1.Clinical characteristics and prediction of Kawasaki disease unresponsive to initial dose of intravenous immunoglobulin

Objective To determine the prediction and clinical characteristics of intravenous immunoglobulin (IVIG) treated Kawasaki disease (KD) failure in initial dose.Methods Retrospective analysis was performed with the clinical data of 1 11 children with KD.The paticnts were divided into sensitive group and unresponsive group according to initial effect of IVIG.The clinical manifestations,laboratory examination and radiological features of the children were compared.Logistic regression analysis was performed in significant different indicators to determine independent correlation factors.In order to seek the reference indexes which indicate unresponsive to IVIG,a receiver operating characteristic (ROC) curve was made,of which the diagnostic cut-off was nine independent correlation factors while grouping was made according to patients' different responses to IVIG.Results (1)There were 90 cases (81.1%)in effective group and 21 cases (18.9%) in unresponsive group.(2) Compared with the sensitive group,hyperpyrexia cases[66.7% (14/ 21 cases) vs 34.4% (31/90 cases),x2 =7.334,P =0.007] and the chances of coronary artery lesions [47.6% (10/ 21 cases) vs 23.3% (21/90 cases),x2 =4.989,P =0.026] were significantly higher in the unresponsive group.(3)Compared with the children administered twice with gamma globulin,the children of single-dose treatment significantly reduced the unresponsive probability [12.5 % (9/72 cases) vs 30.8 % (12/39 cases),x2 =5.504,P =0.019],and there was no difference in the chances of coronary artery lesions[23.6% (17/72 cases) vs 30.8% (12/39 cases),x2 =0.672,P =0.412].(4)Comparing the sensitive group and the unresponsive group,the percentage of neutrophils count [(0.72 ± 0.06) vs (0.76 ± 0.04),t =-2.84,P =0.005],platelet counts [(352.38 ± 42.18) × 109/L vs (373.14 ±36.93) × 109/L,t =-2.076,P =0.040] and C-reactive protein (CRP) [(74.38 ± 12.92) mg/L vs (92.05 ± 11.17) mg/L,t =-5.780,P =0.000] were significantly higher in the unresponsive group,but the level of serum albumin[(34.09 ±3.19) g/L vs (31.61 ±2.03) g/L,t =4.442,P =0.000] was lower.(5) Multivariate Logistic regression analysis indicated that the percentage increase of neutrophils count (P =0.018),CRP (P =0.000) increase and serum albumin(P =0.040) decrease were independent risk factors for unresponsive treatment with gamma globulin.(6)After the area under the ROC curve was calculated,the percentage of neutrophils count,CRP and serum albumin could be effective predictors to IVIG treatment failure in initial dose,and the critical values were 0.72,78.5 mg/L and 33.11 g/L,respectively.Conclusions Treatment with IVIG for the first time may be ineffective in some situations such as the percentage of neutrophils count ≥0.72,CRP ≥78.5 mg/L or serum albumin≤33.11 g/L.

The prosthesis is forced by pneumatic artificial muscle, which can swing like human legs by inputting signals such as square waves and trigonal waves.Its swing performance is tested under such triggerings in order to find out a best motivating curve rendering the prosthesis performance almost the same as what human legs do. Compared with the normal human pace graph, the curve matches very well in the ascending edge but quite a discrepancy in the end of descending edge. According analyzing the reasons of the discrepancies, we could know how to ameliorate the design of the next generation of the bionic legs.
5.Kinematic analysis of simulated knee prosthesis

Knee joint is the largest and most complex joint of human body. Knee prosthesis, as the core component of lower limb, has a great expectation on simulated properties. Based on bionics, this study was designed to analyze kinematic characteristics of simulated knee joint, such as the sliding and rolling ratio of the knee as well as the instantaneous center curve, in order to further investigate an ideal simulated knee joint which was highly similar to normal knee joint. Movement regularity of human knee joint was simplified to introduce biomechanical model of knee joint so as to analyze and deduce formula of sliding ratio, rolling ratio, and instantaneous center curve. The calculation results were compared with parameters of human knee joint, suggesting a high coincidence between simulated and human knee joint. This provides an evidence for analyzing stability of motion and further studying structure and function of simulated knee joint, and this also provides basis for further research artificial simulated leg.
6.Pharmaceutical Care of Parenteral Nutrition Therapy in A Patient with Hyperemesis Gravidarum

