Objective To investigate the relationship of plasma brain natriuretic peptide (BNP) and N-terminal pro-B-type natriuretic peptide (NT-proBNP) levels with the severity of heart dysfunction in elderly patients with heart failure. Methods BNP levels were measured by immunofluorescence in 106 hospitalized patients with heart failure, while NT-proBNP levels were measured by electrochemiluminescence immunoassay in 48 patients. The severity of heart dysfunction was determined by NYHA classification. The left ventricular ejection fraction (LVEF) measured by Color Doppler Ultrasound was used to show the left ventricular systolic function. Results In patients with BNP≤400 ng/L versus BNP>400 ng/L, the rates of LVEF≤45% were 16.4% versus 46.8% (χ~2= 13. 93, P=0. 001), the rates of E/A<1 were 62.3% versus 40. 4% (χ~2 =22. 19,P = 0.024) and the rates of Cr>107 μmol/L were 13.1% versus 38.3% (χ~2 = 11. 31, P = 0. 002) . In patients with NT-proBNP≤400 ng/L versus NT-proBNP>400 ng/L, the rates of LVEF≤45% were 25. 0% versus 58. 3% (χ~2 = 10. 00,P = 0. 019) and the rates of Cr>107 μmol/L were 20. 8% versus 50.0% (χ~2=8. 50,P = 0. 035). Conclusions BNP or NT-proBNP levels increase significantly along the deterioration of the heart failure severity, and it may be important to use BNP or NT-proBNP to predict left ventricular function.

Objective To explore the current condition of psychological problems of patients in cardiology clinic. Methods A total of 347 patients in the cardiology clinic of Prof. HU Da-yi were screened by self-rating depression scale(SDS),Self-rating anxiety scale(SAS),Hamilton rating scale for depression(HRSD)and Hamilton anxiety scale(HAMA). Results The rates of patients with somatic disease only or with psychological disorder only were 52.4%and 12.7%, the rate of somatic disease accompanied with psychological disorder was 27.7%. For the patients with psychological problems, there were 31 cases with depression(8.9%),62 cases with anxiety(17.9%),and 28 cases(8.1%) suffering from mixed depression and anxiety the diagnosis of cardiovascular disease, functional disease or digestive disease was always used by doctors for the patients with psychological problems only,as they always did not accept the diagnosis of psychological disorder. Conclusions It is very common that patients in cardiology clinic department suffered from psychological problems. The patients with psychological disorder only were easily misdiagnosed as somatic disease especially cardiovascular disease.

Chinese Journal of General Practitioners 2010;09(11):793-795
The X-ray computed tomography (CT) appearance of 4 cases with pulmonary cryptococcosis (PC) diagnosed by pathological examination in our hospital was retrospectively analyzed. The appearances of PC on CT were various: solitary lesion in 1 case, multiple lesions in single lobe in 2, and multiple lesions in multiple lobes in 1. There were total 52 lesions in 4 cases; the diameter of nodules or masses was 3 - 75 mm. Cavitations were found in 1 case; lesions appeared obviously enhanced and one lesion showed central necrosis. Two cases underwent pulmonary lobectomy; and 2 cases received core cutting needle biopsies, after antifungal therapy for 3 months to 1 year the lesions showed being absorbed. In summary, the CT appearance of PC is non-specific with various modes and forms. PC should be considered when multiple nodules or masses scattered in subpleural zone, accompanied with ground-glass opacity and obviously enhanced. The examination of pathogen and pathology at the beginning is crucial for improving diagnostic accuracy.
