1.DIAGNOSIS AND THERAPY OF THE PANCREATIC TRAUMA CAUSED BY BLUNT ABDOMINAL LESIONS

Of the 250 cases of blunt abdominal trauma,15 cases of pancreatic lesions were complicatedwhich include 3 cases lacerated on the head of pan-creas, 5 on the body, 6 on the tail and one con-tused. The pancreatic bed drainage was performedfor one case, repair for 8 cases and pancreatic tailresection for 6 cases, In author's experiences, theroutinely inspecting the pancreas was carried outintra operatively, with blunt epigastric trauma, es-pecially the light red hemotoliquid was found bypericentesis. Operative method was classified frompancreatic bed drainage, repair and resection in ac-cordance with severity of the trauma, For prevent-ing the pancreatic juice fistula, the main pancreaticduct must try its best to be ligated. If the traumais on the head, a common bile duct and duodenumdecompression must be done.

Construction of course group,which overcomes the narrow limits of knowledge structure for single course,carries training objectives of comprehensive knowledge and innovation ability,and plays an important role in realizing the goal of higher medical education.Basic course group in clinical laboratory medicine has been built based on the close relation,promotion,and infiltration of five courses contents.It has been explored and reformed from the restructuring and optimizing of course group contents,teaching methods,practice step,and so on.Practice shows that the course group is helpful to improving students' ability of integrated use of knowledge and presents obvious effect for improvement of the cultivation of students' comprehensive quality.
4.Clinical efficacy of navigation of sentinel lymph node combined with endoscopic submucosal dissection in laparoscopic surgery for non curative treatment of early gastric cancer

Objective To explore the clinical efficacy of navigation of sentinel lymph node (SLN) combined with endoscopic submucosal dissection (ESD) for non curative treatment of early gastric cancer.Methods Clinical data of 9 cases with early gastric cancer treated with ESD from August 2015 to December 2015 were enrolled.The SLNs were explored by laparoscopy after nano carbon or indocyanine green(ICG) was injected under endoscopy.Then all stained nodes were resected by laparoscopy.Results The mean number of SLN were 2.75±1.50 in ICG group,which was significantly smaller than the number in nano carbon group(8.40±4.93) according to the laparoscopic pathology,so the accuracy of ICG was higher than that of nano carbon.But ICG performed better in development.The postoperative complications such as postoperative intestinal obstruction,enteritis and gastric paralysis were found in 5 patients of nano carbon group.But no serious complications after operation were found in patients of ICG group.Conclusion Application of ICG can help avoid the excessive removal of tissue,and reduce the incidence of complications.ICG,as a new type of tracer for early gastric cancer,shows good application prospects.

AIM: To investigate the effects of advanced glycation end products (AGEs) on protein and mRNA expression of macrophage inflammatory protein-1? (MIP-1?) in cultured human umbilical vein endothelial cells(HUVECs). METHODS: HUVECs were cultured with AGEs at different concentrations for 24 h and at a concentration of 400 mg/L for different time.The levels of mRNA and protein expression of MIP-1? in cultured HUVEC were detected by in situ hybridization and Western blot, respectively. 7.Effect of carbon monoxide releasing molecule on experimental periodontitis in rats.

OBJECTIVETo evaluate the effects of carbon monoxide releasing molecule-2 (CORM-2) on experimental periodontitis in rats.
METHODSForty-two Wistar rats were divided into three groups. Rats in the normal group (NL group) did not undergo any procedure, whereas the other rats were ligatured and treated with saline solution (NaCl 0.9%) (LO group) or treated with CORM-2 (10 mg kg(-1) per day) (CO group). A 3-0 silk suture was placed around the mandibular first molars. Rats were sacrificed after 3, 7, and 10 d. Blood samples were collected from all animals for tumor necrosis factor-alpha (TNF-alpha) and interleukin-1beta (IL-1beta) analysis. Changes in alveolar bone levels were measured clinically, and periodontal tissues were histopathologically examined to assess the infiltration of inflammatory cells.
RESULTSLigature placement increased alveolar bone loss and inflammatory cell infiltration in periodontal tissue. Alveolar bone loss in CO group was significantly higher than that in NL group, but was lower than that in LO group (P<0.05). The ratio of inflammatory cell infiltration in LO group was significantly higher than that in CO and NL groups, and that in CO group was lower than in LO group (P<0.05). Serum levels of TNF-alpha and IL-1beta in the LO group were significantly higher than those in the CO and NL groups, and those in CO group were lower than in LO group (P<0.05).
CONCLUSIONSystemic administration of CORM-2 reduced periodontal inflammation and alveolar bone loss in experimental periodontitis in rats.

