ObjectiveTo investigate the effect and mechanism of polysaccharide in water extract of Cyclocarya paliurus (CPWP) in inhibiting benign prostatic hyperplasia (BPH). MethodsCPWP was obtained by heating reflux, aqueous extraction, alcohol precipitation, and freeze drying. The chemical composition and structural properties of CPWP were analyzed by high performance liquid chromatography with 1-pheny-3-methyl-5-pyrazolone pre-column derivatization and infrared spectroscopy. Male SD rats were randomly assigned into control, model, finasteride (ig 5 mg·kg-1), and low-, medium-, and high-dose (ig 50, 75, 100 mg·kg-1) CPWP groups, with 8 rats in each group. The BPH model was established by subcutaneously injecting propionate testosterone in castrated rats. The rats in the drug intervention groups were administrated with corresponding drugs, and those in the control group were administrated with an equal volume of normal saline each day. After 30 consecutive days, the rats were sacrificed, and the prostate tissue was separated and weighed. The effects of drug interventions on the body weight, prostate wet weight, and prostate index of rats were examined. The prostate tissue was stained with hematoxylin-eosin (HE) for observation of pathological changes. Enzyme-linked immunosorbent assay was employed to measure the level of dihydrotestosterone (DHT), and immunohistochemical staining was used to detect the expression of steroid 5 alpha-reductase 2 (SRD5A2) and Ki67 in the prostate tissue. ResultsCPWP was identified as a saccharide, with characteristic absorption peaks of saccharides. CPWP showed the total sugar content of 44.15% and molecular weight within the range of 5.5-78.8 kDa, being composed of mannose, rhamnose, galacturonic acid, glucose, galactose, xylose, and arabinose. Compared with the control group, the model group had significantly increased prostate wet weight and prostate index (P<0.01), thick and tall prostate epithelial cells, increased internal wrinkles, papillary expansion into the cavity, an elevation in DHT level in the serum, and up-regulated expression of SRD5A2 and Ki67 in the prostate tissue (P<0.05, P<0.01). Compared with the model group, both the finasteride and CPWP groups showed decreases in prostate wet weight and prostate index (P<0.05, P<0.01), thinned prostate epithelial cells, with only a small portion of internal wrinkles and papillary expansion into the cavity, shortened papillary protrusions, lowered DHT level in the serum, and down-regulated expression of SRD5A2 and Ki67 in the prostate tissue (P<0.01). Moreover, CPWP exerted effects in a dose-dependent manner. ConclusionCPWP inhibits BPH by regulating the expression of SRD5A2.

Schizophrenia is a serious mental disease. The diagnosis of schizophrenia so far relies heavily on subjective evidence, including self-reported experiences by patients, manifestations described by relatives, and abnormal behaviors assessed by psychiatrists. The diagnosis, monitoring of the disease progression and therapy efficacy assessment are challenging due to the lack of established laboratory biomarkers. Based on the current literature, clinical consensus, guidelines, and expert recommendations, this review highlighted evidence-based potential laboratory biomarkers for the diagnosis of schizophrenia, including genetic biomarkers, neurotransmitters, neurodevelopmental-related proteins, and intestinal flora, and discussed the potential future directions for the application of these biomarkers in this field, aiming to provide an objective basis for the use of these biomarkers in the early and accurate diagnosis, treatment, and prognosis and rehabilitation assessment of schizophrenia.

OBJECTIVE:To unscramble the Notice on Standardizing the Allocation and Use of Licensed Pharmacists in Drug Retail Enterprises (called Notice for short )and related document ,and to give some suggestions to provide reference for the industry and policy decision departments. METHODS :The background and content of Notice were described briefly ;the problemsexisting in the corresponding provincial differentiated allocation policy and the difficulties existing in the implementation were analyzed to give some relevant suggestions. RESULTS & CONCLUSIONS :The main contents of Notice contained adhering to the policy of licensed pharmacists ,steadily improving the allocation level of licensed pharmacists ,refining and implementing the allocation requirements ,and strengthening the implementation of supervision and inspection responsibilities ;giving full play to the role of licensed pharmacists and continuously strengthening the team building. At present ,drug administrations of many provinces in China have issued specific policies on the differentiated allocation of licensed pharmacists and pharmaceutical technicians ,which are mainly divided into four schemes. The author believes that the classification method of drug retail enterprises implemented in Shandong and Guangdong provinces are the most scientific. The implementation of differentiated allocation of pharmacists with health(pharmacy)professional title in drug retail enterprises can alleviate the problem of insufficient licensed pharmacists to a certain extent ,but the disadvantage is that it can not achieve accurate registration and control as well as the management of licensed pharmacists ,which is easy to produce policy gaps ;after the “affiliation licensed pharmacist ’s certificate ”phenomenon, the“certificate affiliation of pharmacist with health (pharmacy)professional title ”phenomenon appeared. It is suggested that the provincial drug supervision and administration departments should try the differentiated allocation scheme of hierarchical management;learn from the management experience of licensed pharmacists and pharmaceutical technicians in Guangdong province, develop relevant platforms to manage the information of pharmaceutical technicians uniformly ; strengthen the communication and interaction with the health and human resource security department as well as the mutual promotion mechanism of personnel information screening ,so as to solve the core bottleneck of licensed pharmacists ’quality.

