1.Clinical safety of preoperative lymphatic chemotherapy in the treatment of rectal cancer

Objective To investigate the clinical safety of preoperative lymphatic chemotherapy in the treatment of reetal cancer.Methods The regional and systemic symptoms,postoperatwe stoma healing,haematogenesis.functions of hean,liver and kidney after lymphatic chemotherapy,and the level of CD3+,CD4+,CD8+,CD4+/CD8+,CD(16+56)+in blood 30 minutes before and 48 hours after lymphatic chemotherapv were detected.Results There were no significant effects of lymphatic chemotherapy on the regional and systemic symptoms,postoperative stoma healing,haematogenesis and the functions of heart,liver and kidney.The level of CD4+/CD8+48 hours after lymphatic chemotherapy was significantly increased(t=7.145,P<0.05),while no significant changes of CD3+,CIM+,CD8+,CD(16+56)+were detected(t=1.782,1.151,1.184,0.955,P>0.05),when compared with those 30 minutes before lymphatic chemotherapy.Conclusions Preoperative lymphatic chemotherapy is safe and can enhance patients'immunity in early stage.
2.Research progress of treatment for amblyopia

The most common forms of treatment for amblyopia are occlusion therapy and pharmacologic penalization. But these methods can not recover all the visual deficits of amblyopes and there are some disadvantages of the treatment which need to be considered, including adverse effects, compliance and social stigma. Recently, some novel approaches are investigated for improving the effectiveness and compliance of treatment for amblyopia.
3.Evaluation of visual quality after overnight orthokeratology in pre-adolescent myopes

Background Orthokeratology has been proved to temporarily alter the equivalent sphere,but its effects on visual quality deserve attention.Objective The present study was to investigate the visual quality after overnight orthokeratology in pre-adolescent myopes.Methods Written informed consent was obtained form each subject prior to entering into this series.A descriptive study design was used.One hundred and fifty eyes of 76 teenagers aged (14.90± 1.24)years with low and moderate myopia (-2.79 ± 0.82)D were included in the study.Fitted with Ortho-K contact lens,the lens was wore every night for over 8 hours.Subjective refraction,uncorrected visual acuity (UCVA),contrast sensitivity function,corneal-topography,and aberrometry were examined before,1 week and 3 months after the initiation of orthokeratology.Visual quality was generally evaluated by comparing before wearing,1 week and 3 months after wearing using the National Eye Institute refractive error quality of life instrument (NEI-RQL-42TM).All the procedures were performed by the same clinician.Results Spherical equivalent refractions were (-0.33± 1.02) D and (-0.26 ± 0.60) D 1 week and 3 months after orthokeratology,showing significant decline in comparison with (-2.79±0.82) D of before orthokeratology (P =0.001,0.001).However,no considerable difference was seen between 1 week and 3 months after orthokeratology (P=0.161).Contrast sensitivity function was significantly different in all spatial frequencies before wearing,1 week and 3 months after wearing,and those of 1 week and 3 months after wearing were significantly lower than those of before wearing (1 week:3 cpd P =0.001,6 cpd P=0.001,12 cpd P<0.05,18 epd P<0.05 ;3 months:3 cpd P=0.001,6 cpd P=0.001,12 cpd P<0.05,18 cpd P<0.05).There was no significant change in contrast sensitivity function between 1 week and 3 months of orthokeratology (P>0.05).Flat K,steep K and corneal eccentricity (e) were significantly reduced (P =0.000),and surface asymmetry index (SAI) and surface regularity index (SRI) were significantly improved after orthokeratology in comparison with before orthokeratology (both P =0.001).Root mean square (RMS) of total higher-order,third-order and fourth-order aberrations were significantly increased (P<0.05),but RMS of whole aberration and second-order aberrations significantly decreased after orthokeratology (P< 0.05).There were no significant changes in the fifth-order,sixth-order and seventh-order aberration among pre-wear,1 week and 3 months after orthokeratology.Scales of dark to bright,nocturnal driving,glare,visual fluctuate and halo were lower than those before orthokeratology (P<0.05).The scales of visual fluctuate between 1 month and 3 months after orthokeratology had significant differences (P<0.01),while the rest of the scales had no significant differences (P>0.05).Conclusions Orthokeratology can reduce myopic diopter 1 week after orthokeratology.The overnight wearing of fitted orthokeratology can decrease contrast sensitivity and increase corneal surface irregularity and RMS of third-order and fourth-order.Compared with spectacles,orthokeratology reduces subjective visual quality,especially nocturnal quality.
4.Analysis of eight cases with perinatal pulmonary embolism