8.Comparative analysis on the clinical application of antibiotics before and after hospital special rectification on antibiotics

Objective The article was to promote the rational use of antibiotics by comparative analysis on antimicrobial us -age before and after the special rectification on clinical application of antibacterial drugs in our hospital . Methods Medical record data of 132 inpatients before the rectification and 167 inpatients after the rectification were randomly collected for the rational compara-tive analysis on microorganism submission rate for treatment , usage of antibiotics and medication for typeⅠandⅡincisions . Results The microorganism submission rate was much higher , especially the special grade antibiotics (81.5 %vs 32.1%).There was also a remarkable decrease in the percentage of antibiotics use , the strength of utilization and the average drug cost .The amount of antibiotic prophylaxis without indication and the length of medication for typeⅠincisions showed a remarkable decrease (P<0.05).Remarkable decrease was also observed in the irrational usage rate of antibiotic prophylaxis , dosage, medication time and drug replacement for typeⅠand Ⅱincisions (P<0.05). Conclusion The special rectification measures are effective and practical , which can promote the rational use of antibiotics .
9.Molecular Cloning and Characterization of Phytoene Synthase Gene From a Unicellular Green Alga Haematococcus pluvialis

The unicellular green alga Haematococcus pluvialis accumulates a highly valuable ketocarotenoid, i.e. astaxanthin up to 4%dry weight under stress conditions. Phytoene synthase is considered to be the first rate limiting enzyme in carotenoid biosynthesis pathway in H. pluvialis. The cDNA and genomic genes of phytoene synthase, i.e. psy from H.pluvialis were cloned and characterized.Result showed that psy had one open reading frame of 1 200 bp encoding a putative polypeptide of 400 amino acids which was interrupted by four introns. Phylogenetic analysis revealed that psy from green algae formed a monophyletic clade, and its closer relationship was higher plants. By using genomic walking approach, an approximate 1 kb 5′ flanking region ofpsy gene was cloned and a number of putative cis-regulatory elements were revealed. Fusing a 297 bp internal sequence (-297 to -1 bp from the translation initiation codon ofpsy) with the reporter gene, i.e. lacZ before attemptedintroducing the construct into the green alga via particle bombardment resulted in lacZ transient expression.
10.Significance of serum ischemia-modified albumin level in evaluating liver function in patients with liver cirrhosis of various causes

ObjectiveTo investigate the differences in serum levels of ischemia-modified albumin (IMA) and IMA/albumin ratio (IMAR) in patients with liver cirrhosis of various causes, as well as the significance of IMA measurement and IMAR in evaluating liver function. MethodsA total of 456 patients with liver cirrhosis who were admitted to China-Japan Union Hospital of Jilin University from February 2012 to February 2014 were enrolled. The patients underwent liver function evaluation, and according to the serum albumin (Alb) level, they were divided into normal Alb group (152 patients) and reduced Alb group (304 patients). According to the cause of the disease, the patients were further divided into hepatitis B virus (HBV) group, hepatitis C virus (HCV) group, and alcoholic cirrhosis group, and healthy subjects were enrolled as controls. The serum IMA level was measured for all patients with liver cirrhosis, IMAR was calculated, and the correlations of IMA and IMAR with liver function parameters and Child-Pugh score were analyzed. An analysis of variance was used for comparison of coutinuous data between groups, and the least significant difference t-test was used for further comparision between any two groups; the chi-square test was used for comparison of categorical data between group and the Spearman rank correlation was used to analyze the correlation of IMA、IMAR、Child-Puch score. ResultsCompared with the healthy controls, the patients with liver cirrhosis showed significant increases in the serum IMA level and IMAR(all P<005), and the cirrhotic patients with hypoproteinemia had significantly higher serum IMA level and IMAR than those without hypoproteinemia(all P<005). In addition, the patients with alcoholic cirrhosis had significantly higher serum IMA level and IMAR than those with HBV- or HCV-associated liver cirrhosis (F=6765,8276;P<0.01). The serum IMA level was correlated with Alb, total bilirubin (TBil), international normalized ratio (INR), and cholinesterase (CHE)(r=-0510,0731,0327,-0679;all P<005). IMAR was correlated with TBil, INR, and CHE(r=0488,0327,-0896;all P<005). The serum IMA level and IMAR were positively correlated with Child-Pugh score(r=-0801,0899;all P<005). ConclusionPatients with liver cirrhosis experience a reduced Alb function before the development of hypoproteinemia, and serum IMA level and IMAR differ significantly between the patients with liver cirrhosis of various causes.
