Objective To evaluate the post-operative glucose level and insulin dose of patients undergoing total pancreatectomy. Methods From September 1980 to September 2014, 21 patients underwent total pancrea-tectomy in Peking Union Medical College Hospital, who were enrolled in our study. We reviewed the changes in their insulin dosage and glucose levels after operation, also summarized type and dose of insulin as well as glucose level in stable period. Results The required insulin dose reached peak within 4 days after surgery ( maximum dose 300 U/d).The average dose was (143.5 ±62.8) U/d and decreased gradually.During the perioperative period (needing parenteral nutrition), the blood glucose level fluctuated markedly (1.52-29.06 mmol/L) and the average level was (11.18 ±0.95) mmol/L.During the stable period ( without parenteral nutrition) , patients on average had (5.3 ±2.0) U of preprandial rapid-acting insulin and (8.1 ±2.9) U of long-acting insulin be-fore sleeping;the average fasting blood glucose was (6.69 ±1.48) mmol/L, 2 h postprandial blood glucose was (9.08 ±2.84) mmol/L, bedtime blood glucose was (9.66 ±2.49) mmol/L, and blood glucose level at night was (8.15 ±2.78) mmol/L.67%of the patients had 13 hypoglycemic episodes monthly on average. For those five followed-up patients, the average hemoglobin A1c was (6.15 ±1.20)%.Conclusions Patients undergoing total pancreatectomy may experience marked fluctuation of blood glucose level and short-term increase of insulin need which gradually decreases afterwards. After entering the stable period, the glucose level could be well-con-trolled but with frequent hypoglycemia. There is no diabetic ketoacidosis.

Objective This study was conducted to analyze the clinical characteristics and pituitary function of patients with primary empty sella (PES).Methods The clinical data from 123 hospitalized adult patients with PES from January 2010 to May 2016 were retrospectively studied. Results (1) The average age of the 123 (male 43,female 80) PES patients was (59.2 ± 13.6) years (ranging 24-92 years),among whom 61% patients were in the age group between 50-69 years.(2) The symptoms of the patients included fatigue (56.1%),headache (34.1%),nausea and vomiting (17.9%),gonadal dysfunction (17.1%),visual disturbance (5.7%) and hypopituitarism crisis (3.3%).(3) Hypopituitarism was found in 66 of the 123 patients.Among them,36.6%,31.7% and 17.1% were central hypoadrenalism, hypogonadism, and hypothyroidism, respectively. The percentage of hypopituitarism in complete PES was significantly higher than that in partial PES (P < 0.05).(4) Sixteen patients were concomitant with other autoimmune diseases including 11 patients with Graves' disease and 2 with Cushing's syndrome due to adrenal adenoma. Conclusions The incidence of hypopituitarism in PES was 53.7%,in which the pituitary-adrenal axis hypofunction was more common. An overall evaluation of the pituitary function was essential for the patients who had headache and fatigue,or with suspected PES.The patients with hypopituitarism should be given hormone replacement therapy in time and followed up afterword.

Ovarian Leydig cell tumor(LCT), also known as ovarian testicular stromal cell tumor, is a rare sex cord stromal tumor, accounting for about 0.1% of all ovarian tumors. LCT is often accompanied by clinical manifestations of elevated androgen, and the imaging manifestations sometimes lack specificity. The diagnosis requires histopathological examination. Surgery is the primary treatment method, and postoperative prognosis is generally favorable. This paper retrospectively analyzes the diagnosis and treatment of a patient with LCT in our hospital combining relevant literature, explore the clinical characteristics, diagnosis, and treatment progress of LCT, aiming to improve disease management.

OBJECTIVE To study the effect and potential mechanism of eriodictyol on non-alcoholic fatty liver disease (NAFLD). METHODS Sixteen C57BL/6J mice were randomly divided into control group, NAFLD model group, and eriodictyol low-dose and high-dose groups (50, 100 mg/kg), with 4 mice in each group. Except for control group, the other groups were fed with high fat diet to induce NAFLD model. After four weeks of preprocessing, they were given relevant medicine intraperitoneally (0.01 mL/g), once a day, for 6 consecutive weeks. The body weight and liver weight of mice were measured, and the pathological damage of liver tissue in mice was observed. The levels of aspartate aminotransferase (AST), alanine aminotransferase(ALT), and triglycerides (TG) in serum, as well as the protein expressions of nuclear factor-erythroid 2-related factor 2 (Nrf2) and heme oxygenase-1 (HO-1) in liver tissue were determined. In vitro NAFLD model was established by using 0.5 mmol/L oleic acid (OA) in HepG2 cells. Normal control group, NAFLD model group and eriodictyol low-, medium- and high-concentration groups (50, 100, 150 μmol/L) were set up. HepG2 cells in drug groups were treated with eriodictyol for 24 h at the time of modeling. The lipid deposition was observed in cells, and the levels of TG, malondialdehyde (MDA) and reactive oxygen species (ROS) as well as the phosphorylation levels of the mitogen-activated protein kinase (MAPK) signal pathway related proteins [extracellular signal-regulated kinase (ERK), c- Jun N-terminal kinase (JNK)] and the protein expressions of Nrf2 and HO-1 were all determined. RESULTS In the in vivo experiment, compared with the NAFLD model group, the body weight, liver weight, the serum levels of AST, ALT and TG were all decreased significantly in eriodictyol low- and high-dose groups (except for serum level of AST in eriodictyol low-dose group) (P<0.01); liver lipid deposition was reduced significantly and the protein expressions of Nrf2 and HO-1 in liver tissues were further up-regulated (P<0.01). In the in vitro experiment, compared with the NAFLD model group, the lipid deposition in hepatocytes was reduced in eriodictyol low-, medium- and high-concentration groups (P<0.01), and the levels of ROS, MDA and TG were down-regulated (P<0.05 or P<0.01); the phosphorylation levels of ERK and JNK were significantly down-regulated (P<0.01), while the protein expressions of Nrf2 and HO-1 were up-regulated significantly (P<0.01). CONCLUSIONS Eriodictyol can inhibit MAPK signaling pathway and activate Nrf2/HO-1 signaling pathway to alleviate NAFLD.
