1.Progress of human rhinovirus infection and asthma

Human rhinovirus(HRV)is a common pathogen causing respiratory tract infection.and recent studies have shown that HRV infection is not only a cause of asthma acute exacerbation.but their early infection in infants and young children also plays an important role during the formation of asthma in children.HRV causes asthma relevant symptoms mainly by the following mechanisms,infection of bronchial epitheIial cells.to promote the release of inflammatory mediators.the immune response and to cause airway remodeling.Learning more about the impact of HRV infection on asthma and related mechanisms will help US to find effective prevention and treatment methods to reduce asthma morbidity and acute excerbation.
2.A study of immunocyte subsets and serum cytokine profiles before and after immunal suppression treatment in patients with immune thrombocytopenia

Objective To explore the clinical significance of a series of cytokines and peripheral blood immunocyte subsets before and after immunosuppressive therapy in patients with immune thrombocytopenia (ITP).Methods The percentages of immunocyte subsets in the peripheral blood of 20 patients with ITP and 20 healthy controls were detected by flow cytometry,including CD3+,CD4+,CD8+,CD4+/CD8+,CD1~.ELISA was applied to detect the level of serum TNFα,IL-2,IL-6,IL-4,IL-10,IL-11,IL-17,IL-27,transforming growth factor β (TGFβ),thrombopoietin (TPO) of 20 patients with ITP and 20 healthy controls.Results The percentage of CD3+ T lymphocyte,CD4+ T lymphocyte and the ratio of CD4+ / CD8+ T lymphocyte in patients with ITP were lower than those in healthy controls [(62.66 ± 6.58) % vs (69.93 ± 4.81) %,(29.46 ± 5.02) % vs (39.08 ± 3.50) %,0.97 ± 0.35 vs 1.56 ± 0.26,all P < 0.05].After immunosuppressive therapy,the percentage of CD3+ T lymphocyte,CD4+ T lymphocyte and the ratio of CD4+/CD8+ T lymphocyte [(71.49 ±5.16)%,(39.25 ±3.21)% and 1.56 ±0.28] recovered to the same levels in healthy controls.The percentage of CD8+ T lymphocyte and CD19+ B lymphocyte in patients with ITP were higher than those in the healthy controls [(30.28 ±4.63)% vs (25.90±3.06)%,(18.92 ± 4.27)% vs (13.17 ± 3.64)%,all P < 0.05].After treatment of immunosuppressive therapy,the percentage of CD8+ T lymphocyte and CD19+ B lymphocyte [(25.16 ± 3.45) % and (11.98 ± 3.68) %] recovered to the similar levels in healthy controls.The serum levels of IL-4,IL-6,IL-11,IL-17 and TPO in patients with ITP were significantly higher than those in healthy controls.While TGFβ level was significantly decreased.There was no significant difference of IL-27 between ITP patients and healthy controls.After the treatment of immunosuppressive therapy,IL-4,IL-6,IL-11,IL-17,TPO and TGFβ were down-regulated while IL-27 was up-regulated.There was no significant difference of IFNγ,TNFα,IL-2 and IL-10 among ITP patients before or after immunosuppressive therapy and healthy controls.Conclusions The present study suggests that the aberrant immunocyte subsets and cytokines are involved in the pathogenesis of ITP.Hyper-function of Th2 and Th17,dysfunction of Treg cells,up-regulation of IL-27,IL-11,TPO and other factors may contribute to the pathogenesis of ITP.
3.Congenital preauricular fistula infection: a histopathology observation.

