at home and abroad.
2.Analysis of perioperative risk factors for seizures in children with congenital heart disease following cardiopulmonary bypass

Objective:To examine the occurrence of seizures and perioperative risk factors in children after obtaining cardiopulmonary bypass (CPB).Methods:Electroencephalograph (EEG) and cerebral oxygen saturation (ScO 2) were monitored in 87 patients (aged 3 days-110 months, with the median at 4.4 months) and they were admitted to the Cardiac Intensive Care Unit (CICU) from May 2018 to April 2019, with 77 patients for 72 h after CPB and 10 cases with clinical seizures after CICU discharge for 8-180 h. Clinical data were recorded every 4 hours, including blood pressure, central venous pressure (CVP), temperature, arterial blood gases, lactate, doses of Dopamine, Milrinone, and Adrenalin, and daily C-reaction protein (CRP) and N-terminal pro-brain natriuretic peptide(NT-proBNP). Demographic data, the Society of Thoracic Surgeons-European Association for Cardio-Thoracic Surgery(STAT) mortality classification and clinical outcomes (duration of mechanical ventilation, CICU and hospital stay, and death) were recorded. Results:Seizures occurred in 24 patients (27.6%) within 48 h after CPB.EEG seizures appeared in 7 patients, both EEG and clinical seizures in 7 patients, and clinical seizures in 10 patients.Three patients developed epilepsy.There were 2 hospital deaths and both had EEG seizures.Patients with seizures had higher STAT categories, longer operative time and worse clinical outcomes (all P<0.05). Univariate regression revealed that the occurrence of seizures was positively related to adrenalin, temperature, lactate level (all P<0.01), blood pressure, CVP (all P<0.05) and CRP ( P<0.1), and negatively correlated with arterial carbon dioxide tension[ pa(CO 2)], ScO 2 and dose of Milrinone (all P<0.05). In multivariate regression, only dose of Adrenalin ( P<0.001) and lactate level ( P<0.01) remained significant. Conclusions:The incidence of seizures was high in children after CPB.Numerous perioperative factors that were related to seizures were identified.The most significant risk factors were higher dose of adrenaline and lactate level.Studies on neurological injury after CPB are urgently needed in China in order to reduce brain injury and improve neurodevelopmental outcomes.

4.Analysis of dietary quality of preschool left behind children by using the revised Chinese diet balance index

Objective:
To evaluate the dietary quality by adjusted diet balance index (DBI_16) of preschool left behind children in Anhui Province, and to provide a reference for rapidly and accurately evaluating their nutrition condition.
Methods:
During September to December of 2018, selected 306 left behind children and 598 non left behind children aged 3-6 years old of Anhui Province in total. Four scoring methods (TS total score, LBS Low Bound Score, HBS High Bound Score, DQD Diet Quality Distance) were used to evaluate the dietary quality by Diet Balance Index Revision (DBI_16), and scores were compared to reflect the diet quality of preschool children in LBC group and NLBC group.
Results:
The score of TS (-18.2, -16.1) in LBC group was lower than that of NLBC group, the scores of LBS(24.8, 23.1), HBS (7.9, 6.4) and DQD (35.9, 34.4) in LBC group were higher than that of NLBC group( Z =-46.02, 12.45, 4.14, 4.78, P <0.05). The daily intake of vegetables, fruits, animal food, milk, soybean and drinking water were obviously under the RNI, the dietary intake scores of milk(-4.1, -2.7), animal food (-2.2, -0.8) and food species (-7.4, -6.2) in LBC group were higher than that in NLBC group( Z =-26.42, 13.51, -6.59, P <0.01). About 44.1% of the preschool LBC were in moderate or severe deficit of food intake, 66.0% of the LBC were in the higher level of dietary imbalance, the LBC group s excessive and imbalance problem were of significant differences than those in the NLBC group ( χ 2=15.79, 11.51, P <0.05).
Conclusion
The dietary quality of preschool children in Anhui Province should be improved, the main diet problem was the dietary imbalance, which was related to deficiency in nutrients intake. The scores of DBI_16 in LBC group were significant different with those in NLBC group, it is necessary to take specific intervention to increase the intake of milk, eggs and fruits among preschool children.
5.Cloning and expression pattern of erk2 gene in Inner Mongolia Cashmere goat.