Objective To analyze the incidence,risk factors,clinical characteristics and pregnant outcomes of perinatal pulmonary embolism(PPE).Methods Clinical data of eight patients who were admitted to Beijing Friendship Hospital of Capital Medical University for PPE from January 2006 to March 2016 were collected.General condition,symptoms,laboratory examinations,images,treatments and outcomes of these patients were analyzed retrospectively.Results The ten-year incidence of PPE was 0.029% (8/27 560) in this hospital.Among the eight cases,two cases were diagnosed in the first trimester,and treated successfully by thrombolytic therapy.But one of two cases stopped growth,while the other one was premature labor.There were one case in the third trimester who had successful anticoagulant therapy and five cases in the postpartum period after cesarean delivery.Among the five cases,three cases were recovered after anticoagulant therapy,one case was recovered after thrombolytic therapy and one case died.All of the eight patients were immobilized before the onset of PPE,and five of them were diagnosed after cesarean section.Four out of the eight patients were obese.Five patients had three or more high-risk factors for pulmonary embolism and the other three had two.Conclusions It is necessary to pay close attention to gravidas who have two or more high-risk factors of PPE due to its fatal outcome.
5.Research progress of new isothermal amplification technology in rapid diagnosis of Plasmodium and other parasites

Abstract: At present, nucleic acid detection technology based on the PCR principle is commonly used to detect malaria parasites, the existing Plasmodium detection methods mainly include microscopy, antigen immunoassay, and nucleic acid detection,but due to the long detection time, high personnel and equipment requirements, and other shortcomings, its popularization, and application at the grassroots level are limited. What challenges previous Plasmodium detection methods are the lack of experienced professionals and advanced equipment at the grassroots as well as the requirement of rapid detection of large samples under extreme conditions. The isothermal amplification technology developed in recent years has potential application prospects due to its simplicity, rapidity, high sensitivity, and high specificity. This article attempts to review the principles, characteristics, and prospects of various isothermal amplification technologies, and on this basis, focuses on the introduction of recombinase polymerase amplification (RPA) and recombinase⁃aided isothermal amplification (RAA) assay technologies and proposes the use of such recombinant enzyme amplification technologies to achieve rapid and accurate diagnosis of common Plasmodium species possibility and imagination.
6.Comparison of therapeutic effects between lymphatic chemotherapy and regional extended release chemo-therapy for rectal cancer lymph node metastasis

Objective To compare the treatment effects between lymphatic chemotherapy and regional extended release chemotherapy (RERC) for rectal cancer lymph node metastasis. Methods The lymph nodes at or beside colon (first station), beside (second station) and at the root of mesentery blood vessel (third station) of the patients with rectal cancer in control group (20 cases), lymphatic chemotherapy group (20 cases) and RERC group (20 cases) were removed to compare proliferation index (PI), apoptotic index (AI) and AL/PI of cancer cells in the lymph nodes. Results The PI, AI, and AI/PI of cancer cells in correspondent lymph nodes were not statistically different between lymphatic chemotherapy group and the other 2 groups (F=4.973-7.394, 5.372-8.694, 4.527-5.436, P<0.05; t=3.128-7.688, 3.388-9.615, 6.518-13.180, P<0.05). The PI, AI, and AI/PI of cancer cells in the first and second station lymph nodes in RERC group were significantly different from those in control group (t=5.103, 4.927; 6.938, 6.450; 8.839, 9.021; P<0.05), and from those in the third station lymph nodes in RERC group (F=6.572, 8.964, 5.845, P<0.05; t=3.505, 3.353; 5.397, 4.701; 9.039, 8.629; P<0.05). Sorted by the treatment effects for cancer cells in lymph nodes, the first is each station of the lymphatic chemotherapy group, followed by the first and second station of the RERC group, and then the third station of the RERC group which was almost equal to any station of the control group. Conclusion Lymphatic chemotherapy is better than RERC in the treatment of rectal lymph node metastasis in the aspect of scale and degree.
7.Relationship between Sonic hedgehog signaling pathways and radioresistance of esophageal cancer

