Objective To discuss the efficacy and relative factors that affect the sequential therapy of invasive and non-invasive mechanical ventilation in treating severe respiratory failure. Methods We analyzed the clinical data of the patient who was hospitalized in Respiratory Department of the Sixth Hospital of Luohe on 3th February 2008.Results The patient received invasive mechanical ventilation for 4 days, then changed to mask non-invasive positive pressure ventilation for 10 days, while receiving comprehensive treatment of anti-infection and bronchial expansion, etc. Then the patient was recovered. Conclusion The sequential therapy of invasive and non-invasive mechanical ventilation in treating severe respiratory failure is effective. The efficacy depends on the optimum converting time between invasive and non-invasive mechanical ventilation, the choice of ventilator and the patient′s compliance.

MicroRNAs (miRs) play an important role in regulating diverse cellular processes. It has been reported that miRs are associated with the formation and maturation of erythrocytes, and the expression of globin genes at post-transcriptional level.Compared with normal human enrythrocytes, various miRs are altered in the patients with thalassemia. These changes also happen in the patients with diverse clinical manifestations. In this paper, we systematically summarized the recent progress about the expression dysregulation of miRs in β-thalassemia and their roles in regulating the levels of γ-globin and fetal hemoglobin.During β-like globin gene expression, miRs directly or indirectly regulate the levels of erythroid-specific transcription factors through post-transcriptional action, such as B-cell lymphoma 11A (BCL11A), myeloblastosis oncogene (MYB), specificity protein 1 (Sp1), Kruppel-like factor 3 (KLF3) and GATA1.These effects subsequently regulate the switch between γ-and β-globin gene expression and affect fetal hemoglobin production.Targeting miRs might be a novel therapeutic strategy for β-thalassmeia.

MicroRNAs (miRNAs) are small non-coding RNAs that contain 22 to 25 nucleotides and play important roles in post-transcriptional regulation of target genes. MiRNAs are involved in cell proliferation, differentiation, apoptosis, inflammation, and cancer development. Alveolar bone resorption is the main clinical manifestation of periodontitis. Osteoclasts are unique cells regulated by osteoblasts and inflammatory cytokines and are responsible for bone resorption in periodontitis. Recently, miRNAs have emerged as an important regulator of osteoclast differentiation and function. In this study, we review the recent research progress on the effects of miRNAs on osteoclast differentiation and function, particularly the mechanisms of miRNAs-mediated osteoclast formation and bone loss in periodontitis.

Chinese Journal of Organ Transplantation 2003;0(05):-
Objective To develop an advanced two-cuff technique orthotopic liver transplantation model in rats. Methods On the basis of the previous two-cuff-technique, some hypercoagulable blood was released from the recipient's PV and IVC before the cuffed donor PV and IVC were inserted into the lumen of the recipient's PV and IVC. The anastomosis of recipient's SVC was between the donor's diaphragm venaccaval hiatus and recipient's SVC. Results Every anastomosis was effective. The operative successful rate and one-week survival rate were 96.7?% and 90.0?% respectively. Conclusion This model could be used as an effective measure for the study of liver transplantation in rats.

Objective To investigate the drug resistance of Acinetobacter baumannii strains isolated during 2014 in a hospital of Beijing. Methods Isolated bacteria were cultured by routine method, identified and performed drug susceptibility test by bacteria a-nalysis system.Statistical analysis of Acinetobacter baumannii was conducted by Whonet5.6 software. Results A total of 226 Acin-etobacter baumannii strains were isolated from January 2014 to December 2014 in Fengtai Teaching Hospital of Capital Medical U-niversity.These strains showed the lowest resistance rates to minocycline and Cefoperazone-sulbactam(23.0% and 30.1%).The other antimicrobial resistance rates were more than 50.0%.The resistance rate of Acinetobacter baumannii varied from one depart-ment to another.Pandrug-resistant and multi-drug resistant Acinetobacter baumannii were 71.2%,1 9.0% respectively. Conclusion The resistant rates of Acinetobacter baumannii strains to a variety of antibiotics are high.It is essential to strengthen monitoring the drug resistance of Acinetobacter baumannii,as well as use antibiotics reasonable and separate patients to control Acinetobacter baumannii infection.

Principles of medical equipment course were explored of formative evaluation as the center examination, constructed an examination reform scheme as” One center (formative evaluation as the center),three systems (content system, operation system, monitoring system).” Made students to learn from the center of focus scores change to autonomous learning and culture analysis, solves the question ability and practical ability. At the same time, strengthen the teaching process of teaching and learning to communicate and exchange,Get the students learning feedback information to improve teaching and learning in time,Strengthen the students′ ability of autonomous learning and lifelong learning ability.
9. Research progress on lipodermatosclerosis
Medical Journal of Chinese People's Liberation Army 2020;45(7):767-771
Lipodermatosclerosis (LDS) is a serious skin change associated with the progression of chronic venous insufficiency (CVI) of the lower extremities, and can be clinically divided into acute phase, subacute phase and chronic phase. The disease is common in middle-aged women, prone to occur in the lower leg 1/3, clinical manifestations of the skin flake erythema, induration, pigmentation, skin rough and thickening. The patients often suffer from secondary infection after scratching due to pruritus. The severe ones can form refractory ulcer or even cause skin necrosis, and may be canceration. At present, domestic reports on LDS are less, the clinical manifestations, etiology, pathogenesis and treatment of the LDS are reviewed in present paper, in order to strengthen the understanding of LDS and provide the evidence for clinical diagnosis and treatment of LDS.
