1.Spinal cord injury and MicroRNA:in vitro prefabrication of tissue-engineered spinal cord and repair of spinal cord injury

BACKGROUND:In the whole world, spinal cord injuries caused by trauma lead to more than 180 000 people presenting with permanent impairment annualy. A large number of experiments have confirmed in recent years, under physiological conditions, microRNA has specific expression and plays an important role in the nervous system. OBJECTIVE: To discuss the changes in microRNA expression induced by injuries as wel as the pathophysiological significance in spinal cord injury, and to explore the development potential of microRNA in tissue-engineered and clinical repair of spinal cord injury. METHODS:A computer-based search of PubMed and Chinese Journal Database was performed for related articles published from January 2000 to December 2014 using the keywords of “SCI, microRNA, transcriptional control, clinical research progress” in English and Chinese. Finaly, 38 articles were included for result analysis. RESULTS AND CONCLUSION: Mechanical injury initialy triggers a series of complex secondary damages, including nervous, vascular and immune systems, which can influence the severity of spinal cord injury to a great extent. Secondary damage to the spinal cord is mainly attributed to the activation and deactivation of some specific genes associated with celular and biochemical mechanisms, such as cysteine aspartate specific protease (caspase) gene family, apoptosis related protein Fas and its ligand Fasl system, P53 gene, apoptosis related gene Bcl-2 family. Recent studies have proved that the functional activation of microRNA expression is the key to spinal cord injury. With the development of biological information engineering, studies and controling technologies associated with microRNA expression have been gradualy dominated, some clinical application based on microRNA technology has entered the clinical trial stage. It is believed that with the continuous development of technology and decrease of cost, permanent dysfunction due to spinal cord injury can be regulated and repaired through the microRNA technology at gene level in the future.
2.Effect of hyperbaric oxygen on differentiation and Wnt3 expression of bone marrow mesenchymal stem cells

BACKGROUND:Hyperbaric oxygen(HBO)treatment promotes the proliferation and differentiation of endogenous neural stem cells in neonatal rats following hypoxic/ischemic brain damage(HIBD).The Wnt signaling pathway is associated with neurogenesis.However,there are few data recording the role of HBO in the differentiation of neural stem cells in vitro.OBJECTIVE:To observe the effect of HBO on differentiation and Wnt3 expression of bone marrow mesenchymal stem cells(BMSCs).METHODS:BMSCs were isoiated and cultured.The rat BMSCs of passages 3-5 were cultured in DMEM/F12(1:1)medium with basic fibroblast growth factor,epidermal growth factor and B27 for 24 hours.The induced BMSCs were randomly divided into two groups:control group(no treatment)and HBO group(HBO,0.10 MPa,60 minutes stabilizing pressure with at least 90% oxygen).The neuron specific encloase(NSE),glial fibrillary acidic protein(GFAP)and 04 marked oligodendrocyte immunocytochemistry were detected by immunofluorescent staining,and Wnt3 protein expression was detected by Western-blot.RESULTS AND CONCLUSION:BMSCs cultured in classic medium of neural stem cells could significantly induce the expression of nestin.The expression of NSE and 04 of HBO group was greater than control group(P<0.01),but GFAP expression displayed no significant difference between the groups(P>0.05).Western blot showed HBO could enhance the Writ3 expression (P<0.05).Results show that HBO can induce BMSCs to differentiate into neural cells and oligodendrocyte,which is correlated with the activation of the Wnt3 protein.
3.The role and clinical significance of follicular helper T cells and interleukin-21, CXCL13 in patients with ankylosing spondylitis

Objective To test the level of follicular helper T cells (Tfh) and interleukin (IL)-21,CXCL13 in the peripheral blood of patients with ankylosing spondylitis (AS),and to analyze the relationship between Tfh and clinic features and explore the possible immunological pathogenesis of AS.Methods The Tfh cells were obtained from patients and normal controls and detected by flow cytometry.While the levels of IL-21,CXCL13 in patients and normal controls were measured by enzyme-linked immunosorbent assay (ELISA) tests.Data analysis were performed by Student's t-test,Rank-sum test,Spearman's correlation test.Results The expression of CD4+CXCR5 qCOS + cells (Tfh) (mean rank 33.71) and IL-21 [(299±27) ng/L],CXCL13 [(5.8±1.0) μg/L] in the peripheral blood of AS was significantly higher than normal controls [mean rank 23.54,(176±26) ng/L,(4.2±0.8) μg/L] (Z=-2.258,t=17.221,t=6.464,all P<0.05).It was similar in AS with peripheral joint involvement compared with AS of non-peripheral joint involvement,and there was no difference between AS patients with positive HLA-B27 and those without HLA-B27.Mean -while,no correlation was found between the expression of Tfh,IL-21,CXCL13 and level of ESR,CRP,BASDAI.And there was no significant correlation between the expression of Tfh and IL-21,CXCL13 (P>0.05).Conclusion The expression of Tfh and the levels of IL-21,CXCL13 are increased significantly,but are not closely relatedto disease activity.These results indicate that the abnormality of Tfh may play an important role in the pathogenesis of AS.
4.Effects of remifentanil pretreatment on hemorrhagic shock-induced acute liver injury in rabbits

