1.Relationship Between Arterial Stiffness and Variability of Blood Pressure in Senile Patients With Hypertension

Objective: To investigate the relationship between arterial stiffness and variability of blood pressure (BPV) in senile patients with hypertension.
Methods: A total of 234 senile patients with hypertension were enrolled. The patients were examined by 24h ambulatory blood pressure monitoring (ABPM), BPV was identified by the standard deviation (SD) of blood pressure value. Based on increased percentage of baPWV, the patients were divided into 4 groups: Normal artery group, n=44, Mild arterial stiff group, n=72, Moderate arterial stiff group, n=78 and Severe arterial stiff group, n=40. The 24h ABPM results, BPV, biochemical indexes and their relationship to baPWV were compared among different groups.
Results:①The HDL-C level and 24hDBP, dDBP, 24hSBPSD, 24hDBPSD, dSBPSD, dDBPSD, nDBPSD were significantly different among each group, all P<0.05.②The increased percentage of baPWV was positively related to the age and 24hSBPSD, 24hDBPSD, dSBPSD, dDBPSD, nDBPSD, while negatively related to HDL-C level, all P<0.05.③The age and 24hSBPSD, 24hDBPSD, dSBPSD were the independent risk factors for baPWV elevation.
Conclusion: The arterial stiffness was closely related to BPV in senile patients with hypertension.
2.THE ANTITUMOUR AND IMMUNOMODULATING EFFECT OF EXOPOLYSACCHARIDES PRODUCED BY LACTOBACILLUS DELBRUECCKII SSP.BULGARICUS

Objective To study the antitumour, immunomodulating and enterobacteria regulating effects of exopolysaccharides (EPS). Method The suppressive rate of growth of sarcoma 180, the index of immunity, and the intestinal microflora were determined. Results (1) EPS inhibited the growth of sarcoma 180 by oral for BALB/c mice significantly. The suppressive rate at the dose of 120mg/kg bw?d was 42.7%. (2) At the dose of 120mg/kg bw?d the index of thymus and IL-2 in serum increased significantly. There was increasing trend in respect of TNF-? and killing activation of NK cell. (3) The amounts of Lactobacillus in groups treated with EPS increased significantly, and the amounts of Enterobacteriacea and Enterococcus droped significantly. Conclusion EPS had the antitumour effect in vivo, and could modulate the immune function, improve the microbial balance in the intestinal tract of tumour bearing mice to some extent. So the antitumour mechanism was probably correlated with the immunomodulating effect.
3.Interferon/poly (lactic-co-glycolic acid) and chitosan/poly (lactic-co-glycolic acid) composite membranes for prevention of epidural scar adhesion following laminectomy

BACKGROUND: Epidural resistance materials are an approach to prevent the epidural scar adhesion after laminectomy. A composite membrane of mechanical separation and fibroblast inhibition is promising to be widely studied.OBJECTIVE: To prepare the interferon (IFN)/poly (lactic-co-glycolic acid) (PLGA) and chitosan/PLGA composite membranes, and to observe their effects on preventing epidural scar adhesion after laminectomy in rabbits.METHODS: L2 laminectomy was performed in 120 rabbits, which were randomly and evenly divided into six groups. The control group received nothing on epidurals. In the autologous free fat group, the exposed dura was covered with subcutaneous fat. In the sodium hyaluronate group, 1 mL sodium hyaluronate was dripped onto the dura. In the PLGA, chitosan/PLGA, and IFN/PLGA groups, the exposed dura was covered with one of the three membranes at a sufficient size. The animals were sacrificed at 2, 4, 6, 8 weeks postoperatively. The scar formation and adhesion to the dura in L2 laminectomy sites were observed and scored by gross observation. At 4 weeks postoperatively, fibroblast ultrastructure was observed through the use of transmission electron microscope.RESULTS AND CONCLUSION: Widespread scars formed in the control group. The scars in the autologous free fat and PLGA groups were less compared with the control group, but there was no significant difference between these two groups. In the early stage, the scars in the sodium hyaluronate group were obviously less compared with the autologous free rat and PLGA groups, while in the later period, the difference between them was not significant, but the results from the sodium hyaluronate group were still better compared with the control group. The scars and slight adhesion to the dura were observed in the chitosan/PLGA and IFN/PLGA groups. The effect of preventing scar adhesion in these two groups was remarkably superior to that in the other groups, but there was no significant difference between these two composite membrane groups. At 4 weeks postoperatively, fibroblast morphology and function in the control, autologous free rat, and PLGA groups were better than those in the chitosan/PLGA and IFN/PLGA groups. Chitosan/PLGA and IFN/PLGA composite membranes are effective materials to prevent epidural scar adhesion after laminectomy.
5.Effects of active compression-decompression cardiopulmonary resuscitation on cardiac functions during ventricular fibrillation by two-dimensional echocardiography in dogs

