2.Inflammation induced by degenerative lumbar scoliosis

BACKGROUND:Degenerative lumbar scoliosis patients wil appear to have abnormal changes in various cytokines in the serum of the body, which is closely related to patient’s pathogenesis.
OBJECTIVE: To monitor the changes in the serum levels of tumor necrosis factor-α, interleukin-6 and interleukin-1β during the onset of degenerative lumbar scoliosis.
METHODS: Sixty-six patients with degenerative lumbar scoliosis were enrolled as observation group, and according to Schwab evaluation system, there were 16 cases of type I, 32 of type II, and 18 of type III. Additionally, 60 healthy persons undergoing homochronous physical examination were extracted as control group. Blood samples were taken at preoperative 1, 2, 3 weeks and at postoperative 1, 2, 3, 4, 5, 6 weeks to determine serum levels of tumor necrosis factor-α, interleukin-6 and interleukin-1β as well as to observe the relevant changes.
RESULTS AND CONCLUSION:In the observation group, the levels of tumor necrosis factor-α, interleukin-6 and interleukin-1β were increased gradualy at 1-3 weeks before treatment, and then showed a decrease trend at 6 weeks after treatment. Compared with the control group, in the observation group, the levels of tumor necrosis factor-α were significantly higher at 1-4 weeks after admission (P < 0.05); the levels of interleukin-6 and interleukin-1β were significantly higher at 1-5 weeks after admission (P < 0.05). These findings indicate that the levels of tumor necrosis factor-α, interleukin-6 and interleukin-1 reached the peak at 1 week before treatment, and then gradualy decreased. Changes in the levels of tumor necrosis factor-α, interleukin-6 and interleukin-1β have an important role to assess disease development, which can be used to determine the severity of inflammation based on which we can select the appropriate treatment.

In order to meet the health needs of people, China’s health service system needs continuous reforms and adjustments. Health needs, equity and efficiency, quality of service, and the development history and current situation of the health service system are the four main dimensions to be considered during the building process of the system. This paper argues that building a people-centered health service system is the direction, and it describes its connotation from five different aspects, including human care, interrelated interests, primary health care-oriented, continuous integration, and conditions for support. This paper proposes the strategies and paths to build a people-cen-tered health service system, including to promoting the quality of primary health care, integrating health service sys-tems, and strengthening government’s responsibility.

5.To set up the service management center of ventilators depending on department of respiratory medicine

Objective: To explore how to set up the service management center of ventilators depending on department of respiratory medicine , how to perfect daily management of ventilators, how to rationally and timely allocate ventilators ,how to reduce the fault of ventilators and ensure the safe use of ventilators, and how to cost savings of purchasing ventilators. Methods: To figure out and compare data of before and after the establishment of the service management center of ventilators in 2010 and in 2012. Results: Non machine failure rate was 42.86% in 2012 and 69.35% in 2010, had remarkably decreased. At the same time, the total failures have decreased significantly. Conclusion: Setting up the service management center of ventilators depending on department of respiratory medicine have achieved timely allocation ventilators, have improved the level of medical personnel of using ventilators, have ensured the safe use of ventilators, have cost savings of purchase ventilators, have reduced the fault of ventilators.
6.Applications of SEDECAL mobile X-ray radiographic units

Objective:Mobile X-ray radiographic Units to appear in the clinical application of high voltage circuit judge special case of failure, the analysis in order to provide effective and reliable service basis. Methods: Preliminary judgment based fault location fault code, using stepwise exclusion analyze, judge, and determine the cause of the failure lies. Results:The high-voltage inverter element models of mobile DR, the driver circuit, the output voltage discharge waveform judgment and other issues have a better understanding, to facilitate the exclusion of equipment failure. Conclusion:Special case of X-ray radiographic Units failure high voltage circuit analysis and troubleshooting, especially for the inverter circuit analysis, X-ray radiographic appear to take timely and appropriate repair method when high pressure overload.
7.Therapeutic effect of co-administration of simvastatin,clopidogrel and low molecular weight heparin on patients with unstable angina pectoris