OBJECTIVE:
To investigate the pathology characteristics of congenital preauricular fistula with infection, in order to reduce the recurrence rate after surgery and improve operative technique.
METHOD:
Twenty-five patients diagnosed as congenital preauricular fistula with infection were analyzed. There were 14 patients in infection history group, 9 in infective stage group, and 2 in recurrence group respectively. The whole piece of fistula and scar tissue was completely excised during operation. The specimens were observed by naked eye and serial tissue sections were analyzed.
RESULT:
(1) Macroscopically, in infection history group, initial morphology can be maintained near the fistula orifice, but the distal tissue was dark red scar tissue. In infective stage group, the distal tissue of the specimens was granulation tissue and cicatricial tissue. The granulation tissue was crisp and bright red. In recurrence group, multicystic lesions with severe edema was observed, with a classical dumb-bell appearence. (2) Microscopically, in infection history group and recurrence group, we can see that the distal fistula tissue was discontinuous and was separated by scar tissue. In infective stage group, we can find neo-angiogenesis and infiltration of plasma cells, lymphocytes, neutrophil between interrupted fistula tissues. (3) All patients were followed up for 6-12 month, without recurrence.
CONCLUSION
The fistula tissue of congenital preauricular fistula with infection was divided by the scar tissue, and they did not communicate with each other. Complete delineation of fistula is hardly achieved by methylene blue staining. Radical excision of the fistula and scar tissue may help to avoid leaving viable squamous epithelial remnants and reduce the recurrence rate.

4.Renal injury secondary to Strongyloides stercoralis infection

Objective:To report a rare case of renal injury secondary to Strongyloides stercoralis infection, and investigate common pathological subtypes, pathogenesis and differential diagnosis of Strongyloides stercoralis infection-associated renal injury combined with literature. Methods:The pathological features of renal biopsy were analyzed by immunofloruscence, light microscope and electronic microscope. The pathological changes of digestive tract and pathogen morphology were observed through endoscope and digestive tract biopsy. The correlation between clinical-pathological features and pathological changes of kidney and digestive tract was analized.Results:The 26-year-old male patient presented with nephrotic syndrome. The pathological changes of renal biopsy were consistent with minimal change disease with interstitial focal eosinophil infiltration. Laboratory examination showed that the patient had unexplained eosinophilia and increased IgE level. Hence the patient was treated with glucocorticoid. After 2 months of therapy, proteinuria decreased and turned to negative while the patient developed progressive headache, gastrointestinal bleeding and progressive decrease of hemoglobin. Emergency gastroscopy showed extensive congestion and erosion of the stomach and duodenum. Gastric mucosal biopsy showed a large number of slender "s" shape larvae in the mucosa. The patient also had bilateral lung infection, positive Escherichia coli in cerebrospinal fluid and purplish skin rash around the umbilicus. A serious infection of Strongyloides stercoralis was diagnosed. After antibiotics and anthelmintic treatment, gastrointestinal symptoms and headache disappeared, and no parasite was found in endoscopy. No recurrence of nephrotic syndrome was found during 2 years of follow-up. Conclusions:Strongyloides stercoralis infection might first present with nephrotic syndrome with handful hints of digestive tract combined with eosinophilia and increased IgE levels. Therefore, in epidemic areas or patients with suspicious exposure history, it is necessary to exclude Strongyloides stercoralis infection before immunosuppressive therapy to avoid fatal complications.
5.Investigation on the distribution of critical value limits in adult patients in Beijing

Objective To investigate the distribution of critical values of adults in Beijing, to provide the evidence for the formulation of the Standardized Management Guideline in Critical Values, in order to promote the accurate management of critical values.Methods A total of 110 398 data of critical values from the tertiary and above medical institutions during January 1 to May 31 in 2015 in Beijing were collected by the way of on-site inspection, covering the disciplines of hematology, clinical chemistry, coagulation and blood gas analysis.Fristly, the selected critical values were classified by the factor of admission departments and disease types,then were analyzed by using Kruskal-Wallis test, to compare the differences in each group.Secondly,the combined groups were classified by the factor of gender then were analyzed by using Mann-Whithey U test, to compare the differences in each group.Finally, the stratification thresholds of critical values were established.Results Except for the upper limits of Ca, pH, pCO2, Hb and the lower limits of Glu, pH, the rest of thresholds of critical values had significant differences due to different admission departments and disease types and/or gender.Conclusion Depending on the different admission departmentsces disease types and/or gender, hierarchical limit values on each critical value were formulated.
6.Study on enzyme-linked immunosorbent assay for detecting thrombospondin-1 and its diagnostic value for prostatic carcinoma

