1.Comparison of contrast-enhanced ultrasonography and conventional ultrasonography in diagnosis of polypoid lesions of gallbladder

ObjectiveTo compare the clinical value of contrast-enhanced ultrasonography (CEUS) and conventional ultrasonography in diagnosing polypoid lesion of gallbladder (PLG).MethodsThe postoperative pathology and clinical manifestation of 49 PLG patients were retrospectively analyzed in the First Afifliated Hospital of Harbin Medical University. Of them, 7 cases were maligant lesions, 4 cases were adenomas, 24 cases were polyps of gallbladder and 14 cases were adenomyomatosis of gallbladder. With postoperative pathology as golden standard, the sensitivity, speciifcity and PLG speciifcity were calculatedfor conventional ultrasonography, CEUS and conventional ultrasonography plus CEUS.ResultsThe sensitivity, specificity and PLG specificity were 33.3%, 74.3% and 68.3% for conventional ultrasonography, and 66.7%, 85.7% and 82.9% for CEUS respectively. The conventional ultrasonography plus CEUS were 83.3%, 88.6% and 87.8%.ConclusionCEUS could signiifcantly improve the diagnostic accuracy.

To explore the measures of nursing management in the treatment of large number of infants with urinary calculi. The nursing management measures included launching the preparedness and response project for sudden public health events, formulating scientific and standardized nursing management system,optimizing work flow,strengthening nurse training,focusing on the details in nursing management,implementing disinfection and isolation system seriously,and paying close attention to health education for the parents of minority infants. Scientific nursing management can ensure the treatment effectiveness and nursing safety for the infants with urinary calculi.
3.Tissue distribution of brucine-loaded solid lipid nanoparticles in mice in vivo

AIM To investigate the tissue distribution of brucine-loaded solid lipid nanoparticles in mice in vivo.METHODS Mice were intravenously injected with suspension of prepared brucine-loaded solid lipid nanoparticles and marked by fluorescein isothiocyanate (FITC).The in vivo tissue distribution of nanoparticles was analyzed by having the brucine contents in various tissues (heart,liver,spleen,lung,kidney and bone) determined by HPLC,after which fluorescence confocal laser endomicroscopy was used for further detection.RESULTS Brucine had its the highest (1.64) relative intake efficiency (Re) in mice liver,and the nanoparticles shared all over one value of targeting efficiencies (Te) in various tissues,manifesting a much stronger selectivity to liver than that of brucine solution.With the extension of time,the FITC-narked nanoparticles displayed a rich extracellular to intracellular distribution indicating a positive correlation.CONCLUSION Brucine's increased distribution in the liver tissue of mice due to its solid lipid nanoparticle form shows obvious for liver targeting.

Objective To investigate the effect of different HAART regimens on liver function,viral load and CD4+T cells of HIV infected children with HCV co-infection.Methods A total of 40 patients were divided into 4 groups:13 cases in AZT+3TC+NVP group(group A),16 cases in AZT+3TC+EFV group(group B),5 cases in D4T+3TC+EFV group(group C)and 6 cases in D4T+3TC+NVP group(group D).Liver function,viral load and CD4+T cells of patients in 4 groups were compared between before and after HAART.Results Eight patients(20%)had abnormal liver function before HAART treatment,and all of them had 1 level of liver damage.After receiving HAART,levels of ALT and AST were increased in group A and B(P<0.01);AST levels were increased in group C(P<0.01).CD4+T counts were all increased and HIV viral loads were decreased in 4 groups(P<0.01).There was no significant difference in RNA HCV load before and after HAART treatment(P>0.05).Conclusion Different HAART regimens can increase the liver damage in HIV infected children co-infected with HCV,but have little effect on the treatment efficiency of HAART regimens.
6.Association of related knowledge level and compliance of young and middle-aged hypertensive patients

Objective To evaluate the relevant knowledge level and compliance of young and middle-aged hypertensive patients,and explore the association of them. Method A total of 276 young and middle-aged hypertensive patients were surveyed using a hypertension knowledge level scale and treatment compliance questionnaire. Results The average score of young and middle-aged hypertensive patients on relevant knowledge was (62.55 ± 17.79). From different dimensions, the hypertensive patients got the highest score in the knowledge of their lifestyle,while they got the lowest score in their knowledge for complication.The average score of young and middle-aged hypertensive patients on treatment compliance was (71.53±9.87).From different dimensions,the hypertensive patients got the highest score in medication adherence,while they got the lowest score in exercise and pressure-relieving compliance.It showed that medication and drug compliance knowledge of middle-aged hypertensive patients was closely associated with treatment compliance(r=0.648, P<0.001)). Conclusions Both knowledge level and treatment compliance of young and middle-aged hypertensive patients should be improved.We should strengthen the population of knowledge about hypertension among young and middle-aged patients,especially the guidance of drug knowledge to improve their compliance.
7.Prenatal echocardiographic features and outcomes of congenital ventricular outpouching in ten fetuses

