1.Factors affecting lumbar disc herniation among elderly patients with osteoporosis

Objective:
To investigate the influencing factors of lumbar disc herniation (LDH) among elderly patients with osteoporosis (OP), so as to provide the basis for early prevention of LDH in OP patients.
Methods:
The patients aged 60 years and above with OP who were treated in the clinic of Beijing Jishuitan Hospital from December 2022 to December 2023 were recruited as the study subjects. The data of demographic and clinical information were collected through the hospital medical record system. LDH was diagnosed according to the LDH Diagnosis and Treatment Guidelines. Factors affecting LDH among elderly patients with OP were identified using a multivariable logistic regression model.
Results:
A total of 500 patients with OP were included, with 188 males (37.60%) and 312 females (62.40%). The mean age was (69.05±3.28) years, and the course of OP was (6.37±2.11) years. There were 196 patients living alone, accounting for 39.20%. LDH among elderly patients with OP were detected in 78 cases, accourcing for 15.60%. Multivariable logistic regression analysis showed that the patients who were at ages of 70 years and above (OR=1.465, 95%CI: 1.197-1.793), lived alone (OR=1.777, 95%CI: 1.093-2.889), had a history of fracture (OR=1.799, 95%CI: 1.070-3.023) and scored higher in quality of life (OR=1.405, 95%CI: 1.230-1.605) were at higher risks of LDH, while the patients who had a history of taking calcium supplements (OR=0.562, 95%CI: 0.345-0.915) were at lower risks of LDH.
Conclusion
Age, living situation, history of fracture, history of taking calcium supplements and quality of life were influencing factors for LDH among elderly patients with OP.
2.Epirubicin-loaded chitosan microspheres combined with microwave coagulation for treating hepatocellular carcinoma in mice

BACKGROUND: The surgical resection rate of pdmary hepatic carcinoma is low, thus, the local treatment arose more attention. Microwave coagulation therapy can inactivate rather than kill the hepatic carcinoma ceils. Slow-release chemotherapy has been used in treating primary hepatic carcinoma because it can form local high concentrations and last for a long time. OBJECTIVE: To observe the therapeutic effect of epirubicin-loaded chitosan microspheres combined with microwave coagulation on treating hepatocellular carcinoma in mica. METHODS: Epirubicin-loaded chitosan microspheres were prepared by using emulsion-chemical cross linking technique. The surface morphology and particles size of chitosan microspheres were observed by scanning electron microscope. Ultraviolet speotrophotometer was used to analyze the entrapment efficiency, entrapment efficiency and cumulative release rates of epirubicin-loaded chitosan microspheres. Totally 24 mice with transplanted subcutaneous H22 HCC were divided into 4 groups, which were respectively treated by microwave coagulation therapy, intratumoral injected with physiological saline after microwave coagulation therapy, intratumoral injected with epirubicin after microwave coagulation therapy, intratumoral injected with epirubicin-ioaded chitosan microspheres after microwave coagulation therapy. The tumor inhibitory rate was calculated. RESULTS AND CONCLUSION: The average diameter of chitosan microsphere was 105 μm, with uniformed particle diameter. The ratio of drug loading was 11% and the entrapment was 80%. The in vitro drug cumulative release rate was 84% after 2 weeks. The tumor inhibitory rates of the microwave coagulation combined with physiological saline, epirubicin, and drug carried microspheres groups were 8%, 20%, and 47%. It suggested that chitosan microsphere is a safe and effective slow-release dosage form, which exhibits strong anti-tumor effect when combined with microwave treatment.
3.Analysis on Effect of Free-frying Granules of Traditional Chinese Medicine on Treatment of Chronic Prostatitis

This study was aimed to evaluate the efficacy and safety of free-frying granules of traditional Chinese medicine(TCM) on treatment of chronic prostatitis. A total of 120 chronic prostatitis patients that were diagnosed with the dampness-heat and blood-stasis syndrome and kidney-deficiency syndrome were divided into the TCM group and western medicine group by random digit table. Category II and ⅢA chronic prostatitis were treated with oral administration of Levofloxacin Mesylate Tablets (0.2 g, bid) and the category ⅢB chronic prostatitis was treated with oral administration of Terazosin Hydrochloride Tablets (2 mg, qd). In the TCM group, the dampness-heat and blood-stasis syndrome was treated with Tongluo Qingjie formula ( which was self-made formula containing Rhizoma Dioscoreae Hypoglaucae, Herba Patriniae, Semen Plantaginis, and etc.) and the kidney-deficiency syndrome was treated with Jiawei Wuzhi Yanzong pills (which containing Semen Cuscutae, Chinese Wolfberry, Radix Rehmanniae Preparata, Fructus Rubi and Schisandra chinensis, and etc.). Four weeks was one course of treatment. The results showed that the total effective rate of TCM group was better than that of western medicine group (P< 0.05). Effect of TCM group was obviously better than that of western medicine group in the improvement of prostatitis NIH-CPSI score, the quality of life and symptoms of the patients. TCM may increase lecithin corpuscle in prostatic fluid of ⅢA chronic prostatitis (P< 0.05). Meanwhile, no obvious toxiferous and side reactions were found. It was concluded that the free-frying granules of TCM have good efficacy and safety in the treatment of chronic prostatitis.
4.Laparoscopic nephron sparing surgery in the treatment of renal tumors