Objective To investigate the effects of pretreatment with remifentanil on hemorrhagic shock (HS)-induced acute liver injury in rabbits.Methods Thirty-two New Zealand white rabbits,weighing 2.0-2.5 kg,were randomly divided into 4 groups (n=8 each): sham operation group (group S) ; group HS,low-dose remifentanil group (group RL) ; high-dose remifentanil group (group RH).Remifentanil was infused at 0.66 and 1.32 μg· kg-1 · min-1 for 145 min in groups RL and RH respectively,while the equal volume of normal saline was infused in group C.HS was induced by withdrawing blood from the left femoral artery at 15 min after continuous infusion of normal saline or remifentanil and mean arterial pressure (MAP) was reduced to 40 mm Hg.MAP was reduced to 35-45 mm Hg and maintained at this level for 120 min in groups HS,RL and RH.Blood samples were taken for determination of serum alanine aminotransferase (ALT) and aspartate aminotransferase (AST) activities before administration (T0),immediately before blood letting (T1),and at 10,30,60 and 90 min after blood letting (T3-6).The animals were then sacrificed and the livers were immediately removed for microscopic examination.Results Compared with group S,the serum ALT activity at T5 and T6 and serum AST activity at T4-T6 were significantly increased in the other three groups (P < 0.05).Compared with group HS,the serum ALT activity at T5 and T6,and serum AST activity at T4-T6 were significantly decreased in group RH,and no significant change was found in group RL (P > 0.05).Compared with group RL,the serum ALT activity at T5 and T6,and serum AST activity at T4-T6 were significantly decreased in group RH (P < 0.05).The serum ALT activity at T5 and T6 and serum AST activity at T4-T6 were significantly increased in groups HS,R and RH than those at T0 (P < 0.05 or 0.01).The patho1ogical injury was attenuated in group RH compared with groups HS and RL.Conclusion Remifentanil pretreatment can attenuate HS-induced acute liver injury in rabbits,and the effect is related to the dose.
5.Development and application of portable integral dental unit in ship

Objective To develop a portable integral dental unit in the ship to fulfill medical service during canvoy operation.Methods The unit developed was modified based on WD6232-E dental unit,and comprised of a chair and accessories.An earth box and a seat box were under the chair,and a base was at one side of it.The accessories included a level bar,emesis basin,cold light and etc.The digital unit was modified,which involved in a miniature digital dental X-ray machine and a micro computer.Results The dental unit developed could fulfill dental auxiliary diagnosis and treatment similar to them in garrison hospital.Conclusion The unit gains advantages in portability and easy operation and adapts itself to dental service during expedition,and thus is worthy promoting practically.
6.Clinicopathological features and treatment of postsurgical incidental papillary thyroid microcarcinoma

Objective To analyze clinicopathologic features and treatment strategy of postsurgical incidental papillary thyroid microcarcinoma(PTMC).Methods A total of 305 patients undergoing surgery for thyroid diseases from Apr.2012 to Mar.2013 were retrospectively analyzed.Results Among the 305 patients,19 patients(18 females and 1 male)showed PTMC incidentally after surgery.The mean age was (43.2 ± 12.8)years (ranging from 23 to 67 years old).All patients underwent subtotal thyroidectomy,including 17 endoscopic surgery and 2 open surgery.All frozen sections were benign during operation while pathologic results were PTMC after surgery.The average size of tumors was(2.2 ± 1.5) mm which all was unicentric with no lymph node enlargement intraoperatively.Immunohistochemistry showed that the positive rate of cytokeratin 19,galectin-3,mesothelial cell,and CD56 was 87.5%,43.8%,81.3% and 18.8%,respectively.There were 4 cases of B-raf V600E mutation found in 6 patients.All patients discharged 1 to 3 days after surgical treatment with no complication and received levothyroxine therapy with a suppressive dose of thyrotropin(TSH).131I therapy was not given.Conclusions PTMC is found more common in female and younger age under 45 years.Tumor is usually unicentric and less than 5 mm.Lymph node metastasis is rare.Immunohistochemistry and B-raf V600E mutant test can help the diagnosis.Initial endoscopic subtotal thyroidectomy is effective.There is no need of further surgery and 131I therapy.
7.A Processing Method of EEG Signals Based on Second Generation Wavelet Transform and Blind Signal Separation

