2.Diagnostic value of HbA1C and fasting plasma glucose as early screening parameters for diabetes mellitus

The validity of HbA1C and fasting plasma glucose(FPG)for screening of diabetes in community population was investigated.A total of 1 794 subjects from two sub-districts of Yangpu District,Shanghai,underwent a 75 goral glucose tolerance test(OGTT)and HbA1C determination.Based on 1999 World Health Organization criteria,there were 1 411 subjects with normal glucose tolerance(NGT),111 impaired fasting glucose(IFG),73 impaired glucose tolerance(IGT),33 IFG+IGT,and 166 diabetes.According to the receiver operating characteristic curve,the optimal cut-point of FPG for diagnosing diabetes was 6.15 mmol/L with sensitivity of 89.0%,specificity of 92.8%,and area under the curve of 0.959.The cut-point of HbA1C for diagnosing diabetes was 6.5% with a sensitivity and specificity of 71.1% and 80.2%.The area under the curve was 0.822.The screening model using FPG ≥6.1mmol/L and HbA1C≥6.5% had sensitivity of 66.9% and specificity of 97.0%.When the model was FPG ≥6.1 mmol/L or HbA1C ≥6.5%,the sensitivity and specificity was 96.3% and 76.7% respectively.The results suggest that both FPG and HbA1C have good value for screening diabetes in community,and FPG combined with HbA1C may further promote diagnostic efficacy.
3.Molecular characteristics of Salmonella typhi and Salmonella paratyphi A isolates in Hangzhou area

Objective To determine the molecular characteristics of predominant Salmonella typhi and Salmonella paratyphi A strains prevalent in Hangzhou area from 2002 to 2008.Methods Pulse field gel electrophoresis (PFGE),multi-locus variable-number tandem repeat analysis (MLVA) and multi-locus sequence typing (MLST) were applied for typing as well as analysis of the molecular characteristics of 31 S.typhi isolates and 404 S.paratyphi A isolates from Hangzhou area during 2002 to 2008.Results The 404 S.paratyphi A isolates could be divided into six PFGE types (P1-P6).99.0% of the S.paratyphi A isolates (400/404) belonged to the same one clone family (P1 and P2 types),in which P1 strains occupied 93.3% (373/400) of the isolates.The 31 S.typhi isolates displayed a high diversity,which could be classified into 14 PFGE types,28 MLVA types with 90.3% resolving power and 3 MLST types.The S.typhi strains prevalent in Hangzhou area were similar to those in Southeast Asia but different from those in Europe.The variable number tandem repeat (VNTR) sites with high polymorphism,TR1,TR2 and Sal02,could be used to the markers for diagnosis of S.typhi isolates in the area.The MLST types of 31 S.typhi isolates included all the three types currently found in the world but the ST2 type of S.typhi strains was predominant (23/31,74.2%).Conclusion The paratyphoid A prevalence in Hangzhou area in the recent years is caused by infection of the same clone family of S.paratyphi A whereas the S.typhi strains prevalent in the area display a high diversity.
4.Cost-effectiveness Analysis of Xueshuantong for Injection versus Ginkgo Leaf Extract and Dipyridamole In-jection in the Treatment of Ischemic Stroke

OBJECTIVE:To compare pharmacoeconomic and effect of Xueshuantong for injection and Ginkgo leaf extract and dipyridamole injection in the treatment of ischemic stroke. METHODS:Retrospective study was conducted. Totally 404 inpatients with ischemic stroke were divided into Xueshuantong group(271 cases)and ginkgo leaf extract and dipyridamole group(133 cas-es) according to clinical treatment programs. Based on the conventional treatment,patients in 2 groups were given Xueshuantong for injection and ginkgo leaf extract and dipyridamole injection,respectively. The average treatment course was 10 d. Cost-minimi-zation analysis was performed with the determination index of total effective rate. RESULTS:The total effective rates in Xueshuan-tong group and ginkgo leaf extract and dipyridamole group were 90.77% and 88.72%,respectively,the difference was not statisti-cally significant(P>0.05). The costs in 2 groups were 12 860.21 yuan and 13 155.40 yuan,respectively,and xueshuantong group had lower than ginkgo leaf extract and dipyridamde group. CONCLUSIONS:Both Xueshuantong for injection and Ginkgo leaf ex-tract and dipyridamole injection are effective in the treatment of ischemic stroke. However,the economy of Xueshuantong for injec-tion is superior to the other one.
5.Molecular characterization of a family with beta-thalassemia intermedia

