Objective To sum up the experience of perioperative management of patients with heart transplantation.Methods In recent 3 years 24 cases of dilated cardiomyopathy,4 cases of(serious) valve disease,1 case of serious ischemic cardiomyopathy and 1 case of bi-ventricular assist(received) heart transplantation surgery.The mean by pass time was 75?24 min and the mean aortic clamp time was 72?8 min.Cyclosporine,MMF and prenisodone were adopted as anti-immune(response) strategy and the dose of cyclosporine was adjusted according to the serum concentration.(Intramyocardial) electrograms and tissue Doppler ultrasound technique were used to monitor the(immune) response situation.The myocardial biopsy was performed in case of the suspect of acute(rejection).Results In 5 postoperative deaths,3 cases died of low cardiac output,1 died of infection-(induced) multiple organ failure and 1 case died of serious bleeding.The postoperative complications(included) right heart failure in 6 cases,renal failure in 13 cases,and acute rejection in 4 cases.(Conclusion) The effective prevention and management of complications is the key point of success of heart transplantation.Application of comprehensive monitoring techniques is beneficial to the management of the immune rejection after transplantation.

Objective To evaluate the efficacy of combined red and blue light therapy with acne film coating in treatment of moderate and severe acne.Methods Totally 547 patients with moderate or severe acne were enrolled in the study from March 2012 to July 2014.Using open control,according to the cycle of time sequence will patients were divided into three groups:183 patients (study group) were received alternatively red and blue light therapy twice a week,and acre film coating was applied once a day;182 patients in control group 1 received film coating alone and another 182 patients (control group 2) received light therapy alone.The treatment lasted for 6 weeks in all three groups.The numbers of skin lesions were recorded before and 2,4,6 w after treatment,and the curative effect was evaluated 10 w after treatment.Results The number of skin lesion were significantly reduced 6w after treatment in all three groups,that for study group was most markedly.For patients with moderate acne,the total effective rates in study group and in control groups 1 and 2 were 84.6％ (110/130),58.5％ (76/130) and 51.9％ (67/129),respectively (x2 =21.836,and 31.955,P ＜0.01).For patients with severe acne the total effective rates in study group and in control groups 1 and 2 were 84.9％ (45/53),57.7％ (30/52) and 56.6％ (30/53),respectively (x2 =9.525 and 10.258,P ＜0.01).There are 20 cases in observation group therapy for different degree of erythema,itching,burning,tingling,incidence of side effects was 10.9％ (20/183),control group 1 and 2 were 7.1％ (14/182),12.1％ (22/182),are tolerated.Conclusion The combination of red/blue light therapy with acne film coating is more effective than the application of single method alone for patients with moderate or severe acne.

Objective To establish the model of pancreatoduodenal allotransplantation in pigs with enteric drainage (ED) and portal venous drainage (PVD). Methods Forty-six hybrid landraces were divided into two groups (donor and recipient groups) randomly, for pancreatoduodenal allotransplantation. Donors were perfused via abdomial aorta without clamping the portal venous outflow with UW solution after heparinization. Whole pancreatoduodenal graft was harvested with segments of abdomial aorta and portal vein and shaped under cold UW solution. Then, the end-to-end anastomosis was performed with the donor iliac artery bifurcation “Y” graft to the recipient superior mesenteric arteries and celiac artery. Furthermore, type Ⅰdiabete model was made by removal of the recipient pancreas. The venous anastomosis was reconstructed between the donor portal vein and the recipient superior mesenteric vein. Meanwhile, the end-to-side anastomosis was performed with the donor common iliac artery bifurcation “Y” graft to the recipient abdomial aorta and the side-to-side intestinal anastomosis was performed between the donor duodenum and the recipient jejunum. External jugular vein was intubated for transfusion. The levels of blood glucose, insulin and glucagon in blood were measured before and during the operation and 1, 3, 5, 7 d after operation. Results Twenty-three cases of pancreatoduodenal allotranplantations were performed on pigs. One died from complication of anesthesia. Success rate of operation was 95.7%.Complications of operation happened in 2 cases in which one was phlebothrombosis, incidence 4.5% and the other was duodenojejunal anastomotic leak, incidence 4.5%. The level of blood glucose increased within 30 min and recovered on the 2nd day after removal of pancreas. The levels of insulin and glucagon decreased within 30 min and recovered on the 2nd day after removal of pancreas. Rejection curred at the 1st day and reached the worst level on the 9th day after transplantation without the change of insulin and glucagon in blood and clinical symptoms of rejection. Conclusion Pancreatoduodenal transplantation in pigs can treat type Ⅰ diabete. ED and PVD can keep the function of endocrine in normal. The techni- que of duodenal transplantation with ED and PVD may pave the way for the further development of pancreas transplantation in clinic.