Objective:To summarize the echocardiographic features and outcomes in fetuses with congenital ventricular outpouching (CVO).Methods:This retrospective study enrolled ten fetuses diagnosed with CVO by fetal echocardiography in the Affiliated Hospital of Qingdao University and Qingdao Women and Children's Hospital from January 2015 to April 2022. Clinical data were analyzed, including echocardiographic features, other intracardiac and extracardiac malformations, karyotypes, and pregnancy outcomes. Data were analyzed by descriptive statistics.Results:All ten cases were single, including eight ventricular diverticula and two ventricular aneurysms. Five cases had the anomaly in the left ventricular and the other five in the right. Five cases were isolated malformations, and the other five were complicated by other intra- or extracardiac malformations. A pathogenic copy number variation was detected in one case. Three pregnancies were terminated, and one was lost to follow-up. The other six fetuses were born alive and showed no obvious clinical symptoms or abnormalities in growth and development during 3-70 months of follow-up. The right ventricular diverticulum spontaneously disappeared in one case. One case with the right ventricular aneurysm was also diagnosed with noncompaction of the left ventricular myocardium by echocardiography at six months.Conclusions:Fetal CVO presents with typical echocardiographic features and can be diagnosed prenatally. Regular follow-up during pregnancy is recommended to observe the sizes of outpouchings and the occurrence of complications in fetuses with CVO after excluding other structural and chromosomal abnormalities to avoid unnecessary termination. Attention should also be paid to postnatal follow-up.
8.Application of perioperative nutrition management strategy in patients with hepatic cystic echinococcosis

Objective:To explore the application of perioperative nutrition management strategy in patients with hepatic cystic echinococcosis.Methods:From October 2017 to February 2019, 53 patients with hepatic cystic echinococcosis who met the inclusion criteria were selected as the control group. Another 53 patients from June 2018 to January 2019 were collected as the experimental group, the nutritional index, liver function, complication rate and postoperative recovery were compared between the two groups.Results:The levels of retinol-binding protein, prealbumin and transferrin were (24.32 ± 3.76) μg/L, (167.00 ± 24.12) mg/L, (2.08 ± 0.43) μg/L on the day of admission in the experimental group, and one day before the operation were (27.78± 4.98) μg/L, (245.00 ± 22.02) mg/L,(2.47 ± 0.54)μ g/L, there was no significant difference ( t=0.576-3.552, P < 0.05). The incidence of infection, biliary leakage and hemorrhage in the experimental group were 0, 1.89% (1/53) and 0 respectively, which were lower than 9.43% (5/53), 13.21% (7/53), 11.32% (6/53) in the control group ( χ2 value was 4.867, P < 0.05). The aspartate aminotransferase and alanine aminotransferase of the experimental group on the 7th day after operation were (51.50 ± 6.30), (29.54 ± 2.03) U/L, which were significantly different from (69.53 ± 7.07), (43.72±3.67) U/L of the control group ( t value was 2.032, 2.015, P<0.05). Anal exhaust, defecation time, hospitalization time and expenses in the experimental group were (15.89±8.34) h, (49.12±10.56) h, (8.69 ± 1.69) d, (2.84±1.37) thousand yuan, which were significantly different from (34.13±7.13) h, (63.45±11.03) h, (11.51±4.18) d, (3.76±1.53) thousand yuan in the control group. There was statistical significance ( t values were 3.372-12.592, P<0.05). Conclusions:The application of perioperative nutrition management strategy in patients with hepatic cystic echinococcosis can improve the nutritional status of patients during perioperative period, promote the recovery of liver function and reduce the occurrence of postoperative complications.
10.The relationship between compliance and perceived social support in young patients with newly treated pulmonary tuberculosis based on the Southern Xinjiang Model: the mediating role of medication self-efficacy

Objective:To explore the mediating effect of medication self-efficacy between perceived social support and compliance behavior in young patients with newly treated pulmonary tuberculosis under the Southern Xinjiang Model.Methods:A cross-sectional survey was conducted in Kashgar Tuberculosis Prevention and Control Institute Hospital and Tuberculosis Specialist Hospital of Hetian from January 2022 to April 2023. A total of 302 young patients with initial treatment of pulmonary tuberculosis in were selected as the research objects by convenience sampling method. The General Information Questionnaire, the Tuberculosis Compliance Behavior Rating Scale, Perceived Social Support Scale and Self-efficacy for Appropriate Medication Use scale were used to investigate, and the correlation analysis and mediating effect analysis were carried out.Results:The total scores of compliance behavior, self-efficacy and perceived social support were (119.09 ± 13.49), (23.48 ± 6.25) and (55.67 ± 11.33) respectively. All dimensions of medication self-efficacy and perceived social support were positively correlated with compliance behavior ( r values were 0.555 - 0.943, all P<0.01), and medication self-efficacy was positively correlated with perceived social support ( r = 0.981, P<0.01). Medication self-efficacy played a partial mediating effect between perceived social support and compliance behavior, accounting for 74.46% of the total effect. Conclusions:The medication self-efficacy of young patients with newly treated pulmonary tuberculosis is the mediating variable between perceived social support and compliance behavior. Medical staff should focus on and carefully evaluate the medication self-efficacy and perceived social support of patients, so as to give supportive intervention in time, so as to change or even reverse the non-compliance behavior and improve the compliance level of patients.