Objective To analyze a rare genotype with β-thalassemia intermadia.Methods Phenotypic analysis was performed using routine hematological tests to measure red blood cell parameters and high performance liquid chromatography (HPLC)was used to measure hemoglobin fractions.The β-globin gene mutations were conducted using reverse-dot-blot and DNA sequencing of the breakpoint region were characterized with gap-PCR.Results The proband had a trait of thalassemia intermedia with reduced hemoglobin (86 g/L).The proband's father had a trait of microcytic hypochromic with reduced mean corpuscular volume(MCV),mean corpuscular hemoglobin (MCH) (63.7 fl and 20.4 pg,respectively) and an elevated level of HbA2.He had the phenotype of heterozygosity for β-thalassemia.The preband's mother,grandmother and sister had a trait of heterezygote for hereditary persistence of fetal hemoglobin (HPFH) with elevated level of HbF,which were 28.3%,21.1% and 33.7%,respectively.After molecular characterization of the family members,the proband was identified as a patient with β-thalassemia intermedia caused by co-existence of β-thalassemia(β41-42) and HPFH-6.The father was heterozygoas for β-thaiassemia (β41-42/βN) and the mother,grandmother and sister were all heterozygons for HPFH-6.Condusions A rare β-thalassemia intermedia case resulting from compound heterezygote of β41-42 with HPFH-6 is found.This study may provide clinical experiences for antenatal diagnosis.
6.Effect of metabolic syndrome in the bone mineral density in elderly male type 2 diabetics

Objective To explore the impact of metabolic syndrome (MS)in bone mineral density (BMD) in elderly male type 2 diabetics (T2DM). Methods One hundred and fourty cases were divided into MS group and non-MS(NMS)group according to with or without MS. Height,weight were measured, body mass index(BMI) were calculated and the total cholesterol (TC), triglyceride (TG), high-density lipoprotein cholesterol (HDL-C), low density lipoprotein cholesterol (LDL-C), glycated hemoglobin (HbA1c)were tested. The bone mineral density (BMD) was measured by dual-energy X-ray absorptiometry (DEXA). The differences among these groups were compared and the risk factors for MS were analyzed. Results The MD, BMI, TC,TG in MS group was higher than that in NMS group (P < 0.05 vs. P < 0.01),and HDL-C was lower than NMS group (P < 0.05). Correlation analysis showed that BMD was negatively correlated with age , TG , HDL-C , and was positively correlated with BMI.The results of multiple stepwise regression analysis showed that age ,HDL-C,BMI were independent risk factors for BMD. Conclusion The BMD significantly increases in T2DM with MS.Obesity and low HDL-C have positive effects in bone mass.
7.Literature Analysis on Adverse Drug Reactions of Xueshuantong Injection and Analogous Analysis of Adverse Drug Reactions in Lanzhou Region

Objective To review the occurrence and relevant factors of adverse drug reactions (ADR) of Xueshuantong Injection. Methods Articles and documents in CNKI, VIP, and CBM were searched in June 2014 according to incorporation and exclusion standard. The dose, indication, medicating path and method, solvent, as well as the duration of treatment course and adverse reaction of Xueshuantong Injection were analyzed. The national information system for monitoring ADR was searched to collect adverse reaction cases of Xueshuantong Injection (2004.9-2014.9) reported in Lanzhou region. Cases were analyzed and under analogy with literature results. Results Totally 66 articles involving 4686 patients were included (except for patients of control group). Adverse reactions occurred in 767 patients, including skin damage (402 cases), systemic damage (221 cases), gastrointestinal system damage (75 cases). All of these were relieved after treatment. There were 11 adverse reaction cases of Xueshuantong Injection from Lanzhou region reported in the national information system for monitoring ADR. Conclusion There is a high incidence of adverse reactions in the clinical application of Xueshuantong Injection and ratio of serious adverse events report.
9.Effect of obstetrics outpatient process reforming on the clinic time and satisfaction degree of pregnant women

Objective To study the clinic time and patient satisfaction degree after implemented outpatient process reforming so as to improve the quality of nursing.Methods The questionnaire investigation was used,and 246 pregnant women before outpatient process reforming implemented and 283 pregnant women after it implemented were recruited and investigated.The clinic time and satisfaction degree of pregnant women were compared before and after it implemented.Results After the outpatient process reformed,the clinic time and satisfaction degree of pregnant women were ( 156.53 ± 9.23 ) min and 79.86%,and that of before it implemented were( 180.82 ± 8.86 ) min and 67.07%,and the differences were statistically significant ( t =16.87,x2 =68.48;P < 0.05).Conclusions Outpatient process reformed can reduce the clinic time of pregnant women and improve their satisfaction degree,and provide high-quality nursing service and indicate the principle of patient-centered.
