1.Diagnosis,prevention and therapy of primary osteoporosis

Osteoporosis is a skeletal disorder characterized by compromised bone strength predisposing a person to an increased risk of fracture.Bone strength primarily reflects the integration of bone density and bone quality.Here the current status of diagnosis,prevention and treatment on osteoporosis is introduced briefly.The diagnosis based on the criteria of World Health Orgnization by bone mineral density measurement(Dual-energy X- ray absorptiometry).Prevention is of the primary importance,pay more attention to the education to people and medical doctors for correcting their misleading concept.There are different opinions about intervention threshold which has to be investigated further.More progress has been made on osteoporosis therapy,according to the feature of illness,evidence-based medicine,analysis on reducing fracture risk and cost-effectiveness help to determine the drug selection.

Vitamin D deficiency and insufficiency are the global problem.It can induce muscle weakness,falls and fracture due to both osteoporosis and falls.Supplementation of vitamin D and calcium are effective prevention measures for falls,osteoporosis and fractures.It has become important to identify the optimal serum level of 25(OH)D and the suitable amount of vitamin D intake in Chinese.

8.Treatment of total knee replacement following failed high tibial osteotomy

Objective To investigate the effect of total knee arthroplasty ( TKA) following high tibial osteotomy ( HTO) and to analyze the factors that may influence the operation .Methods A total of 16 patients (19 knees) who had undergone a previous HTO were treated with TKA between 2000 and 2010.The American Knee Society Scores ( KSS), femorotibial angle ,quadriceps angle ,posterior slope angle of tibial plateau ,Insall Salvatti Ratio and knee range of motion ( ROM) were evaluated for each patient pre-and post-operatively and the results were evaluated .Results The KSS scores improved from 73.20 ±3.56 scores preoperatively to 153.00 ±3.39 scores postoperatively (P <0.05).The femorotibial angle was corrected from 160.57°±3.30°preoperatively to 169.85°±1.34°postoperatively(P<0.05).The quadriceps angle was corrected from 24.57°±2.07°preoperatively to 17.28°±1.11°postoperatively (P<0.05).The posterior slope angle of tibial plateau was corrected from 2.25°±0.50°preoperatively to 3.75°±0.95°postoperatively (P<0.05).The Insall Salvatti Ratio improved from 0.80 ±0.08 preoperatively to 1.05 ±0.12 postoperatively ( P <0.05).The knee ROM improved to 38.25°±8.88°postoperatively from 96.25°±4.34°preoperatively(P<0.05).Conclusion TKA following HTO demands adequate soft tissue balancing , good limb alignment and appropriate prosthesis .
9.Primary hyperparathyroidism in childrens complicated with vitamin D deficiency: an evidence-based case report

Primary hyperparathyroidism (PHPT) in children is a rare condition,co-existing with vitamin D deficiency can lead to more diagnostic uncertainty.Here we report a case of a boy with PHPT complicated with vitamin D deficient rickets.The diagnosis and treatment of this patient was analyzed against literature review to summarize evidence-based clinical features of PHPT in children.We found that compared with adult PHPT,PHPT in children is associated with severer symptoms,higher serum calcium level,lower parathyroid hormone,preponderance of single adenoma in pathology,and higher cure rate of surgery.Co-existence of osteomalacia may induce reduction of the serum calcium level to the normal range,but cause more severe bone lesions.
