1.Effect of single and low dose propofol intravenously on cardiovascular and adverse response during extubation in patients undergoing ovarian cancer operation

Objective To observe the effect of single and low dose propofol administered near the end of surgery on the cardiovascular and adverse response during extnhation in patients undergoing ovarian cancer operation. Methods Fifty ASA Ⅰ - Ⅱ patients undergoing selective ovarian cancer operation were randomly allocated into group S and group P,with 25 patients in each group. In group S, inhalation of sevoflurane was discontinued at about 2 minutes before the end of operation. In group P, inhalation of sevoflurane was discontinued at about 5 minutes before the end of operation,and at the same time,0.5 mg/kg of propofol was administered intravenously. The systolic blood pressure (SBP),diastolic blood pressure (DBP),heart rate (HR) and recovery time of consciousness were recorded at 5 min before extubation (t_1),during extubation (t_2), 1 min after extubation (t_3), and 5 rain after extubation (t_4). The incidence of agitatian,bucking,nausea and vomiting were recorded by an observer who was blind to this study. Results There were statistical differences in HR,SBP,DBP at t_2-t_4 between two groups. The recovery time was (10.39 ± 1.26) min in group P,and(9.57 ± 1.25) min in group S,there was significant difference between group P (2 cases) and group S (9 cases)(P< 0.05). There was significant difference in the incidence of agitation between two groups (P< 0.05). Conclusion Low dose propofol before extubation can inhibit stress and cardiovascular response and decrease the incidence of agitation, bucking, nausea and vomiting.
2.Protective effects of G CSF on lungs in rats with acute cholangitis

Objective To investigate the protective effects of G CSF on lungs of rats with acute cholangitis and changes in blood TNF and TXA 2/PGI 2 by treating the rats with G CSF. Methods Seventy two Wistar rats were randomized into the control, infected and treated groups. We established the model of cholangitis in the rats with injection of E. coli 25922 into the bile duct. The animals in the treated group were pretreated with G CSF 500 ng/day bid) for 5 days before the E. coli injection. The levels of blood TNF and TXA 2/PGI 2 were measured at the 3rd, 6th and 12th h after the injection. Meanwhile, the morphological changes in lungs of the rats were observed. 3.Expression and biological significance of COX-2, PCNA, CD44v6 protein in different procancerous lesion and esophageal squamous cell carcinoma tissue

Objective To examine the expression of COX-2, PCNA, CD44v6 protein in different procancerous lesion and esophageal squamous cell carcinoma tissue, and to investigate the relationship between their expression and the biological level,lymph node metastasis in esophageal squamous cell carcinoma tissue. Methods Immunohistochemistry was demonstrated to show these tumor markers expression in different procancerous lesions and esophageal squamous cell carcinoma tissue. Results Stepwise over-expression of COX-2, CD44v6, PCNA protein were observed in the multi-step process of esophageal tissue. Significant difference in expression of these tumor markers(P <0.05) was observed between normal esophageal epithelia and dysplasia. The expression of COX-2 (P <0.05) had statistical different in biological level and lymph node metastasis.The expression of CD44v6 (P <0.05)had statistical different in lymph node metastasis. The relationship between COX-2 and CD44v6 was statistical significant in esophageal squamous cell carcinoma tissue(P <0.01). Conclusion Stepwise over-expression of COX-2,CD44v6 PCNA protein have relation with esophageal squamous cell carcinoma. The overexpression of COX-2 and CD44v6 have effect on prognosis of esophageal squamous cell carcinoma. The expression of COX-2 have relationship with that of CD44v6 in esophageal squamous cell carcinoma.
4.Clinical study of hyperthermic peritoneal perfusion with oxaliplatin and interleukin-2 combined with chemotherapy for treatment on malignant ascites in eider patients

Objective To evaluate the effects and toxicities of the hyperthermic peritoneal peffusion with oxaliplatin (L-OHP) and interleukin-2 (IL-2) combined with chemotherapy for treatment malignant asci-tes. Methods 42 patients with malignant aacites from assimilation system tumor were catheterized,drained and flushed with L-OHP 85 mg/m2 in 2 000 ~2 500 ml of 5 % glucose and IL-2(2 MU),NRL-001 Double RF tumor hyperthermia system was applied to heat the abdominal part for 60 ~ 120 minutes at an intraperitoneal temperature of 41 -43 ℃ ,next day,all the patients were treated with calcium folinate(CF)0. 2 g/m2 by 2 hours intravenous infusion,5-fluorouracil (5-FU) 0. 4 g/m2 intravenously, followed by 46 hours continuous infusion of 5-FU (1 600 mg/m2), 2 weeks a cycle. Results The overall response rate was 66. 67%, including CR : 6 cases, PR:22 cases, NC:6 cases, and PD:8 cases. MST is 5.6 months. The main toxicity was impatient abdomen ache,paralysis intestinal obstruction. Conclusion Hyperthermic peritoneal perfusion with oxaliplatin and inter-leukin-2 combined with chemotherapy is efficient and safe in the treatment malignant ascites and can improve the quality of life. It is worthing wildly using and research.
5.Effects of preoperative intravenous different doses of pantoprazole on acidity of gastric fluid