Objective:To investigate the association between age and cardiac tamponade after radiofrequency ablation of atrial fibrillation(RAAF).Methods:Clinical data of patients undergone de novo AF ablation procedures at Beijing Anzhen Hospital from January 2013 to December 2016 were retrospectively collected.Patients were divided into an elderly group(age ≥60 years)and a non-elderly group(age <60 years). Logistic regression analyses were used to evaluate the association between old age and the risk of cardiac tamponade complicating RAAF.Results:A total of 5 313 patients were involved in this study, including 41 patients(0.77%)with cardiac tamponade. The proportion of cardiac tamponade was higher in the elderly group than in the non-elderly group(1.1% or 32/2 950 vs.0.4% or 9/2 363, χ2=8.489, P=0.004). One patient with cardiac tamponade in the elderly group required immediate surgical repair whereas none in the non-elderly group did.No patient died in hospital.Multivariate Logistic regression analysis showed that the risk of cardiac tamponade increased in the elderly group, compared with the non-elderly group( OR=2.570, 95% CI: 1.190-5.570, P=0.017). Stratified analysis revealed that among females and patients with oral anticoagulants, left atrium dimension < 40 mm or procedure duration≥ 120 min in the elderly group carried a higher risk of cardiac tamponade than those in the non-elderly group( OR=1.011, 2.914, 3.922 and 3.244, P<0.05). Conclusions:Old age(age ≥60 years)is an independent risk factor for cardiac tamponade complicating RAAF.

Objective:To compare and observe the changes in choroidal thickness between healthy pregnant women and healthy non-pregnant women. Methods:A prospective clinical study. From January 2019 to August 2019, healthy pregnant women (pregnant women group) and healthy non-pregnant women age-matched were enrolled during the same period (the normal group) in the obstetrics of Zhuji People's Hospital. All patients were enrolled with their right eyes. Frequency-domain OCT-enhanced depth imaging technology was used to measure the subfoveal macular and 1000 μm above, below, nasal, and temporal choroidal thickness and foveal retinal thickness (CMT). The choroidal thickness and CMT of the pregnant women group and the normal group were compared by t test, and the choroidal thickness and CMT of the normal group and the eyes of different gestational weeks were compared by one-way analysis of variance. Results:The pregnant women group and the normal group included 161 patients (161 eyes) and 40 patients (40 eyes). According to the different gestational weeks, the pregnant women were divided into the first trimester group, the second trimester group, and the third trimester group, with 47 patients (47 eyes), 66 patients (66 eyes), and 48 patients (48 eyes) respectively. There was no significant difference in age, axial length, intraocular pressure, and CMT between the different gestational week groups and the normal group ( F=1.433, 1.558, 0.416, 2.288; P>0.05). The subfoveal choroidal thickness (SFCT) of the pregnant women group and normal group were 317.7±73.9 μm and 279.7±44.1 μm, respectively, and the difference was statistically significant ( t=3.113, P=0.002). Compared with the normal group, the choroid at the upper, lower, nasal, and temporal sides of the pregnant group 1000 μm from the fovea was thickened. The difference between the upper, nasal and temporal sides was statistically significant ( t=2.699, 3.474, 2.595; P<0.05). The SFCT of the eyes in the first trimester group, the middle group, and the late group were 305.8±72.3, 327.7±69.8, 315.8±80.5 μm, respectively. Compared with the normal group, the difference was statistically significant ( F=4.180, P=0.007). Pairwise comparison between the two groups, the second trimester group was significantly different from the normal group ( P=0.003). There was no significant difference among the first trimester group、the third trimester group and the other groups ( P>0.05). Conclusion:The choroidal thickness of pregnant women is thicker than normal, and the choroidal thickness in the second trimester reaches the maximum value; while the macular CMT during pregnancy has no significant change.