Objective To compare the effect of active compression-decompression cardiopulmonary resuscitation(ACD-CPR) with standard- cardiopulmonary resuscitation(S-CPR) on ventricular function in a canine ventricular fibrillation model. Methods Ventricular fibrillation was induced in anesthetized and instrumented canine. Twenty-four dogs were randomly assigned to either ACD-CPR group or S-CPR group.After 4 minutes of untreated VF,two-dimension echocardiography was used to evaluate the left ventricular end-diastolic volume(LVEDV),left ventricular end-systolic volume(LVESV) and left ventricular ejection fraction (LVEF) of every canine of the two groups when they were undergoing cardiopulmonary resuscitation. Results During ventricular fibrillation, both ACD-CPR group and S-CPR group showed decreased LVEDV compared with pre-ventricular fibrillation, but not statistically significant( P >0.05).LVEDV was increased in ACD-CPR group compared with S-CPR group, but not statistically significant (P> 0. 05). Both ACD-CPR group and S-CPR group showed significantly increased LVESV compared with pre-ventricular fibrillation,of which the difference was statistically significant ( P <0. 001). Both ACD-CPRgroup and S-CPR group showed significantly decreased LVEF compared with pre-ventricular fibrillation,of which the difference was statistically significant (P <0. 001). LVEF was increased in ACD-CPR group compared with S-CPR group,of which the difference was statistically significant ( P = 0.019). Conclusions Compared with S-CPR,ACD-CPR resulted in higher LVEF.
7.Effects of different doses of dexmedetomidine on cerebral oxygen saturation and pulmonary shunt fraction in patients undergoing one-lung ventilation

Objective To investigate the effects of different doses of dexmedetomidine on cerebral oxygen saturation and pulmonary shunt fraction in patients undergoing one-lung ventilation (OLV). Methods Sixty ASAⅠ-Ⅱpatients, aged 46-71 years, with body mass index (BMI)18-24 kg/m2 and scheduled for thoracotomy were randomly divided into three groups (n=20 each):high dose dexmedetomidine group (group D1), low dose dexmedetomidine group (group D2) and control group (group C). Dexmedetomidine 1μg/kg was infused in group D1 after anesthesia induction, and then a rate of 0.5μg·kg-1·h-1 was continuously infused. Dexmedetomidine 0.5μg/kg was infused in group D2 after anesthesia induction, and then a rate of 0.3μg · kg-1 · h-1 was continuously infused. Group C was received the equal volume of normal saline. Anesthesia was main?tained with propofol-remifentanil and intermittent iv boluses of rocuronium. Arterial and jugular venous blood samples were collected before anesthesia induction (T0), at 15 min after two-lung ventilation (T1), at 5 min (T2) and 30 min (T3) of OLV for blood gas analysis. Value of Qs/Qt was calculated and SctO2 was recorded at the same time. Results Compared with group C and group D2, Qs/Qt was decreased at T2 in group D1 (P<0.05). Qs/Qt was lower at T3 in group D1 and D2 than that of group C, and which was lower in group D1 than that of group D2 (P<0.05). In group C and group D1 a significant de?crease in SctO2 was observed at T2 and T3 compared to that at T0 and T1 (P<0.05). SctO2 was significantly higher at T2 and T3 in group D2 than that in group C and group D1 (P<0.05). Conclusion Dexmedetomidine given during OLV undergoing thoracotomy can improve oxygenation, decrease pulmonary shunt fraction and reduce the occurrence of low SctO2.
8.Clinical and muscular pathological features with chronic graft-versus-host disease-related polymyositis: one case report