Objective To approach the effects of the combination of simvastatin,clopidogrel and low molecular weight heparin on patients with unstable angina pectoris(UAP). Methods A total of 86 patients presented with UAP in Department of Internal Medicine of Tianjin Hongqiao Hospital were divided into control group and therapy group(each 43 cases)by random number table. The patients in the control group were treated with routine therapies such as oxygen,anti-platelet agents,nitrates,β-blockers,calcium channel antagonists,et al,while the patients in treatment group received besides the above conventional treatments,they additionally took simvastatin, clopidogrel orally and subcutaneous injection of low molecular weight heparin. The frequency of recurrence of angina, improvement of cardiac ischemia,the change of cholesterol,adverse reaction and the occurrence of complication were observed during 4 weeks of treatment. Results Compared to the control group,the frequency of angina(time/week:2.77±1.12 vs. 3.78±2.57,P<0.05)and consumption of nitroglycerin(mg/d:1.16±0.46 vs. 2.89±1.89,P<0.05) were obviously less,the improvement of cardiac ischemia(95.3%vs. 79.1%,P<0.05)and electrocardiogram(ECG:90.7%vs. 62.8%,P<0.05)was superior and the total effective rate was higher obviously in the therapy group. After treatment,plasma cholesterol levels were decreased obviously in treatment groups compared with before treatment (mmol/L:3.78±1.23 vs. 5.76±1.74,P<0.05),but the statististical difference in cholesterol levels in control group was found between before and after treatment(5.66±1.14 vs. 5.69±1.89,P>0.05). Before and after treatment,there were no significant changes of platelet count(PLT),activated partial thromboplastin time(APTT),liver functions and renal functions,and after treatment,bleeding and allergy serious adverse reactions were not seen in patients of both groups. Myocardial infarction occurred in 2 patients in the control group. Conclusion The addition of combined application of simvastatin,clopidogrel and low molecular weight heparin onto routine therapies has better effects for treatment of UAP.
8.Application of nitinol rib claws embracing in the treatment of multiple rib fractures and flail chest

Objective To observe the effect of nitinol rib claws embracing in the treatment of multiple rib fractures and flail chest .Methods 43 patients who received nitinol rib fixation claw embracing treatment for multiple rib fractures,flail chest(the internal fixation group) and 41 cases with non-internal fixation(the non-internal fixation group) were chosen.The efficacy and complications were analyzed and compared .Results After 12 weeks, the results of X-ray showed that in the internal fixation group ,bone fractures were all healed ,1 case of pulmonary atelecta-sis,1 case of pneumonia ,no patients died;In the non-internal fixation group ,12 patients had pulmonary atelectasis , 19 cases of pneumonia,1 case died,the incidence rate of complications between the two groups had significant differ -ence(χ2 =5.64,P<0.05).The hospitalization periods,analgesics volume and respiratory support in fixation group were lower than the non-fixation group,the differences were statistically significant (t =4.86,6.75,6.88,all P<0.05).Conclusion The clinical effect of nitinol embracing claw rib fixation in the treatment of multiple rib frac-tures,flail chest is superior to non-internal fixation group ,with a high clinical value .
9.Effect of simvastatin and atorvastatin on reducing blood lipid in patients with coronary heart disease

Objective To evaluete the effect of atorvastatin on reducing blood lipid in patients with coronary heart disease.Methods 150 cases of coronary heart disease patients collected in Liaocheng Third People’s Hospital of Shandong province from January 2013 to January 2014 were randomly divided into A,B two groups,each had is cases.Group A were treated with simvastatin,and group B were treated with atorvastatin.Blood lipid changes in two groups before and after treatment were compared. Results Compared with before treatment,the clinical sypotoms after treatment in two groups were allivate and TC,LDL-C indexes were significantly decreased (P <0.05 ).But there was no statistical significance between A and B groups. Conclusion Simvastatin and atorvastatin can effectively improve the blood lipid status in patients with coronary heart disease.
