Objective To investigate the characteristics of clopidogrel-associated thrombocytopenia to provide references for clinically safe drug use. Methods The case reports of thrombocytopenia induced by clopidogrel published in PubMed,Embase,CNKI,Wanfang and VIP were searched from the establishment of each database to November 2022,and their occurrence was collated and analysed. Results A total of 44 cases from 43 articles were identified and included in the analysis. There were 30 males(68.2%)and 14 females(31.8%).Ages ranged from 37 to 88(65.0±11.4)years,of which 30(68.2%)were ≥60 years old.Thrombocytopenia was found from 8 h to 9 months after medication,of which 29 cases(65.9%)appeared within two weeks. There were 31 cases(70.5%)with severe thrombocytopenia and 38 cases(86.4%)with complications,of which 24 cases(63.2%)with bleeding and 19 cases(50.0%)with thrombotic thrombocytopenic purpura(TTP).The platelet countof41 cases(93.2%)returned to normal after drug withdrawal and symptomatic treatment, and 3 cases(6.8%)died finally. Conclusion Clopidogrel related thrombocytopenia is mainly severe thrombocytopenia, and often accompanied by bleeding or thrombotic thrombocytopenic purpura(TTP),but the overall outcome is good.Platelet count should be regularly monitored within the first two weeks after medication.Clopidogrel should be stopped and symptomatic treatment should be given in case of any abnormality.

Objective To summarise the best evidence for postpartum follow-up acquired from patients with gestational diabetes mellitus so as to provide an evidence-based reference for establishment of a postpartum follow-up program. Methods Literature on postpartum blood glucose follow-up in patients with gestational diabetes mellitus was retrieved from 17 major databases including BMJ Best Practice,UpToDate,Joanna Briggs Institute(JBI),International Guide Collaboration Network,National Institute for Health and Care Excellence(NICE),American Diabetes Association website,Medlive,Cochrane Library,Embase,OVID,PubMed,Web of Science,Quanfang local PubMed,CNKI,Wanfang Data,VIP,and Sinomed from the inception of databases to July 2023.The literature to be retrieved included guidelines, expert consensus, evidence summaries, systematic reviews, clinical decisions, and the original studies. Results A total of 9 articles, five practice guidelines,a systematic review,an evidence summaries,an expert consensus and a clinical decisions, were included.Totally,17 pieces of best evidence were summarised from the five aspects:postpartum blood glucose, lifestyle guidance, breastfeeding, drug treatment and health education. Conclusions The summarised best evidences can provide evidence-based references for postpartum follow-up of patients with gestational diabetes.Nursing staff in hospitals and community health centres should formulate relevant follow-up plans according to the actual postpartum conditions of patients in order to prevent the incidence of postpartum Type Ⅱ diabetes.

Objective:To analyze the influencing factors of post stroke cognitive impairment (PSCI) and their correlation with cognitive scores in patients with acute ischemic stroke. Methods:In this cross-section study, 36 patients diagnosed with acute ischemic stroke and post stroke cognitive impairment (PSCI) admitted to the Department of Vascular Neurology of Beijing Tiantian Hospital Affiliated to Capital Medical University from June 1, 2022 to September 30, 2022 were selected as the PSCI group. And one to one matching was performed for patients without PSCI (PSNCI group) with an age±1 year and same gender admitted to the hospital during the same period (as control, 36 cases). Basic clinical data of the two groups were collected, the laboratory and imaging examinations were completed. Mini Mental State Examination (MMSE) and Montreal Cognitive Assessment Scale (MoCA) were used for cognitive evaluation by neuropsychologists. Hypothesis testing was used to compare the differences in basic data, laboratory tests and lesion sites between the two groups. Multi-factor conditional logistic regression was performed to analyze the influencing factors of PSCI, and Spearman correlation analysis was carried out to analyze the correlation between influencing factors of PSCI and the cognitive scores. Results:Compared with those in PSNCI group, the proportion of patients with stroke/transient ischemic attack history, hyperhomocysteinemia (HHcy), apolipoprotein E(ApoE) ε4 carriers and the ratio of temporal lobe and thalamus infarction were higher in PSCI group (41.7% vs 13.9%, 36.1% vs 2.8%, 30.6% vs 5.6%, 22.3% vs 2.8%, 25.0% vs 5.6%), the MMSE and MoCA scores were lower in PSCI group [16.50 (8.25, 19.00) vs 28.00 (27.00, 30.00), 10.00 (4.25, 14.50) vs 27.00 (25.00, 28.00)] (all P<0.05). Logistic regression analysis showed that HHcy was a positive correlation factor for PSCI ( OR=2.342, 95% CI=1.186-4.622, P=0.014). Spearman correlation analysis showed that MMSE ( r=-0.415) and MoCA ( r=-0.417) scores were negatively correlated with homocysteine (Hcy) (both P<0.05). Conclusion:HHcy is an important factor affecting the occurrence and development of PSCI in patients with acute ischemic stroke, and Hcy level is negatively correlated with cognitive scores in those patients.

ObjectiveTo explore the sensory processing characteristics of preschool children with autism spectrum disorder (ASD), and to provide a theoretical basis for early screening and intervention training of ASD. MethodsA total of 215 preschool children with ASD and170 typically developed (TD) children were investigated with a basic situation questionnaire and sensory processing measure (SPM). The two groups were stratified according to age and gender, and the differences of scores in sensory domains were compared to analyze the sensory processing characteristics of preschool children with ASD. ResultsThe scores of social participation, vision, hearing, touch, taste and smell, body awareness, balance and motion, planning and ideas, and total sensory system in children with ASD were all higher than those in children with TD (all P<0.01). The highest degree of abnormality was found in hearing and the lowest degree in taste and smell in children with ASD. The results of Spearman correlation analysis showed that in the 4-year-old and 5-year-old children with ASD, the scores of vision (rs=-0.200, P= 0.033) and hearing (rs=-0.194, P=0.040) decreased with age. There was no correlation between the scores of other developmental quotients and age (all P>0.05). Boys and girls with ASD had higher scores in all developmental quotients than TD children (P<0.01). However, there was no significant gender difference in any developmental quotients of ASD children (all P>0.05). ConclusionSensory processing abnormalities are common in preschool children with ASD, which are different from those of TD children in multiple sensory domains. Sensory processing abnormalities may be used as an indicator for early screening of ASD, and it is necessary to conduct corresponding intervention training for sensory processing abnormalities in children with ASD.