The study aims at cloning the CDS fragment of erk2 gene cDNA in Inner Mongolia Cashmere Goat and analyzing its tissue-specific expression, erk2 gene cDNA was cloned by RT-PCR. The nucleotide sequence was analyzed by Blast and amino acid sequence was analyzed by online softwares SMART and Psite. The tissue-specific expression pattern of erk2 was analyzed by quantitative RT-PCR. The expression of erk2 in testis of goat was detected by Immunohistochemistry. The cloned erk2 gene cDNA (GenBank Accession No. JX569765) was 1 083 bp in length, including a complete ORF encoding 360 amino acids residues. The amino acid sequence shares 100% identity with the Bos Taurus ERK2 (Bos Taurus BC133588.1). Analysis by SMART suggests that the encoded protein contained a "TEY" structure and an S-TKc domain possessing serine/threonine kinase catalytic activity. Analysis with Psite indicates one cAMP-/cGMP-dependent protein kinase phosphorylation site, 3 protein kinase C phosphorylation sites, 5 casein kinase II phosphorylation sites, 2 protein kinases ATP-binding region signatures and one serine/threonine protein kinases active-site signature in this protein. Analysis by Psort (k-NN prediction) suggestes that this protein most probably is localized in cytoplasm. The results of quantitative RT-PCR show that the expression of erk2 mRNA was higher in heart, skin and breast, whereas lower in spleen and kidney. ERK2 protein was detected in testis by immunohistochemistry.

6.Research on the accuracy of using student standardized patients in objective structured clinical examination assessment

Objective To discuss the accuracy and objectivity of student standardized patients (SSP) in objective structured clinical examination(OSCE). Methods On March 30 and 31, 2013, 168 seven-year program medical students of class 2006 and 2007 took part in OSCE. Differences in as-sessment results between SPP and those given by professional doctors at 3 SP sites(angina pectoris SSP station, acute cholecystitis SSP station, depression SSP station) were analyzed. Each site had 4 items for assessment, with a total score of 100. Scores were given in strict accordance with a set of unified scoring rules. Counting data were presented as x±s. Data were verified using t test. P<0.05 was considered statis-tically considerable. Results At angina pectoris station, respective scores of SSP and professional physi-cians were 85.2±7.1 and 85.5±6.6, P=0.688. At acute cholecystitis station, respective score of SSP and professional physicians were 89.1±5.2 and 88.2±6.2, P=0.150. At depression station, respec-tive score of SSP and professional physicians were 79.8±7.5 and 78.2±7.0, P=0.078. Conclusion There is no statistical difference between scores given by SSP and those given by the physicians in OSCE. This proves that SSP who received standardized training delivers fair and accurate results in OSCE , and therefore is recommended for future application.
7.Analysisof clinical risk factors of moderate and severe bronchopulmonary dysplasia in premature infants

Objective To explore the high risk factors of different degrees of premature infants with bronchopulmonary dysplasia (BPD) and to provide theoretical basises for the clinical prevention of BPD.Methods The clinical datas of 64 cases patients with BPD who were diagnosed and hospitalized in Neonatology Department of Affiliated Hospital of Qingdao University from June in 2009 to March in 2016 were retrospective analyzed,from several aspects to analyze the BPD's high risk factors,such as the mother's factors,the perinatal factors,treatments after birth and complications.Results There were 27 moderate and mild cases,19 moderate cases and 18 severe cases in children who were diagnosed BPD;different degrees of BPD patients in gender,5 minute's apgar score,the repeated application of PS,long time of mechanical ventilation,high concentration of oxygen inhalation,neonatal pneumonia,blood transfusion and neonatal anemia's differences were significant in the severity of BPD difference (P =0.003,0.033,0.006,0.002,0.001,0.000,0.001,0.001,0.036,0.004).Conclusion Strengthen resuscitation in delivery room,shorten the mechanical ventilation time and reduce the high concentration oxygen inhalation,prevent and reduce the infection after delivery,reduce latrogenic blood loss and the number of blood transfusion are important measures to alleviate the severity of BPD.
8.Expression of DNMT3B gene in hepatocellular carcinoma and its effect on proliferation, invasion and metastasis of hepatoma cells

Objective To investigate the expression of DNA methyltransferase 3b (DNMT3B) in hepatocellular carcinoma (HCC) and its effect and mechanism on the proliferation,invasion and migration of HCC cells.Methods The expression of DNMT3B gene was detected by qRT-PCR in 46 cases of HCC tissues and corresponding adjacent tissues;the results and clinical pathological parameters were analyzed.SiRNA targeting DNMT3B was transfected into MHCC97-H cells by RNA interference (RNAi) technique.The mRNA and protein expression levels of related genes were detected by qRT-PCR and Western blot.The cell proliferation was measured by MTT assay,and the invasion and migration abilities were measured by Transwell assay.Results In 46 HCC patients,the expression of DNMT3B (73.91%) was significantly higher in HCC than in adjacent normal tissue.The high expression of DNMT3B gene was associated with histological type and tumor size of HCC (all P<0.05).Inhibition of DNMT3B gene expression decreased proliferation,invasion and migration of MHCC97-H cells.Interference with DNMT3B gene increased the expressions of tumor suppressor genes RASSFA1,APC and MTSS1 at mRNA and protein levels.Conclusion DNMT3B is associated with the progression of HCC.It may inhibit the proliferation,invasion and migration of HCC cells by regulating the methylation of downstream tumor suppressor gene.
9.Immunological synapse formation inhibits the apoptosis of macrophages in rheumatoid arthritis