Objective To evaluate the operative techniques and the methods of the renal function protection in laparoscopic nephron sparing surgery for the treatment of renal tumors.Methods Thirty-six renal tumor patients accepted transperitoneal laparoscopic partial nephrectomy at the 70 degree lateral decubitus position.There were 17 tumors in the upper pole,13 in the lower pole and 6 in the center of kindey.The mean tumor diameter was 2.6 cm(from 0.8 to 4.0 cm).Peke forceps were used to clamp the renal artery and achieve warm ischemia.Cold scissors was used to resect tumors and Hem-o-lok was used to clamp and control the artery bleeding in surgical bed instead of ligature when suturing the kidney parenchyma.Ulinastatin was used to prevent and relieve the ischemical reperfusion injury.The operative time,estimated blood loss,warm ischemia time,intraoperative and postoperative complications and the operative efficacy were recorded.Results All operations were completed successfully,no case was converted to open surgery.The mean operative time was 128 min(95 to 186 min),mean estimated blood loss was 130 ml (40 to 600 ml),mean warm ischemia time was 21 min(16to 28 min).There was no hemorrhage and urinary leakage after surgery.The post-operative renal function was normal in all the cases.The histopathological examination showed that 36 cases were renal cell carcinoma including 31 pT1a,cases and 5 pT1b cases.There was no positive surgical margin.The mean follow-up time was 16 months (from 6 to 30 months).There was no recurrence and metastasis found and renal function was normal in all cases during the follow-up.Conclusions Laparoscopic nephron sparing surgery for renal tumors is a safe and feasible treatment option.This procedure can resect tumor safely and preserve renal function efficiently.The use of Hem-o-lok instead of ligature can significantly reduce the operative time.Ulinastatin can help reducing the ischemical reperfusion injury and thus preserve renal function.
5.Effects of yogurt supplementation on the growth of preschool children in Beijing suburbs.

OBJECTIVETo investigate the effect of yogurt supplementation on the growth of preschool children in Beijing suburbs.
METHODSFour hundred and two preschool children (217 males, 185 females), aged 3-5 years, whose height for age and/or weight for age were less than the reference level, were selected as subjects from 7 kindergartens in Beijing Fangshan District. The subjects were divided randomly into control group (CG, 201) and yogurt supplemented group (YG, 201). Each subject in YG was given one serving of yogurt (125 g) for 5 days a week from March to December in 2001, while nothing additional was provided to CG. All subjects kept their usual diet during the study. Anthropometry (body height and weight and upper-arm circumference) and the bone mineral density (BMD) of forearm were measured every 3 months. Disease status and dietary intake were also recorded and assessed.
RESULTSThe intake of calcium, zinc, and vitamin B2 in YG was significantly higher than that in CG. The incidence and duration of upper-respiratory infection and diarrhea of children in YG were significantly less than those in CG. The height gain of children in YG was significantly higher than that in CG after yogurt was supplemented for 3, 6, and 9 months (P<0.05) (1.90+/-0.49 cm vs 1.77+/-0.54 cm, 3.83+/-0.57 cm vs 3.64+/-0.66 cm and 5.43+/-0.69 cm vs 5.24+/-0.76 cm, respectively). The weight gain of children in YG was significantly higher than that in CG after yogurt was supplemented for 3, 6, and 9 months (P<0.05) (0.70+/-0.43 kg vs 0.49+/-0.35 kg, 0.98+/-0.62 kg vs 0.80+/-0.60 kg and 1.42+/-0.76 kg vs 1.20+/-0.67 kg, respectively). The BMD of children in YG was significantly higher than that in CG after yogurt was supplemented for 9 months (P<0.05) (0.415+/-0.058 g/cm2 vs 0.400+/-0.065 g/cm2).
CONCLUSIONYogurt is beneficial to the improvement of calcium, zinc, and vitamin B2 intake, the decreasing of the incidence and duration of upper-respiratory infection and diarrhea, and the promotion of the health and the growth and development of preschool children.

6.The prediction of maternal HBV transmission by breast milk of postpartum women with chronic HBV infection

Objective To investigate the prediction of maternal HBV transmission by breast milk of postpartum women with chronic HBV infection.Methods HBV DNA levels in serum and breast milk weredetected by fluorescent quantitative polymerase chain reaction in 64 postpartum women with chronic HBV infection.HBV DNA≥1.0×103copies/ml was defined as positive,and correlation analysis was conducted.Results HBV DNA positive rate was 78.1%and 62.5%in serum and breast milk respectively,with a HBV DNA range of 1.05×103~3.87 ×104copies/ml in breast milk.When HBV DNA in serum was 1.0×105~1.0×107copies/ml,the HBV DNA positive rate in breast milk reached to 94.9%;however,when HBV DNA in serum was 1.0×103~1.0×104copies/ml,the positive rate in breast milk was only 18.2%.Conclusion The HBV DNA positive rate of breast milk in postpartum women with chronic HBV infection is correlated with the HBV DNA levels in serum;and breast-feeding should be avoided for postpartum women with HBV DNA≥1.0×105copies/ml in the serum.So serum HBV DNA detection is necessary in antenatal care for women with chronic HBV infection.
7.In vitro infection of human hepatoma (Hep G2) cell line by hepatitis B virus positive serum.

