1.Analysis of incidence and influencing factors of congenital heart disease in perinatal infants in Henan province

Objective To analyze the incidence, trend and influencing factors of congenital heart disease(CHD) in perinatal infants in Henan province. Methods From 2011 to 2020,1 356 838 perinatal infants born from 28 weeks of pregnancy to 7 days after delivery were selected from 37 national birth defect monitoring points in Henan province as the research subjects, and the incidence of CHD in perinatal infants from different regions, fetal sex and maternal age were compared. The Joinpiont regression model was established to analyze the temporal change trend of the incidence rate of CHD in perinatal infants in Henan province from 2011 to 2020; the annual percentage change (APC) represented the internal trend of each segment, and the average annual percentage change (AAPC) represented the overall change trend. The trend of gap in incidence rate of CHD in different regions, mother's age and perinatal sex was compared by the Joinpiont parallel test. Results From 2011 to 2020,19 004 cases of perinatal infants with CHD were detected in Henan province, with a total incidence of 140.06/10 000; the incidence of CHD in perinatal infants showed an increasing trend year by year (AAPC=43.3% ,P<0.05) From 2011 to 2020, the incidence of CHD in urban perinatal infants in Henan province was significantly higher than that in rural areas, the incidence of CHD in male perinatal infants was significantly higher than that in female perinatal infants,and the incidence of CHD in perinatal infants in older mothers was significantly higher than that in younger mothers (X2=7 259.160,5 415.473,499.520; P<0.05). From 2011 to 2020,the incidence of CHD in perinatal infants in urban and rural areas of Henan province showed an increasing trend year by year (AAPC=42.5% ,44.5% ;P<0.05); the difference between urban and rural areas in the incidence of CHD in perinatal infants was increasing year by year(P<0.05). From 2011 to 2020,the incidence of CHD in male perinatal and famale perinatal infants in Henan province showed an increasing trend year by year(AAPC=44.3%,42.7% ;P<0.05). From 2011 to 2020, the incidence of CHD in perinatal infants of non-elderly and elderly pregnant women in Henan province showed an increasing trend year by year (AAPC=42.9% ,42.7% ;P<0.05).the difference between the elderly arid non-elderly pregnant women in the incidence of CHD In perinatal infants was increasing year by year (P<0.05). Conclusion From 2011 to 2020 ,the incidence of CHD in perinatal infants in Henan province showed an upward trend, and the regional distribution, fetal sex and maternal age were related to the incidence of perinatal CHD.
2.Clinical study on the application of guide catheter window guidance technology in intracranial artery stent angioplasty

Objective:To explore the application value of guide tube fenestration and drainage technology in intracranial artery stenting surgery.Methods:A total of 120 patients with symptomatic intracranial atherosclerotic stenosis admitted to the Xingtai Third Hospital from January 2021 to December 2022 were selected and randomly divided into an observation group ( n=60) and a control group ( n=60). Both groups were treated with intracranial artery stenting, with the observation group receiving guidance catheterization and window opening technique during the surgery. Two groups of surgeries were observed and compared: the degree of vascular stenosis before and after surgery, the Montreal Cognitive Assessment Scale (MoCA) scores before and one month after surgery, intraoperative complications, and prognosis. Results:The surgical time and catheter placement time in the observation group were (110.20±23.32)minutes and (11.32±2.01)minutes, respectively, which were significantly shorter than those in the control group (all P<0.05). The stent placement rate and operation success rate in the observation group were 95.00% and 96.67%, respectively, significantly higher than those in the control group (all P<0.05). The degree of postoperative vascular stenosis in the observation group was (32.29±7.11)%, significantly milder than that in the control group [(44.43±8.15)%, P<0.05]. One month after surgery, the MoCA scores of both groups improved significantly compared to before surgery (all P<0.05), and there was no statistically significant difference between the groups ( P>0.05). There was no statistically significant difference in the incidence of collateral circulation occlusion and vascular rupture between the two groups (all P>0.05). During the follow-up period, there was no statistically significant difference in the incidence of restenosis, intracranial hemorrhage, and recurrent ischemic stroke between the observation group and the control group (all P>0.05); The good prognosis rate of the observation group was 73.33%, significantly higher than that of the control group (53.33%, P<0.05). Conclusions:The application of guided catheter fenestration technique in intracranial artery stenting has good value, which is beneficial for improving the stent placement rate and operation success rate, and improving the degree of vascular stenosis.
3.Multivariate analysis for pelvic floor dysfunction.

OBJECTIVE:
To investigate the risky factors for pelvic floor functional disorder.
METHODS:
A total of 346 patients, who undergone surgery for pelvic floor reconstruction from January 2007 to December 2013 in the Third Affiliated Hospital of Zhengzhou University, were enrolled for this study and served as an experimental group. Meanwhile, 349 female healthy subjects were selected as a control group. The data including the times of gestation and parity, history of menopause, hysterectomy surgery, uterine fibroid and leucorrhea were recorded.
RESULTS:
Compared with the controls, the numbers of people with hysterectomy, menopause and clean vagina≥III degree, lack of lactobacillus as well as the times of gestation and parity in the experimental group were significantly increased (P<0.05). The history of hysterectomy, the times of gestation and parity and lack of lactobacillus in vagina were the independent risk factors of pelvic floor dysfunction.
CONCLUSION
The history of hysterectomy, the times of gestation and parity, and clean vagina may involve in the occurrence and development of pelvic floor functional disorder, and the existence of lactobacillus in the vagina might be a protective factor.