Objective To use enzyme-linked immunosorbent assay (ELISA) for measuring thrombospondin-1 (TSP-1),and to analyze its diagnostic value for prostatic carcinoma.Methods The possible difficulties and the way to solve the difficulties with ELISA spot were explored first.Three agents which could segregate idio-antigen and one technique which could depurate proteinum were designed and developed.The non- idio- proteinum cross reaction problems were solved and the routine method to measure TSP-1 with ELISA was set up successfully.The serum TSP-1 was measured in 14 patients with benign prostatic hyperplasia (BPH) and 18 patients with prostatic carcinoma.Results The TSP-1 values were (73.77±12.72)% and (121.86±-19.47)% in prostatic carcinoma group and benign prostatic hyperplasia group,respectively (t= 8.44,P<0.01).The diagnostic sensitivity and specificity of TSP-1 and prostate specific antigen (PSA) for prostatic cancer were 92.7%,88.9% and 85.7%,66.7%,respectively (P<0.01).The area under the receiver operating characteristic curve (ROC) of TSP-1 and PSA were 0.9663 and 0.7421 (P<0.05).Conclusions The determination of TSP-1 with ELISA is feasible.TSP-1 is an ideal diagnostic parameter for prostatic carcinoma and it may distinguish BPH from malignant prostatic disease more exactly than PSA.
7.Comparison of intubation of Shikani optical stylet and Clarus Video Stylet in patients with cervical spine immobilization

Objective To compare the application of Shikani optical stylet(SOS)and Clarus Video Sty-let(Tracway)in patient with cervical spine immobilization in tracheal intubation. Method Sixty patients,ASAⅠ~Ⅱ,undergoing cervical internal fixation operation,were randomly divided into Shikani optical stylet group (Group S,n = 30)and Clarus Video Stylet group(Group T,n = 30). MAP,HR and RPP(The rate-pressure product)were recorded at the point before induction of anesthesia(T1),before intubation(T2),at the immediate time of intubation(T3)and 1 min(T4),3 mins(T5),and 5 mins after intubation(T6).The intubation time,one-time success rate of intubation,the number of intubation times and the incidence of sore throat and other complica-tions were observed. Results The one-time intubation time in group S was obviously shorter than that in group T (P<0.05).The incidence of mild sore throat and intubation throat injury rate were lower in group S than those in group T(P<0.05).Compared with those at T1,MAP and RPP decreased significantly at point of T2~T6in both of two groups(P<0.05).There was no significantly difference in MAP,HR and RPP at any points of time between the two groups. The one-time success rate of intubation,the number of intubation times and the incidence of air-way complications in two groups(P>0.05)were no significantly different. Conclusions Compared with Clarus Video Stylet(Tracway),Shikani optical stylet can shorten the intubation time in patients with cervical spine immo-bilization,but no difference was found in regard to the hemodynamic influence on intubation,success rate of intu-bation,the intubation times,the sore throat and the other related complications.
8.Expression and significance of Neuritin and HSP60 in repair of acute liver injury

