Objective To investigate the clinical value of the 18F-FDG SPECT-CT combined with the image of CT in the diagnosis of lung cancer.Methods Retrospectively analysis images of 18F-FDG SPECT-CT and CT of 162 patients pulmonary tumor-like lesions from January 2013 to October 2013 were conducted.All the patients have been confirmed by the pathological mechanism or cytologic examination.Each patient has been firstly diagnosed by the 18F-FDG SPECT-CT along and then combined with the images of CT.The sensitive rate and the accurate rate of two methods were compared by x2 test.Results There were 40 benign lesions and 114 malignant ones in 162 patients.The sensitive rates of the two methods were 86.0％ and 96.5％ (x2 =6.63,P ＜ 0.05).The accurate rates of the two ways were 79％ and 96.3％ (x2 =7.76,P ＜ 0.05).The differences were statistically significant.Conclusion 18F-FDG SPECT-CT combined with the images of CT can increase the diagnostic sensitivity and accuracy.It has important clinical value.

Objective To evaluate the precision of the ACCU-CHEK Performa glucose monitor,the Optium Xceed glucose monitor,the Breeze glucose monitor and the Contour glucose monitor.Methods A total of 102 patients (diabetic patients or un-diabetic patients) who visited our endocrinology outpatient department were randomly selected.The venous blood samples were collected using the laboratory auto-analyzer,and the synchronous finger tip capillary blood were collected using the ACCU-CHEK Performa glucose monitor and the Optium Xceed glucose monitor,or using the Breeze glucose monitor and the Contour glucose monitor.Results The correlation coefficients between VPG by the laboratory auto-analyzer and CBG by the four kinds of glucose monitors was good,and R values were 0.990,0.985,0.963,0.952,and there was no significant difference between the CBG of Breeze glucose monitor and the same VPG(P ＞ 0.05).When the blood glucose concentration was higher than 4.2mmol/L,and compared with the VPG detected by synchronous measurement laboratory,95％ of the CBG test results detected by four kinds of blood glucose meters were in the range of ± 20％.Conclusion The four kinds of glucose monitors can provide high accurate.

Objective Target-controlIed-infusion(TCI) system makes administration of intravenous anesthetics simpler, more accurate and adjustable. Bispectral index (BIS) is generally accepted as a measurement of depth of sedation and anesthesia. The aim of this study was to evaluate the usefulness of BIS as a feedback control in propofol anesthesia with TCI for patients undergoing laparoscopic cholecystectomy. Methods Forty ASA Ⅰ - Ⅱ patients (12 male, 28 female) aged between 20-70 yr were randomly allocated to one of two groups of twenty each: feedback TCI group and TCI group. In feedback TCI group, BIS value of 50 was set as the control variable. A bolus of 3 ?g?kg-1 fentanyl was given Ⅳ 3 min before induction. The target concentration of propofol was set at 3 ?g?ml-1 throughout anesthesia. Real time BIS, the highest and lowest systolic and diastolic pressure during operation, doses of adjunctive drugs, time for emergence and orientation, awareness during operation and the standardized unit dose of propofol were recorded and compared between groups. Objective:To observe the outcome of posterior staphyloma (PS) marginal retinal photocoagulation in pars plana vitrectomy (PPV) for high myopia macular hole retinal detachment eyes accompanied with PS.Methods:From January 2017 to June 2019, 49 patients (49 eyes) with high myopia macular hole retinal detachment accompanied with PS who were undergone PPV operation from Tianjin Eye Hospital were included in this study. There were 13 males (13 eyes) and 36 females (36 eyes). All patients underwent best corrected visual acuities (BCVA) and optical coherence tomography examinations. The standard logarithmic visual acuity chart was used for BCVA examination, and the visual acuity was converted to minimum resolution angle in logarithmic (logMAR) when recorded. The patients were randomly divided into two groups according to surgical options: conventional PPV with internal limiting membrane (ILM) peeling (group A, 24 eyes), PS marginal retinal photocoagulation in PPV with ILM peeling (group A, 25 eyes). The mean preoperative logMAR BCVA of group A and B were 1.87±0.28 and 1.80±0.37, the difference was not statistically significant ( t=0.604, P=0.551). The patients in the group A received 23G PPV, triamcinolone acetonide staining during the operation, the epiretinal membrane was peeled off, indocyanine green assisted staining, the posterior macular ILM was peeled off, and the peripheral retina was examined in detail during the operation. Areas with retinal degeneration were reinforced by laser photocoagulation, and the subretinal fluid was drained through the macular hole and filled with silicone oil. The eyes of the group B were subjected to retinal photocoagulation for 2 to 3 rows at the edge of the PS in addition to the usual surgical procedures. The average follow-up time was 8.34±3.21 months. Surgical outcome were estimated by the average number of operation, retinal reattachment rate, macular hole closure rate and BCVA. The χ2 test or Fisher exact probability was used to compare the count data. Independent sample t test was used to compare the measurement data. Results:Retinal reattachment was obtained in 17 eyes (70.8%, 17/24) and 24 eyes (96.0%, 24/25) in group A and B after first surgery respectively, the difference was statistically significant ( χ2=3.984, P=0.046). Final retinal reattachment was obtained in all 49 eyes. Final macular hole closure was in 15 eyes (62.5%, 15/24) and 19 eyes (76.0%, 19/25) in group A and B, respectively, the difference was not statistically significant ( χ2=1.051, P=0.305). The mean postoperative logMAR BCVA of group A (1.20±0.47) and B (1.08±0.39) were all improved than preoperative BCVA, the differences were all statistically significant ( t=2.899, 5.327; P=0.001, 0.000), the differences of mean postoperative logMAR BCVA between two groups was not statistically significant ( t=0.675, P=0.506). The mean number of operation of group A (2.63±0.88) was more than group B (2.08±0.28), the difference was statistically significant ( t=3.003, P=0.006). Conclusion:In comparison with conventional PPV, combined PS marginal retinal photocoagulation can improve retinal reattachment rate after first surgery, and reduce the number of reoperations.