AIM:To investigate the potential role of Sonic hedgehog (Shh) signaling pathways in the radioresistance of esophageal cancer.METHODS:Radioresistant cell line Eca109R was established by repeating X-ray irradiation at dose of 60 Gy in total using Eca109 cells as parental cells.The radiosensitivity of the parental and radioresistant cells was confirmed by colony formation assay.The cell viability was detected by CCK-8 assay.The intracellular protein levels of Shh and Gli1 were determined by Western blot and immunofluorescence.RESULTS:The survival fractions at dose of 2 Gy for Eca109R cells and Eca109 cells were 0.937±0.013 and 0.499±0.042, respectively.The inhibitory rate of cell viability decreased gradually in the Eca109R cells (P<0.05), suggesting that the radioresistant cell line was successfully established.The results of Western blot indicated that the protein expression of Shh and Gli1 was much higher in the Eca109R cells than that in the Eca109 cells (P<0.05).Immunofluorescence staining showed that Gli1 was expressed in the cytoplasm and nucleus, and presented nuclear clustering in the Eca109R cells.The positive rate of Gil1 expression in Eca109 cells was 52.3%± 0.035%, while that in Eca109R cells was 87.6%±0.021% (P<0.05).CONCLUSION:The radioresistance of esophageal cancer may be related to the activation of Shh signaling pathways with over-expression of Gli1 and other related proteins.
8.Treatment of superior sulcus tumors using the improved antero-cervical parasternal approach

ObjectiveTo investigate the experience of resection of superior sulcus tumors using the improved antero-cervical parastemal approach.MethodsThe clinical data of 3 patients with superior sulcus tumor from July 2005 to May 2010 were analyzed retrospectively.Three patients with superior sulcus tumor underwent en bloc resection using the improved antero-cervical parastemal approach.The 1st to 3rd rib were excised,1 case with 1/5 centrum vertebra excision,1 case with the 1st transverse process of thoracic vertebra excision,1 case with the 1 st and 2nd transverse process of thoracic vertebra excision,1 case with T1 nerve root and sellate ganglion excision.ResultsAll the patients recovered well after operation.There was no serious complication except for 1 case with secondary Horner syndrome.The mild paradoxicalbreathing was found in 3 cases postoperative dressing change,but they had no dyspnea and tolerance well.The paradoxical breathing was disappeared when the mediastinum was fixed after 14 d thoracic wall pressure dressing.The pathological diagnosis after operation:2 cases with squamous-celled carcinoma,1 case with adenosquamous carcinoma,there was no mediastinal lymph node metastasis,cutting edge was negative.Stage:2 cases with T3N0M0,1 case with T4N0M0.Three cases were treated with chemotherapy (NP plan) for 4 cycles after operation without radiotherapy.Three cases were followed up for 53,37, 13 months after operation,they were all survival without recurrence and good quality life.ConclusionsFor the treatment of especially anterior and middle seated superior sulcus tumors,improved antero-cervical parasternal approach provides a safe and effective exposure.The improved antero-cervical parasternal approach is worthy of promotion.
9.Anti-infective reconstituted bone xenograft combined with external fixator for treatment of posttraumatic long bone infection in lower extremities

Objective To observe the long-term efficacy of anti-infective reconstituted bone xenograft (ARBX) combined with external fixation of posttraumatic long bone infection in lower extremities.Methods This retrospective case series study included 36 patients with posttraumatic long bone infection in lower extremities followed up for more than 18 months after receiving one-stage ARBX bone grafting combined with ring external fixation from January 2004 to December 2013.There were 21 male and 15 female patients,at the age of 19-72 years (mean,35.8 years).Multiple fractures were seen in 24 patients and single fractures in 12 patients.Bone and functional results were evaluated using the association for the study and application of the method of Ilizarov (ASAMI) classification.Results Follow-up ranged from 18 to 72 months (mean,38 months).Bone union was seen in 33 cases in a mean period of 5.2 months,and infection was completely cured.Length of limbs in all patients reached the expected extension length with the bone extension length of 3-10 cm (mean,5.2 cm).All the extended areas showed bone healing.According to the ASAMI classification,bone result was excellent in 20 patients,good in 13 and fair in 3,with the excellent and good rate of 92%,and limb function recovery was excellent in 18 patients,good in 11 and fair in 7,with the excellent and good rate of 81%.Conclusion ARBX adjuvant external fixator treatment of posttraumatic long bone infection in lower extremities improves bone healing rate and limb function recovery rate and shortens bone healing time at one stage.