OBJECTIVETo establish a culture system of HBV positive serum infected Hep G2 cells in vitro.
METHODSHep G2 cells were seeded into six-well cluster dishes, at 1 x 10(-6) cells per well and incubated with 3 ml 10% fetal calf serum/ Dulbecco's modified Eagle's medium (10% FCS/DMEM) at 37 degrees in 5% CO2 air. At 24 h after plating, infection group Hep G2 cells were cultured with 0.5 ml HBV positive serum, in control group HBV negative serum was used, 24 h later the inoculums was removed. The cells were then extensively washed with 0.01 mol/L phosphate-buffered saline (PBS). After washing with PBS, 4 ml 2% FCS/DMEM were added to each well and the medium was collected every 12 h. ELISA method was used to detect HBsAg in culture medium. HBV DNA in cells and culture medium was detected by PCR.
RESULTSIn infection group, HBsAg could be detected from cell culture medium from 12 h (after PBS washed) to 84 h. HBV DNA could be detected by PCR in culture medium and cells.
CONCLUSIONInfection of Hep G2 cells by HBV positive serum is feasible.

8.70 degrees recumbent position transperitoneal laparoscopy for treatment of upper urinary tract transitional cell carcinoma.

OBJECTIVETo study the effect of 70 degrees recumbent position transperitoneal laparoscopy for treatment of upper urinary tract transitional cell carcinoma (TCC).
METHODSFrom May 2004 to January 2007, 70 degrees recumbent position transperitoneal laparoscopy combined with urethral resectoscope was used to treat 31 cases of upper urinary tract transitional cell carcinoma. At the same time titanium clip to occlude the two extremities of ureter tumor was used, extracting specimen by oblique incision of lower quadrant.
RESULTSAll operations were finished successfully, no one was turned to open surgery; mean operation time was 140 min, mean blood loss 80 ml, mean hospital stay time 8 d, without complications of urine leakage and intestinal fistula and so on.
CONCLUSIONS70 degrees recumbent position transperitoneal laparoscopy for resection of whole kidney and ureter is worth of general clinical application because it could provide large space for operation, simplify the treatment of renal pedicle vessels, decrease operation risk, reduce operation trauma and offer early recovery. But its effect on tumor spread and recurrence will still need long term follow-up.

9.The mechanism of HBV infection of human trophoblast cell.

OBJECTIVETo observe the changes of human trophoblast cells after infected with hepatitis B virus.
METHODSHBV positive serum was used to infect human trophoblast cells in vitro. HBsAg in cell culture medium were detected by ELISA method and HBV DNA in cell culture medium and cells were detected by PCR method. HBV fluorescence polymerase chain reaction diagnose kit were used to detect the HBV DNA concentration. Ultra structure of trophoblast cells were observed with transmission electron microscopy (TEM).
RESULTSHBsAg could be detected in infection group by ELISA. Infection group cell culture medium and infection group cells were HBV DNA positive. HBV DNA concentrations in HBV infection cell culture medium in 0, 12, 36, 60, 84 h after extensively PBS washed were < 10(3), 3 x 10(4), 6 x 10(5), 5 x 10(5), 3 x 10(5) copies/mL. HBV infected trophoblast cells were found many forms of endosomes, some of which contents virus like particle.
CONCLUSIONHBV might take advantage of clathrin-mediated endocytosis to enter trophoblast cell, which might lead to cell infection or across the cell bar by transcytosis.

10.Expression of protein 4.1 family in melanoma cell lines and its effect on cell proliferation.

OBJECTIVETo detect the expression of protein 4.1 family members in mouse melanoma cell lines and evaluate their effect on cell proliferation.
METHODSPCR and Western blot were used to detected to the expression of protein 4.1 family members (4.1R, 4.1B, 4.1G, and 4.1N) at the mRNA and protein levels in B16 and B16-F10 cell lines. The expression plasmid vector pEGFP-N1-EPB41L3 carrying 4.1B gene sequence amplified from genomic RNA of mouse embryo fibroblasts was constructed and transiently transfected into mouse melanoma cells. The change in cell proliferation was assessed using MTT assay.
RESULTSThe mRNA and protein expressions of all the protein 4.1 family members, with the exception of 4.1B, were detected in both B16 and B16-F10 cells. Transfection of cells with the eukaryotic expression vector pEGFP-N1-EPB41L3 markedly inhibited cell proliferation as compared with the non-transfected cells.
CONCLUSIONThe eukaryotic expression vector carrying EPB41L3 sequence is capable of inhibiting the proliferation of mouse melanoma B16 and B16-F10 cells.
