1.Effect of individualized nutritional intervention on body weight gaining during pregnancy and pregnant outcome

Objective To explore the effects of individualized nutrition intervention on body weight gaining and pregnant outcome in normal pregnant women. Methods Two hundred and twelve normal pregnant women admitted in our hospital before the 12th gestational week were divided into the observation group and the control group. Eighty-eight pregnant women admitted between October 2013 and January 2014 were assigned into the observation group and another 124 pregnant women admitted between June 2013 and September 2013 into the control group. The observation group was given nutrition assessment and nutritional guidance one by one and face to face by the nutritionist at three time points:around the 12 thgestational week, between the 22nd to 24th gestational week, and between the 32nd to 34th gestational week. The control group was given regular antenatal visits and lectures during pregnancy. These two groups were followed up until delivery. The comparisons were down between the groups in terms of body weight gain and complications during pregnancy, delivery mode and the change of perinatal fetus outcome. Results In the observation group, the body weight gaining of the pregnant women during pregnancy, the incidence of gestational diabetes mellitus (GDM) and the cesarean section rate were significantly lower than those of the control group (all P<0.05). The neonatal birth weight and the incidence of macrosomia were significantly lower than those of the control group (all P<0.05). Conclusion Individualized nutritional intervention given by the nutritionist can control body weight gaining during pregnancy, decrease the incidence of GDM, improve the pregnancy outcome and ensure the maternal and child health.
2.An analysis of 49 cases of patients with sysmetic lupus erythematosus and pregnancy

Objective To investigate the pregnancy opportunity and outcome of patients with sysmetic lupus erythematosus (SLE).Methods We carried out a retrospective analysis of the clinical information of patients who suffered from SLE complicated with pregnancy and were hospitalized to People's Hospital of Peking University from December of 1992 to February of 2012.Chi-square test were used for statistical analysis.Results Forty-nine cases of patients with SLE complicated with pregnancy had 52 pregnancies in total.In 27 cases of planned pregnancies,24 cases(89% )resulted in live births and 5 cases( 18% ) had flares during pregnancy.Of the 24 live births,3 cases( 12%)were premature deliveries and 4 cases(17%) had low birth weight infants.In 25 cases of unplanned pregnancies,12 cases (48%) resulted in live births and 20 cases (80%) had flares during pregnancy.Of the 12 live births,6 cases(50% ) were premature deliveries and 6 cases (50%) were low birth weight infants.The patients in the planned pregnancy group tended to have flares during pregnancy more frequently than those in the unplanned group (P<0.01),the former were more likely to have live births than the latter (P<0.01) and were less likely to have premature delivery (P<0.05).Conclusion Patients with SLE should have planned pregnancy and need collaborative supervision of both rheumatology and immunology department and obstetric department to improve live birth rate,decrease premature delivery and the activity of SLE during pregnancy.
3.Application of failure mode and effect analysis in nursing care of children with tracheal foreign body removal

Objective To identify the potential risks in the process of nursing care for children with tracheal foreign body removal and improve the skills and the process, to promote the quality of nursing care and the success rate of tracheal foreign body removal. Methods The failure mode and effect analysis (FMEA) were used to analyze the potential risks in children with tracheal foreign body removal, preoperative, intraoperative and postoperative care, and to find out the possible risks in each medical treatment. For pediatric venous puncture technology was not skilled, surgical position placement inappropriate (especially in the case of foreign body removal surgery posture), surgical instruments and equipment management, intraoperative nursing cooperation and observation of the disease, postoperative transfer and other aspects of the transfer of training and improvement. Results The medical risk value (RPN) in the treatment of pediatric tracheal foreign body removal in the process of nursing in the control group was reduced from 1 284.2 to 213.2 of the experimental group. The average value of doctors' satisfaction was increased from 79.33% in the control group to 93.33%in the experimental group, P<0.05, the difference was statistically significant. Conclusions The use of FMEA in the treatment of pediatric tracheal foreign body removal surgery care and medical risk management improve the quality of surgical care and help improve the success rate of tracheal foreign body removal. It also ensures the safety of the children in the operation process and improves the quality of nursing work.
4.Generation of CD14+ dendritic cells in vitro with GM-CSF and IL-4.