Objective To explore the role of clinical pharmacists in nutrition support therapy in the patients with hyperemesis gravidarum. Methods The clinical pharmacist played a positive role in nutrition support care of a patient with hyperemesis gravidarum by analysising disease characteristics and adverse drug reactions, providing suggestion on the selection of fat emulsion and offering an individualized pharmaceutical care. Results The clinical pharmacist recognized the potential risk in nutrition support plan, took modifications timely, and prevented the occurrence of unfavorable clinical outcomes. Conclusion The participation of clinical pharmacists in nutrition support therapy of the patients with hyperemesis gravidarum is beneficial to improve the efficacy and safety of nutrition support and promote the rational use of drugs.

Liver and gastrointestinal tumors greatly threaten human health and have poor therapeutic outcomes and prognosis.Tumor necrosis factor -alpha -induced protein 8 -like 2 (TIPE2)can negatively regulate innate immunity and adaptive immunity and maintain immune ho-meostasis.Recent studies have found that TIPE2 can affect various signaling pathways and thus exerts an inhibitory effect on the development and progression of tumors.This article briefly introduces the structure and function of TIPE2 and its regulatory effect on tumor -related sig-naling pathways such as Ras,Ral,and Rac and downstream molecules,as well as the role of TIPE2 in liver and gastrointestinal tumors,re-lated signaling pathways,and research advances.

Background and purpose:Endometrial carcinoma is a common malignant tumour in women and expresses COX-2,CDK4;the expressions are associated with the development of the tumour. Mifepristone can inhibit the growth of the tumours, but its mechanisms are not very clear, We investigated the inhibitory effect of antiprogestins mifepristone n the growth of human endometrial carcinoma cell line HHUA xenografted in nude mice of and the expressions of COX-2,CDK4,the purpose was to find out whether or not mifepristone could influence the expressions of COX-2,CDK4. Methods:Human endometrial carcinoma HHUA cells were cultured in vitro.The models of xenografted tumor were established by the transplantation of human endometrial carcinoma HHUA cell on nude mice. The nude mice were randomly divided into two groups to receive either refined peanut oil (control group), or daily mifepristone for six weeks, respectively . The sizes of xenograft were measured in the pre-and post-treatment. The changes of morphology were observed by glass microscopy. 10.Clinical Effect of Interferon α-2 b on Scar Formation after Glaucoma Filtering Surgery

Objective:To investigate the preventive efficacy of interferon α -2b on scar formation after glaucoma filtering opera-tion. Methods:Totally 62 patients with primary glaucoma(68 eyes)were randomly divided into the observation group(31 cases with 36 eyes)and the control group(31 cases with 32 eyes). The patients in the observation group were given wet compress with a piece of cotton infiltrated by interferonα-2b,while the control group was given wet compress with a piece of cotton infiltrated by mitomycin C. After 4-month postoperative follow-up,the type of filtering bleb,complications and the changes of intraocular pressure were compared between the two groups. Results:The observation group was mainly with type II bleb(58. 33%),while the control group was mainly with type I bleb(53. 13%),and there was significant difference in the filtering bleb type between the two groups(P<0. 05). After the operation,the intraocular pressure at each time point in the two groups was decreased significantly when compared with that before the operation(P<0. 05),while the difference between the two groups was not statistically significant(P>0. 05). The percentage of visual invariableness or improvement in the observation group was significantly higher than that in the control group(P<0. 05),and the occurrence of complications after the operation in the observation group was significantly lower than that in the control group( P<0. 05). Conclusion:Interferon α-2b is an ideal drug for anti-scarring formation after glaucoma filtration surgery.
