1.Countermeasures in enhancing intelligence cultivation among stomatological students

Intelligence is the precondition for mastering knowledge and the foundation for acquiring technical skills.Intelligence cultivation among students majored in stomatology which includes constructing sound objective system,setting up supporting environmental condition and establishing scientific cultivation mechanism should be considered as an important teaching task.These methods can establish affluent language environment for students,set up opening mathematical logic training room,provide teaching environment with multiple visual space,develop the guidance for interpersonal communication and broaden the space for exploration of natural secrets.Combined with overall training of stomatology,the cultivation mechanism can be classified into four parts:establishing scientific mechanism for elective courses,providing auxiliary mechanism,adopting scientific evaluation and introducing effective incentives.

3.Research progress on the mechanism of traditional Chinese medicine in lung cancer

This article summarized the mechanism of traditional Chinese medicine treatment for lung cancer in recent years. Related literatures were from the immune regulation, inhibiting growth and metastasis of lung cancer cells and promoting apoptosis and reversing drug resistance. Review showed that traditional Chinese integrated with western medicine for lung cancer has gradually become the consensus of clinical practioners.
4.Progress of transesophageal echocardiography applied in perioperative monitoring

The application of transesophageal echocardiography (TEE) in perioperative monitoring was introduced,and its involvement in non-cardiac surgery was described.The advantages and disadvantages of TEE were analyzed when applied to cardiac and non-cardiac operations for congenital heart disease,coronary disease,vascular heart disease,aorta disease and etc.The abuse of vasoactive drug or fluid is avoided due to subjective evaluation on cardiac function and volume load.TEE facilitates the anesthetist in diagnosis and evaluation of the patient with heart diseases,and the surgeon in immediate assessment of the operation,which can be used for real-time monitoring of cardiac function and hemodynamics.
5.Animal implantation with a new type of chitosan microspheres/calcium phosphate cement

Objective:To investigate bioactivity,biodegradation,bone conductive properties,and clinical maneuverability of a novel injectable chitosan microspheres/ calcium phosphate cement(CPC).Methods: The bone defect of ??4 mm?4 mm?6 mm was made at both thighbones of 12 rabbits,and experimental material(chitosan microspheres/CPC) or control material(?-tricalcium phosphate cement,?-TCP) was injected into the defect respectively.The filling situation was evaluated by X-ray 3 days after operation.The rabbits were divided into three groups,four for each group.The samples with chitosan microspheres and the control with ?-TCP were collected at 8,16,24 weeks after operation.Histological examination and scanning electron microscope(SEM) evaluation were performed.Results: The absorption of implants with chitosan microspheres was observed at 8 weeks,and became more apparent at 16 weeks.Different size of cavities were observed in CPC after the degradation of chitosan microspheres.The chitosan microspheres/CPC was dramatically degradated after 24 weeks with a few chitosan microspheres,and new bone replaced the degradated materials.The implants with ?-TCP were absorbed slowly compared with the chitosan microspheres/CPC.The cavities in ?-TCP were small.Conclusion: The chitosan microspheres/calcium phosphate cement has the characters of good biocomptable and osteocombinative ability.Compared with the control material,adding chitosan microspheres into CPC could enhance its degradability and facilitate the new bone formation.
6.Study Progress in Combined Application of Chinese Medicines and 5-Hydroxytryptamine Reuptake Inhibi-tors

At present, selective 5-hydroxytryptamine (5-HT) reuptake inhibitors (SSRIs) and 5-HT and noradrenaline (NA) re-uptake inhibitors ( SNaRI) are the first choice for the clinical treatment of depression, however, the clinical adverse effects of the drugs have become a prominent problem affecting the clinical efficacy. Chinese medicines have some experience in the treatment of depres-sion, and the combined use with the above antidepressants can enhance the efficacy and / or reduce the adverse effects. The article re-viewed the status of the combination of SSRIs and Chinese medicines in order to provide reference for the clinical treatment of depres-sion.
7.Research progress in non-drug treatment of essential hypertension by Traditional Chinese Medicine

Non-drug therapy of traditional Chinese medicine (TCM) mainly includes exercise therapy, such as Baduanjin and Taichi. Some TCM techniques adjunct toanti-hyperhension drugs show effectiveness with safety, such as moxibustion, acupuncture, acupoint application, auricular point application, massage, cupping, scraping, herbal tea, emotional therapy, and music therapy. In future, long-term follow-up of trials and standardization of research are needed. In the other way, in-depth research on patients with grade 3 hypertension and secondary hypertension is still needed.
8.Clinical characteristics and prediction of Kawasaki disease unresponsive to initial dose of intravenous immunoglobulin