AIM: To confirm that the inflammation response after mechanical arterial injury correlates with the neointimal hyperplasia in animal model. METHODS: Male Wistar rats underwent left common carotid balloon angioplasty were injected twice with a bacterial lipopolysaccharide (LPS, 50 ng/rat) before and after surgery. Next, just after neointima formation, the animals were sacrificed for the evaluation by morphometric analysis, histological observation and immunostaining. Western blot was used to investigate the protein expression of several known mediators of apoptosis. RESULTS: Serum interleukin-1 beta levels as a marker of inflammation were increased after LPS treated. Early neotimal lesions were characterized by intimal thickening and the presence of SMCs. Neointima with smooth muscle alpha-actin negative were observed at 7 days after injured. These areas of neointima demonstrated a relatively high proliferation index by proliferating cell nuclear antigen (PCNA) antibody staining, whereas the proliferation index in media was low. Neointimal thickness was significantly increased at 4 weeks after injury in LPS treated animals compared with controls, from (151.2?14.5 to 173.9?15.3) ?m2. Activation of caspase-3 was observed, indicating that smooth muscle cells of neointima was associated with apoptosis. Immunofluorescence analysis revealed NF-?B expression located to the adventitia. CONCLUSIONS: Our results indicate that nonspecific stimulation of low-level LPS facilitates neointimal formation and may be an important factor in the restenosis of angioplasty.

Two p-phenylenediamine (p-PD)-imprinted polymers, P (MAA) and P (AA), were synthesized using methacrylic acid (MAA) and acrylamide(AA) as functional monomer, respectively, in order to prepare molecular recognition material with high selectivity for p-PD and explore the feasibility of methods such as molecular spectrometry and computational approach of quantum chemistry for the selection of functional mono mer with high imprinting efficiency.The molecular recognition properties of the imprinted polymers were evaluated by high performance liquid chromatography.The results indicated that P(AA) exhibited no imprint ing effect for p-PD, while P(MAA) can bind p-PD selectively(k' =3.57), which showed remarkable imprint ing effect (IF=2.95), and p-PD and its analogues o-phenylenediamine and p-aminobenzoic acid can almost realize baseline separation on P (MAA) column in the mobile phase of methanol.Furthermore, we made a comparative study on the interaction of p-PD with MAA and AA by spectroscopic techniques such as UV and fluorometry as well as HF/6-31G~* computational approach.The results demonstrated that the complex of p-PD-MAA was more stable than that of p-PD-AA, which can give a good explanation for the molecular recog nition properties of P (MAA) and P (AA).The study indicated that both molecular spectrometry (UV and fluorometry)and computational approach of quantum chemistry can be employed as efficient means for the selection of efficient functional monomer.The results showed that fluorometry is sensitive and convenient for the choice of functional monomer if the template molecule is fluorescent.

Using a micro-column packed with polyethylenimine aminated glycidyl methacrylatE-grafted-polytetrafluoroethylene fiber made by ourselves as the adsorption material, the adsorption behaviors of Pb and Cd under different conditions were studied by inductively coupled plasma optical emission spectrometry. The conditions for the preconcentration of Pb and Cd such as pH, flow-rate, time and eluent acidity were optimized. The results show that the PTFE-g-GMA-PEI Fiber has excellent adsorption property and high adsorption capacity for Pb2+ and Cd2+. The Enrichment factors for Pb2+ and Cd2+ were 30 and 80, respectively and the detection limits were 3.5 μg/L and 0.15 μg/L, respectively, the relative standard derivations were 1.5% and 0.6%(n=9)respectively. When the single ion concentration was 50 μg/L, the sampling frequency was 9 and 18 times/h, respectively. The method was applied to the simultaneous determination of trace lead and cadmium of several Chinese traditional medicine samples, the standard recovery of samples was 90%-108%.