Objective To determine whether macrophages can behave as antigen presenting cells participating the formation of immunological synapse in rheumatoid arthritis (RA) and whether this process can affect the apoptosis. Moreover, this study was aimed to observe the function of cyclophilin A (CypA) in immunological synapse formation and its role in regulating the apoptosis of macrophages. Methods human acute monocytic leukemia cell line (THP-1) induced macrophages were coated with staphylococcal enterotoxin B(SEB) (100 ng/ml) and co-cultured with activated Jurkat T cells (human acute T-cell leukemia cell line), then incubated in the RPMI-1640 for 16 hours to induce apoptosis. The apoptosis of the macrophages were analyzed by flow cytometry by Annexin V-PI staining. The macrophages cultured in the RPMI-1640 alone were used as control. Meanwhile, CypA (200 ng/ml) were added to or not added in order to observe the apoptosis of macrophages. The function of CypA and the apoptosis of macrophages isolated from RA peripheral blood were also investigated through co-culture with CD4+T cells isolated by immunomagnetic beads. Comparisons between groups were performed by two-sample t-tests. Results In the peripheral blood of healthy people and RA patients, the apoptosis of macrophages which participated immunological synapse was (32.9±2.8)%, (24.7±1.6)%, (14.5±1.2)% respectively, which was significantly lower than the apoptosis of macrophages cultured alone [ respectively for (61.4±2.4)%, (45.5±2.6)%, (22.9±1.5)%, (P<0.05) ]. After CypA was added, the apoptosis of macrophages in cell lines, healthy people and RA patients decreased to (27.2±2.1)%, (20.1±1.1)%, (12.9±1.0)%, lower than the apoptosis of macrophages which participated immunological synapse formation (P<0.05). Conclusion In RA, the macrophages participate in the formation of immunological synapse by interacting with CD4+ T cells. They can significantly reduce the apoptosis on themselves. CypA can enhance this effect. These results provide a new theoretical foundation for prolonged survival of macrophages in RA, which can secrete a variety of cytokines to enhance inflammation and joint destruction.
10.Clinical significance of the measurement of peripheral blood Epstein-Barr virus load in patients with HBV infection

ObjectiveTo investigate the clinical significance of co-infection with hepatitis B virus (HBV) and Epstein-Barr virus (EBV) in HBV-related liver diseases such as chronic hepatitis B (CHB), liver cirrhosis, and hepatocellular carcinoma (HCC). MethodsA retrospective analysis was performed for the clinical data of 487 patients with HBV infection who were diagnosed in Zhongnan Hospital of Wuhan University from May 2016 to August 2018, among whom 194 (39.8%) had co-infection with HBV and EBV. The patients were divided into groups according to the copy number of EBV DNA (>400 IU/ml), Child-Pugh class (Child-Pugh class A, B, and C), and progression of liver disease (CHB, liver cirrhosis, and HCC), and related indices were compared between groups. The t-test was used for comparison of normally distributed continuous data between two groups; an analysis of variance was used for comparison between multiple groups, and the least significant difference t-test was used for further comparison between two groups. The Kruskal-Wallis H test was used for comparison of non-normally distributed continuous data between groups, and the Dunn-Bonferroni test was used for further comparison between two groups. The chi-square test was used for comparison of categorical data between groups. ResultsThe patients with CHB had a significantly higher copy number of HBV-DNA than those with liver cirrhosis or HCC (t=2.417 and 3.258, P=0.017 and 0.001), while the patients with HCC tended to have a higher copy number of EBV DNA than those with CHB or liver cirrhosis, but there was no significant difference between the three groups (F=1.161, P=0.315). After adjustment for liver function based on Child-Pugh class, the HCC patients with Child-Pugh class A liver function had a significantly higher copy number of EBV DNA than the CHB patients and the patients with liver cirrhosis (t=2.062 and 2.615, P=0.041 and 0.010), the liver cirrhosis patients with Child-Pugh class C liver function had a significantly higher copy number of EBV DNA than the CHB patients (t=2.647,P=0.012). ALT/AST, globulin, and lymphocyte percentage were specific clinical indices for co-infection with HBV and EBV. ConclusionThere is an increase in EBV load in HCC patients, and both EBV and HBV are involved in the progression of liver diseases. Dynamic quantification of EBV DNA in patients with HBV infection has a certain significance in early intervention of the progression of liver diseases.