Objective To study a processing method for EEG signals mixed with EOG and ECG signals disturbance.Methods First,the EEG was denoised by the hard threshold method,the soft threshold method,the compromise threshold method and the ? law threshold method in the second generation wavelet,and then the denoised EEG which still contained EOG and ECG was separated by fast independent component analysis( FastICA) algorithm.Results The ? law threshold method of the second generation wavelet had better denoising effect and FastICA algorithm had more ideal separate performance.Conclusion It is an effective preprocessing method for EEG in denoising with the ? law threshold method of the second generation wavelet and then in separating disturbance of independent source with FastICA algorithm.
8.Changes and Significances of Serum Cystatin C and Transforming Growth Factor-β1 Levels in the Neonatal Asphyxia

Objective:To investigate the changes and significances of serum cystatin C and transforming growth factor-β1 levels for the neonatal asphyxia.Methods:Forty-six asphyxia newborns were chosen as the asphyxia group,and thirty healthy newborns were selected as the control group.The TGF-β1,CysC,BUN,Scr,and GFR levels of both groups were detected on the 1st,3rd,7th day after hospitalization.According to the renal injury,the 46 newborns were divided into normal group and asphyxia group,and the serum indexes were detected and analyzed.Results:On the 1st,3rd,7th day after hospitalization,the TGF-β1,GFR of asphyxia group was obviously increased and was lower than those of the control group (P<0.05);the level of CysC,BUN,Scr in both groups were decreased,and the change degree in asphyxia group were higher than that of the control group (P<0.05);the CysC,BUN,Scr in renal injured group were higher than those of normal group,and TGF-β1,GFR were much lower (P<0.05).Additionally,TGF-β1 level of renal injured group was negatively correlated to the BUN and Scr,and positively correlated with the GFR (P<0.05).The level of serum CysC in renal injured group was positively correlated to BUN and Scr and negatively correlated to GFR (P<0.05).Conclusion:The serum TGF-β1,CysC in asphyxia newborns had significant changes compared with the healthy newborns and was correlated to the renal injured indexes,which had clinical directive significance on the early diagnosis,condition judgment,and prognosis of neonatal asphyxia with renal injury.
9. UPLC fingerprint-based analysis of differences in chemical components from different parts of Stephania tetrandra

Objective: To analyze and compare UPLC fingerprints of root, rhizome, stem, and leaf of Stephania tetrandra, learn the differences in chemical component types and contents of main active components, and provide basis for rational development and utilization of S. tetrandra. Methods: UPLC was used to obtain characteristic chromatograms of different parts; The Similarity Evaluation System for Chromatographic Fingerprints of Traditional Chinese Medicine (Version 2012) was run to capture the common peaks of different parts and calculate their similarity and analyze the characteristic peaks of different parts. SPSS 23.0 was run to compare the difference in component contents of the roots and rhizomes using the paired sample t-test. Results: The similarity in chemical composition between root and rhizome was 0.928, indicating they have similar chemical composition, and both of them contained tetrandrine and fangchinoline, the index components. The similarity between rhizome and leaf was 0.947; The similarity was low between stem, leaf and root and rhizome, and there were no tetrandrine and fangchinoline in the first two parts. The results of paired samples t-test show that the total content of chemical components in rhizome was higher than that in roots, and the mainly difference came from other non-index components, but there was no significant difference between tetrandrine and fangchinoline. Conclusion: Significant differences are present in chemical composition types and contents of different medicinal parts of S. tetrandra; The type of chemical components in rhizome is similar to that in root, and the content of some components in rhizome is significantly higher than that in root, which means that rhizomes can be used as an equivalent of roots. Stems and leaves cannot be a substitute for roots because they do not contain tetrandrine and fangchinoline, but they contain many other chemical components which can be utilized as a new resource.
10.Safety of treating cervical spondylosis with rotational manipulation

ObjectiveTo explore the clinical therapeutic effect and safety of treating the vertebral artery type cervical spondylosis with rotational manipulation.Methods180 patients with vertebral artery type cervical spondylosis were treated by rotational manipulation.ResultsBy follow-up, 89 patients were evaluated as excellent, 73 as good and 18 as fair. The total rate of excellent and good results was 81%.ConclusionRotational manipulation is an effective method. The correct diagnosis and indication are keys to take good treatment results and safety.