Thirty nine compounds in the volatile oils of the flowerbuds of Panax quinquefolium L.cultivated in China were isolated and identified with gas chromatography/mass spectre metry. To our great surprise,β-famensene,a sesquiterpene compound of them,alone account for 48.67% of the total volatile oils contents. This experimental result was contrasted with the volatile oils components of the different parts of Panax quinquefolium L.and Panax ginseng,anidentical genus plant,on chemical components and relative quantities.It was discovered that the only one common component is β-famensene in them and it is the highest value in determined relative contents. Above result provides scientific basis for biological synthesis of triterpenoid saponins.

Objective To evaluate the applicative value of ultrasound elastography in diagnosis of focal lesions in the parotid and submandibular glands.Methods 30 patients(30 lesions) were scanned by real-time ultrasound elastography and were analyzed in elastography evaluated criteria (5 score method).All the results were compared with the pathological types.Results The elastographic grades of most benign nodules were 1 - 3,while most of malignancy were 4 - 5.The elastographic grades of benign and malignant lesions were of significant difference statistically( P ＜0.01 ).If elastographic grade 4 or 5 were diagnosed as malignancies,the sensitivity,specificity and accuracy for diagnosing malignant l(e)sions in the parotid and submandibular glands were 72.73 ％,84.21 ％,80.00 ％,respectively.Conclusions Ultrasound elastography is useful in the differential diagnosis of focal lesions in the parotid and submandibular glands.

Objective To investigate human papilloma virus(HPV) infection in normal endometrium, atypi-cal hyperplasia of endometrium and endometrial carcinoma. Methods By the nucleic acid hybridization,we detected 28 pairs of endometrial carcinomas, 21 pairs of atypical hyperplasia of endometrium. Normal endometrium from 16 pa-tients with uterine myomas were as control. Results HPV16/18 DNA was detected in 25 endometrial carcinoma and 2 atypical hyperplasia of endometrium and 1 normal endometrium. Conclusions Endometrial carcinoma HPV16/18 DNA were significantly higher than those infected with atypical endometrial hyperplasia and normal endometrium. Note the occurrence of endometrial cancer,the development may be associated with HPV infection.

Objective To conclude the results of electrophysiological (EP) mapping and ganglianated plexis (GPs) ablation during the minimally invasive atrial fibrillation (AF) surgery.Methods During the period of Sep 2007 to May 2010 to-tally 185 paroxysmal atrial fibrillation cases were recruited receiving the minimally invasive AF surgery with video assistance.The EP mapping was used to test the dual direction block after ablation and the distribution of GPs was recorded.The sinus rhythm rate was followed up after surgery.Results All the minimally invasive surgery were performed successfully withoutdeath in the hospital.There was no permanent pacemaker implant and other serious complications.EP mapping results showed92％ pulmonary vein (PV) potential to the left atrium (LA) could be isolated with the dual direction block for the paroxysmal atrial fibrillation cases.And there were 89％ GPs positive,higher rate than the persistent AF.There were more GPs positive in the right PV area than the left and 86％ GPs lied at the LA domain,not the PV orifice.At least 5 or 6 ablation lesions were needed to get the dual direction block and denervation of the GPs.The sinus rhythm rate after surgery after 3,6,12 and 24months is 83.7％,82.4％,85.4％ and 83.9％.The 12 months follow up data showed the cases with more GPs positive (＞6) had 81.3％ sinus rate and the less GPs positive (≤2) group with 52.6％.Conclusion The EP mapping and GPsablation techniques during the minimally invasive AF ablation surgery could get good sinus rhythm rate during the long term follow up.
