Object To study the chemical constituents of the underground part of Glehnia littoralis Fr. Schimidt ex Miq. Methods Eight compounds were isolated from EtOAc-soluble fraction of ethanol extract of G. littoralis by repeated chromatography over silica gel. Their structures were determined on the basis of their spectral data. Results They were identified as falcalindiol (Ⅰ), (8E)-1, 8-heptadecadiene-4, 6-diyne-3, 10-diol (Ⅱ), bergapten (Ⅲ), cnidilin (Ⅳ), xanthotoxin (Ⅴ), salicylic acid (Ⅵ), vanillic acid (Ⅶ), ferulic acid (Ⅷ). Conclusion Compounds Ⅱ and Ⅵ-Ⅷ were isolated from this plant for the first time.
3.Discussion on the optimal retrieval time of Cook Medical inferior vena cava filters

Objective To discuss the optimal retrieval time of the indwelling Gunther Tulip and Cook Celcet inferior vena cava filters (VCF). Methods During the period from March 2013 to April 2015 at Shengli Oilfield Central Hospital, the implantation of retrievable inferior vena cava filter was performed in 58 patients. Among the 58 patients, Gunther Tulip VCF was used in 13 and Cook Celcet VCF was employed in 31. Twenty-one patients followed the doctor's advice to receive retrieval procedure of VCF within three months after the implantation. Results Among the 21 patients, successful retrieval of VCF was obtained in 19. The mean indwelling time of Gunther Tulip VCF was 54.4 days, the longest time being 79.0 days. Gunther Tulip VCF was successfully removed in 3 patients and retrieval of VCF failed in 2 patients, with a retrieval success rate of 60%. The mean indwelling time of Cook Celcet VCF was 37.6 days, the longest time being 67.0 days. Cook Celcet VCF was successfully removed in 16 patients, with the success rate of retrieval being 100%. Conclusion Despite many VCFs that have been indwelled for a long time can be safely retrieved, retrieval procedure should be performed as early as possible in order to improve the retrieval success rate of VCF. It seems that the use of Cook Celcet VCF is a better choice although it is more expensive.

AIM: To prepare glycine liposome microparticle and observe the effect of glycine liposomes on cardiomyocyte injury induced by hypoxia/reoxygenation. METHODS: (1) Reverse-phase evaporation method was used to produce glycine liposomes, the effects of different organic solvents: aether, chloroform and two mixtures of aether/chloroform on entrapment efficiency were evaluated, transmission electron microscope was used to detect the particle diameter of glycine liposomes. 5.Effect analysis of multi-interventional modes mainly with mechanical thrombectomy for large artery occlusive acute cerebral infarction

Objective To investigate the effectiveness and safety in patients with largeartery occlusive acute cerebral infarction who received multi-interventional modes mainly with mechanical thrombectomy and its related factors affecting prognosis. Methods The clinical data of 56 patients with large artery occlusive acute cerebral infarction were analyzed retrospectively. The clinical characteristics (gender,age,and underlying diseases),timing of treatment (time from ictus to puncture,time from puncture to recanalization), multi-interventional mode therapies (intra-arterial thrombolysis,thrombectomy,balloon dilation,and stenting, etc. ),and distribution of offending vessels were observed. The modified Thrombolysis in Cerebral Ischemia Scale (mTICI)grade was used to evaluate revascularization. The National Institute of Health Stroke Scale (NIHSS)score was used to observe the neurological function at 24 h before and after procedures. The modified Rankin scale (mRS)was used to evaluate the prognosis at 3 months after procedure. The safety of the treatment was evaluated with operative complications (mainly symptomatic intracranial hemorrhage)and mortality. The patients were divided into either a good prognosis group (n = 34;mRS≤2)or a poor prognosis group (n =22;mRS≥3)according to the prognosis at 3 months after procedure. They were analyzed with univariate analysis. The factors influencing the prognosis were further analyzed with multivariate logistic regression analysis. Results (1)The recanalization rate in 56 patients was 78. 6%(n = 44),in which basilar artery was the highest,reaching 93. 8% (15 / 16),middle cerebral artery was 87. 0% (20 / 23). The NIHSS score at 24 hours was 10 ± 7,it was lower than 16 ± 6 on admission. There was significant difference (t =6. 401,P <0. 01). At 3 months,34 patients (60. 7%)had good prognosis,4 (7. 1%)died,and 8 (14. 3%) had symptomatic intracranial hemorrhage. (2)Multiple factor analysis showed that the high level of recanalization was a protective factor for good prognosis (OR,0. 465,95% CI 0. 267 -0. 809,P =0. 007). Diabetes was an independent risk factor for poor prognosis (OR,5. 535,95% CI 1. 101 -27. 835, P = 0. 038). Conclusion Acute large artery occlusive cerebral infarction treated with the intra-arterial multi-interventional modes may quickly and effectively restore intracranial blood flow. It has the characteris-tics of high recanalization rate and good prognosis,and the higher the level of recanalization,the better the prognosis. Diabetes is an independent risk factor for poor prognosis.
6. Clinical efficacy of injected autologous fat and buried polydioxanone threads for nasolabial fold filling