This study was conducted to get high quality and sufficient numbers of mature dendritic cells from healthy donor peripheral blood. The experiment began on culturing of plastic-adherent monocytes isolated from healthy donor peripheral blood with granulocyte-monocyte clony-stimulating factor (GM-CSF 150 ng/ml) and interleukin 4 (IL-4 800 U/ml) without fresh medium feeding and cytokines for 7 days. After 7 days, CD14+ monocytes not only differentiated into high purity DC but also expressed HLA-I and HLA-II molecules, costimulating molecules, adherent molecules and its progenitor marker CD14 molecule highly. These cells displayed all phenotypic and morphologic characteristics of mature dendritic cells and were most potent stimulatory cells in allogeneic mixed leukocyte reactions. The endocytosis ability of these DCs peaked at the third day in culture and decreased remarkably afterwards. These results provide evidence for the first time that CD14+ monocytes differentiated in vitro from peripheral blood monocytes exhibit dendritic cells characteristics and still express its progenitor marker CD14 molecules highly. The results of this experiments may facilitate further studies of CD14+ DC and its clinical applications.

5.Humoral immune responses in rabbits with an experimental SARS-CoV inactivated vaccine

Objective:To evaluate the immunogenicity of an experimental SARS-CoV inactivated vaccine.Methods:The virus suspension of F69 strain was inactivated with 0.4% formaldehyde and purified,then used as the immune antigen combined with Freund′s adjuvant.Eight adult New Zealand rabbits were immunized 4 times with this vaccine.12 sets of rabbit serum were sampled from the third day to 74th day after first immunization.Titers of specific IgG antibody and neutralizing antibody were determined by indirect ELISA and micro-cytopathic effect neutralizing test.Results:Rapid and potent humoral immune responses were induced by F69 inactivated vaccine in all eight immunized rabbits.Both specific IgG antibody and neutralizing antibody all peaked just with 2 vaccinations,with the maximum titer of 1∶81 920 and 1∶20 480 respectively about 6 weeks after first immunization.Across neutralizing reaction existed between F69 and Z2-Y3 strains.Conclusion:F69 inactivated vaccine owns strong immunogenicity.Similar antigenic epitopes exist between the F69 strain and Z2-Y3 strain,which ensured the cross neutralizing reaction.
6.New type recombinant antibody fragment scFv multimer and cancer targeting.

New type recombinant antibody single chain variable fragment (scFv) is formed by the joined VH and VL domains of immunoglobulin with the used of a polypeptide linker that is at least 12 residues in length. scFv is the smallest functional unit of antibody and has shown a fine prospect for the radioimmunoscintigraphy of cancer because of its special characteristics including increased tumour penetration and fast clearance rates compared with parent Ig. A scFv molecule with a linker of 3-12 residues cannot fold into a functional Fv domain and instead associates with a second scFv molecule to form a bivalent dimer (Diabody). Reducing the linker length below three residues can force scFv association into trimers (Triabody) or tetramers (Tetrabody) depending on linker length, composition and V-domain orientation. This review describes linker length and V-domain orientation of scFv, expression and stability of scFv multimers, size of scFv multimers and its effect on in vivo pharmacokinetics, flexibility and avidity of scFv multimers, in vitro application of multimeric murine scFv, multispecific scFv multimers and cancer targeting.

