1.Networked health information on diabetes: layer analysis

Objective Networked health information on diabetes updated by some websites was evaluated to provide evidence for its improvement.Methods Twenty-four domestic diabetes-related websites were identified,and primary index (media index,health information index and diabetes information index) and secondary index (practicability index and novelty index) were evaluated by using hierarchal analysis method.Results Of three primary index,compared with media and health information index,diabetes information index showed higher scores.Moreover,health information and diabetes information had strong causality.For the secondary index,practicability and novelty got relatively lower scores.The website Diabetic Club and Health for All got the highest (3.2) and the slowest scores (1.7),respectively.Conclusion At present,domestic diabetes-related websites may be far from satisfaction and need improvement in technology and human resources and more inputs.
2.Clinicopathological analysis of six cases of bullous systemic lupus erythematosus

Objective To summarize the clinical features,treatment and prognosis of bullous systemic lupus erythematosus (BSLE).Methods Clinical data were collected from 6 cases of BSLE between May 2009 and September 2015,and were analyzed retrospectively.Results Among the 6 patients,2 were male and 4 were female.The mean age of onset was 34 years.All the 6 patients presented with tense blisters and bullae arising on an erythematous base or normal skin,which were arranged in an annular pattern in 3 patients.The SLE Disease Activity Index (SLEDAI) score was > 4 in all the 6 patients.Histopathological examination showed subepidermal blisters or fissures in all the patients.Direct immunofluorescence (DIF) revealed continuous linear deposition of immunoglobulin G (IgG),IgM and IgA in all the patients,linear deposition of C3 in 4 patients,and linear deposition of Clq in 2 patients at the basement membrane zone (BMZ).All the 6 patients were treated with oral glucocorticoids and hydroxychloroquine,and 2 patients were additionally treated with cyclophosphamide.During the follow-up period,adverse reactions to different extents were observed in the 6 patients.Conclusions BSLE mainly occurs in young and middle-aged people.Histopathologically,subepidermal blisters or fissures can be observed with linear deposition of IgG,IgM,IgA,or C3 along the basement membrane zone on DIF.
3.Application of evidence- based nursing in nasopharyngeal carcinoma patients undergoing radiotherapy

Objective To discuss application of evidence-based nursing in nasopharyngeal carcinoma patients undergoing radiotherapy. Methods Evidence-based nursing was used in 33 nasopharyngeal carcinoma patients undergoing radiotherapy. Results After evidence-based nursing, all the patients actively cooperated with the doctors and nurses, lived through the treatment process smoothly, and radiotherapy complications were reduced or avoided. Conclusions Evidence-based nursing is an idealized nursing method which could reduce or avoid the complications and alleviate the pain of patients and improve the quality of nursing.
4.Study of Abnormal Heart Rate Variability and Its Acupuncture Intervention in Anxiety Disorder

Heart rate variability (HRV) is a reliable, innocuous and sensitive new index to measure cardiac autonomic nervous function. Anxiety disorder is often accompanied by autonomic nerve dysfunction and its main sign is abnormal HRV. Acupuncture can affect HRV indices, correct abnormal HRV and improve cardiac autonomic nerve dysfunction to relieve anxiety. This article sorts out and analyzes recent years’ studies on abnormal HRV in anxiety disorder, the effect of acupuncture on HRV and acupuncture regulation of abnormal HRV in anxiety disorder to provide a therapeutic basis for clinical acupuncture intervention in abnormal HRV in anxiety disorder.
5.Effects of umbilical cord blood monocytes transplantation in neonatal rats with hypoxic-ischemic brain damage on rat astrocyte proliferation and its correlation with bone morphogenetic protein 4