Objective To determine the prediction and clinical characteristics of intravenous immunoglobulin (IVIG) treated Kawasaki disease (KD) failure in initial dose.Methods Retrospective analysis was performed with the clinical data of 1 11 children with KD.The paticnts were divided into sensitive group and unresponsive group according to initial effect of IVIG.The clinical manifestations,laboratory examination and radiological features of the children were compared.Logistic regression analysis was performed in significant different indicators to determine independent correlation factors.In order to seek the reference indexes which indicate unresponsive to IVIG,a receiver operating characteristic (ROC) curve was made,of which the diagnostic cut-off was nine independent correlation factors while grouping was made according to patients' different responses to IVIG.Results (1)There were 90 cases (81.1%)in effective group and 21 cases (18.9%) in unresponsive group.(2) Compared with the sensitive group,hyperpyrexia cases[66.7% (14/ 21 cases) vs 34.4% (31/90 cases),x2 =7.334,P =0.007] and the chances of coronary artery lesions [47.6% (10/ 21 cases) vs 23.3% (21/90 cases),x2 =4.989,P =0.026] were significantly higher in the unresponsive group.(3)Compared with the children administered twice with gamma globulin,the children of single-dose treatment significantly reduced the unresponsive probability [12.5 % (9/72 cases) vs 30.8 % (12/39 cases),x2 =5.504,P =0.019],and there was no difference in the chances of coronary artery lesions[23.6% (17/72 cases) vs 30.8% (12/39 cases),x2 =0.672,P =0.412].(4)Comparing the sensitive group and the unresponsive group,the percentage of neutrophils count [(0.72 ± 0.06) vs (0.76 ± 0.04),t =-2.84,P =0.005],platelet counts [(352.38 ± 42.18) × 109/L vs (373.14 ±36.93) × 109/L,t =-2.076,P =0.040] and C-reactive protein (CRP) [(74.38 ± 12.92) mg/L vs (92.05 ± 11.17) mg/L,t =-5.780,P =0.000] were significantly higher in the unresponsive group,but the level of serum albumin[(34.09 ±3.19) g/L vs (31.61 ±2.03) g/L,t =4.442,P =0.000] was lower.(5) Multivariate Logistic regression analysis indicated that the percentage increase of neutrophils count (P =0.018),CRP (P =0.000) increase and serum albumin(P =0.040) decrease were independent risk factors for unresponsive treatment with gamma globulin.(6)After the area under the ROC curve was calculated,the percentage of neutrophils count,CRP and serum albumin could be effective predictors to IVIG treatment failure in initial dose,and the critical values were 0.72,78.5 mg/L and 33.11 g/L,respectively.Conclusions Treatment with IVIG for the first time may be ineffective in some situations such as the percentage of neutrophils count ≥0.72,CRP ≥78.5 mg/L or serum albumin≤33.11 g/L.
9.Transcranial Approach for Resection of Tumors in Area of the Anterior Skull Base, Orbit and Paranasal Sinuses

Objective To investigate the feasibility and complications of the transcranial approach for resection of tumors that involve the anterior skull base, orbit and paranasal sinuses. Methods Nine cases of tumors involving the anterior skull base, orbit and paranasal sinuses, which were resected by transcranial approach alone in our hospital from July 2000 to April 2001, were retrospectively analyzed. All patients had the lesion of the anterior skull base and orbit, and 5 cases had additional paranasal sinuses involvement. Results The tumors in all patients were removed completely. Complications occurred in three cases, including one case of cerebrospinal fluid leaking and two cases of blepharoptosis. One died of tumor recurrence one year after operation. Eight were tumor-free survival during the follow-up period of 20~29 months. Conclusion The transcranial approach can completely remove the tumors widely invading the anterior skull base, orbit and paranasal sinuses. Separation of the periosteum of the orbital roof can cause blepharoptosis, which leads to failure of cosmetics. Extending approach must be avoided if it is possible. The reconstruction of the skull base with pedicle pericranial flap may be the key factor to reduce complications.
10.Coracoclavicular screw fixation for treatment of unstable distal clavicle fractures

[Objective]To retrospectively analyze of functional and radiographic outcome of distal calvicle fractures (Neer type II) treated with coracoclavicular screw fixation.[Method]Fifteen patients with Neer type II distal clavicle fracture were treated surgically between March 2001 and March 2004.Operative treatment included open reduction and fixation of the proximal clavicular fragment to the coracoid process using a 6.5-mm cancellous screw and repair of the coracoclavicular ligaments.Perioperative radiographs were performed to ensure that the screw was introduced properly into the coracoid.Following the operation,the arm was placed in a sling for two weeks.After two weeks,the sling was discontinued and the patient was permitted to use the arm for daily activities except for lifting,pulling,and pushing for 6 weeks.The patient is instructed not to perform any heavy lifting,pushing,pulling or participate in contact sports for 3 months.Data were collected to allow scoring according to the American Shoulder and Elbow Surgeons shoulder index 6 months after operation.[Result]All the patients were followed up at an average of 16 months (range:9~26 months).Fracture union occurred at a mean of 7 weeks (range:6~9 weeks)postoperatively without any serious complications such as screw-migration,pseudarthrosis,infection,neurovascular damage,or skin necrosis.All patients returned to the pre-injury level of activity with no residual pain or dysfunction.The mean American Shoulder and Elbow Surgeons shoulder index was 97 points ( range: 90-100 points) 6 months after operation.Two patients showed radiologically ossification of the acromioclavicular joint with no clinical symptoms.[Conclusion]Coracoclavicular screw fixation is a relatively simple,reliable,and safe technique of open reduction and internal fixation of the type II distal clavicle fracture.