Objective To report the clinical and pathological features of chronic graft-versus-host disease-related polymyositis by summarizing the clinical data of the patient with chronic graft-versus-host disease-related polymyositis. Methods One patient with chronic graft-versus-host disease-related polymyositis was hospitalized in our hospital on December 29,2010.The patient,40 years old,female,underwent allogeneic haematopoietic stem cell transplantation because of acute granulocytic-monocytic leukemia.Fourteen months later she manifested as slowly progressive muscle weakness and myalgia in all limbs.Serum creatine kinase level was between 426-1948 U/L. Myositis antibody EJ was strongly positive.Electromyogram showed a neurogenic impairment and slow peripheral nerve conduction speed.Muscle biopsies were carried out in the left biceps brechii.In addition of standard histological and enzyme histochemical staining for the muscle sections,immunohistochemical workup was performed with mouse antiCDs,anti-CD20,anti-CD68 and anti major histocompatibility complex- Ⅰ ( MHC- Ⅰ ) monoclonal antibodies as first antibodies.Results The muscle biopsy showed large variation of fiber size,with muscle fiber necrosis,regeneration.Some angular fibers distributed in small cluster.The inflammatory cells infiltrated around the small vessel or in the endomysium,mainly CD8+ T-lymphocytes and CD6+8 macrophages.The most muscle fibers were MHC-Ⅰ positive. Conclusion The graft-versus-host disease-related polymyositis manifests as chronic myositis process with neurogenic lesions.
9.Clinical application of ultrasound-guided minimally invasive operation for benign breast tumor

Objective To investigate the clinical effect of Ultrasound-guided minimal invasion technique used in the diagnosis and treatment of benign breast tumor .Methods 60 cases of benign breast tumors , including single or multiple breast lumps ,were treated using minimally invasive technology under the guidance of ultrasound . The average operation time was 25min.Through 3-6months after operations ,any residue and recurrence at local lesion were not found by clinical and ultrasonic examination , and incision scar formation was not clear .Minimally invasive operation can completely amputate breast tumors .Results Under real time ultrasound monitoring ,all the 60 cases of operation had 100%of display rate.The tumor pathologic classification is as follows:25cases of fibroadenomas ,2 ca-ses of cyst ,3 cases breast cystic hyperplasia ,12 cases of adenosis of mammary glands ,3 cases of lipoma of the breast and one case of phyllodes tumor .The other 14 cases have both two kinds of pathological types .Conclusion The system of ultrasound-guided minimally invasive technology can accurately puncture benign breast tumor with high re -section rate,small trauma and is operated safely .It is a therapeutic method and consistent with the opinion of cosme-tology.
10.Evaluation of Effect of Neonatal Hypoxic-Ischemic Encephalopathy Admitted to Neonatal Intensive Care Unit

Objective To investigate a cure for neonatal hypoxic-ischemic encephalopathy(HIE) by observing the curative effect on neonates with HIE admitted to neonatal intensive care unit (NICU).Methods Fifty neonates with HIE from May to Dec.in 2006 admitted to NICU for treatment(NICU group),which contrast to the 42 neonates with HIE in corresponding periods of 2005 year(control group).The curative effect of three supports and three expectant treatment were evaluated by observing vital signs,oxygen saturation,biochemical test,and the neonatal beha-vioral nerve assessment (NBNA) respectively; Clinical curative effect contained markedly effective and effective which as the total curative effect statistics.Results There were no obvious difference between each group from the means of blood sugar data while hospitalizing as well as the NBNA in 3 days.