Objective To investigate the effect of umbilical cord blood monocytes(UCBMC) transplantation in neonatal rats with hypoxic-ischemic brain damage (HIBD) on rat astrocyte proliferation and its correlation with bone morphogenetic protein(BMP) 4.Methods Forty 7-day-old SD rats with the random number table,were divided into normal control(CON) group,hypoxic-ischemic(HI) group,normal(N) + UCBMC group,HI + UCBMC group,10 rats in each group.HIBD model was prepared according to the Rice method.Twenty-four hours after hypoxia,the UCBMC group and HI + UCBMC group were injected with 3 × 106 UCBMC via the lateral ventricle.Seven days after transplantation,changes in the number of neurons were observed by Nissl staining;the expression of glial fibrillary acidic protein (GFAP) was observed by Western blot;the astrocyte proliferation was observed by proliferating cell nuclear antigen (PCNA)/GFAP,BMP4/GFAP immunofluorescence double staining,and their correlation was analyzed.Results Nissl staining showed that the neurons at cerebral cortex and hippocamp were irregularly arranged and decreased in the HI group,and the number of Nissl-stained cells were significantly less than that in the CON group(tcortex =26.54,thippocamp =32.26,all P <0.05) ;but the Nissl-stained cells were well arranged in the HI + UCBMC group and more Nissl-stained cells were observed as compared with the HI group (tcortex =10.18,thippocamp =12.56,all P < 0.05) ; Western blot showed that the expression of GFAP in the HI group was significantly higher than that in CON group(t =5.50,P < 0.05) ;but the expression of GFAP in the HI + UCBMC group was significantly lower than that in the HI group (t =3.04,P < 0.05) ; immunofluorescence double staining showed that there were more PCNA + GFAP + cells in the HI group compared with the CON group(t =10.39,P < 0.05),but fewer PCNA + GFAP + cells were observed in the HI + UCBMC group than those in the HI group(t =3.72,P < 0.05).And there were more BMP4 + cells in the HI group compared with the CON group (t =5.52,P < 0.05),but fewer BMP4 + cells were observed in the HI + UCBMC group than those in the HI group(t =2.33,P <0.05).The fluorescence intensity of GFAP were correlated with that of BMP4 in HI + UCBMC group (r =0.84,P < 0.05).Conclusions UCBMC transplantation can decrease the proliferation of the astrocytes,thus promote brain damage repair and its mechanism might be collected with the decreased expression of BMP4.
6.Relationship of calcineurin expression between T-lymphocyte and myocardium in patients with heart failure

Objective Congestive heart failure (CHF) is the final common pathway of various heart diseases.Calcineurin,a calcium/calmodulindependent phosphatase consisting of a catalytic subanit A (CnA) and a regulatory calcium-binding subunit B (CnB),is activated in heart failure.This study aimed to investigate the relationship between mRNA level of calcineurin in circulating T-lymphocyte and that in myocardium in patients with CHF. MethodsA total of 38 patients with CHF (aged from 29 to 62 years) were included in this study.The mRNA levels of alpha-and beta-isoform of CnA in left ventricular anterior papillary muscle and peripheral lymphocytes were determined by semi-quantitative reverse transcription polymerase chain reaction.Pearson linear correlation analysis was performed,and difference was considered statistically significant at a P value <0.05. ResultsCalcineurin mRNA levels in lymphocytes were positively correlated with those in myocardium (for CnA-alpha mRNA,r=0.820;for CnA-beta mRNA,r=0.875;both P<0.01).CnA-beta mRNA levels in both circulating lymphocytes and myocardium increased significantly with increasing NYHA class (r=0.877 for peripheral blood and r=0.805 for cardiac muscle;both P<0.01). ConclusionsThe mRNA level of CnA-beta in circulating lymphocytes is positively correlated with that in myocardium and is a promising marker for the severity of cardiac dysfunction in patients with CHF.
7.Clinicopathologic analysis of four cases of desmoplastic melanocytic nevus

Objective To investigate pathological characteristics and differential diagnosis of desmoplastic melanocytic nevus.Methods Four cases of desmoplastic melanocytic nevus were analyzed based on the clinical manifestations and histological,immunohistochemical and fluorescence in situ hybridization (FISH) features.Results Of the 4 cases,2 were male and 2 were female.Their age ranged from 19 to 30 years with the average age being 26.5 years.The skin lesions were located on the extremities in 3 cases,on the vulva in 1 case.Histologically,the lesions were bilaterally symmetrical intradermal nevus.Nevus cells appeared epithelioid and/or fusiform,some were clustered or scattered in the proliferative fibrous tissue.None of lymphocyte aggregation,necrosis or ulceration was observed.Immunohistochemical examination showed positive staining for S100 and Melan A in 3 cases,positive staining for P16 in 2 cases,Ki-67-1abeling index less than 5% in all the 4 cases,and negative staining for factor XⅢ (FXⅢ) and CD34 in 2 cases.FISH assay showed no copy-number variations in gene loci 6p25 (RREB1),6q23 (MYB),6p11.1-q11.1 (Cep6) and 11q13 (CCND1) in desmoplastic nelanocytic nevus.Conclusion Desmoplastic melanocytic nevus is a kind of histologically unique,benign melanocytic nevus,and immunohistochemical staining for Ki-67,S-100,Melan A and FXmand FISH assay on melanoma can be helpful for the differential diagnosis between cutaneous fibrous histiocytoma and melanoma.
9.Impacts of mesalazine, montmorillonite powder, clostridium and on serum cytokine of rats with experimental ulcerative coliti