Objective To explore the expression changes and possible molecular mechanisms of Neuritin and HSP60 during the repair of liver injury,and to provide an experimental basis for the study of the repair of liver injury. Methods Forty-eight Sprague-Dawley(SD)rats were randomly divided into control group without any treatment(n=6)and experimental group(n=42)underwent 70% hepatectomy to induce acute liver injury,and 6 h,12 h,24 h,48 h,3 d,7 d and 14 d after the operation,the left lobe resection of the residual liver was per-formed. Immunoblotting technique (Western blot) was used to detect the expression difference of Neuritin and HSP60 in liver tissue of the corresponding time points. Hematoxylin eosin staining(hematoxylin-eosin staining, HE)was used to observe the expression changes at each time point in liver pathology,and tail vein blood to detect ATL and AST changes. Results (1)Compared with those in the control group,the serum ALT and AST levels in the experimental group were increased at 6 h,12 h,24 h and 48 h after the operation,and reached the peak at 48 h postoperatively but those began to decrease 3 d postoperatively and was almost normal 7 d postoperatively. The difference was statistically significant(P < 0.05). Pathological findings:compared with that in the control group, the hepatic lobule structure in the experimental group was disorderly. The obvious balloon like change reached the peak 48 h postoperatively.(2)In the whole process of repair of liver injury,the expression of Neuritin and HSP60 showed differences in the opposite. There was a significantly negative correlation between Neuritin expression and the repair or the aggravation of the injury(P<0.001)and the lowest expression was observed 48 h postoperatively. There was a significantly positive correlation between HSP60 expression and the repair or the aggravation of the injury(P < 0.001)and the highest expression was observed 48 h postoperatively. Conclusions (1)With simple 70% left hepatic lobectomy,the repair of liver injury model of SD rats is successfully established and the heaviest injury is observed 48h postoperatively. With a high success rate(100%,42/42),simple and practical,the method provides a reliable and convenient animal model for the study of liver regeneration,liver injury and liver transplan-tation.(2)The expression of Neuritin decreases gradually after liver injury and reaches the lowest 48 h postopera-tively,while the expression of HSP60 increases gradually and reaches the highest 48 h postoperatively.(3)The change of expression of Neuritin and HSP60 is closely related to the process of liver injury repair(P < 0.001), showing a certain change rule. They may have some biological effects through interactions,and participate in and promote the regeneration and repair of liver injury.
9.Research progress of respiratory tract infection induced by respiratory syncytial virus in children

Respiratory syncytial virus (RSV) is a major cause of respiratory tract infection in children,and RSV infection is an important inducement to recurrent cough or wheezing in children.Nevertheless,the pathogenes is not clear,the clinical presentations are significantly different.Its test is not effective and fast,and the positive therapies to control RSV airway infection are lacking.Therefore,to control the respiratory tract disease of the children,it is important to know the research progress of the pathogenesis,the clinical manifestation,the examination,and the prevention and therapy of RSV infection.
10.Investigation and analysis of internal quality control on clinical chemistry, clinical immunology and clinical hematology of mutual recognition laboratories in 142 medical institutions in Beijing

Objective To investigate the internal quality control ( IQC ) on clinical chemistry , clinical immunology and clinical hematology in mutual recognition laboratories in medical institutions in Beijing.Methods By means of questionnaire survey and on -site investigation, fresh frozen serum and whole blood samples with assigned values by reference method were measured to investigate the status of IQC on clinical chemistry , clinical immunology and clinical hematology in 142 mutual recognition laboratories in medical institutions of Beijing,and results were analyzed.Results 142 copies of questionnaireson clinical chemistry, clinical immunology and clinical hematology were send out and 120, 97, and 101 laboratories returned the questionnaires respectively .The information feedback rate was 84.5%, 68.3% and 71.1%respectively .All the questionnaires were effective .Questionnaires survey results showed that more than 50%laboratories set up quality control goals and the most of the goals were probability for error detection ( Ped) 95%, probability for false rejection(Pfr)5%;About 70% laboratories usecd the same quality control plan for different tests ;The most frequently used quality control rules are 12s/13s/22s.On-site investigation showed that ,take the results of clinical chemistry for example , based on the desirable biological variation and WS/T 403 -2012 , most of the tests can't meet the quality control goalsunder the existing quality controlcondition.Conclusion Clinical laboratories should consider their actual situations , assess their own qualitylevels that they can reach , set reasonable quality standards for themselves , and make appropriateindividualized quality control plan.