Objective:
To investigate the clinical application of injected autologous fat and buried polydioxanone threads for nasolabial fold filling.
Methods:
From April 2016 to October 2017, 64 cases of mild and moderate nasolabial fold beauty seekers from the First People's Hospital of Jiande were divided into group A and group B with their attitude. Group A (28 cases) were only treated with buried polydioxanone threads for nasolabial fold filling; Group B (36 cases) were treated with injected autologous fat and buried polydioxanone threads for nasolabial fold filling. The surgical effect was recorded for 18 months.
Results:
The most common complications were swelling, bruising and pain immediately after the surgery. Polydioxanone threads exposure occurred in two cases in each group 2 weeks after the surgery. There were satisfactory results in skin color and skin texture in both groups. 9 cases showed inadequate correction in group A while only 2 cases showed inadequate correction in group B in long-term follow-up (>3 months).
Conclusions
Combined treatment of autologous fat and polydioxanone threads is safe and effective for nasolabial fold filling.
7.Expression of glycine receptor in rat cardiomyocytes

AIM: To study the expression of glycine receptor (GlyR) in adult rat cardiac tissue and cultured cardiomyocytes from neonatal rat hearts. METHODS: Total RNA and membrane protein were extracted from cardiac tissue of adult rats and primary cultured cardiomyocytes from neonatal rat hearts, ?1 and ? subunits mRNA and protein of GlyR were detected with the method of reverse transcription nest DNA polymerase chain reaction (RT-PCR) and Western blotting. RESULTS: The mRNA and protein expression of ? subunit were identified in the adult rat heart tissue and cultured cardiomyocytes, similar to the sequence and immunogenicity generated from GlyR ? subunit of the rat spinal cord; for ?1 subunit, mRNA was found in only cultured cardiomyocytes. CONCLUSION: These data demonstrate that adult rat cardiac tissue and cultured cardiomyocytes from neonatal rat hearts express mRNA and protein of GlyR subunits similar to the GlyR that expresses in the spinal cord, suggesting that GlyR exists in the membrane of the rat cardiomyocytes.

AIM: To investigate whether glycine receptor is involved in the protection of glycine against(anoxia/reoxygenation) injury in cardiomyocytes by detecting oxygen free radical metabolism,apoptosis and intracellular calcium overload.METHODS: The neonatal rat cardiomyocytes were cultured and exposed to anoxia and reoxygenation((A/R)) in the presence of glycine receptor antagonist,glycine or in free chloride buffer.The superoxide dismutase(SOD) activity,the contents of malondialdehyde(MDA) and nitric oxide(NO),the intracellular free calcium concentration and the apoptotic rate in the cardiomyocytes were determined.RESULTS: SOD activity and NO content in cardiomyocytes were lower,but MDA content,intracellular free calcium concentration and apoptotic rate in cardiomyocytes were higher in A/R group than those in control.Pretreatment with glycine inhibited the above changes caused by A/R,which was reversed by strychnine treatment and in the free chloride medium.CONCLUSIONS: Glycine inhibits free radical production,attenuates calcium overload,decreases apoptotic rate and increases SOD activity and NO release in cardiomyocytes exposed to(A/R).These findings suggest that glycine exerts a protective effect against A/R injury via glycine receptor and glycine protects the neonatal rat cardiomycytes from A/R-induced injury in a chloride-dependent manner.
9.Aerobic exercise reduces hepatic lipid deposition in non-alcoholic fatty liver disease