7.Study on the immunogenicity of group A plus group C meningococcal glycoconjugate vaccine

Objective To investigate the immunogenicity of group A plus group C meningococeal glycoconjugate vaccine,namely dosage,immune memory and compatibility. Methods The mice were injected with group A plus group C meningococcal glycoconjugate vaccine with different dosages. Blood samples were taken on the 14th day after the last injection for testing the antibodies against polysaccharide A and C. After the optimal immunization dosage had been decided,the mice were inoculated separately with the monovalent group A and the monovalent group C and the bivalent group A plus group C glycoconjugate vaccine with one,two or three injections for observation of the effectiveness of different injections and the compatibilities. Results The dosage of 1.25 μg of each polysaccharide of group A and group C in bivalent glycoconjugate vaccine appears to be immunologically optimal to vaccinate the mice. Immunological memory could be induced in mice inoculated with the glycoconjugate vaccine,and the antigenic immunogenicity of the group A component and group C component in the formulation of group A plus group C meningococcal glycoconjugate vaccine was not affected. Conclusion Group A plus group C meningococcal glycoconjugate vaccine have good immunogenicity,immune memory and compatibility.
8.The study of 5-Hydroxytryptamine 2B receptor-nanodisc self-assembling and its ligand binding activity

Objective To explore the application of nanodisc in functional and drug discovery research of G protein-coupled receptor (GPCR).Methods The purified recombinant 5-Hydroxytryptamine 2B receptor (5-HT2BR) was reconstituted into nanodisc complex.Sodium dodecyl sulfate polyacrylamide gel electrophoresis (SDS-PAGE) and size exclution chromatography were performed to evaluate the reconstitution reaction,followed by the use of surface plasmon resonance to validate the ligand-binding activity of 5-HT2BR after reconstitution.Results 5-HT2B R was effectively self-assembled into nanodisc while maintained its binding activity toward the antagonist SB204741.Conclusions The presented study provided potential application of 5-HT2B R-nanodisc for the development of subtype-selective drugs against 5-HT2B R and the fundamental of utilizing nanodisc for GPCR structural and functional studies as well as drug discovery.
9.Immune responses of dendritic cells after loaded with cytotoxicity T lymphocyte epitope based peptide of human alpha-fetoprotein (hAFP).

OBJECTIVETo study the immune responses of lymphocytes after activated by dendritic cells (DCs) loaded with cytotoxicity T lymphocyte (CTL) epitope based peptide of human alpha-fetoprotein (hAFP, 218-226 LLNQHACAV).
METHODSGet high purity DCs by cultured plastic-adherent monocytes isolated from healthy donor of HLA-A2(+) peripheral blood with granulocyte-monocyte colony-stimulating factor (GM-CSF) and interleukin-4 (IL-4) for 7 days. Stimulate self-lymphocytes with DCs that loaded with CTL epitope based peptide of hAFP under the culture medium contains interleukin-2 (IL-2) for 7 days. Analyse IL-12 and TNF in culture medium and also the specific lysis activity of lymphocytes against four strains of primary hepatocellular carcinoma cells.
RESULTSAfter stimulated by DC loaded with CTL epitope based peptide derived from hAFP, lymphocytes appeared a good characteristics and the culture medium of activated lymphocytes contained a high level Th1 type cytokines of IL-12 and TNF. Activated lymphocytes not only specifically lysed HLA-A2(+) HepG2 line but also had the cytotoxicity against other three primary hepatocellular carcinoma cell lines and T2 target cell loaded with peptide of hAFP.
CONCLUSIONSThe results of this research supply the basic materials for the DC based vaccine with HLA-A2 restricted peptide epitope derived from hAFP against AFP positive primary hepatocellular carcinoma.

10.Prenatal diagnosis of 22q11 microdeletion syndrome.

OBJECTIVETo establish a method for the prenatal diagnosis of 22q11 microdeletion syndrome.
METHODSBACs-on-Beads (BoBs) and fluorescence in situ hybridization (FISH) were performed on a fetus for whom amniotic chromosomal culturing has failed and a pair of twin fetuses suspected for 22q11 deletion syndrome.
RESULTS22q11 microdeletion was detected in all 3 fetuses by prenatal BoBs as well as FISH, with only one red signal detected at the DiGeorge/VCFS N25 site and two green signals on the 22q13.3 ARSA site.
CONCLUSIONThe combination of prenatal BoBs and FISH can provide a method for the prenatal diagnosis of 22q11 microdeletion.