Objective:To establish the model of rats with UC by 2,4,6 trinitrobenzene sulfonic acid ( TNBS)/ethanol to understand the changes of IL-17,IL-23,TGF-β1,IFN-γin pathogenesis of ulcerative colitis (UC) and the impacts of mesalazine,mont-morillonite powder and clostridium on that.Methods:85 rats were divided randomly into 6 groups including the model ,mesalazine, montmorillonite,clostridium,mesalazine combined with montmorillonite powder group (15 rats per group ) and an additional control group of 10 normal rats.TNBS/ethanol was used to establish the model of rats with UC.The dosage of model, mesalazine, montmorillonite powder ,clostridium and mesalazine combined with montmorillonite powder were converted by surface area of rat and given to the rats by gastric-tube daily.All rats were executed at the 12th day of the molding and treatment ,and the blood and colon samples were collected.The content of IL-17,IL-23,TGF-β1,IFN-γwas measured in the blood by the ELISA method.The different changes of the four cytokines were compared separately.Results: ( 1 ) TNBS/ethanol was used to establish the model of rats with UC.The obvious inflammation can be observed on distal colon of rats by pathologic sections of HE stained .(2) The levels of plasma IL-17,IL-23 and IFN-γin the model,mesalazine,montmorillonite,clostridium and mesalazine combined with montmorillonite powder group were higher than ones in the control gruop (P<0.05),meanwhile,the levels of plasma TGF-β1 are lower (P<0.05).3.Comparing with the model group,the levels of plasma IL-17,IL-23 and IFN-γin the mesalazine,montmorillonite,clostridium and mesalazine combined with montmorillonite powder group were lower , and the levels of TGF-β1 were higer.The differences have statistic significative ( P<0.05).(4)Comparing the efficacy of treatment among mesalazine ,montmorillonite powder,clostridium and mesalazine combined with montmorillonite powder ,mesalazine and mesalazine combined with montmorillonite powder gets the best results ( P<0.05 ).The efficacy between montmorillonite powder and clostridium is similar.Conclusion:(1) TNBS/ethanol can be used to establish the model of rats with UC and can be successfully approved by the measurement of symptoms and pathologic investigation .(2)The IL-17,IL-23 and IFN-γare very active higher in the blood of rats with experimental ulcerative colitis induced by TNBS /ethanol, and have a positive correlation with inflammation.The TGF-β1 is in decrease in the blood of rats with colitis , and has a Negative Correlation with inflammation.(3)Mesalazine,montmorillonite powder,clostridium and mesalazine combined with montmorillonite powder may renovate the damage of inflammatory tissues of rats with experimental ulcerative colitis , and relieve the symptom of inflammation by reducing exudation of proinflammatory cytokines IL-17, IL-23 and immune regulatory cytokines IFN-γby producing the exudation of proinflammatory cytokines TGF-β1 to be good for recovery of UC.
10.Performance of four simplified screening methods of elevated blood pressure among children and adolescents

Objective:
To evaluate the performance of four simplified screening methods of elevated blood pressure commonly used among children and adolescents according to Chinese guidelines for prevention and treatment of hypertension (revised in 2018), so as to provide a reference for the early detection of the elevated blood pressure among children and adolescents.
Methods:
Stratified cluster random sampling method was used to monitor the physical fitness of 5 211 children and adolescents in a city of Shanxi Province from October to November 2021. Chinese guidelines for prevention and treatment of hypertension was considered as gold standard, and sensitivity, specificity, area under the curve (AUC) and Kappa value were calculated to evaluate the screening effectiveness of formula method, height specific method, age group specific method, sex and age specific method for screening elevated blood pressure.
Results:
The detection rates of elevated blood pressure among children and adolescents screened by gold standard, formula method, height specific method,age group specific method, sex and age specific method were 21.9%, 24.0%, 21.1%, 24.5% and 20.2%, respectively. There was no significant difference between prevalence of elevated blood pressure screened by formula method, sex and age specific method and gold standard( χ 2=1.21, 1.41, P >0.05), whereas height specific method and age group specific method had significant differences with gold standard ( χ 2=20.39, 67.09, P <0.05). AUC was the largest for height specific method [0.94(95% CI =0.93-0.95)], and the smallest for age group specific method [0.87(95% CI = 0.86 -0.88)]. The Kappa values of height specific method (0.89) and sex and age specific method (0.89) were both greater than 0.85 , which were more consistent with the screening effectiveness of gold standard. When comparing by sex, age and body mass index (BMI), the screening effectivenesses were consistent with the overall in boys, 6-11 years and normal body weight groups, while the screening effectivenesses were different in girls, 12-17 years, overweight and obese groups. The AUC (0.87), Kappa value (0.71) and sensitivity (82.33%) of age group specific method were the lowest and the screening effectiveness was the worst.
Conclusion
Height specific method is more effective and can be used for early identification and self detection of blood pressure abnormalities among children and adolescents.