Objective:To observe any effect of aerobic exercise on lipid deposition in the liver and monoacylglycerol O-acyltransferase 1 (MGAT1) signaling in non-alcoholic fatty liver disease (NAFLD).Methods:Male Sprague-Dawley rats were fed a 45% diet fat for 6 weeks, after which they were confirmed to have NAFLD. The rats were then randomly divided into an exercise group, a sedentary group and a diet adjustment group. The exercise and sedentary groups remained on the high-fat diet, but the exercise group underwent 8 weeks of aerobic exercise, while the diet adjustment group returned to a normal diet without any exercise. After the intervention, lipid accumulation in liver tissues was detected by hematoxylin-eosin staining and hepatic steatosis indices were calculated. Liver MGAT1 and the expression of peroxisome proliferator activated receptor γ (PPARγ) protein were detected using western blotting.Results:Liver steatosis indices in the exercise and the diet adjustment groups had decreased significantly after the eight weeks. The expression of MGAT1 protein had decreased significantly in the exercise group and the expression of PPARγ protein had increased significantly. Compared with the sedentary group, no significant changes were observed in the expression of MGAT1 protein in the livers of the diet adjustment group, though their average PPARγ protein expression had increased significantly. Compared with the diet adjustment group, the average liver steatosis index had increased significantly in the exercise group, but the expression of MGAT1 protein had decreased significantly.Conclusions:Aerobic exercise can significantly improve liver lipid deposition in NAFLD, at least in rats. The mechanism may be related to inhibition of the MGAT1 signal pathway. Aerobic exercise may be a rehabilitation intervention for NAFLD patients.
10.Effect of PCSK9 inhibitors on early intensive lipid-lowering therapy in patients with symptomatic intracranial atherosclerotic stenosis

Objective To investigate th effect of PCSK9 inhibitors on early intensive lipid-lowering therapy in patients with symptomatic intracranial atherosclerotic stenosis(ICAS).Method One hundred and thirty-nine symptomatic ICAS patients who attended the Xuzhou Central Hospital from October 2022 to August 2023 were collected.According to the choice of lipid-lowering regimen,they were divided into statin drug group(n=54),PCSK9 inhibitor group(n=42)and combination drug group(n=43).Baseline data,laboratory indices,adverse drug reactions,clinical neurological function scores and endpoint events of the three groups were collected and analyzed.Results There was no statistically significant difference in the clinical data among the three groups before treatment.Compared with pre-treatment,total cholesterol(TC)and low-density lipoprotein cholesterol(LDL-C)levels were significantly lower in the three groups after treatment(all P<0.01),and peripheral blood leukocyte counts were significantly lower in the PCSK9 inhibitor group after treatment(P<0.05).After treatment,the differences in TC and LDL-C levels and the proportions of LDL-C<1.8 mmol/L after 2 weeks of medication and LDL-C<1.4 mmol/L on the 90th day were statistically significant among the three groups(all P<0.01).Compared with the statin drug group,the differences in TC and LDL-C levels and the proportions of LDL-C<1.8 mmol/L after 2 weeks and LDL-C<1.4 mmol/L on the 90th day in the PCSK9 inhibitor group and the combination drug group were statistically significant(all P<0.05).The differences in the occurrence of endpoint events,the proportion of recurrent AIS or TIA,cardiovascular events,and the time to endpoint events among the three groups were statistically significant on the 180th day after treatment(all P<0.05).Differences in the occurrence of endpoint events,proportion of cardiovascular sexual events,and time to endpoint events on day 180 after treatment were statistically different in the PCSK9 inhibitor group and the combination drug group compared with the statin drug group(all P<0.05).There was a statistically significant difference among the survival analysis curves of the three groups,with higher event-free survival in both groups with PCSK9 inhibitors alone or in combination(Log-Rank=13.95,P=0.0009).Conclusion Early intensive lipid lowering with PCSK9 inhibitors is effective and safe in patients with symptomatic ICAS,and can further reduce LDL-C,TC,and white blood cell counts,as well as improve lipid compliance,and significantly reduce the rate of end-point events with a significantly prolonged time to occurrence.