Gindenosides are the active ingredients of Panax ginseng. 20 (S) -protopanaxadiolocotillol type epimers are the main metabolites of 20 (S) -protopanaxadiol. The previous studies showed that there are stereoselectivity difference in pharmacodynamics and pharmacokinetics between 24R-epimer and 24S-epimer. The purpose of this study was to explore the excretion of the epimers in bile, feces and urine of rat. Liquid chromatography tandem mass spectrometry method has been performed for determination of 24R-epimer and 24S-epimer in bile, feces and urine. 24R-epimer or 24S-epimer was intragastric administered to rats at a single dose of 10 mg x kg(-1). Results showed that after administration the recovery of 24R-epimer and 24S-epimer in feces was 17.69% and 17.09%, respectively, while both of the two epimers were hardly detected in urine. The 48 h cumulative biliary excretion rate of 24R-epimer was 8.01% after administration, while only 1.47% for 24S-epimer. It indicated that there are stereoselectivity in biliary excretion of the epimers with intragastric administration.

Objective: To observe the effect of ethanol extract of Crataegus Pinnatifida Bge. Var. major N. E. Br. from Hei-longjiang province on hemodynamics of acute myocardial ischemia caused by ligation and platelet aggregation induced by isoproterenol. Methods:After the model establishment of acute myocardial ischemia by ligation in rats, the systolic blood pressure ( SBP) ,diastolic blood pressure ( DBP ) , heart rate ( HR ) , left ventricular systolic pressure ( LVSP ) , left ventricular end-diastolic pressure ( LV-EDP) , left ventricular peak systolic ( +) and diastolic ( -) rate ( ± dp ·dt max-1 ) were recorded. Meanwhile, the acute myocar-dial ischemia model induced by isoproterenol was established. The platelet aggregation was measured by a platelet aggregation instru-ment. Results:The ethanol extract of Crataegus Pinnatifida Bge. Var. major N. E. Br. from Heilongjiang province could significantly increase the SBP, DBP, LVSP, +dp · dtmax-1 and HR, and reduce LVEDP,-dp·dtmax-1 in high dose group with statistical sig-nificance (P<0. 05). The medium dose group had small impact on the DBP, while showed statistically significant effect on the other hemodynamic indices. The low dose group had statistically significant effect on the SBP, LVSP and ± dp·dtmax-1 (P<0. 05), while showed no notable effects on the other indices. The results of aggregation rate in 1 min and 5 min after the determination and the maxi-mum one in the high and medium dose groups showed that the ethanol extract could significantly inhibit the platelet aggregation with sta-tistical significance (P<0. 05). Conclusion:The ethanol extract of Crataegus Pinnatifida Bge. Var. major N. E. Br. can improve the blood flow of acute myocardial ischemia in rats and reduce platelet aggregation after ischemia.

OBJECTIVETo evaluate clinical outcomes of anterior cruciate ligament (ACL) and posterior cruciate ligament (PCL) reconstruction under arthroscopy and repair of the injured posteromedial complex structure of the knee joint in the treatment of posterolateral knee dislocation with multiple ligament injuries.

METHODSFrom March 2008 to August 2012,22 patients (16 males and 6 females, ranging in age from 20 to 53 years old, with an average of 30.5 years old) with posterolateral dislocation of the knee were treated with primary reconstruction of ACL and PCL, combined with the repair of injuries in the posteromedial complex and soft-tissue. Eight patients had injuries caused by sports,5 patients road accidents and 9 patients falling down. The ACL was reconstructed using the gracilis and semitendinosus tendons. The PCL was reconstructed using LARS artificial ligaments (14 cases), or gracilis and semitendinosus tendons (8 cases). Suture repair was performed in 17 patients with posteromedial ligament injuries,and self-semitendinosus strengthening operations were performed in 5 patients. Continuouspassive montion (CPM) and active exercises were executed after operation at early stage. The IKDC and Lysholm system were used to evaluate therapeutic effects.

RESULTSAll the patients were regularly followed up, and the duration ranged from 11 to 56 months (averaged, 39 months). According to the IKDC scale,9 patients got a grade A result, 10 got a grade B result, and 3 got a grade C result. The IKDC subject score was 89.6±3.1 and the Lysholm scores was 90.7±1.8 at the latest follow-up, which were both better than those before operation.

CONCLUSIONReconstructing the ACL and PCL and repairing injured posteromedial complex of the knee followed by an active rehabilitation is an effective method to treat posterolateral knee dislocation.