Objective To investigate the effects of preoperative iv different doses of pantoprazole on acidity of gastric fluid in patients undergoing elective surgery. Methods Sixty ASA Ⅰ -Ⅲ patients aged 38-80 yr,weighing 44-72 kg undergoing elective surgery were randomly allocated into 3 groups (n = 20 each): group Ⅰ control (group C); group Ⅱ pantoprazole 40 mg(group P1) and group Ⅲ pantoprazole 80 mg(group P2). The patients received intravenous normal saline (NS) 100 ml, pantoprazole 40 and 80 mg in NS 100 ml respectively in group C, P1 and P2 at 30 min before surgery. Gastric content was aspirated using a gastric tube on the patients'arrival in the operating room and at 1,2 h and the end of surgery. The pH of the gastric fluid was measured. The adverse effects including dizziness, diarrhea, PONV and incidence of stress ulcer were recorded. Results The pH was significantly increased in group P1 and P2 as compared with control group but there was no significant difference in pH between group P1 and P2 . There were no significant difference in the incidence of stress ulcer and pH of the gastric fluid≤2.5 among the 3 groups. Conclusion Preoperative intravenous pantoprazole 40 mg can effectively increase pH of gastric juice and is helpful in reducing the incidence of stress ulcer and the risk of acid aspiration.
6.Efficacy Comparison of Xiaoshi Lidan Capsules and Ursodeoxycholic Acid Capsules in the Treatment of Chronic Cholesterol Gallstone Cholecystitis

OBJECTIVE:To compare clinical efficacy of Xiaoshi lidan capsules and Ursodeoxycholic acid capsules in the treat-ment of chronic cholesterol gallstone cholecystitis. METHODS:120 patients with chronic cholesterol gallstone cholecystitis in our hospital were selected and divided into observation group and control group according to random number table,with 60 cases in each group. Observation group was given Xiaoshi lidan capsules 1.2 g,po,tid(after the meal);control group was given Ursodeoxycholic acid capsules 250 mg,po,qd(after dinner). Both group received treatment for 6 months. Effective rate of litholysis were observed in 2 groups as well as abdominal pain score [PRI,VAS score,present pain intensity(PPI)],the thickness of gallbladder wall before and after treatment. Clinical efficacy and the occurrence of ADR were compared between 2 groups during treatment. RESULTS:3 pa-tients withdrew from the observation group and 1 patient withdrew from the control group. Before treatment,there was no statistical significance in PRI,VAS score,PPI and the thickness of gallbladder wall between 2 groups(P>0.05). After treatment,above index-es of 2 groups were decreased significantly,while PRI,VAS score and PPI in observation group was significantly lower than in con-trol group,with statistical significance (P<0.05). The effective rate of litholysis (64.91%) in observation group was significantly lower than in control group (79.67%),with statistical significance (P<0.05). Total effective rate of observation group (57.89%) was slightly higher than that of control group(54.24%),without statistical significance(P>0.05). There was no statistical signifi-cance in the incidence of ADR between 2 groups(P>0.05). CONCLUSIONS:Xiaoshi lidan capsules are similar to Ursodeoxycholic acid capsules in clinical efficacy for chronic cholesterol gallstone cholecystitis with good safety,and can be used as optional drug ex-cept for chronic cholesterol gallstone cholecystitis.
7.Application of systematic health education in the ultrasound guided interventional therapy for patients with cysts

Objective To investigate the effect of systematic health education on the ultrasound guided interventional therapy for pa -tients with cysts.Methods Totally 181 patients with cysts who received the ultrasound guided interventional therapy in our hospital ,were randomly divided into observation group and control group .The patients in the observation group received systematic health education ,while patients in control group received convention health education .Results The differences of pain score between two groups were statistical significance(P<0.05).By health education,the patients in the observation group had a deeper knowledge of the operation and became more cooperative(P<0.05).Conclusion Systematic health education could alleviate the pain and improve the psychological indisposition reac -tions of patients,and enhance therapeutic compliance of patients with ultrasound guided interventional therapy .In the meantime,systematic health education could improve the quality of work and enhance the diathesis of paramedic .

9.CT diagnosis of cardiac rhabdomyoma in childhood

Objective To investigate the CT findings of cardiac rhabdomyoma in childhood. Methods Five children with cardiac rhabdomyoma confirmed by operation was retrospectively analysed. Enhanced electron beam CT (EBCT) was performed in 4 children and enhanced 64-slice helical CT (MSCT) scan was performed in 1 infant. Three dimensional reconstruction was applied for the full display of the lesions based on the transverse section images. Results ( 1 ) Multiple masses or nodules originated from atrioventricular septum were observed in 4 children including 1 nodule from the anterior walls of the left and right ventricles. A mass was derived from the mitral valve in 1 infant (2) The CT value of the lesions after enhancement ranged from 80 to 180 HU and was similar to neighbouring left ventricular myocardium. (3) The ventricular outlet obstruction was found in 3 children including 1 infant with pericardial effusion and pulmonary inflamation of the right lower lobe. Conclusion The rhabdomyoma should be considered first when masses or nodules originated from atrioventricular septum and with the enhancement similar to neigbouring left ventricular myocardium in children.
